osteoarthritis(oA) is the most common type of arthritis found in the United States' population and is also the most common disease of joints in adults throughout the world with the knee being the most frequently affected of all joints. As the United States' population ages along with the increasing trends in obesity prevalence in other parts of the world, it is expected that the burden of oA on the population, healthcare system, and overall economy will continue to increase in the future without making major improvements in managing knee oA. Numerous therapies aim to reduce symptoms of knee oA and continued research has helped to further understand the complex pathophysiology of its disease mechanism attempting to uncover new potential targets for the treatment of oA. This review article seeks to evaluate the current practices for managing knee oA and discusses emerging therapies on the horizon. These practices include non-pharmacological treatments such as providing patient education and self-management strategies, advising weight loss, strengthening programs, and addressing biomechanical issues with bracing or foot orthoses. oral analgesics and anti-inflammatories are pharmacologicals that are commonly used and the literature overall supports that some of these medications can be helpful for managing knee oA in the short-term but are less effective for long-term management. Additionally, more prolonged use significantly increases the risk of serious associated side effects that are not too uncommon. Diseasemodifying osteoarthritis drugs are being researched as a treatment modality to potentially halt or slow disease progression but data at this time is limited and continued studies are being conducted to further investigate their effectiveness. objective To describe whether or not there are gender differences in lower urinary tract symptoms(LUTS)prevalence and risk factors in community-dwelling older Chinese *** We performed a secondary analysis of a prospective cross-sectional population-based survey of Chinese Americans aged 60 years and older between January 2011 and December 2013 in English,Mandarin,Cantonese,Taishanese,or Teochew.A clinical review of systems was used to assess LUTS,which included urinary frequency,urgency,burning and/or pain,blood in urine,and urinary *** of the total 3157 people queried,42%were men and 58%were *** men reported LUTS compared to women(32.9%vs.28.6%,p=0.01).In a multivariable analysis,female gender(adjusted odds ratio[aoR]0.60,95%confidence interval[CI]0.49-0.73),being married(aoR 0.79,95%CI 0.65-0.97),and smoking(aoR 0.66,95%CI 0.49-0.88)were found to be protective,while traditional Chinese medicine use(aoR 1.51,95%CI 1.28-1.78),heart disease(aoR 1.54,95%CI 1.24-1.91),and anxiety(aoR 1.69,95%CI 1.25-2.28)were most strongly associated with increased odds of *** examining genders separately,being married was found to be protective only in ***,unique factors found in men were hypertension,heart disease,and practice of Tai *** In this large population-based study,LUTS were more prevalent in older Chinese American men than *** also found gender-specific factors that influenced the odds of reporting LUTS;however,traditional Chinese medicine use was the only factor that was shared by both *** longitudinal investigations are needed to elucidate these underlying mechanisms to provide evidence-based and culture-specific guidelines for this rapidly growing population.

BACKGRoUND: Transesophageal echocardiography(TEE) is used in the emergency department to guide resuscitation during cardiac arrest. Insertion of a TEE transducer requires manual skill and experience, yet in some residency programs cardiac arrest is uncommon, so some physicians may lack the means to acquire the manual skills to perform TEE in clinical practice. For other infrequently performed procedural skills, simulation models are used. However, there is currently no model that adequately simulates TEE transducer insertion. The aim of this study is to evaluate the feasibility and efficacy of using a cadaveric model to teach TEE transducer placement among novice ***: A convenience sample of emergency medicine residents was enrolled during a procedure education session using cadavers as tissue models. A pre-session assessment was used to determine prior knowledge and confidence regarding TEE manipulation. Participants subsequently attended a didactic and hands-on education session on TEE placement. All participants practised placing the TEE transducer until they were able to pass a standardized assessment of technical skill(SATS). After the educational session, participants completed a post-session assessment. RESULTS: Twenty-five residents participated in the training session. Mean assessment of knowledge improved from 6.2/10 to 8.7/10(95% confidence interval [CI] of knowledge difference 1.6–3.2, Ponfidence improved from 1.6/5 to 3.1/5(95% CI of confidence difference 1.1–2.0, Po relationship between training level and the delta in knowledge or ***: In this pilot study, the use of a cadaveric model to teach TEE transducer placement methods among novice users is feasible and improves both TEE manipulation knowledge and confidence levels.

objective:To assess prevalence and intensity of soil-transmitted helminths(STH) in school age children of two southern districts as baseline information prior to implement a deworming program against intestinal parasites as part of an integrated country development ***:Children randomly selected from urban and rural schools in Southern Belize provided one stool sample each,analysed by the Kato-Katz method to assess prevalence and intensity of STH *** Info software was used for data analysis;Chi-square test and Fischer exact test were applied to compare group proportions;Ponsidered of statistical significance;descriptive statistics were expressed as ***:A total of 500 children from 10 schools participated in the study from May to *** of STH ranged between 40%and 82%among schools,with a median of 59.2%;the majority of light intensity,and with 2.2%high intensity *** and Axaris infections presented similar frequency in children aged from 6 to 9 years old;hookworm infections tended to be more frequent in the older group 10 to 12 years *** significances(P≤0.01)were found in children in rural schools infected with any species of STH,in moderate Trichuris infections,in hookworm infections in rural areas with strong Mayan presence and in Ascaris infections in children of Mayan ***:High prevalence of STH in Southern Belize provided sound ground for implementing an integrated deworming control program.

Experimental evidence indicates that chronic mechanical sub-occlusion of the intestine may damage the enteric nervous system （ENS）, although data in humans are lacking. We here describe the first case of enteric degenerative neuropathy related to a congenital obstruction of the gut. A 3-year and 9-mo old girl began to complain of vomiting, abdominal distension, constipation with air-fluid levels at plane abdominal radiology. Her subsequent medical history was characterized by 3 operations： the first showed dilated duodeno-jejunal loops in the absence of occlusive lesions; the second （2 years later） was performed to obtain full-thickness biopsies of the dilated intestinal loops and revealed hyperganglionosis at histopathology; the third （9 years after the hyperganglionosis was identified） disclosed a Ladd＇s band which was removed and the associated gut malrotation was corrected. Repeated intraoperative full-thickness biopsies showed enteric degenerative neuropathy along with reduced interstitial cells of Cajal network in dilated loops above the obstruction and a normal neuromuscular layer below the Ladd＇s band. one year after the latest surgery the patient tolerated oral feeding and did well, suggesting that congenital （partial） mechanical obstruction of the small bowel in humans can evoke progressive adaptive changes of the ENS which are similar to those found in animal models of intestinal mechanical occlusion. Such ENS changes mimic neuronal abnormalities observed in intestinal pseudoobstruction.

The increased use of mechanical circulatory support has led to a rise in infectious complications in the recent years predisposing this population of patients to higher morbidity and mortality. In this study, we report a significantly lower rate of driveline infections of 0.12 episodes/patient-year as compared to the rates reported in the existing literature in patients with the HMII (Heartmate II). The study is limited by the fact that it is based on a small population of patients and was conducted retrospectively making recall bias hard to rule out. It is strictly restricted to one type of continuous flow pump (HMII) solely to decrease variations in the data reviewed. The exact cause of the low infection rate noted in this study is difficult to define. However, detailed teaching to the caregivers and the patient as well as close follow-ups in the perioperative period may substantially contribute to the outcome noted. Continued research limited to similar continuous flow ventricular assist devices in larger study populations would shed light on defining causes of infections in this population and developing robust algorithms to prevent such complications.
