Background:The AbSeS-classification defines specific phenotypes of patients with intra-abdominal infection based on the(1)setting of infection onset(community-acquired,early onset,or late-onset hospital-acquired),(2)presence or absence of either localized or diffuse peritonitis,and(3)severity of disease expression(infection,sepsis,or septic shock).This classification system demonstrated reliable risk stratification in intensive care unit(ICU)patients with intra-abdominal *** study aimed to describe the epidemiology of ICU patients with pancreatic infection and assess the relationship between the components of the AbSeS-classification and ***:This was a secondary analysis of an international observational study(“AbSeS”)investigating ICU patients with intra-abdominal *** patients with pancreatic infection were included in this analysis(n=165).Mortality was defined as ICU mortality within 28 days of observation for patients discharged earlier from the *** with mortality were assessed using logistic regression analysis and reported as odds ratio(OR)and 95%confidence interval(CI).Results:The overall mortality was 35.2%(n=58).The independent risk factors for mortality included older age(OR=1.03,95%CI:1.0 to 1.1 P=0.023),localized peritonitis(OR=4.4,95%CI:1.4 to 13.9 P=0.011),and persistent signs of inflammation at day 7(OR=9.5,95%CI:3.8 to 23.9,Pwithin the first week(OR=4.0,95%CI:1.3 to 12.2,P=0.013).Gramnegative bacteria were most frequently isolated(n=58,49.2%)without clinically relevant differences in microbial etiology between survivors and ***:In pancreatic infection,a challenging source/damage control and ongoing pancreatic inflammation appear to be the strongest contributors to an unfavorable short-term *** this limited series,essentials of the AbSeS-classification,such as the setting of infection onset,diffuse

Chemically modified STM tips for atomic-resolution imaging of ultrathin NaCI films

Cl-functionalized scanning tunneling microscopy （STM） tips are fabricated by modifying a tungsten STM tip in situ on islands of ultrathin NaCI（100） films on Au（111） surfaces. The functionalized tips are used to achieve clear atomic- resolution imaging of NaCI（100） islands. In comparison with bare metal tips, the chemically modified tips yield drastically enhanced spatial resolution as well as contrast reversal in STM topographs, implying that Na atoms, rather than C1 atoms, are imaged as protrusions. STM simulations based on a Green＇s function formalism reveal that the experimentally observed contrast reversal in the STM topographs is due to the highly localized character of the Cl-pz states at the tip apex. An additional remarkable characteristic of the modified tips is that in dI/dV maps, a Na atom appears as a ring with a diameter that depends crucially on the tip-sample distance.

water use efficiency in an arid watershed: a case study

Integrated water Resource Management （IwRM） becomes increasingly important in arid watersheds, where water resources are scarce and demands are rising under increased population pressure. In this case study, we present the results from the Elqui Valley in Chile, where water resources are shared between agriculture, mining operations, hydropower generation and drinking water for nearby cities. An IwRM model was applied to visualize water resources distribution in the catchment and determine irrigation and water use efficiencies. After calibration with observed surface and groundwater measurements, the model showed large differ- ences in efficiencies between different irrigation sectors. Most notably, irrigation sectors with the highest irrigation security, i.e., higher water rights per hectare, were characterized with lower water use efficiencies. Frequent water deficits were present in sec- tors downstream of the main reservoir, which was related to limited water rights per hectare, lower irrigation efficiencies and higher water demands due to larger agricultural areas. In subsequent scenario analyses, the model was used to evaluate the impact of improved irrigation and water use efficiencies in these downstream sectors, to confirm an important reduction in water deficits, except for drought years when surface water resources are insufficient. In a third scenario, groundwater resources were allowed to compensate for water deficits, effectively reducing these in most irrigation sectors, especially during droughts. Expansion of the current agricultural area by 37% was evaluated, as a future scenario, and was found to be unsustainable, as aquifer levels are low- ered to such levels that make annual recharge insufficient.

Effect of introducing an online system on the follow-up of elbow arthroplasty

AIM To evaluate the effect of introducing a structured online follow-up system on the response *** Since June 2015 we have set up an electronic followup system for prosthesis in orthopedic patients. This system allows prospective data gathering using both online and paper questionnaires. In the past all patients received questionnaires on paper. This study includes only patients who received elbow arthroplasty. Response rates before and after introduction of the online database were compared. After the implementation,completeness of the questionnaires was compared between paper and digital versions. For both comparisons Fisher's Exact tests were *** A total of 233 patients were included in the study. with the introduction of this online follow-up system,the overall response rate increased from 49.8% to 91.6%(P was comparable to 90.7% in the online group(P > 0.05).Paper questionnaires had a completeness of 54.4%,which was lower compared to the online questionnaires where we reached full completeness(P with a mean age of 52 years compared to a mean age 62 years of responders(P w-up system increased the response rate. Moreover,online questionnaires are more complete than paper questionnaires.

Palaeozoic tectonic evolution of the Tianshan belt,Nw China

The Chinese Tianshan belt is a major part of the southern Central Asian Orogenic Belt, extending westward to Kyrgyzstan and Kazakhstan. Its Paleozoic tectonic evolution, crucial for understanding the amalgamation of Central Asia, comprises two stages of subduction-collision. The first collisional stage built the Eo-Tianshan Mountains, before a Visean unconformity, in which all structures are verging north. It implied a southward subduction of the Central Tianshan Ocean beneath the Tarim active margin, that induced the Ordovician-Early Devonian Central Tianshan arc, to the south of which the South Tianshan back-arc basin opened. During the Late Devonian, the closure of this ocean led to a collision between Central Tianshan arc and the Kazakhstan-Yili-North Tianshan Block, and subsequently closure of the South Tianhan back-arc basin, producing two suture zones, namely the Central Tianshan and South Tianshan suture zones where ophiolitic melanges and HP metamorphic rocks were emplaced northward. The second stage included the Late Devonian-Carboniferous southward subduction of North Tianshan Ocean beneath the Eo-Tianshan active margin, underlined by the Yili-North Tianshan arc, leading to the collision between the Kazakhstan-Yili-NTS plate and an inferred Junggar Block at Late Carboniferous-Early Permian time. The North Tianshan Suture Zone underlines likely the last oceanic closure of Central Asia Orogenic Belt; all the oceanic domains were consumed before the Middle Permian. The amalgamated units were affected by a Permian major wrenching, dextral in the Tianshan. The correlation with the Kazakh and Kyrgyz Tianshan is clarified. The Kyrgyz South Tianshan is equivalent to the whole part of Chinese Tianshan （CTS and STS） located to the south of Narat Fault and Main Tianshan Shear Zone; the so-called Middle Tianshan thins out toward the east. Size-dependent dissociation of small cobalt clusters on ultrathin NaCI films

Mass-selected anionic cobalt clusters of two different sizes （Co2 and Co13） were deposited onto ultrathin NaC1 films grown on an Au（111） substrate. Using scanning tunneling microscopy experiments and density functional theory simulations, we show that the deposited Co2 cluster dissociates and that the resulting Co atoms dope the NaCI surface by substituting Na ions. In contrast, the larger Co13 cluster does not dissociate and remains stable on top of the NaC1 film. The size- dependent fragmentation of clusters is an important aspect in the understanding of the chemical interaction between size-selected small aggregates of atoms and supporting surfaces.
