Polysomnographic assessment of respiratory disturbance during deep propofol sedation for endoscopic submucosal dissection of gastric tumors

AIM To investigate that polysomnographic monitoring can accurately evaluate respiratory disturbance incidence during sedation for gastrointestinal endoscopy compare to pulse oximetry *** This prospective observational study included 10 elderly patients with early gastric cancer undergoing endoscopic submucosal dissection(ESD) under propofol sedation. Apart from routine cardiorespiratory monitoring, polysomnography measurements were acquired. The primary hypothesis was tested by comparing the apnea hypopnea index(AHI), defined as the number of apnea and hypopnea instances per hour during sedation, with and without hypoxemia; hypoxemia was defined as the reduction in oxygen saturation by ≥ 3% from *** Polysomnography(PSG) detected 207 respiratory disturbances in the 10 patients. PSG yielded a significantly greater AHI(10.44 ± 5.68/h) compared with pulse oximetry(1.54 ± 1.81/h, P < 0.001), thus supporting our hypothesis. Obstructive AHI(9.26 ± 5.44/h) was significantly greater than central AHI(1.19 ± 0.90/h, P < 0.001). Compared with pulse oximetry, PSG detected the 25 instances of respiratory disturbances with hypoxemia 107.4 s earlier on *** Compared with pulse oximetry, PSG can better detect respiratory irregularities and thus provide superior AHI values, leading to avoidance of fatal respiratory complications during ESD under propofol-induced sedation.

＜正＞As part of the Japan-China Joint Studies on Origin and Transport of Aeolian Dust and its Impact on Climate（ADEC）,a Mie-scattering depolarization lidar was developed and operated in Aksu,Xinjiang,China（40.62 N,80.83 E,1028 m above mean sea level）to investigate the vertical structure of the dust layer over the Taklimakan ***,an oasis city,is located on the northern part of the Taklimakan Desert.A strong dust storm occurred in Aksu on 13-16 April *** this study,by directly solving the lidar equation,we estimated the backscattering ratio at the lowest height during the dust storm.

In this study, waste glycerol from biodiesel pretreatment was tested as catalyst for the supercritical water gasification. Hydrogen fermentation solid residue of food waste（FW-HFSR） was gasified in the supercritical water gasification reactor with and without FW-HFSR. The FW-HFSR showed good catalytic effect. Considering reagent glycerol does not show any catalytic effect, KOH in waste glycerol should have worked as catalyst. Based on this finding, first-order rate equation has been developed, and good agreement with experimental result.

检验了这样一个假设:局部地震复发时间极小值T_L和等效的局部概率极大值P_L可用来测绘日本关东和东海地区的凹凸体。在这两个地区,国家地球科学与灾害预防研究所(NIED)的地震目录在M=1.5(M1.5)级以上是完整的。以网格间距为0.01°的各节点20km内的地震计算α值和b值,以此为基础绘出M7目标地震的T_L(P_L)图。只选用了上部33km深度内的地震。在若干地区,随着深度的增加b值显著上升。因此,若整个地壳孕震区的数据混到一起的话,有些T_L(P_L))异常就不会被揭示出来。这样,我们对顶部15km和其余深度范围,以及整个地壳孕震区分别测绘了T_L(P_L)。所得的T_L和P_L图表明,整个区域约有12％呈现出异常短的复发时间。在1890年以来发生的6个M≥6.5的目标浅震中,5个位于T_L<450年的异常区内。我们将此解释为具有异常短T_L的地区勾划出了凹凸体,未来这些地区比其他地区更有可能发生目标地震。该结果系由偶然性造成的可能性几乎不存在。1923年关东特大地震的破裂似乎发端于我们在研究区内所绘出的最为显著的凹凸体之中。一个异常位于未来可能的东海地震破裂区的东北部,另一异常位于其西南角上。计算出的T_L绝对值是不确定的,因为它们取决于计算所用区域的大小。

Two-dimensional(2D) ferromagnetic materials have been discovered with tunable magnetism and orbital-driven nodal-line features. Controlling the 2D magnetism in exfoliated nanoflakes via electric/magnetic fields enables a boosted Curie temperature(TC) or phase transitions. One of the challenges, however, is the realization of high TC2D magnets that are tunable, robust and suitable for large scale fabrication. Here, we report molecular-beam epitaxy growth of wafer-scale Fe3+XGeTe2films with TCabove room temperature. By controlling the Fe composition in Fe3+XGeTe2, a continuously modulated TCin a broad range of 185–320 K has been achieved. This widely tunable TCis attributed to the doped interlayer Fe that provides a 40% enhancement around the optimal composition X = 2. We further fabricated magnetic tunneling junction device arrays that exhibit clear tunneling signals. Our results show an effective and reliable approach, i.e. element doping, to producing robust and tunable ferromagnetism beyond room temperature in a large-scale 2D Fe3+XGeTe2fashion.

Neck neuromuscular treatment for depressive disorders with cervical muscular tension or chronic neck pain—A pilot study

BACKGROUND: An intimate relationship between depressive disorders and chronic pain is well known but often neglected. We studied patients with depressive disorders accompanying cervical muscular tension or neck pain. They also complaints of various physical, psychosomatic, and psychiatric signs such as anxiety and non-specific complaints related to autonomic imbalance. These complaints or symptoms are commonly resistant to pharmacological treatment, and thus we tried to treat these conditions by reducing their cervical muscular tension or pain. METHODS: We evaluated 30 complaints of 138 patients suffering from depressive disorders with cervical muscular abnormality (52 men and 86 women). The presence of the 30 complaints was evaluated as ‘positive’ or ‘negative’ before and during treatment. The necks of all patients were treated using low-frequency stimulation, micro-wave therapies, electric needle application, and acupuncture. RESULTS: Reducing cervical muscular tension remitted or cured the 30 complaints including depressive mood. The reduction of cervical muscle pain or tension initially ameliorated the depressive symptoms and anxiety, and subsequently improved their autonomic imbalances. Discriminant analysis of the first and second examinations correctly classified 95.3% of original grouped cases. CONCLUSION: Reducing cervical muscular tension is an effective treatment for patients suffering from depressive disorders with cervical neuromuscular pain or rigidity.

Usefulness of Immuno-Magnetic Beads Conjugated with Anti-EpCAM Antibody for Detecting Endometrial Cancer Cells

A simple and non-invasive method for detecting endometrial cancer in women with abnormal uterine bleeding is required. For this purpose, we prepared immuno-magnetic beads conjugated with anti-human EpCAM rat monoclonal antibody (mAb) for isolating exfoliated endometrial cells including endometrial cancer cells in vaginal discharge. The affinities of the anti-human EpCAM rat mAbs were analyzed by flow cytometry and immunocytochemistry and then magnetic beads were conjugated with the mAbs. The rate of retrieval of endometrial cells using the immuno-magnetic beads was calculated. Endometrial cells were isolated using the immuno-magnetic beads from the vaginal discharges of 22 patients with endometrial cancer and 16 non-malignant controls. The isolated cells were stained using endometrial cancer specific-mAbs and analyzed by flow cytometry and imaging cytometry. The immuno-magnetic beads conjugated with high-affinity mAb (clone 1456) appeared to have very low auto-fluorescence. Sufficient enrichment of Ep-CAMpositive cells using immuno-magnetic beads was observed in both simulation and clinical samples. The overall sensitivities of flow cytometry and imaging cytometry to detect endometrial cancer cells were 72.7% and 45.5%, respectively. Meanwhile, the overall specificities of flow cytometry and imaging cytometry for healthy controls were 75.0% and 81.3%, respectively. Our immuno-magnetic beads have very low auto-fluorescence, so they could be useful for fluorescent analysis, such as fluorescent immunochemical staining. In the future, these novel immuno-magnetic beads could be used for cytological study.

Efficacy of computed image modification of capsule endoscopy in patients with obscure gastrointestinal bleeding

AIM:To investigate whether flexible spectral color enhancement(FICE) improves diagnostic yields of capsule endoscopy(CE) for obscure gastro-intestinal bleeding(OGIB).METHODS:The study subjects consisted of 81 *** FICE,there were three different sets with different *** randomly selected sets of FICE,images of CE were evaluated again by two individuals who were not shown the conventional CE reports and *** difference between FICE and conventional imaging was ***:The overall diagnostic yields in FICE sets 1,2,3 and conventional imaging(48.1%) were 51.9%,40.7%,51.9% and 48.1%,respectively,which showed no statistical difference compared to conventional *** total numbers of detected lesions per examination in FICE imaging and conventional imaging were 2.5 ± 2.1 and 1.8 ± 1.7,respectively,which showed a significant difference(P = 0.01).CONCLUSION:The diagnostic yield for OGIB is not improved by ***,FICE can detect significantly more small bowel lesions compared to conventional imaging.
