Perioperative intensive insulin therapy using artificial endocrine pancreas in patients undergoing pancreatectomy

Perioperative glycemic control is important for reducing postoperative infectious complications. However, clinical trials have shown that efforts to maintain normoglycemia in intensive care unit patients result in deviation of glucose levels from the optimal range, and frequent attacks of hypoglycemia. Tight glycemic control is even more challenging in those undergoing pancreatic resection. Removal of lesions and surrounding normal pancreatic tissue often cause hormone deficiencies that lead to the destruction of glucose homeostasis, which is termed pancreatogenic diabetes. Pancreatogenic diabetes is characterized by the occurrence of hyperglycemia and iatrogenic severe hypoglycemia, which adversely effects patient recovery. Postoperatively, a variety of factors including surgical stress, inflammatory cytokines, sympathomimetic drug therapy, and aggressive nutritional support can also affect glycemic control. This review discusses the endocrine aspects of pancreatic resection and highlights postoperative glycemic control using a closed-loop system or artificial pancreas. In previous experiments, we have demonstrated the reliability of the artificial pancreas in dogs with total pancreatectomy, and its postoperative clinical use has been shown to be effectiveand safe, without the occurrence of hypoglycemic episodes, even in patients after total pancreatectomy. Considering the increasing requirement for tight perioperative glycemic control and the recognized risk of hypoglycemia, we propose the use of an artificial endocrine pancreas that is able to monitor continuously blood glucose concentrations with proven accuracy, and administer automatically substances to return blood glucose concentration to the optimal narrow range.

Certain perovskite-type oxynitrides have bandgaps suitable for renewable hydrogen production via photocatalytic and photoelectrochemical water splitting under visible *** the ordering of oxide and nitride anions in these materials is important because this ordering affects their semiconductor ***, the numerous possible orderings complicate systematic analyses based on density functional theory(DFT) calculations using defined elemental *** work shows that anion ordering in large-scale supercells within perovskite-type oxynitrides can be rapidly predicted based on machine learning, using BaNbO2N(capable of oxidizing water under irradiation up to 740 nm) as an *** learning allows the calculation of the total energy of BaNbO2N directly from randomly selected initial atomic placements without costly structural optimization, thus reducing the computational cost by more than 99.99%.Combined with the Metropolis Monte Carlo method, machine learning permits exploration of the stable anion orderings of large supercells without costly DFT *** work therefore demonstrates a means of predicting the properties of functional materials having complex compositions based on the most realistic elemental arrangements in conjunction with reasonable computational loads.

A 67-year-old woman underwent right radical neck dissection for cervical lymph node metastasis from maxillary gingival carcinoma. Two months later, metastasis in the left superior internal jugular lymph nodes were discovered, and left radical neck dissection was performed. Postoperatively, airway obstruction occurred despite performing extubation after confirming that the patient had fully recovered from anesthesia. Bilateral hypoglossal nerve palsy was diagnosed and the patient was reintubated. After extubation on the following day, airway obstruction was relieved, but slurred speech and impaired swallowing were persistent. In view of this, hypoglossal nerve function should be examined before the second radical neck dissection on the contralateral side.

Remifentanil Has Sufficient Hypnotic and Amnesic Effect for Induction of Anesthesia by Itself

With a small-dose remifentanil, some patients showed no reaction and did not remember it postoperatively. We, therefore, hypothesized that remifentanil may decrease the level of consciousness and/or exhibit amnesic effect when stimulations are avoided. Thirty-patients were divided into two groups: non-stimulation group and stimulation group. Anesthesia was induced with 1 micro-g·kg?1·min?1 of remifentanil using no additional hypnotic agent. In the non-stimulation group, patients were left free from any stimulation except non-invasive blood pressure monitoring. In the stimulation group, patients were asked to follow verbal commands. The level of consciousness was evaluated with electroencephalogram and BIS-value derived from it. In the non-stimulation group, all patients reached the decreased level of consciousness in 5 minutes. In the stimulation group, however, 14 patients were judged to be still conscious. 10 patients could open their mouth at the 5th minute, but 9 of these 10 patients did not remember it postoperatively. In conclusion, remifentanil, with no additional anesthetics, exhibited hypnotic and amnesic effects when stimulations were kept minimal.

Effect of super-chilling storage on maintenance of quality and freshness of the Pacific oyster(Crassostrea gigas)

The quality changes of shelled Pacific oysters(Crassostrea gigas)were examined in relation to the effects of superchilling storage at-1 C for 28 d by measuring changes in biochemical properties(microbial analysis,adenosine triphosphate(ATP)-related compounds,pH,free amino acids)and sensory evaluations in this *** results indicated that microorganism growth was significantly inhibited during superchilling *** diphosphate(ADP)and adenosine monophosphate(AMP)accumulated while ATP rapidly decreased in the adductor *** and ADP were the primary components in the other 3 tissues including mantle,gill,and body trunk of oysters,and they remained relatively stable over *** pH and adenylate energy charge in the adductor muscle could be utilized as freshness indicators for shelled ***,there were no significant differences(P>0.05)among the free amino acids during whole *** to the sensory evalu-ations,oysters could be alive and tolerated up to 21 d at-1℃*** study demonstrated that superchilling storage at-1℃could better maintain the eating quality of shelled oysters and the shelf life was extended to 21 d.

AIM:To identify factors that influence long-term liver function following radiofrequency ablation(RFA)in patients with viral hepatitis-related hepatocellular ***:A total of 123 patients with hepatitis B virus-or hepatitis C virus-related hepatocellular carcinoma(HCC)(n=12 and n=111,respectively)were *** rates of worsening Child-Pugh(CP)scores(defined as a 2-point increase)were ***:CP score worsening was confirmed in 22patients over a mean follow-up period of 43.8±*** analysis identified CP class,platelet count,and aspartate aminotransferase levels as significant predictors of a worsening CP score(P=0.000,P=0.011 and P=0.024,respectively).In contrast,repeated RFA was not identified as a risk factor for liver function ***:Long-term liver function following RFA was dependent on liver functional reserve,the degreeof fibrosis present,and the activity of the hepatitis condition for this ***,in order to maintain liver function for an extended period following RFA,suppression of viral hepatitis activity is important even after the treatment of HCC.

Cell Proliferation Inhibition by Sericin from the Wild Silkworm, Cricula trifenestrata

Sericin is a major silk protein, and recent studies have shown that sericin has numerous physiological activities against cultured cells. For example, it accelerates mammalian cell proliferation and protects insect cells against cell death. In this study, we examined the activities of sericin prepared from the wild silkworm, Cricula trifenestrata. The results presented here indicated that C. trifenestrata sericin has anti-proliferative activity against feline kidney cells, while sericin from Bombyx mori promoted cell proliferation. The components of the sericin preparation that played a role in inhibition of proliferation were analyzed by fractionation through partition chromatography (Folch method) and reversed-phase chromatography. Our analysis showed that at least two components in the C. trifenestrata sericin preparation promoted the anti-proliferative activity in an additive or even synergistic manner. Therefore, differences in components between insect species may contribute to different activities against cultured cells.
