Safety and Efficacy of a Transdermal Rotigotine for the Treatment of Fatigue and Quality of Life in Patients with Parkinson’s Disease

Aim: To evaluate safety and efficacy of a transdermal rotigotine for the treatment of fatigue and quality of life (QOL) in patients with Parkinson’s disease (PD). This was a multi-sites open-label study of 58 PD patients (male 26, female 32) who met a Japanese PD diagnosis criterion. They received a transdermal rotigotine 4.5 mg/day for 8 weeks. We added a rotigotine on the previous anti-Parkinson’s drugs. Clinical signs were evaluated by Hoehn-Yahr (H-Y) stage, unified Parkinson’s disease rating scale (UPDRS), fatigue severity scale (FSS), and Euro quality of life (QOL). The scores of UPDRS improved from 35.2 ± 8.0 (mean ± SD) to 31.8 ± 8.3 (P = 0.14). There was no significant improvement or worsening of the H-Y stages. The scores of FSS improved from 57.3 ± 12.7 (mean ± SD) to 50.1 ± 11.8 (P = 0.061). The scores of QOL improved from 38.1 ± 11.1 to 48.3 ± 10.0 (P = 0.068). Our data demonstrate that, in a small sample size, administration of a transdermal rotigotine was associated with few side effects and was modestly effective for the treatment of fatigue and QOL in patients with PD.

International Conservation Working Holidays in Fukuoka, Japa...

＜正＞International Conservation Working Holidays （ICWH）, which were carried out in cooperation with the British Trust for Conservation Volunteers （BTCV）, were held nine times between 1997 and 2005 in Kurogi-cho, Fukuoka Prefecture, Japan. They were investigated in this study. Firstly, questionnaire surveys were conducted among the Japanese participants to evaluate the activities done during the ICWHs. Secondly, the number of repeat volunteers from previous years and the number of staff with their assigned responsibilities were analyzed. Finally, changes in the BTCV’s international strategy as a result of having launched the Conservation Volunteers Alliance Accreditation Program are described. The purpose of this investigation is to clarify various problems related to the ICWHs and to demonstrate the necessity of commencing leadership training and of establishing self-sufficient regional organizations to overcome these problems. The number of valid responses to the questionnaire survey was 207 which represents 80.9% of the total number of returned questionnaires. The highest response to the question regarding activities was "Interaction with other people", although the highest reason given for participation was "Do actual environmental conservation work". It is conjectured that the safe and pleasant management not only enabled conservation tasks, but also fostered communication. Regarding the repeats status of volunteers, between 80 to 90% of the participants intended to participate in the ICWH program again, while 10% were interested in being involved in project management. The actual number of repeat volunteers increased each year from 1998 to 2005. The re-participation rate was approximately 10% and the proportion of return volunteers who returned as staff （as opposed to a normal participants） was approximately 50%. Primary cardiac epithelioid hemangioendothelioma (EHE) is extremely uncommon. To our knowledge, only fourteen cases have been reported. We report a case of a primary EHE of the right atrium which incidentally detected in 77-year-old woman. Echocardiograhpy revealed a 26 × 16 mm right atrial mass attached to the base of the interatrial septum and moderate regurgitation of the tricuspid valve. The tumor and stalk were completely resected from the interatrial septum with a partial resection of the septal leaflet of the tricuspid valve. The patient had an uneventful postoperative course and no signs of recurrence nor metastasis of EHE 4 years after operation. Because of malignant potential of EHE, a longer period of close follow up is necessary.

In recent years, stronger interest has been focused on the development of field application of polymer impregnation for the purpose of improving the durability of various reinforced concrete structures. Field polymer impregnation system is applied as the surface treatment of old or new cast-in-place reinforced concrete structures. The conventional field polymer impregnation system generally consists of base concrete drying, impregnant penetration and polymerization processes. The authors have developed a new labor- and energy-saving system in which the impregnant is penetrated immediately after the drying of the base concrete, and then the polymerization is accomplished by the heat or thermal energy remaining in the dried concrete in a short time. The purpose of this investigation is to establish the practical procedures for such a rapid field polymer impregnation system. In the laboratory tests, base concrete slabs are dried under various conditions by use of an LPG (liquefied petroleum gas) torch burner and a far infrared heater, and penetrated with methyl methacrylate-based impregnant by ponding method, subsequently polymerized by the remaining thermal energy. The behavior of the impregnant under heating conditions and the effects of the impregnant formulations and working life, base concrete temperature and remaining thermal energy on polymer penetration depth are examined. As a result, it is concluded that the polymer penetration depth can be controlled by the working life of the impregnant and the drying temperature-time product of the base concrete regardless of the drying methods and conditions of the base concrete. The temperature-time product expresses the approximate level of the remaining thermal energy. There are only a few reports about upper extremity fractures in patients with Parkinson’s disease (PD). This is a case report of a PD patient with severe tremors who had proximal humeral fracture. We performed surgery for the left side and conservative treatment for the right side. The patient was a 73-year-old woman who had been diagnosed with PD 10 years prior to presentation. Open reduction internal fixation was conducted for the left proximal humeral fracture. 7 months after the left shoulder fracture, she fell resulting in a right proximal humeral fracture for which she underwent conservative treatment. Although bone union was obtained bilaterally, anterior subluxation occurred in the operated side 9 months postoperative. Screw remove was performed because perforation of the screw was observed in the humeral head and was causing pain. Currently, restriction in range of motion and subluxation may be seen on the side that received conservative treatment, although pain is absent and patient satisfaction is high. On the operated side, there is a marked restriction in range of motion, subluxation, and pain. Our experience revealed that conservative treatment was effective for proximal humeral fractures and that optimal bone union might be obtained even in patients with suboptimal PD control.

Classification of Intervertebral Disc Degeneration in Low Back Pain Using Diffusional Kurtosis Imaging

Degenerative disc disease is the most common cause of low back pain. Intervertebral disc abnormalities are commonly evaluated by magnetic resonance imaging (MRI), and Pfirrmann’s system involves the use of T2-weighted images (T2WI) to classify disc degeneration. However, as this classification is based on visual evaluation, it is not possible to quantify degeneration using this method. The present study was performed to establish an MRI-based intervertebral disc classification system using diffusional kurtosis imaging (DKI), to quantify intervertebral disc water content according to the Pfirrmann classification. Sagittal mean diffusional kurtosis (MK) mapping was performed for the L3/4, L4/5, and L5/S1 intervertebral discs in 32 patients (15 female, 17 male;age range, 24 - 82 years;mean age, 57.7 years). The degree of disc degeneration was assessed in the midsagittal section on T2WI according to the Pfirrmann classification (grade I - V). The relationships between MK values, which are correlated with intervertebral disc composition changes, and grade of degeneration determined using the Pfirrmann classification were analyzed. The MK values tended to decrease with increasing grade of degeneration, and differed significantly between grades I and IV, but not between grade IV and V (P < 0.05, Mann-Whitney U test). DKI is an effective means of detecting the early stages of disc degeneration. Therefore, DKI may be a useful diagnostic tool for quantitative assessment of intervertebral disc degeneration.

In this paper,a navigation and a position estimation algorithm based on SOM are introduced into the path planning *** efficiency of the system is examined and discussed through the collision avoidance and position estimation experiment using the distances obtained from the ultrasonic range sensors and the magnetic field data.

＜正＞ RAMP2 and -3 enable CRLR to function as an adrenomedullin （AM） receptor （CRLR/RAMP2 or -3）. We examined the functions of the transmembrane （TM） domain and cytoplasmic C-terminal tails （C-tails） of RAMP2 and -3 by cotrans-fecting their various mutants and chimeras into HEK-293 cells stably expressing human CRLR. FACS analysis revealed that substituting a Thr-Val sequence in the RAMP3 TM with the corresponding region （Ile-Pro） from RAMP2 significantly enhances AM-induced intemalization of CRLR. suggesting the RAMP2 sequence participates in the positive regulation of CRLR intemalization. Deletion of the C-tail from RAMP2 disrupted CRLR transport from the endoplasmic reticulum to the cell surface, markedly reducing 125I AM binding and evoked cAMP accumulation. Deletion of the C-tail from RAMP3 markedly enhanced CRLR intemalization. though there was no change in agonist affinity. The highly conserved Ser-Lys sequence within RAMP C-tails is involved in the cellular trafficking of the two AM receptors, but deleting the C-tails from RAMPs had no effect on lysosomal sorting of CRLR. Thus, the respective C-tails of RAMP2 and -3 differentially affect CRLR surface delivery and intemalization.
