Adverse events of sacral neuromodulation for fecal incontinence reported to the federal drug administration

AIM:To investigate the nature and severity of AE related to sacral neurostimulation(SNS).METHODS:Based on Pubmed and Embase searches,we identified published trials and case series of SNS for fecal incontinence(FI)and extracted data on adverse events,requiring an active *** problems were operationally defined as infection,device removal explant or need for lead and/or generator *** addition,we analyzed the Manufacturer and User Device Experience registry of the Federal Drug Administration for the months of August-October *** were included if the report specifically mentioned gastrointestinal(GI),bowel and FI as indication and if the narrative did not focus on bladder *** classification,reporter,the date of the recorded complaint,time between initial implant and report,the type of AE,steps taken and outcome were extracted from the *** cases of device removal or replacement,we looked for confirmatory comments by healthcare providers or the ***:Published studies reported adverse events and reoperation rates for 1954 patients,followed for 27(1-117)*** rates were 18.6%(14.2-23.9)with device explants accounting for 10.0%(7.8-12.7)of secondary surgeries;rates of device replacement or explant or pocket site and electrode revisions increased with longer follow *** the period examined,the FDA received 1684 reports of AE related to SNS with FI or GI listed as indication.A total of 652 reports met the inclusion criteria,with 52.7%specifically listing *** or loss of benefit(48.9%),pain or dysesthesia(27.8%)and complication at the generator implantation site(8.7%)were most commonly *** led to secondary surgeries in 29.7%of the *** were performed to explant(38.2%)or replace(46.5%)the device or a lead,or revise the generator pocket(14.6%).Conservative management changes mostly involved changes in stimulation parameters(44.5%),which successfully addressed concerns in 35.

AIM： To test the hypothesis that pain and affect rather than impaired emptying determine symptom severity in patients with gastroparesis. METHODS： Adult patients with documented gastroparesis were enrolled prospectively in a single center and asked to complete the Gastroparesis Cardinal Symptom Index （GCSt）, Hospital Anxiety and Depression Scale （HADS）, the Short Form 12 （SF-12） as quality of life index, rate pain severity and answer 10 open-ended questions. RESULTS： A total of 55 patients （44 women） participated. Idiopathic （n = 29） or diabetic （n = 11） gastroparesis and connective tissue disease （n = 8） were the most common underlying causes. Antiemetics （n = 30） and prokinetics （n = 32） were most often prescribed. Seventeen patients used opioids on a daily basis. Nausea and/or vomiting （n = 28）, pain （n = 24） and bloating （n = 14） were most commonly listed as dominant symptoms. Patients subjectively attributed symptom improvement to nutritional and dietary therapy （n= 11）, prokinetics （n = 11）, antiemetics （n = 10） or analgesic agents （n = 3）. In univariate analyses, the physical subscore of the SF-12 and HADS, but not gastric emptying delay or symptom duration significantly correlated with disease severity as measured by the GCSI. In multivariate analyses, the combination of vomiting, bloating and depression best predicted the overall impact on quality of life. CONCLUSION： The study confirms the importance of pain and affect in gastroparesis, which requires novel approaches to improve more effectively the quality of life in patients with this disorder.

Warm-wet climatic conditions are widely regarded as conducive to remarkable tree growth, alleviating climatic pressures. However, the notable decline in tree growth observed in the southern edge of boreal forests has heightened concerns over the spatial-temporal dynamics of forest decline. Currently, attaining a comprehensive grasp of the underlying patterns and their propelling factors remains a formidable challenge. We collected tree ring samples from a network of 50 sites across the Greater Xing'an Mountains. These samples were subsequently grouped into two distinct clusters, designated as Groups A and B. The percentage change of growth (GC, %) and the proportion of declining sites were utilized to assess forest decline. The decline in tree growth within Larix gmelinii forests exhibits significant regional variation, accompanied by temporal fluctuations even within a given region. Group A exhibited a pronounced increase in frequency (59.26%) of occurrences and encountered more severe declines (21.65%) in tree growth subsequent to the 1990s, contrasting sharply with Group B, which observed lower frequencies (20.00%) and relatively less severe declines (21.02%) prior to the 1980s. The primary impetus underlying the opposite radial growth increments observed in Larix gmelinii trees from the interplay between their differential response to temperatures and wetter climatic conditions, which is significantly influenced by varying stand densities. In cold-dry conditions, low-density forests may experience soil water freezing, exacerbating drought conditions and thereby inhibiting tree growth, in Group B. Trees growth in high-density stands is restrained by warm-wet conditions, in Group A. These results provide new insights into the variability at the southern edge of the boreal forest biome with different responses to density and climate.

Dieback of spruce plantations: Deadwood stands and management heterogeneity enhance beetle diversity and habitat connectivity

Forest ecosystems are important for biodiversity conservation and human societies, but are under pressure due to climate change and human interventions. This applies to natural forests as well as tree plantations. The latter are globally widespread and therefore gaining increasing importance for biodiversity conservation. However, even after dieback due to increasing disturbance frequencies, such plantations are primarily managed for economic returns, leading to growing conflicts among stakeholders. In particular, the impact of forest management on biodiversity is being discussed. This study investigates the effects of five management approaches in a landscape severely affected by spruce (Picea abies L.) dieback on beetle diversity, conservation, and community composition. We considered direct effects of management and indirect effects of environmental parameters separately in ground-dwelling and flight-active beetles. Beetle diversity was strongly affected by forest management, with nonintervention deadwood stands being most beneficial for beetles. In addition, we show indirect effects of environmental factors. In general, parameters related to salvage logging (e.g. open canopies, tree stumps) influenced beetle diversity and conservation negatively, while positive effects were found for soil nutrient availability and plant species richness. Community composition differed strongly among management categories and indicated a lack of landscape connectivity for open habitat species, as we found only low proportions of such species even on salvage-logged sites. We propose a mixture of management approaches after bark beetle outbreaks, including a substantial proportion of non-intervention deadwood stands, to increase landscape heterogeneity and connectivity. This may increase overall biodiversity while addressing the concerns of both forestry and species conservation.

Zn-1.0Cu-0.5Ca(TA15)alloy has shown promising characteristics of enhanced mechanical properties and biodegradability for absorbable cardiovascular stents,endovascular devices,and wound closure devices *** this study,the TA15 alloy for bioabsorbable biomedical applications is *** the conventionally cast TA15(TA15-C)alloy,CaZn_(13) phase are present as a large dendritic network with an average size of 73.25±112.84μm,Hot rolling of the TA15-C alloy has broken the long and dendritic network of the CaZn_(13) phases,however,the refined phases are observed as segregations and the distribution is *** segregated CaZn_(13) suffered heavy localised corrosion which lead to poor mechanical properties in the as-fabricated condition and after *** treatment(UST)during casting is identified as an effective technique for the refinement and redistribution of CaZn_(13) particles in TA15 alloy,which successfully reduce the size of the CaZn_(13) phase to 10.91±4.65μm in the as-solidified *** hot rolling,the UST processed TA15(TA15-UST)shows improved mechanical properties due to grain refinement and the reduction in microstructural defects,*** broken CaZn_(13) *** of 8-week immersion corrosion tests showed that both alloys possess very similar corrosion ***,TA15-UST has markedly improved corrosion homogeneity compared to TA15-N which favours the retention of mechanical properties even after prolonged exposure to physiological fluids.
