Associations among pain catastrophizing, muscle strength, and physical performance after total knee and hip arthroplasty

AIM To investigate whether reductions in pain catastrophizing associated with physical performance in the early period after total knee arthroplasty(TKA) or total hip arthroplasty(THA).METHODS The study group of 46 participants underwent TKA or THA. The participants were evaluated within 7d before the operation and at 14d afterwards. Physical performance was measured by the Timed Up and Go(TUG) test, and 10-m gait time was measured at comfortable and maximum speeds. They rated their knee or hip pain using a visual analog scale(VAS) for daily life activities. Psychological characteristics were measured by the Pain Catastrophizing Scale(PCS). Physical characteristics were measured by isometric muscle strength of knee extensors and hip abductors on the operated side. The variables of percent changes between pre-and postoperation were calculated by dividing post-operation score by pre-operation *** Postoperative VAS and PCS were better than preoperative for both TKA and THA. Postoperative physical performance and muscle strength were poorer than preoperative for both TKA and THA. The percent change in physical performance showed no correlation with preoperative variables. In TKA patients, the percent change of PCS showed correlation with percent change of TUG(P = 0.016), 10-m gait time at comfortable speeds(P = 0.003), and 10-m gait time at maximum speeds(P = 0.042). The percent change of muscle strength showed partial correlation with physical *** percent change of VAS showed no correlation with physical performances. On the other hand, in THA patients, the percent change of hip abductor strength showed correlation with percent change of TUG(P =0.047), 10-m gait time at comfortable speeds(P = 0.001),and 10-m gait time at maximum speeds(P = 0.021). Efficacy and adverse events of cold vs hot polypectomy: A meta-analysis

AIM: To compare previously reported randomized controlled studies (RCTs) of cold and hot polypectomy, we systematically reviewed and clarify the utility of cold polypectomy over hot with respect to efficacy and adverse ***: A meta-analysis was conducted to evaluate the predominance of cold and hot polypectomy for removing colon polyps. Published articles and abstracts from worldwide conferences were searched using the keywords “cold polypectomy”. RCTs that compared either or both the effects or adverse events of cold polypectomy with those of hot polypectomy were collected. The patients’ demographics, endoscopic procedures, No. of examined lesions, lesion size, macroscopic and histologic findings, rates of incomplete resection, bleeding amount, perforation, and length of procedure were extracted from each study. A forest plot analysis was used to verify the relative strength of the effects and adverse events of each procedure. A funnel plot was generated to assess the possibility of publication ***: Ultimately, six RCTs were selected. No significant differences were noted in the average lesion size (less than 10 mm) between the cold and hot polypectomy groups in each study. Further, the rates of complete resection and adverse events, including delayed bleeding, did not differ markedly between cold and hot polypectomy. The average procedural time in the cold polypectomy group was significantly shorter than in the hot polypectomy ***: Cold polypectomy is a time-saving procedure for removing small polyps with markedly similar curability and safety to hot polypectomy.

Non-steroidal anti-inflammatory drug-induced small bowel injuries identified by double-balloon endoscopy

AIM： To clarify clinical features of the NSAID-induced sma bowel lesions using a new method of endoscopy. METHODS： This is a retrospective study and we analyzed seven patients with small bowel lesions while taking NSAIDs among 61 patients who had undergone doubleballoon endoscopy because of gastro-intestinal bleeding or anemia between September 2000 and March 2004, at .lichi Medical School Hospital in Japan. Neither conventional EGD nor colonoscopy revealed any lesions of potential bleeding sources including ulcerations. Double-balloon endoscopy was carried out from oral approach in three patients, from anal approach in three patients, and from both approaches in one patient. RESULTS： Ulcers or erosions were observed in the ileum in six patients and in the jejunum in one patient, respectively. The ulcers were multiple in all the patients with different features from tiny punched out ulcers to deep ulcerations with oozing hemorrhage or scar. All the patients recovered uneventfully and had full resolution of symptoms after suspension of the drug. CONCLUSION： NSAIDs can induce injuries in the small bowel even in patients without any lesions in both the stomach and colon.

Molecular Dynamics Study on Transmission Mechanism of Torsional Deformation in Cellulose Nanofibers with Hierarchical Structure

Cellulose nanofiber (CNF) is a fibrous and nano-sized substance produced by decomposition of bulk-type cellulose which is a main component of plants. It has high strength comparable to steel, and it shows low environmental load during a cycle of production and disposal. Besides it has many excellent properties and functions such as high rigidity, light-weight, flexibility and shape memory effect, so it is expected as a next-generation new material. Usually it is composed of many cellulose micro fibrils (CMFs) in which molecular chains of cellulose are aggregated in a crystal structure, the knowledge of mechanical properties for each CMF unit is important. Since actual fibrils are complicatedly intertwined, it is also crucial to elucidate the transmission mechanism of force and deformation not only in one fibril but also in between fibrils. How the dynamic and hierarchical structure composed of CMFs responds to bending or torsion is an interesting issue. However, little is known on torsional characteristics (shear modulus, torsional rigidity, etc.) concerning CMF. In general, in a wire-like structure, it is difficult to enhance torsional rigidity and strength, compared with tensile ones. Therefore, in this study, we try to build a hierarchical model of CNF by multiplying CMF fibers and to conduct molecular dynamics simulation for torsional deformation, by using hybrid model between all-atom and united-atoms model. First, shear modulus was estimated for one CMF fibril and it showed a value close to the experimental values. Also, we assume a state in which two CMFs are ideally arranged in parallel, and create a hierarchical structure. We evaluate the dependence on the temperature for the bond strength and toughness in the hierarchical structures. Furthermore, we mentioned the transmission mechanism between components of a hierarchical structure.

A 56-year-old male visited our hospital for evaluation of an occipital *** computed tomography showed hypervascular enhancement with osteolytic change in the skull and a huge enhanced mass in the *** resonance imaging showed bone metastasis in the thoracic *** for hepatitis B surface antigen and hepatitis B core antibody were positive and his liver condition was Child-Pugh grade *** diagnosis was hepatocellular carcinoma(HCC) with skull and vertebrae metastases on chronic hepatitis *** was treated with radiation therapy for bone metastases and transcatheter arterial chemoembolization for *** he developed acute respiratory failure because of aspiration pneumonia,congestion and oedema with haemorrhage of the lungs and *** showed HCC with multiple bone *** liver tumor was categorized as well-differentiated HCC,Edmondson classification Ⅰ,trabecular type and pseudoglandular *** the liver mild infiltration of lymphocytes was seen in Glisson's capsules which were signif icantly enlarged with well preserved limiting *** necrosis was not *** fibrosis was *** 8 cm × 7 cm × 3 cm metastatic lesion had formed in the left occipitotemporal part of the cranial *** lesion was osteolytic and showed invasion into the dura *** the subdural cavity nor the brain showed involvement from the metastatic ***,skull metastasis from HCC is very rare and it affects the patient's prognosis and the quality of ***,it is very important to make an early diagnosis and carry out proper management of skull metastasis from HCC.

An Experimental Study on Measurement Methods of Bulk Density and Porosity of Rock Samples

Density and porosity are fundamental and important physical properties of rocks in various geological problems, and affect the other physical properties. Therefore, measurements of density and porosity of rock samples are important investigation items in both geo-science and geo-engineering areas. Several measurement techniques of the density and porosity are available and being applied currently. To ensure the data quality and to conduct its quality assessment, comparison of measurement results by different measurement techniques is necessary since the techniques are based on different principles and test procedures. In this study, we collected eight types of rock samples including a gabbro, a granite, four sandstones, a welded tuff and a mudstone as study materials, and also prepared several metal specimens for the experimental comparison. The porosities of the eight rocks covered a very wide range from 0.3% to 50% approximately. We employed three methods (caliper, buoyancy and helium-displacement pycnometer) to measure volumes of regularly-shaped specimens and to determine their bulk densities and porosities. As a result, the three techniques yielded almost same bulk densities and porosities for all the specimens. In addition, we also applied mercury intrusion porosimetry to measure density and porosity as well as to determine pore size distribution of the rock samples. Porosity values obtained by the porosimetry method were underestimated in the case of high-porosity (soft) rock samples and overestimated for the very low-porosity rock samples. Ability to determine pore size distribution, however, is a very important advantage of the porosimetry method.

Congenital median raphe cysts: Coexistence of cystic lesions and canal-like lesions

Median raphe cyst of the perineum is a rare congenital anomaly and was first reported in 1895. We report a 9-year-old male with median raphe cysts in the perineum which had become swollen and then spontaneously ruptured several times since he was 3 years old. There were two separate lesions;one was a cystic lesion and the other was a canal-like lesion. They were completely excised and the inner layer of the cysts was composed of transitional epithelium.

This review article provides doctors and dentists with an introduction to the clinical applications and sig- nificance of magnetic resonance cisternography (MRC) for the evaluation of neurovascular compression of the trigeminal nerve in patients with trigeminal neu- ralgia (TN). Specifically, the method and characteris- tics of MRC are first explained using the relevant MR sequences. Next, clinical applications for the diagnosis of TN are discussed. Finally, the significance and util- ity of MRC for effective treatment of patients with TN, such as carbamazepine administration and/or local anesthetic blocks, are presented.
