AIM:To compare histological endpoint assessment using noninvasive alternatives to biopsy during treatment in a chronic hepatitis C virus(HCV)***:Patients with chronic HCV were randomized to receive interferon-based therapy for 24(genotypes 2/3)or 48(genotype 1)***～TM(FS)was assessed at baseline and at week-12 post-treatment *** biopsy for METAVIR was assessed by a single ***～ transient elastogra-phy(TE)was performed during treatment in a patient ***:Two thousand and sixty patients(n = 253 in Asia)were classif ied as METAVIR F0-1(n = 1682)or F2-4(n = 378).For F2-4,FS(n = 2055)had sensitiv-ity and specif icity of 0.87 and 0.61,respectively,with area under the receiver-operating curve of 0.82;corre-sponding values for TE(n = 214)and combined FS/TE(n = 209)were 0.77,0.88 and 0.88,and 0.93,0.68 and *** FS/TE agreement for F2-4 was 71%(κ = 0.41)and higher in Asians vs non-Asians(κ = 0.86 vs 0.35;P < 0.001).Combined FS/TE had 97% accuracy in Asians(n = 33).Baseline FS(0.38 vs 0.51,P < 0.001)and TE(8.0 kPa vs 11.9 kPa,P = 0.006)scores were lower in patients with sustained virological response than in nonresponders,and were maintained through ***:FS and TE may reliably differentiate mild from moderate-advanced disease,with a potential for high diagnostic accuracy in Asians with chronic HCV.

Octa-<i>O</i>-Methoxy Resorcin [4] Arene Amberlite XAD-4 Polymeric Chelating Resin for Solid Phase Extraction, Preconcentration, Separation and Trace Determination of Ni(II), Cu(II), Zn(II) and Cd(II) Ions

Synthetic resin, Amberlite XAD-4 was linked covalently with the third generation supramolecule, octa-O-methoxy resorcin [4] arene through -N=N-group to form chelating resin, which has been characterized and effectively used for the separation and preconcentration of metal ions such as Ni(II), Cu(II), Zn(II) and Cd(II). Critical parameters such as pH, flow rate, sorption capacity, breakthrough studies, distribution coefficient, preconcentration factor, concentration of eluting agents responsible for quantitative extraction of metal ions were optimized. The synthesized resin showed good binding affinity towards Ni(II), Cu(II), Zn(II) and Cd(II) under selective pH conditions. Good breakthrough capacity and fast exchange kinetics of the resin lead to effective separation of metal ions from their binary and ternary mixture by column method on the basis of pH and eluting agents. The resin could be reused for about 8 -10 cycles. The proposed method having the analytical data with the relative standard deviation (RSD) 2% and with recoveries of analytes higher than 98%, reflects upon the reproducibility and reliability of the method which has been successfully applied in the separation and determination of Ni(II), Cu(II), Zn(II) and Cd(II) ions in synthetic, natural and ground water samples.

A Validated Model for the Imaging Diagnosis of Cystic Lung Disease

Rationale and Objectives: Cystic lung disease may be accurately diagnosed by imaging interpretation of specialist radiologists, without other information. We hypothesized that with minimal training non-specialists could perform similarly to specialist physicians in the diagnosis of cystic lung disease. Methods: 72 cystic lung disease cases and 25 cystic lung disease mimics were obtained from three sources: 1) a prospective acquired diffuse lung disease registry, 2) a retrospective search of medical records and 3) teaching files. Cases were anonymized, randomized and interpreted by 7 diffuse lung disease specialists and 15 non-specialist radiologists and pulmonologists. Clinical information other than age and sex was not provided. Prior to interpretation, non-specialists viewed a short PDF training document explaining cystic lung disease interpretation. Results: Correct first choice diagnosis of 85%-88% may be achieved by high-performing specialist readers and 71%-80% by non-specialists and lower-performing specialists, with mean accuracies in the diagnosis of LAM (91%, p Conclusion: With specific but limited training, non-specialist physicians can diagnose cystic lung diseases from CT appearance alone with similar accuracy to specialists, correctly identifying approximately 75% of cases.
