Colonoscopy can miss diverticula of the left colon identified by barium enema

AIM: To identify the diagnostic value of colonoscopy for diverticulosis as determined by barium enema. METHODS: A total of 65 patients with hematochezia who underwent colonoscopy and barium enema were analyzed, and the diagnostic value of colonoscopy for diverticula was assessed. The receiver operating characteristic area under the curve was compared in relation to age (< 70 or ≥ 70 years), sex, and colon location. The number of diverticula was counted, and the detection ratio was calculated. RESULTS: Colonic diverticula were observed in 46 patients with barium enema. Colonoscopy had a sensitivity of 91% and specificity of 90%. No significant differences were found in the receiver operating characteristic area under the curve (ROC-AUC) for age group or sex. The ROC-AUC of the left colon was significantly lower than that of the right colon (0.81 vs 0.96, P=0.02). Colonoscopy identified 486 colonic diverticula, while barium enema identified 1186. The detection ratio for the entire colon was therefore 0.41 (486/1186). The detection ratio in the left colon (0.32, 189/588) was significantly lower than that of the right colon (0.50, 297/598) (P < 0.01). CONCLUSION: Compared with barium enema, only half the number of colonic diverticula can be detected by colonoscopy in the entire colon and even less in the left colon.

Hepatocellular carcinoma(HCC),the predominant form of primary liver cancer,is the fifth most common cancer worldwide and the second leading cause of cancer-related death. Despite the high incidence,treatment options remain limited for advanced HCC,and as a result prognosis continues to be poor. Current therapeutic options,surgery,chemotherapy and radiotherapy,have only modest efficacy. New treatment modalities to prolong survival and to minimize the risk of adverse response are desperately needed for patients with advanced HCC. Tumor immunotherapy is a promising,novel treatment strategy that may lead to improvements in both treatment-associated toxicity and outcome. The strategies have developed in part through genomic studies that have yielded candidate target molecules and in part through basic biology studies that have defined the pathways and cell types regulating immune response. Here,we summarize the various types of HCC immunotherapy and argue that the newfound field of HCC immunotherapy might provide critical advantages in the effort to improve prognosis of patients with advanced HCC. Already several immunotherapies,such as tumor-associated antigen therapy,immune checkpoint inhibitors and cell transfer immunotherapy,have demonstrated safety and feasibility in HCC patients. Unfortunately,immunotherapy currently has low efficacy in advanced stage HCC patients; overcoming this chal lenge will place immunotherapy at the forefront of HCC treatment,possibly in the near future.

Hepatocellular carcinoma(HCC) is the fifth most common cancer and the second leading cause of cancer-related deaths worldwide. Although the prognosis of patients with HCC is generally poor, the5-year survival rate is > 70% if patients are diagnosed at an early stage. However, early diagnosis of HCC is complicated by the coexistence of inflammation and cirrhosis. Thus, novel biomarkers for the early diagnosis of HCC are required. Currently, the diagnosis of HCC without pathological correlation is achieved by analyzing serum α.fetoprotein levels combined with imaging techniques. Advances in genomics and proteomics platforms and biomarker assay techniques over the last decade have resulted in the identification of numerous novel biomarkers and have improved the diagnosis of HCC. The most promising biomarkers,such as glypican-3, osteopontin, Golgi protein-73 and nucleic acids including microRNAs, are most likely to become clinically validated in the near future. These biomarkers are not only useful for early diagnosis of HCC, but also provide insight into the mechanisms driving oncogenesis. In addition, such molecular insight creates the basis for the development of potentially more effective treatment strategies. In this article,we provide an overview of the biomarkers that are currently used for the early diagnosis of HCC.

Cultivation of microalgae for biodiesel production:A review on upstream and downstream processing

Fluctuating market price of fossil fuel and overwhelming emission of greenhouse gases to the atmosphere have resulted in climate change and have been a global concern in this decade. Hence, biodiesel has become an alternative option to fossil diesel as it is renewable and environmentally friendly. Nevertheless, this alternative fuel that is usually derived from terrestrial oil crops will cause shortage in food supply and deforestation if mass production is realized. In recent years, cultivation of aquatic microorganism(particularly microalgae) to produce biodiesel is considered as a practical solution due to their high growth rate and ability to synthesize large quantity of lipid within their cell. However, the development of energy and cost-efficiency of microalgae cultivation system are the main issues in producing renewable microalgae biodiesel. Of late, wastewater or organic compost has been used as the cultivation medium as it can provide sufficient nutrients to sustain microalgae *** cultivation method and system are vitally important as these factors undoubtedly affect the final microalgae biomass and lipid yield. In this review, the cultivation system of microalgae, nutrients demanded for microalgae production, cell harvesting and drying, microalgae oil extraction, and utilization of microalgae biomass for biodiesel production are introduced and discussed. It is anticipated to convey clearer perspectives in upstream and downstream processes in microalgae-derived biodiesel production.

Eastern Asian endemic seed plant genera and their paleogeographic history throughout the Northern Hemisphere

We review the fossil history of seed plant genera that are now endemic to eastern Asia. Although the majority of eastern Asian endemic genera have no known fossil record at all, 54 genera, or about 9%, are reliably known from the fossil record. Most of these are woody (with two exceptions), and most are today either broadly East Asian, or more specifically confined to Sino-Japanese subcategory rather than being endemic to the Sino-Himalayan area. Of the "eastern Asian endemic" genera so far known from the fossil record, the majority formerly occurred in Europe and/or North America, indicating that eastern Asia served as a late Tertiary or Quaternary refugium for taxa. Hence, many of these genera may have originated in other parts of the Northern Hemisphere and expanded their ranges across continents and former sea barriers when tectonic and climatic conditions al- lowed, leading to their arrival in eastern Asia. Although clear evidence for paleoendemism is provided by the gymnosperms Amentotaxus, Cathaya, Cephalotaxus, Cunninghamia, Cryptomeria, Glyptostrobus, Ginkgo, Ketel-eeria, Metasequoia, Nothotsuga, Pseudolarix, Sciadopitys, and Taiwania, and the angiosperms Cercidiphyllum, Choerospondias, Corylopsis, Craigia, Cyclocarya, Davidia, Dipelta, Decaisnea, Diplopanax, Dipteronia, Em-menopterys, Eucommia, Euscaphis, Hemiptelea, Hovenia, Koelreuteria, Paulownia, Phellodendron, Platycarya, Pteroceltis, Rehderodendron, Sargentodoxa, Schizophragma, Sinomenium, Tapiscia, Tetracentron, Toricellia, Trapella, and Trochodendron, we cannot rule out the possibility that neoendemism plays an important role especially for herbaceous taxa in the present-day flora of Asia, particularly in the Sino-Himalayan region. In addition to reviewing paleobotanical occurrences from the literature, we document newly recognized fossil occurrences that expand the geographic and stratigraphic ranges previously known for Dipelta, Pteroceltis, and Toricellia.

The purpose of this paper is, first of all, to show what V. E. Frankl calls meanings rooted in three values： creativevalues, experiential values, and attitudinal values. Creative values are what one finds by creating a work or doing adeed. Experiential values are realized by experiencing something or encountering someone. Attitudinal values arewhat a person discovers by the attitude she/he takes toward unavoidable suffering. The author points out that thesethree kinds of values have something in common： connectedness or relationship. Creative values （e.g., an artisticmasterpiece） show the worker＇s connectedness with the work accomplished. Experiential values also stand for therelationship between, say, a person and natural beauty. Frankl describes a person＇s attitudinal values by stating ＂Hemay turn a personal tragedy into a triumph.＂ In short, we can say that attitudinal values indicate relationshipbetween the sufferer and suffering itself. The author states the core of Frankl＇s three values is the aforementionedconnectedness or relationship. In other words, what Frankl calls meanings indicates manifestation of connectednessor relationship in the form of creations, experiences, and attitudes. However, in this paper, the author insists thateven if Frankl＇s three kinds of values are not realized, there is another value to realize beyond Frankl＇s three kindsof values, which is coexistence. Within this context, Frankl fails to mention cases where one cannot find meaning inlife by a deed or work they does, an experience they encounter, or an attitude they takes in the midst of *** three types of meanings in life need a skill, a chance, and/or ego-strength. We can discover meaning in life justby being together, and the author proposes the fourth meaning in life, or coexistent values. It can be defined as＂values realized by having meaningful relationships or togetherness with others.＂ Careful analysis of Frankl＇writings shows connectedness aspects of logotherapy

Many prior research findings indicate that audit quality differs between the Big 4 and non-Big 4 audit firms using an indicator variable. However, most previous research focuses on only outcome measures, such as audit fees, going concern reports, and non-audit services. This study investigates audit quality differentiation between the Big 4 and non-Big 4 audit firms hypothesis from an audit objective point of view. One of the material objectives of Japanese internal control audit institutions is to facilitate assessment and improvement of internal controls by corporations themselves. The findings of this study indicate that the Big 4 audit firms accomplish this objective better than non-Big 4 audit firms. Consequently, most Big 4 clients do not disclose significant deficiencies （SDs）, implying that they improve the quality of internal controls through internal controls auditing. This paper concludes that Big 4 firms produce a higher audit quality level than non-Big 4 firms, and this quality difference iS related to how an audit objective is interpreted and implemented.

The interactive eff ects of ozone,soil nutrient availability and root microorganisms on physiological,growth,and productivity traits were studied for the fi rst time for Japanese larch(Larix kaempferi)seedlings grown in containers over a growing season,using a free air ozoneconcentration enrichment exposure *** nutrient availability altered leaf and root nutrient dynamics and enhanced plant growth;however,it also enhanced seedling susceptibility to damping-off disease compared to low nutrient *** eff ects of elevated ozone,as compared with ambient ozone,on leaf gas exchange and plant stem form were neither off set nor exacerbated by soil nutrient availability and root *** negative eff ects suggest that elevated ozone may have implications for ecological health even when plant vigor is limited by factors other than *** of roots with ectomycorrhizae had negligible infl uence on the eff ects of either soil nutrient availability or ***,this lack of eff ect may be upon impeded formation of complete mycorrhizal root tips due to factors other than the manipulated variables.B and Na appeared to have an important role in stress responses,so further studies to examine their link with physiological mechanisms as a function of *** study provides an important perspective for designing forestry practices to enhance seedling health.

This paper describes an extendable graphical framework,aflak,which provides a visualization and provenance management environment for the analysis of multi-spectral astronomical *** its node editor interface,aflak allows the astronomer to compose transforms on input datasets queryable from public astronomical data repositories,then to export the results of the analysis as Flexible Image Transport System(FITS)files,in a manner such that the full provenance of the output data be preserved and reviewable,and that the exported file be usable by other common astronomical analysis *** is the standard of data interchange in *** embedding aflak’s provenance data into FITS files,we both achieve interoperability with existing software and full reproducibility of the process by which astronomers make discoveries.
