BACKGROUND Metabolic dysfunction-associated steatotic liver disease(MASLD)and metabolic dysfunction-associated steatohepatitis(MASH)are a growing health burden across a significant portion of the global patient ***,these conditions seem to have disparate rates and outcomes between different ethnic *** combination of MASLD/MASH and type 2 diabetes increases the risk of hepatocellular carcinoma(HCC),and Hispanic patients experience the greatest burden,particularly those in South *** To compare outcomes between Hispanic and non-Hispanic patients in the United States,while further focusing on the Hispanic population within Southeast Texas to determine whether the documented disparity in outcomes is a function of geographical circumstance or if there is a more widespread reason that all clinicians must account for in prognostic *** This cohort analysis was conducted with data obtained from TriNetX,LLC(“TriNetX”),a global federated health research network that provides access to deidentified medical records from healthcare organizations *** cohort networks were used:University of Texas Medical Branch(UTMB)hospital and the United States national database collective to determine whether disparities were related to geographic regions,like Southeast *** This study findings revealed Hispanics/Latinos have a statistically significant higher occurrence of HCC,type 2 diabetes mellitus,and liver fibrosis/cirrhosis in both the United States and the UTMB Hispanic/Latino *** mortality in Hispanics/Latinos was lower within the United States group and not statistically elevated in the UTMB *** This would appear to support that Hispanic patients in Southeast Texas are not uniquely affected compared to the national Hispanic population.

Environmental assessment of impacts, management, and policy are important aspects of protection of human health and the environment. Assessing the impacts of human activities requires selection of bioindicator species that can be used to assess, manage, and develop public policies that ensure ecosystem integrity, and therefore sustainability of social, cultural, and economic systems. With the use of Chinook Salmon (Oncorhynchus tshawytscha), Pacific Cod (Gadusmacrocephalus), Mallard (Anas platyrhynchos), and Red Knot (Calidris canutus rufa), we explore assessment and measurement endpoints, and their relationship to management and development of public policy. This combination of fish and birds provides a diversity of life histories, ecosystem roles, human values, and resource use to explore their use as bioindicators and endpoints. It also allows examination of 1) conservation and protection of species and biodiversity, 2) protection of ecosystems, 3) provision of goods and services, and 4) societal well-being.

BACKGROUND Distal humerus elbow fractures are one of the most common traumatic fractures seen in pediatric patients and present as three main types:Supracondylar(SC),lateral condyle(LC),and medial epicondyle(ME)*** To evaluate the epidemiology of pediatric distal humerus fractures(SC,LC,and ME)from an American insurance claims *** A retrospective review was performed on patients 17 years and younger with the ICD 9 and 10 codes for SC,LC and ME fractures based on the IBM Truven MarketScan®Commercial and IBM Truven MarketScan Medicare Supplemental *** from 2015 to 2020 were queried for treatments,patient age,sex,length of hospitalization,and *** A total of 1133 SC,154 LC,and 124 ME fractures were *** fractures had the highest percentage of operation at 83%,followed by LC(78%)and ME fractures(41%).Male patients were,on average,older than female patients for both SC and ME *** In the insurance claims databases used,SC fractures were the most reported,followed by LC fractures,and finally ME *** was identified to be a factor for how a pediatric distal humerus fractures,with patients with SC and LC fractures being younger than those with ME *** peak age per injury per sex was similar to reported historic central tendencies,despite reported trends for younger physiologic development.

Artificial light at night(ALAN) pollution alters bat lunar chronobiology:insights from broad-scale long-term acoustic monitoring

Background The timing of behavior and habitat use of nocturnal animals can be influenced by the lunar cycle in nature. The prevalence of artificial light at night(ALAN) has been recognized as a source of environmental pol-lution. The interaction between ALAN and the lunar cycle on bat behavior is important for understanding anthro-pogenic effects on bats. We utilized a decade(2012–2022) of acoustic monitoring data collected in North Carolina, United States, to investigate the relationship between bat activity, lunar cycle, and light pollution. We examined whether the amount of lunar illumination affected species-specific nightly activity and whether hourly bat activity patterns varied between nights with different moon phases. We further investigated if the relationship between bat activity and the lunar cycle might be altered by light *** We found that seven bat species showed activity variation across nights in relation to the amount of moon illumination when ALAN was absent. In general, bats were less active on full moon nights compared to new moon nights. Light pollution interacted with the bat–lunar relationship in five of the seven species, masking the effect of the lunar cycle. We identified delayed bat activity patterns on nights with a full or waxing moon in seven species, and light pollution altered that pattern in four species. Overall, ALAN was associated with decreased bat activity inde-pendent of lunar cycle *** Our study demonstrated that at a broad spatial scale, ALAN negatively affected many North American temperate bat species and altered their lunar chronobiology. As light pollution is spreading to historically dark areas and habitats, ALAN might couple with other threats, such as the white-nose syndrome or climate change, to cause cascading damage in the environment that depends on ecosystem services such as pest control provided by bats. Asbestos Exposure during Gasket Removal from Aircraft Engine

We have previously evaluated asbestos exposure associated with various maintenance procedures on light aircraft. The purpose of this study was to evaluate asbestos exposure during engine maintenance on light aircraft. This test was designed to evaluate the potential for asbestos exposure to mechanics and others who remove asbestos-containing engine gaskets from reciprocating style aircraft engines. Utilized in this test was an air cooled, horizontally opposed, aviation gasoline burning engine, assembled during 1986 and operated intermittently up into 2015, having accumulated 1680 hours run time. Nearly 75% of the asbestos-containing gaskets installed during 1986 were still in place at the time of testing. Chrysotile asbestos contents of such gaskets ranged from 55% to 60% by area, for those of sheet style and 5% by area, for the spiral wound metal/asbestos style. Despite the levels of effort required to effect gasket removals, the professional aircraft mechanic was not exposed to airborne asbestos fibers at the lower limits of sampling and analytical detection achieved;all of which were substantially less than the current Occupational Safety and Health Administration Permissible Exposure Limits for asbestos. The results of this testing indicate an absence of gasket related asbestos exposure risk to mechanics who work with light aircraft engines, including those having asbestos-containing gaskets. These results are consistent with the findings of Mlyarek and Van Orden who studied the asbestos exposure risk occasioned during overhaul of larger radial style reciprocating aircraft engines [1].

Management of a Retained Epidural Catheter in a Pregnant Patient: A Case Report and Review of Literature

Background: Epidural anesthesia is an effective intervention to treat labor pain and provide analgesia for orthopedic procedures. A rare complication of this technique is epidural catheter retention. Case Presentation: In this case report, we present an otherwise healthy 21-year-old G2P0010 woman at 39 weeks and 3 days gestation whose anesthetic management was complicated by a symptomatic retained epidural catheter fragment. Computed tomography (CT) imaging of her spine showed the fractured catheter within her paraspinal muscles and neurosurgery recommended no surgical intervention at that time. The patient reported resolution of back pain while in the postpartum unit which she continues to endorse two weeks after discharge. Conclusion: Retained epidural catheter is a medical emergency that requires immediate imaging and neurological evaluation. While magnetic resonance imaging is typically regarded as the standard imaging modality for this complication, in epidural sets that contain metallic components, we suggest using CT to avoid the risk of thermal nerve damage. In cases where the patient does not report any acute complaints, nonsurgical management may be appropriate but close follow-up is required to monitor for catheter migration.
