Weight loss and cachexia are common problems in colorectal cancer patients;thus,parenteral and enteral nutrition support play important roles in cancer ***,the impact of nonessential amino acid components of nutritional intake on cancer progression has not been fully *** this study,we discovered that gastrointestinal cancer patients who received cysteine as part of the parenteral nutrition had shorter overall survival(Ply promotes colon cancer cell growth in vitro and in immunodeficient mice,predominately by inhibiting SESN2 transcription via the GCN2-ATF4 axis,resulting in mTORCl *** inhibitors Rapamycin and Everolimus block cystine-induced cancer cell *** addition,cystine confers resistance to oxaliplatin and irinotecan chemotherapy by quenching chemotherapy-induced reactive oxygen species via synthesizing *** demonstrated that dietary deprivation of cystine suppressed colon cancer xenograft growth without weight loss in mice and boosted the antitumor effect of *** findings indicate that cyst(e)ine;as part of supplemental nutrition,plays an important role in colorectal cancer and manipulation of cyst(e)ine content in nutritional formulations may optimize colorectal cancer patient survival.

＜正＞The talk will review the study of human brain aging and animal brain aging（rodent and monkey）,utilizing different approaches including fMRI,immunocytochemistry（apoptotic markers,transmitters and peptides） as well as western blotting to analyse the events of *** development to adulthood,one could see a change from disorderly to orderly pattern as revealed by *** progressive aging,the activated area for a specific activity *** immunohistochemistry,silver staining and western blotting,the mechanisms and events of degeneration begin to unfold, cell death and cell death effectors（caspases） increase,contacts decrease,hypertau proteins appear and the decline of transmitters in the cortex and brainstem,affecting the latter in different nuclei which control the fundamental,visceral and sensory ***,some of these events may be akin to other kinds of degeneration,*** drug *** is hoped that this talk may illustrate the simple research effort over the years by anatomists employing different approaches.

Introduction Nasopharyngeal carcinoma （NPC） is an epithelial-originated malignancy occuning with a high incidence in Southeast Asia, in particulat, in southern China. The molecular basis for the disease is not well understood and recent epidemiology studies have correlated the disease to Epstein-Barr virus （EBV） infection, consumption of salted fish, preserved foods and smoking'. NPC is almost entirely of the undiffenentiated squamous cell types and the discase is highly curable by radiation therapy, with survival rates of 80-90％2. Effect of Initiation of Basal Insulin Glargine on Glycemic Control in Patients with Diabetes: Real life Experience from Hong kong

Introduction: To assess the changes in glycemic control after initiating or switching to a basal insulin analogue in patients with diabetes mellitus. Methods: A retrospective, observational analysis was conducted using electronic data from a Hong kong regional hospital. Data from adult patients with type 1 and 2 diabetes mellitus (T1DM and T2DM, respectively) who had been prescribed with basal insulin glargine in 2008-2010, with recorded HbA1c levels at the time of initiation, at 6 and 12 months thereafter, were analysed. Results: Data from 106 eligible patients were analysed. Substantial reduction in HbA1c and fasting sugar levels were reported in both T1DM (Δ HbA1c = 1.5%, Δ FBG = 1.3 mmol/l p l/l p l of 42% of T1DM and 26% of T2DM patients achieved HbA1c levels lin naive achieved a statistically greater HbA1c reduction (Δ = 1.7%) than those who previous treated with premixed or basal bolus insulin (Δ = 0.3%) (p lycaemia reduced from 69% to 62% in T1DM but increased from 26% to 36% in T2DM patients. All hypoglycaemic episodes recorded were either asymptomatic or mild and self-limiting. Only 4% of the patients discontinued treatment at the end of 12 months. Conclusions: In real life clinical practice, a single daily basal insulin analogue therapy provided effective glycemic control with an acceptable risk of mild hypoglycaemia.

Effect of angiotensin II type I receptor blocker losartan on bone deterioration in orchiectomized male hypertensive and normotensive rats

Background Epidemiological study showed that the use of angiotensin-converting enzyme inhibitors was associated with higher bone mineral density （BMD） in older people, especially male subjects, which suggested that angiotensin II may have a detrimental effect on bone. Therefore, blocking its effect may have a beneficial effect on bone health. Methods Six-month-old male spontaneously hypertensive rats （SHR） and normotensive Wistar kyoto rats （WkY） were used. Animals of each model were randomly assigned to the following four groups： Group 1, SHAM operated＋vehicle; Group 2, orchidectomy （ORX）＋vehicle; Group 3, ORX＋low-dose Iosartan （10 ***）; and Group 4, ORX＋high-dose Iosartan （25 mg-kg-l.d-1）. Blood pressure was recorded weekly. SHAM and ORX operations were performed, followed by daily Iosartan and vehicle treatment from day 4 after operation for 16 weeks. Serum and 24-hour urine samples were collected for measurement of bone turnover markers before euthanasia and then the left femur was collected for measurements of BMD and microarchitecture before mechanical test. Results Urine deoxypyridinoline/urine creatinine （DPD/Cr） ratio was significantly higher in SHR than in WkY. BMD and microarchitecture parameters also showed bone deterioration in SHR. After ORX, serum osteocalcin concentration decreased and urine DPD/Cr ratio increased significantly accompanied by a significant decrease in cortical and trabecular BMD and cortical bone thickness in both WkY and SHR. High-dose Iosartan significantly increased DPD in urine in both SHR and WkY. Apart from marginal favorable changes in bone architecture in WkY treated with high-dose Iosartan, Iosartan did not show significant effect on BMD, bone area, bone microarchitecture, and mechanical properties in both SHR and WkY. Conclusion Angiotensin II type I receptor blocker Iosartan was not able to demonstrate significant effect on ORX-induced bone deterioration in both hypertensive and normotensive rats.

Obesity is a major risk factor for cancers including hepatocellular carcinoma(HCC)that develops from a background of non-alcoholic fatty liver disease(NAFlD).Hypercholesterolemia is a common comorbidity of *** cholesterol biosynthesis mainly occurs in the liver,its role in HCC development of obese people remains *** high-fat high-carbohydrate diet-associated orthotopic and spontaneous NAFlD-HCC mouse models,we found that hepatic cholesterol accumulation in obesity selectively suppressed natural killer T(NkT)cell-mediated antitumor *** analysis of human liver revealed aberrant cholesterol metabolism and NkT cell dysfunction in NAFlD ***,cholesterol-lowering rosuvastatin restored NkT expansion and cytotoxicity to prevent obesogenic diet-promoted HCC ***,suppression of hepatic cholesterol biosynthesis by a mammalian target of rapamycin(mTOR)inhibitor vistusertib preceded tumor regression,which was abolished by NkT inactivation but not CD8^(+)T cell ***,sterol regulatory element-binding protein 2(SREBP2)-driven excessive cholesterol production from hepatocytes induced lipid peroxide accumulation and deficient cytotoxicity in NkT cells,which were supported by findings in people with obesity,NAFlD and *** study highlights mTORC1/SREBP2/cholesterol-mediated NkT dysfunction in the tumor-promoting NAFlD liver microenvironment,providing intervention strategies that invigorating NkT cells to control HCC in the obesity epidemic.

Meiosis is a specialized cell division for producing haploid gametes in sexually reproducing *** this study,we have independently identified a novel meiosis protein male meiosis recombination regulator(MAMERR)/4930432 k21 Rik and showed that it is indispensable for meiosis prophase I progression in male *** super-resolution structured illumination microscopy,we found that MAMERR functions at the same double-strand breaks as the replication protein A and meiosis-specific with OB domains/spermatogenesis associated 22 *** generated a Mamerr-deficient mouse model by deleting exons 3 e6 and found that most of Mamerrà/àspermatocytes were arrested at pachynema and failed to progress to diplonema,although they exhibited almost intact synapsis and progression to the pachytene stage along with XY body *** mechanistic studies revealed that the recruitment of DMC1/RAD51 and heat shock factor 2 ebinding protein in Mamerrà/àspermatocytes was only mildly impaired with a partial reduction in double-strand break repair,whereas a substantial reduction in ubiquitination on the autosomal axes and on the XY body appeared as a major phenotype in Mamerrà/à*** propose that MAMERR may participate in meiotic prophase I progression by regulating the ubiquitination of key meiotic proteins on autosomes and XY chromosomes,and in the absence of MAMERR,the repressed ubiquitination of key meiotic proteins leads to pachytene arrest and cell death.

目的:6 min步行距离(6MWD)是临床上常见的功能性运动能力测试,特别是对于慢性呼吸系统疾病患者。一些文献记载了关于白种人的6MWD的最小临床重要差异(MCID),但对亚洲人群的相关信息未见记载。本研究旨在建立亚洲成人慢性呼吸道疾病患者6MWD的最小临床重要差异。方法:采用"锚定"法和"分布"法对新加坡中央医院肺康复计划登记处的资料进行了分析。"锚定"法收集确诊为慢性呼吸道疾病的患者50例,其中男38例,女12例;平均年龄(64.2±12.3)岁,通过电话采访及使用整体变化评估量表(GROC)评价患者肺康复计划后的自觉改善程度,再将患者在肺康复计划中6MWD的改变数值与相对应的患者自觉改善程度进行比较,这种关系用操作特征曲线(ROC)统计表示。通过"分布"法确定6MWD的MCID,从门诊和住院的肺康复计划(PRP)参与者中检索医疗记录136例,其中男103例,女33例;平均年龄(60.1±14.1)岁。结果 :ROC曲线表明6MWD出现临床显著性变化的最佳临界值为17.4 m,其对应的敏感性为79.4%,特异性为81.3%,阳性似然比为4.23。由"分布"法确定6MWD的最小临床重要差异值为18.6 m。结论:18.6 m为新加坡华人6MWD的最小临床重要差异的估计值。

背景:香港政府结核病与胸部疾病中心的结核病控制规程。目的:确定香港对结核病家庭接触者的检查结果。设计:从在胸部疾病中心登记治疗的5757例中随机选择970首发病例,对其所有家庭接触者进行回顾性队列研究。结果:在确定的2678名家庭接触者(首发病例的3倍)中,检查率为90％;发现41例活动性病例,比率为1720/100000(95％CI1238-2329)。比率在两端年龄组最高,≤5岁的儿童组为3604/100000(95％CI 990-3615),在>60岁年龄组的人群中为3547/100000(95％CI 1456-6489)。痰涂片和培养阳性的首发病例其接触者病例率最高,为2904/100000(95％CI 1869-4873);但细菌学为阴性病人的接触者病例率也高,为1478/100000(95％CI678-2789)。通过接触者追踪确定的阳性病例可能是源病例而不是继发病例。≤5岁的儿童结核菌素为阳性为8％,香港对所有的新生儿都予BCG接种,在过去的30年覆盖率为99％,很难估算这些儿童的感染率。结论:香港作为结核病的中等负担地区,对接触者进行调查是主动发现活动性病例的一个非常有效的措施。
