Factors Influencing Academic Motivation among UOS Medical Students

Motivation has been found to play a major role in determining student performance in the academic process. The aim of this study is to identify the intrinsic ＆ extrinsic motivational factors among medical students in the University of Sharjah. A Cross Sectional Study was conducted among university students of medical colleges in the period between February 2011 and March 2011. A total of 452 students （337 females and 115 males） were selected using the Quota sampling method equally between each college. A self-administered questionnaire consisting of 13 demographic questions, an agreement scale of 32 questions about the motivational factors and an open ended question asking students about what the university can do to help motivate them were developed. The most common intrinsic factors chosen by students were grade expectation （94%）, interest in material （92%） and leading an honorable life （88%）. While on the other hand the most common extrinsic factors were upcoming exams （88%）, family expectations （88%） and praise （82%）. The most common factors were similar between all students regardless of their major, gender, or performance. However, differences were visible in other factors such as in high performing students; rewards scored a 71.4% while low performing students scored it at 50%. It also shows that females consider praise as an important factor （86%） on the contrary males gave it a 72%. While the most common intrinsic factors showed to be grades, interest in material and leading an honorable life, the most common extrinsic factors on the other hand were upcoming exams, family expectations and praise.

Primary Anorectal Melanomas Interest of Targeting C-KIT in Two Cases from a Series of 11 Patients

Background: The anorectal location of melanomas is extremely rare (1% - 3% of all melanomas), and the prognosis remains poor because of the aggressiveness and the high metastatic potential of those tumors. The discovery that the KIT oncogene may be aberrantly activated in a subset of patients with anorectal melanoma creates a realm of possibility for the development of targeted molecular therapy. Aim: to show the epidemiologic, clinico-radiological, histological features and treatment management especially in patients with over-expression of C-KIT treated by Imatinib. Methods: It is a retrospective study conducted in the department of medical oncology at Hassan II University Hospital between January 2007 and January 2012, including all patients with histologically proven melanoma of the anorectal area. Results: 11 cases were collected, with slight female predominance. Nine patients were metastatic at the moment of diagnosis, and only two with local stage, but evolution was marked by local and distant recurrence less than 12 months after abdo-minoperineal resection. First line of chemotherapy was based mainly on paclitaxel, carboplatine and dacarbazine. Response was modest with only 3 partial responses, 4 patients with disease stability, and 4 patients with disease progression. Two patients, with over expression of C-KIT, received Imatinib as second line of treatment with significant improvement of symptoms and radiological response reaching 50%. Seven patients died with a median survival of 11 months from diagnosis to the date of death. Conclusion: Primary anorectal melanomas are very rare, with high aggressiveness and poor prognosis. Treatment management is still a big challenge given to the modest efficacy of conventional chemotherapy. Better understanding of carcinogenesis and signaling pathways will allow development of new targeted therapies.

Tomato is an important field crop,and nutritional imbalances frequently reduce its *** and Recommendation Integrated System(DRIS),uses ratios for nutrient deficiency diagnosis instead of absolute concentration in plant *** this study,local DRIS norms for the field tomatoes were established and the nutrient(s)limiting tomatoes yield were *** leaves were analyzed for nutrients,to identify nutritional status using the DRIS *** hundred tomatoes fields were selected from Chatter Plain Khyber Pakhtunkhwa and the Sheikupura Punjab *** first fully matured leaf was sampled,rinsed,dried and ground for analyzing P,K,Ca,Mg,Cu,Fe,Mn and Zn using an Inductively Coupled Plasma Atomic Emission Spectrophotometer(ICP AES).Plant tissue N and S were measured by the combustion *** tomatoes yields were recorded at each *** data were divided into high-yielding(≥3.79 kg/10 plant)and low-yielding(elding plant population had a statistically greater mean S and Fe than the low-yielding *** average balance index,the sum of functions,for S and Fe were−11.04 and−5.17 which reflected deficiency of S and *** nutrients norms established may optimize plant nutrition in field tomatoes for high yield.

This study investigates if the anti-tumor effect of Pterostilbene in the SKOV3 ovarian cancer(OC)cell line involves inhibition of cell metabolism and tested in this effect involves modulating AMPK and Akt-induced regulation of ***,SKOV3 cells were cultured in the humidified conditions in DMEM media for 24 h with or without increasing concentration of ***,the cells were incubated with Pterostilbene(IC_(50)=50μM)under similar conditions with or without pre-incubation with Dorsomorphin,an AMPK *** a dose-dependent manner,Pterostilbene inhibited SKOV3 cell survival and increased their lysate levels of lactate dehydrogenase(LDH)and single-stranded DNA(ssDNA).When SKOV3 cells were treated with 50μM Pterostilbene,Pterostilbene significantly suppressed cell migration and invasion,reduced lysate levels of lactic acid and the optical density of Oil Red O staining,and increased lysate glucose *** also increased levels of malondialdehyde(MDA),reactive oxygen species(ROS),and induced intrinsic cell apoptosis by upregulating protein levels of Bax and cleaved caspase-3 and reducing protein levels of ***,Pterostilbene reduced mRNA levels of sterol regulatory element-binding protein 1(SREBP-1),fatty acid synthase(FAS),acetyl CoA carboxylase-1(ACC-1),and AMP-activated protein kinase(AMPK).Furthermore,Pterostilbene increased the protein levels of p-AMPK,p-p53,p-raptor,p-TSC-2,but significantly decreased protein levels of p-Akt,p-TSC-2,p-mTOR,p-S6K1,and *** with Dorsomorphin(CC)abolished all the anti-tumorigenesis effects afforded by Pterostilbene and prevented Pterostilbene-induced phosphorylation of Akt,p53,and *** conclusion,the tumorsuppressive effect of Pterostilbene in SKOV3 cells involves the induction of ROS and inhibition of dysregulation cell metabolism mainly due to AMPK-induced Akt-dependent or independent suppression of mTOR.

A 60-year-old woman presented to the emergency department of our university hospital for several episodes of upper gastro-intestinal bleeding (hematemesis and melena). The endoscopic examination (Figure 1) showed a large ulcer at the front of the bulb, with a calculation within it and a visible vessel on the banks of the ulcer treated by putting two clips. Computer tomography (CT), demonstrated a sclerotic, atrophic and multi gallstone gallbladder, with a calculation protruding in the duodenum. The patient underwent surgery which consisted on partial cholecystectomy with duodenal suture and closing of cystic duct.

Empty Sella Syndrome: A Case Report and Literature Review

An empty sella occurs due to herniation of the arachnoid through an incompetent diaphragma sellae. Over time, cerebrospinal fluid (CSF) pulsations may enlarge the sella and compress the gland against the floor of the sella. Empty sella syndrome is considered as a less common entity and is usually asymptomatic and an incidental finding. However, it can be a manifestation of increased intracranial pressure and can be occasionally severe. Compression of the pituitary gland may affect function, or traction on the optic chiasm and nerves may cause visual symptoms. An empty sella may be classified as primary when this occurs in persons who have not received pituitary radiation or pituitary surgery, while an empty sella discovered following such procedures is classified as secondary empty sella. In this report, we presented a 41-year-old multiparous patient who presented to us with symptoms of headache and left sided hemi-sensory disturbance. Examination was unrevealing except for the fact that she was obese. On evaluation, she was detected to have impaired blood sugars, dyslipidemia and vitamin insufficiency. MRI brain revealed presence of Empty Sella. Further hormonal analysis was normal. She was treated conservatively and regular follow-up was advised.

Effect of Ketamine Instillation on Acute and Chronic Post Mastectomy Pain, a Dose Finding Clinical Study

Background: Uncontrolled acute postoperative pain is considered a risk factor for the development of chronic pain afterward. Objectives: To explore the most effective dose of ketamine instillation (1 of 3 doses: 1, 2, or 3 mg/kg) for acute and chronic post mastectomy pain (PMP). Methods: Ninety female patients with cancer breast, aged (18 - 60 yrs), weighted (50 - 90 kg), scheduled for modified radical mastectomy, randomly allocated into 3 groups to receive ketamine instillation after surgical homeostasis before wound closure (1 of 3 doses;1, 2, or 3 mg/kg as A, B or C groups respectively) patients were followed up for 48 h for acute pain (total morphine consumption, the first request of analgesia and visual analog scale at rest and movement (VASR/M), chronic pain by Leeds assessment of neuropathic signs and symptoms (LANSS) for six-months, hemodynamics, and side effects. Results: Median total dose of morphine consumption was 8 mg (5 - 10) versus 6 mg (6 - 7) in A and B groups respectively in the first 48 h postoperatively. Lowest VASR/M was recorded in C then B and lastly A group (P = 0.037). No patients in the C group requested analgesia versus thirty (100%) and nine (30%) patients in the A and B groups respectively with the first request of analgesia was 12 h (5 - 36) in the A group versus 30 h (12 - 36) in the B group respectively (P Conclusion: Ketamine instillation effectively controlled acute post mastectomy pain (PMP) in a dose-dependent manner and reduced the incidence and severity of chronic pain in patients who undergoing a modified radical mastectomy.

Worldwide, large studies have shown that the menopause is mainly associated with substantial changes in body composition that result in an increase in waist circumference, fat accumulation and specially weight gain. This overweight could be exacerbated by fat intake. Thus, this study was planned to evaluate the impact of the consumption of olive and argane oils on anthropometric profile and body composition of postmenopausal women. The nutritional intervention was conducted over a period of eight weeks, involving daily consumption of 25 mL of argane oil The anthropometric profile （weight, height and body mass index or olive oil in 151 postmenopausal women （55.49 ± 6.18 years old）. （BMI）） and body composition （fat mass （FM） and lean mass （LM）） were determined at 0 week （baseline）, and after the 4th and 8th weeks of nutritional intervention. Results clearly demonstrated that argane oil or olive oil consumption did not affect the anthropometric parameters and the body composition of postmenopausal women. Thus, argane and/or olive oils＇ regular diet does not lead to weight gain, and postmenopausal women could benefit from their impact on health for a better quality of life and to overcome all menopause associated problems. A longer period of nutritional intervention is required to confirm the trend down that was recorded.
