AIM: To examine the long term survival of geriatric patients treated with percutaneous endoscopic gastrostomy (PEG) in Japan. METHODS: We retrospectively included 46 Japanese community and tertiary hospitals to investigate 931 consecutive geriatric patients (≥ 65 years old) with swallowing difficulty and newly performed PEG between Jan 1st 2005 and Dec 31st 2008. We set death as an outcome and explored the associations among patient’s characteristics at PEG using log-rank tests and Cox proportional hazard models. RESULTS: Nine hundred and thirty one patients were followed up for a median of 468 d. A total of 502 deaths were observed (mortality 53%). However, 99%, 95%, 88%, 75% and 66% of 931 patients survived more than 7, 30, 60 d, a half year and one year, respectively. In addition, 50% and 25% of the patients survived 753 and 1647 d, respectively. Eight deaths were considered as PEG-related, and were associated with lower serum albumin levels (P = 0.002). On the other hand, among 28 surviving patients (6.5%), PEG was removed. In a multivariate hazard model, older age [hazard ratio (HR), 1.02; 95% confidence interval (CI), 1.00-1.03; P = 0.009], higher C-reactive protein (HR, 1.04; 95% CI: 1.01-1.07; P = 0.005), and higher blood urea nitrogen (HR, 1.01; 95% CI: 1.00-1.02; P = 0.003) were significant poor prognostic factors, whereas higher albumin (HR, 0.67; 95% CI: 0.52-0.85; P = 0.001), female gender (HR, 0.60; 95% CI: 0.48-0.75; P < 0.001) and no previous history of ischemic heart disease (HR, 0.69; 95% CI: 0.54-0.88, P = 0.003) were markedly better prognostic factors. CONCLUSION: These results suggest that more than half of geriatric patients with PEG may survive longer than 2 years. The analysis elucidated prognostic factors.

We present a novel framework for the multidomain synthesis of artworks from semantic layouts.One of the main limitations of this challenging task is the lack of publicly available segmentation datasets for art synthesis.To address this problem,we propose a dataset called ArtSem that contains 40,000 images of artwork from four different domains,with their corresponding semantic label maps.We first extracted semantic maps from landscape photography and used a conditional generative adversarial network(GAN)-based approach for generating high-quality artwork from semantic maps without requiring paired training data.Furthermore,we propose an artwork-synthesis model using domain-dependent variational encoders for high-quality multi-domain synthesis.Subsequently,the model was improved and complemented with a simple but effective normalization method based on jointly normalizing semantics and style,which we call spatially style-adaptive normalization(SSTAN).Compared to the previous methods,which only take semantic layout as the input,our model jointly learns style and semantic information representation,improving the generation quality of artistic images.These results indicate that our model learned to separate the domains in the latent space.Thus,we can perform fine-grained control of the synthesized artwork by identifying hyperplanes that separate the different domains.Moreover,by combining the proposed dataset and approach,we generated user-controllable artworks of higher quality than that of existing approaches,as corroborated by quantitative metrics and a user study.

This paper investigates the production scheduling problems of allocating resources and sequencing jobs in the seru production system(SPS).As a new-type manufacturing mode arising from Japanese production practices,seru production can achieve efficiency,flexibility,and responsiveness simultaneously.The production environment in which a set of jobs must be scheduled over a set of serus according to due date and different execution modes is considered,and a combination optimization model is provided.Motivated by the problem complexity and the characteristics of the proposed seru scheduling model,a nested partitioning method(NPM)is designed as the solution approach.Finally,computational studies are conducted,and the practicability of the proposed seru scheduling model is proven.Moreover,the efficiency of the nested partitioning solution method is demonstrated by the computational results obtained from different scenarios,and the good scalability of the proposed approach is proven via comparative analysis.

Assessing mixing uniformity in microreactors via in-line spectroscopy

Mixing behavior is critical for enhancing the selectivity of fast chemical reactions in microreactors.A high Reynolds number(Re)improves the mixing rate and selectivity of the reactions,but some exceptions of increasing side product yield with the higher Re have been reported.This study investigated the mixing uniformity in microreactors with in-line UV-vis spectroscopy to clarify the relationship between reaction selectivity and chaotic mixing with the higher Re.A colorization experiment of thymolphthalein in an acidic solution was conducted with an excess acid amount to the base to indicate a non-uniformly mixed region.Non-uniformity significantly increased with Re.At the same time,the degree of mixing,which was measured by a usual decolorization experiment,showed that the mixing rate increased with Re.The in-line analysis of the Villermaux-Dushman reaction during the mixing clarified that side product yield significantly increased with Re at around 300 and then decreased at around 1100.These results suggest the compensation effect between the mixing uniformity and mixing rate on the selectivity of the mixing-sensitive reactions.Faster mixing,characterized by a larger Re,can disturb mixing uniformity and,in some cases,decrease reaction selectivity.

BACKGROUND Bone metastasis has various negative impacts.Activities of daily living(ADL)and quality of life(QOL)can be significantly decreased,survival may be impacted,and medical expenses may increase.It is estimated that at least 5%cancer patients might be suffering from bone metastases.In 2016,we published the Comprehensive Guidelines for the Diagnosis and Treatment of Bone Metastasis.Since then,the therapeutic outcomes for patients have gradually improved.As life expectancy is a major determinant of surgical intervention,the strategy should be modified if the prolongation of survival is to be achieved.AIM To monitor how bone metastasis treatment has changed before and after launch of our guidelines for bone metastasis.METHODS For advanced cancer patients with bone metastasis who visited the Department of Clinical Oncology at Akita University hospital between 2012 and 2023,parameters including the site and number of bone metastases,laboratory data,and survival time,were extracted from electronic medical records and the Katagiri score was calculated.The association with survival was determined for each factor.RESULTS Data from 136 patients were obtained.The 1-year survival rate for the poor prognosis group with a higher Katagiri score was 20.0%in this study,which was 6%and an apparent improvement from 2014 when the scoring system was developed.Other factors significantly affecting survival included five or more bone metastases than less(P=0.0080),and treatment with chemotherapy(P<0.001),bone modifying agents(P=0.0175)and immune checkpoint inhibitors(P=0.0128).In recent years,advances in various treatment methods have extended the survival period for patients with advanced cancer.It is necessary not only to simply extend survival time,but also to maintain ADL and improve QOL.CONCLUSION Various therapeutic interventions including surgical approach for bone metastasis,which is a disorder of locomotor organs,are increasingly required.Guidelines and scoring system for prognosis need to be revised promptly.

BACKGROUND The success of liver resection relies on the ability of the remnant liver to regenerate.Most of the knowledge regarding the pathophysiological basis of liver regeneration comes from rodent studies,and data on humans are scarce.Additionally,there is limited knowledge about the preoperative factors that influence postoperative regeneration.AIM To quantify postoperative remnant liver volume by the latest volumetric software and investigate perioperative factors that affect posthepatectomy liver regenera-tion.METHODS A total of 268 patients who received partial hepatectomy were enrolled.Patients were grouped into right hepatectomy/trisegmentectomy(RH/Tri),left hepa-tectomy(LH),segmentectomy(Seg),and subsegmentectomy/nonanatomical hepatectomy(Sub/Non)groups.The regeneration index(RI)and late rege-neration rate were defined as(postoperative liver volume)/[total functional liver volume(TFLV)]×100 and(RI at 6-months-RI at 3-months)/RI at 6-months,respectively.The lower 25th percentile of RI and the higher 25th percentile of late regeneration rate in each group were defined as“low regeneration”and“delayed regeneration”.“Restoration to the original size”was defined as regeneration of the liver volume by more than 90%of the TFLV at 12 months postsurgery.RESULTS The numbers of patients in the RH/Tri,LH,Seg,and Sub/Non groups were 41,53,99 and 75,respectively.The RI plateaued at 3 months in the LH,Seg,and Sub/Non groups,whereas the RI increased until 12 months in the RH/Tri group.According to our multivariate analysis,the preoperative albumin-bilirubin(ALBI)score was an independent factor for low regeneration at 3 months[odds ratio(OR)95%CI=2.80(1.17-6.69),P=0.02;per 1.0 up]and 12 months[OR=2.27(1.01-5.09),P=0.04;per 1.0 up].Multivariate analysis revealed that only liver resection percentage[OR=1.03(1.00-1.05),P=0.04]was associated with delayed regeneration.Furthermore,multivariate analysis demonstrated that the preoperative ALBI score[OR=2.63(1.00-1.05),P=0.02;per 1.0 up]and liver resection percentage[OR=1.02(1.00

Reverse shoulder arthroplasty (RSA) is an effective treatment for rotator cuff tears. Despite its advantages, complications occur at a high rate. Complications requiring revision include a high rate of base plate failure, 38% of which are due to instability. The primary stability the base plate ensures is a crucial factor and, thus, is the subject of much debate in clinical studies and biomechanical research. This study is aimed to provide data that will contribute to the base plate’s pri-mary stability and glenoid longevity by clarifying the stresses at the scapular fossa and base plate interface associated with elevation after RSA. A 3D finite element model was created from the DICOM data for the scapulohumeral joint and SMR shoulder system. For loading conditions, 30 N was applied for each posi-tion with abduction angles of 0, 45, 90, and 135 degrees. A three-dimensional fi-nite element analysis was performed using the static implicit method with LS-DYNA. The von Mises stresses in the scapular fossa were found not to exceed the yield stress on the bone even after elevation to an abduction angle of 135 de-grees after RSA. It is rough to uniformly compare the yield stress and the von Mises stress, but it was inferred that the possibility of fracture is low unless a large external force is applied. A maximum von Mises stress showed 0 degrees of abduction, suggesting that the lowered position is in a more severe condition than the elevated position. If better improvement is desired, it may be necessary to devise ways to reduce the stress on the upper screw. .

Bulk Mg-based hydrogen storage materials have the potential to provide a low-cost solution to diversify energy storage and transportation.Compared to nano powders which require handling and processing under hydrogen or an inert gas atmosphere,bulk Mg-based alloys are safer and are more oxidation re-sistant.Conventional methods and existing infrastructures can be used to process and handle these ma-terials.However,bulk Mg alloys have smaller specific surface areas,resulting in slower hydrogen sorp-tion kinetics,higher equilibrium temperatures,and enthalpies of hydride formation.This work reviews the effects of the additions of a list of alloying elements and the use of innovative processing meth-ods,e.g.,rapid solidification and severe plastic deformation processes,to overcome these drawbacks.The challenges,advantages,and weaknesses of each method and future perspectives for the development of Mg-based hydrogen storage materials are discussed.

耐腐蚀性能和拉伸性能提升的α型钛合金设计

通过2个电子参数(结合次数Bo_(t)和d轨道能级Mdt)提出了新设计的α型钛(α-Ti)合金。新设计合金Ti-5Al-4Zr-3.6Sn、改性合金Ti-5Al-3Sn-1.9Zr和参考合金Ti-5Al-2.5Sn具有相同的Bo_(t)值(3.847)以及不同的Mdt值(2.430,2.426,2.422)。测试了3种α-Ti合金的极限抗拉伸强度(σ_(UTS))、断裂应变(ɛ_(f))和热盐腐蚀性能。3种α-Ti合金均采用冷坩埚悬浮熔炼技术进行制备。结果表明,3种合金样品均具有均匀的微观结构。在3种α-Ti合金中测量到的α单相晶粒尺寸约为600μm。Ti-5Al-4Zr-3.6Sn合金的σ_(UTS)和ɛ_(f)值为801 MPa和16%,Ti-5Al-3Sn-1.9Zr合金的σ_(UTS)和ɛ_(f)值为708 MPa和15%,Ti-5Al-2.5Sn合金的σ_(UTS)和ɛ_(f)值为603 MPa和15%。热盐腐蚀测试进行28.8ks后显示Ti-5Al-4Zr-3.6Sn、Ti-5Al-3Sn-1.9Zr和Ti-5Al-2.5Sn合金的失重率为2.61%、2.83%和3.10%。σ_(UTS)、ɛ_(f)和耐热盐腐蚀结果表明,新设计合金Ti-5Al-4Zr-3.6Sn是一种有实际应用潜力的钛合金材料。
