Background The endoscopic surgery for inner-side cancer of the breast is usually performed by periareolar approach, but it often makes deformation or malposition of nipple and areola. The trans-axillary approach is favorable without making any injuries on breast skin. Furthermore, we devised a new approach of retro-mammary route without subcutaneous exfoliation, from axillary skin incision, to preserve skin touch sensation.Methods We have performed video-assisted breast surgery （VABS） on 200 patients since December 2001. The newly devised trans-axillary retromammary-route approach （TARM） was performed on 12 patients of early breast cancer. After endoscopic sentinel lymph node biopsy, we lengthened the axillary skin incision to 2.5 cm, and dissected retromammary tissue from superficial pectoral fascia onto major pectoral muscle below the tumor. The working space was made by lifting traction sutures through the gland. We cut the gland vertically at free margin 2 cm apart from the tumor edge, and dissect skin flap over the tumor. The breast reconstruction was done by filling absorbable fiber cotton.Results Traction sutures made it easier to cut the mammary gland vertically. We did not experience any skin damages like burn. All surgical margins were negative. The operation time was needed longer but the blood loss was not different. The postoperative esthetic results were good. The sensory disturbance was minimal. All patients were satisfied with this operation.Conclusion This newly devised TARM approach need no injury on whole breast, and can become a single standard method for breast conserving surgery wherever the cancer situated.

This paper reports stick-slip behaviors of Indian gabbro as studied using a new large-scale biaxial friction apparatus, built in the National Research Institute for Earth Science and Disaster Prevention （NIED）, Tsukuba, Japan. The apparatus consists of the existing shaking table as the shear-loading device up to 3,600 kN, the main frame for holding two large rectangular prismatic specimens with a sliding area of 0.75 m^2 and for applying normal stresses an up to 1.33 MPa, and a reaction force unit holding the stationary specimen to the ground. The shaking table can produce loading rates v up to 1.0 m/s, accelerations up to 9.4 m/s^2, and displacements d up to 0.44 m, using four servocontrolled actuators. We report results from eight preliminary experiments conducted with room humidity on the same gabbro specimens at v = 0.1-100 mm/s and an = 0.66-1.33 MPa, and with d of about 0.39 m. The peak and steady-state friction coefficients were about 0.8 and 0.6, respectively, consistent with the Byerlee friction. The axial force drop or shear stress drop during an abrupt slip is linearly proportional to the amount of displacement, and the slope of this rela- tionship determines the stiffness of the apparatus as 1.15 × 10^8 N/m or 153 MPa/m for the specimens we used. This low stiffness makes fault motion very unstable and the overshooting of shear stress to a negative value was recognized in some violent stick-slip events. An abrupt slip occurred in a constant rise time of 16-18 ms despite wide variation of the stress drop, and an average velocity during an abrupt slip is linearly proportional to the stress drop. The use of a large-scale shaking table has a great potential in increasing the slip rate and total displacement in biaxial friction experiments with large specimens.

AIM To determine whether oral glutathione(GSH)administration can alleviate the effects of fasting-induced intestinal atrophy in the small intestinal mucosa. METHODS Rats were divided into eight *** group was fed ad libitum,another was fed ad libitum and received oral GSH,and six groups were administrated saline(SA)or GSH orally during *** height,apoptosis,and cell proliferation in the jejunum were histologically evaluated.i NOS protein expression(by immunohistochemistry),nitrite levels(by high performance liquid chromatography,as a measure of NO production),8-hydroxydeoxyguanosine formation(by ELISA,indicating ROS levels),glutathione/oxidized glutathione(GSH/GSSG)ratio(by enzymatic colorimetric detection),andγ-glutamyl transpeptidase(Ggt1)mR NA levels in the jejunum(by semi-quantitative RT-PCR)were also estimated. The English language has a history of about one thousand and five hundred years since it began to be spoken from around the middle of the fi fth century in England. During that time many internal and external events which are associated with the history, the culture, and society of England infl uenced the language at each stage. These events have brought changes to the system of the language and qualitative improvement directly or indirectly in every stage of the history of the English language. This paper focuses on the period stretching from Old English to Early Modern English, and its purpose is to discuss what sort of events were involved in the forming of Present-day English.

BACKGROUND The oral nucleos(t)ide analogue,entecavir(ETV)was demonstrated to reduce the rate of hepatocellular carcinoma(HCC)in patients with hepatitis B virus(HBV)-associated liver ***,the reduction of HCC differs in various regions of the *** To investigate the reduction of HCC development due to ETV therapy by *** We surveyed the differences in HCC development following ETV treatment based on published articles using PubMed(2004-2019).RESULTS The regions with the most marked reduction in HCC development due to ETV therapy were Spain(1.0%/year)and Canada(Southern part,1.3%/year),and the most ineffective areas were South Korea(3.6%-3.8%/year),China(3.3%/year),Taiwan(2.4%-3.1%/year),and Hong Kong(2.8%/year).Following ETV administration,the incidence of HCC in genotype D regions(1.89%±0.28%/year,mean±SE)was significantly lower than that in genotype C regions(2.91%±0.24%/year,P<0.01).With regard to the initial HBV-DNA level,in genotype C patients(average:5.61 Log10IU/mL)this was almost the same as that in genotype D patients(average:5.46 Log10IU/mL).Moreover,there was no association between the prevalence ratio of HBV and the incidence of HCC on ETV *** The effectiveness of ETV in preventing HCC development in HBV-associated liver cirrhosis is genotype-dependent.

A lipoplex （i.e., pDNA#1/lipid complex and transfection reagent for pDNA delivery） containing galactosylceramide （GalCer） and an amidine-bearing lipid （TRX） was examined whether the bound pDNA was specifically ingested by hepatocyte via asialoglycoprotein receptor （ASGPR） and then expressed protein. Gel electrophoresis and small-angle X-ray scattering （SAXS） confirmed that the TRX-GalCer liposome#2 complexed with pDNA and the resultant lipoplex took a hexagonally packed inverted cylinder structure when the GalCer composition was less than 20 wt.% of the total lipid. When the lipoplex carrying pGL3 （luciferase-cording pDNA） was administrated to HepG2, the luciferase activity was increased with increasing the GalCer composition until it reached 3 wt.% and then decreased upon further addition of GalCer. When we added galactose itself as a competitor, the luciferase activity was decreased, while glucose did not show such decrease, suggesting that HepG2 ingested the lipoplex via ASGPR-mediated endocytosis. This paper indicated that the hexagonally packed inverted cylinder structures of lipoplex may not always provide excellent transfection and presented a possibility that the TRX lipoplex#3 can obtain a cellulartargeting ability through the receptors for oligosaccharide.
