AIM： To investigate the relationship between postendoscopic resection （ER） scars on magnifying endoscopy （ME） and the pathological diagnosis in order to validate the clinical significance of ME. METHODS： From January, 2007 to June, 2008, 124 patients with 129 post-ER scar lesions were enrolled. Mucosal pit patterns on ME were compared with conventional endoscopy （CE） findings and histological results obtained from targeted biopsies. RESULTS： CE findings showed nodular scars （53/129）, erythematous scars （85/129）, and ulcerative scars （4/129）. The post-ER scars were classified into four pit patterns of sulci and ridges on ME：（ Ⅰ ） 47 round; （Ⅱ） 54 short rod or tubular; （Ⅲ） 19 branched or gyrus-like;and （Ⅳ） 9 destroyed pits. Sensitivity and specificity were 88.9% and 62.5%, respectively, by the presence of nodularity on CE. Erythematous lesions were high sensitivity （100%）, but specificity was as low as 36.7%. The range of the positive predictive va ue （PPV） on CE was as low as 10.6%-25%. Nine type N pit patterns were diagnosed as tumor lesions, and 120 cases of type Ⅰ-Ⅲ pit patterns revealed non-neoplastic lesions. Thus, the sensitivity, specificity, and the PPV of ME were 100%. CONCLUSION： ME findings can detect the presence of tumor in post-ER scar lesions, and make evident the biopsy target site in short-term follow-up. Further large-scale and long-term studies are needed to determine whether ME can replace endoscopic biopsy.

背景:经皮冠状动脉介入治疗(PCI)越来越多地用于治疗无保护性左冠状动脉主干狭窄的治疗,虽然冠状动脉旁路移植术(CABG)已被公认为治疗的选择。方法:我们将无保护性左冠状动脉主干狭窄的患者随机分配至西罗莫司药物洗脱支架组(300例)和接受CABG组(300例)。使用非劣效性宽界限,在随访1年时,比较了两组之间的主要不良心脏或脑血管事件的复合终点(全因死亡率、心肌梗死、脑卒中或缺血驱动的靶血管血运重建)。随访2年时,比较了两组相关事件的发生率。

Global pandemics caused by influenza or coronaviruses cause severe disruptions to public health and lead to high morbidity and *** remains a medical need for vaccines against these ***(cytomegalovirus)is aβ-herpesvirus that induces uniquely robust immune responses in which remarkably large populations of antigen-specific CD8+T cells are maintained for a ***,CMV has been proposed and investigated as a novel vaccine vector for expressing antigenic peptides or proteins to elicit protective cellular immune responses against numerous *** generated two recombinant murine CMV(MCMV)vaccine vectors expressing hemagglutinin(HA)of influenza A virus(MCMV^(HA))or the spike protein of severe acute respiratory syndrome coronavirus 2(MCMV^(S)).A single injection of MCMVs expressing either viral protein induced potent neutralizing antibody responses,which strengthened over ***,MCMV^(HA)-vaccinated mice were protected from illness following challenge with the influenza virus,and we excluded that this protection was due to the effects of memory T ***,we show here that MCMV vectors induce not only long-term cellular immunity but also humoral responses that provide long-term immune protection against clinically relevant respiratory pathogens.

Purpose: To compare nerve growth factor (NGF) levels in tears and on the ocular surfaces of normal control and non-sj gren’ s type keratoconjunctivitis sicca subjects,and to investigate the effect of 0.1% prednisolone eyedrops on NGF levels in keratoconjunctivitis sicca patients. Design: Prospective,double-masked,randomized,comparative clinical trial. Participants: Forty-one keratoconjunctivitis sicca patients and 23 age-and gender-matched healthy subjects. Methods: Baseline tear NGF levels were measured in keratoconjunctivitis sicca patients and healthy control subjects using enzyme-linked immunosorbent assays. Keratoconjunctivitis sicca patients received 0.1% prednisolone drops in one eye and 0.1% hyaluronic acid drops in the other,3 times a day for 28 days. Also,impression cytology (IC) and immunostaining for NGF on conjunctival epithelium were performed on both groups. Main Outcome Measures: Tear NGF/total tear protein (TP)-concentration ratio,IC and NGF immunocytologic staining,subjective symptom scale,tear breakup time,and Schirmer values. Results: Keratoconjunctivitis sicca patients were found to have baseline tear NGF concentrations higher than those of age-and gender-matched healthy control subjects (65.9± 14.5 vs. 122.1± 45.3 pg/μ g,P<0.0001). In keratoconjunctivitis sicca patients,prednisolone treatment for 28 days resulted in a decrease in tear NGF levels,symptom scores,and IC scores,whereas hyaluronic acid treatment had no such effect (68.2± 25.0 pg/μ g vs. 108.0± 43.4 pg/μ g,P< 0.0001 for tear NGF/TP ratio; 2.16± 1.01 vs. 3.39± 1.50,P=0.0014 for symptom scale; 1.05± 0.67 vs. 1.61± 0.86,P=0.0317 for IC). Measurements taken at both 14 and 28 days indicated that neither prednisolone nor hyaluronic acid treatment affected breakup time or Schirmer values. Conclusion: Keratoconjunctivitis sicca patients showed elevated levels of tear NGF,which were decreased by treatment with 0.1% prednisolone.
