Chains are typically used for tension load transfer. They are very flexible and allow easy length adjustment by hooking at the links. Steel is the traditional material for chains. Recently, synthetic link chains made from ultra-strong polyethylene fibers, branded as Dyneema®, are commercially available. These chains offer a highly improved strength to weight ratio. So far, one type of such chains is available, and it has a Working Load Limit of 100 kN. 50 of such chains, containing 6 links were tested to fracture. The strength of each chain and the location of the failed link were documented during testing for later interpretation. Weibull statistics was applied in order to extrapolate towards the allowable load for very low failure risks (high reliability). Two approaches were used. One extrapolation was based on all results;the other was applied after recognition that the end links failed under a slight negative influence by the connection to the testing equipment. Thus, in fact two populations are mixed, the chains with failing end links and the chains with failing central links. So considering the population without the failing end links is more representative for pure chain behavior without clamping effects. The results from this latter consideration showed a higher Weibull exponent, thus a more realistic extrapolation behavior. Both methods indicate that the reliability at the working load limit of 100 kN is very good.

Background ST-segment elevation （STE） in the right precordial leads is an ECG manifestation of acute pul- monary embolism （APE） and some patients present with concomitant STE in the inferior leads. This clinical enti- ty apparently deserves careful attention. Methods We included 42 APE patients presenting with i〉 0.1 mV STE in leads V1-V3/V4. Clinical and ECG characteristics in 15 patients with and 27 without STE in the inferior leads were included for comparison. Results Of the 42 patients, 98% were classified as high or intermediate risk patients, 79% showed ECG signs of right ventricular strain （RVS） and 83% showed the maximal amplitude of STE in leads V1-V2. The patients with STE in the inferior leads presented with faster heart rate （131±30 vs. 108_＋21 beats/min, P=0.015）, lower systolic blood pressure （107±22 vs. 123_＋26 mmHg, P=0.043）, higher incidence of ele- vated troponin （87% vs. 56%, P=0.040） and need to intensify therapy （73% vs. 33%, P=0.013）. Conclusions STE in the right precordial leads is an ECG manifestation of intermediate to high risk in APE patients. The ECG characteristics include the maximal amplitude of STE in leads V1-V2 and the RVS pattern. Simultaneous STE in the inferior and right precordial leads is associated with hemodynamic instability and need for intensified therapy.

Clinical Predictors for Recurrence after Curative Resection for Colorectal Cancer

Purpose: To identify clinical predictors of recurrence of colorectal cancer after curative surgical treatment. Methods: Retrospective follow-up-study of 925 consecutive patients treated with R0-resection for colorectal cancer Stage I, II and III from 1990 until 2000 with a mean follow-up of 60 ± 37 months. Predictors for cancer recurrence were identified in a pilot-sample of these patients, followed by analyses of the rest of the patients (test-sample), and finally with a concluding analyses of the entire patient group. Data were analyzed with Pearson Chi-square test (χ2), Cox regression analyses and log rank test. Results: Tumor stage (Stage I: HR 0.10 (0.05;0.19), Stage II: HR 0.31 (0.24;0.41)) and postoperative reoperations due to complications due to other causes than anastomotic leakage (HR 2.02 (1.21;3.36)) were significant predictors of cancer recurrence in the multivariate Cox regression model. The association between reoperations and recurrence was strongest for the patients with the best prognosis: Stage I and Stage II-cancers. Long duration of surgery, strongly associated with blood-loss and infusions of liquid and blood-products, reoperation due to anastomotic leakage as well as right colon/transversum localization were significant at a trend-level (10%). Conclusions: Tumor stage and reoperations due to postoperative complications other than anastomotic leakage are significant predictors for recurrence after curative surgery for colorectal cancer.

Purpose: To identify clinical predictors for reduced long-term survival and describe the cause of death after surgical treatment for rectal cancer. Methods: A retrospective follow-up study of 442 consecutive, unselected patients treated for rectal cancer at a tertiary centre from 1990 until 2000 and followed for 17 years or until death. Predictors for death were assessed by Cox regression analysis. The cause of death was obtained from the Norwegian Cause of Death Registry. Results: 254 men and 188 women with a median age of 71 years (21 - 95 years) were resected for rectal cancer with low anterior resection (n = 266), abdominoperineal resection (n = 125), Hartmann’s procedure (n = 19) or diverting stoma only (n = 32). Median follow-up was 5 years (0 - 17 years). The relative five-year survival rates for stages I, II, III and IV was 83.9%, 65.2%, 41.1% and 9.3%, respectively. The proportion of deaths due to recurrence from colorectal cancer in stages I, II, III and IV was 23.5%, 55.8%, 72.3% and 98.0%, respectively. Heart, lung and cerebrovascular disease and other malignancies were the cause of death in the other patients. Background: A proportion of patients with acute pancreatitis develop severe organ dysfunction requiring admittance in an intensive care unit. Patient characteristics associated with ICU admittance were evaluated in this consecutive series of patients. Methods: The clinical course of patients with first attack of acute pancreatitis in urban Norway between 01.01.1996 and 31.12.2006 was reviewed. Patient characteristics, comorbidity, pre hospital delay and likely aetiology of acute pancreatitis were analysed as risk factors for ICU admittance. Results: This study includes 565 patients, 299 women and 266 men (p = 0.089), with a median age of 60 years (range: 17 - 98). 50 (9%) of the patients were admitted to the ICU. Men were more at risk than women for ICU admittance (OR 2.34;95% CI: 1.27 - 4.32). Patients with alcohol associated pancreatitis (OR 5.94;95%, CI: 2.61 - 13.53), miscellaneous aetiologies (OR: 2.81, 95% CI: 1.02 - 7.73) and non-assessed aetiology (OR: 2.71, 95%, CI: 1.26 - 5.82) were more at risk of ICU admittance than patients with bile stones associated pancreatitis. Pre hospital delay increased the risk of ICU admittance amongst patients with alcohol associated pancreatitis (OR: 4.23;95%, CI: 1.11 - 16.18). Two comorbid conditions conveyed increased risk (OR: 8.78;95%, CI: 1.87 - 41.22). The case fatality rate was 24% in the ICU and 1.6% in the ward (OR 20.01, 95% CI: 7.71 - 51.93). Conclusions: Aetiology of acute pancreatitis, especially alcohol, pre-hospital delay and male gender are associated with increased of risk of ICU admittance and increased fatality rate during first attack of AP.
