Endoscopic characteristics of small intestinal malignant tumors observed by balloon-assisted enteroscopy

BACKGROUND Capsule endoscopy and balloon-assisted enteroscopy(BAE) enable visualization of rare small bowel conditions such as small intestinal malignant ***,details of the endoscopic characteristics of small intestinal malignant tumors are still *** To elucidate the endoscopic characteristics of small intestinal malignant *** From March 2005 to February 2017,1329 BAE procedures were performed at Keio University Hospital. Of these procedures,malignant tumors were classified into three groups,Group 1: epithelial tumors including primary small intestinal cancer,metastatic small intestinal cancer,and direct small intestinal invasion by an adjacent organ cancer; Group 2: small intestinal malignant lymphoma; and Group 3,small intestinal gastrointestinal stromal tumors. We systematically collected clinical and endoscopic data from patients' medical records to determine the endoscopic characteristics for each *** The number of patients in each group was 16(Group 1),23(Group 2),and 6(Group 3),and the percentage of solitary tumors was 100%,43.5%,and 100%,respectively(P < 0.001). Patients' clinical background parameters including age,symptoms,and laboratory data were not significantly different between the groups. Seventy-five percent of epithelial tumors(Group 1) were located in the upper small intestine(duodenum and ileum),and approximately 70% of gastrointestinal stromal tumors(Group 3) were located in the jejunum. Solitary protruding or mass-type tumors were not seen in malignant lymphoma(Group2)(P < 0.001). Stenosis was seen more often in Group 1,(68.8%,27.3%,and 0%;Group 1,2,and 3,respectively; P = 0.004). Enlarged white villi inside and/or surrounding the tumor were seen in 12.5%,54.5%,and 0% in Group 1,2,and 3,respectively(P = 0.001).CONCLUSION The differential diagnosis of small intestinal malignant tumors could be tentatively made based on BAE findings.

One-Dimensional Analysis of Supersonic Two-Stage HVOF Process

The one-dimensional calculation of the gas/particle flows of a supersonic two-stage high-velocity oxy-fuel(HVOF) thermal spray process was performed. The internal gas flow was solved by numerically integrating theequations of the quasi-one-dimensional flow including the effects of pipe friction and heat transfer. As for the supersonicjet flow, semi-empirical equations were used to obtain the gas velocity and temperature along the centerline. The velocity and temperature of the particle were obtained by an one-way coupling method. The material ofthe spray particle selected in this study is ultra high molecular weight polyethylene (UHMWPE). The temperaturedistributions in the spherical UHMWPE particles of 50 and 150 m accelerated and heated by the supersonic gasflow was clarified.

Laparoscopic retroperitoneal lymph node dissection versus open retroperitoneal lymph node dissection for testicular cancer:A comparison of clinical and perioperative outcomes

Objective:This study was performed to evaluate the clinical and perioperative outcomes of laparoscopic retroperitoneal lymph node dissection(L-RPLND)and open retroperitoneal lymph node dissection(O-RPLND)performed by one surgeon at a single ***:We evaluated 30 patients with stage IIA germ cell tumors who underwent retroperitoneal lymph node dissection(15 underwent L-RPLND and 15 underwent O-RPLND)at our institution between April 1,2010 and March 31,*** clinical parameters were compared between patients who underwent L-RPLND using the retroperitoneal approach and those who underwent O-RPLND using the transperitoneal *** were no significant differences in the background characteristics of the two groups except for the median follow-up duration(46 months for L-RPLND and 71 months for O-RPLND,p=0.02).Results:L-RPLND was associated with a shorter mean operative time(mean 222 min for L-RPLND vs.453 min for O-RPLND,p<0.001).There was significantly less blood loss during surgery in the L-RPLND group compared to the O-RPLND group(mean 165 mL for L-RPLND vs.403 mL for O-RPLND,p<0.001).Parameters related to postoperative recovery were significantly better for the L-RPLND group than for the O-RPLND *** were no differences in the histopathological characteristics between the two *** patients in either group exhibited disease ***:Patients who underwent L-RPLND had more rapid recovery,and shorter hospital stay compared to those who underwent O-RPLND;complications were comparable between the two groups.L-RPLND is an efficient procedure with the benefits of minimally invasive surgery.

Cooling Curve's Effect on Hardening Behavior of Steel

Quenching Oils are comprised mostly of mineral oil as major component, but mineral oil alone result in lower cooling performance. Therefore, various additives are formulated as required for each particular case. Cooling curves of mineral oil as determined by JIS method can be divided into three stages, vapor film stage, boiling stage and convection stage. And there are many types of additives used for quenching oil, one effective to shorten vapor film stage and another to shift boiling stage toward lower temperature side. As rapid quenching required temperature range for particular steel varies pending on its shape of CCT curve"1, it is possible to provide higher quench hardening performance using quenching oil formulated of adequate additives.

Characterization of a Microhollow Cathode Discharge Plasma in Helium or Air with Water Vapor

Microhollow cathode discharge （MHCD） plasmas were generated in gas mixtures containing water vapor at pressures of up to 100 kPa of He or 20 kPa of air. The cathode diameter was 1.0 mm with a length of 2.0 ram. The electricM characteristics showed an abnormal glow mode. Spectroscopic measurements were carried out to examine the plasma and radicals. An analysis of the spectral profile of Ha at 656.3 nm enabled a derivation of the electron densities, namely 2× 1014 cm-3 （at 10 kPa） and 6× 1014 cm-3 （at 4 kPa） for the helium and air atmospheres, respectively, in the negative glow region. By comparing the observed OH radical spectra with those calculated by the simulation code L[FBASE, the gas temperature was deduced to be 900 K for 4 kPa of He at a discharge current of 50 mA.

AIM:To evaluate the safety of unsedated transnasal small-caliber esophagogastroduodenoscopy(EGD)for elderly and critically ill bedridden patients. METHODS:One prospective randomized comparative study and one crossover comparative study between transnasal small-caliber EGD and transoral conventional EGD was done(Study 1).For the comparative study, we enrolled 240 elderly patients aged>65 years old. For the crossover analysis,we enrolled 30 bedridden patients with percutaneous endoscopic gastrostomy (PEG)(Study 2).We evaluated cardiopulmonary effects by measuring arterial oxygen saturation(SpO2)and calculating the rate-pressure product(RPP)(pulse rate×systolic blood pressure/100)at baseline,2 and 5 min after endoscopic intubation in Study *** assess the risk for endoscopy-related aspiration pneumoniaduring EGD,we also measured blood leukocyte counts and serum C-reactive protein(CRP)levels before and 3 d after EGD in Study 2. RESULTS:In Study 1,we observed significant decreases in SpO2 during conventional transoral EGD, but not during transnasal small-caliber EGD(0.24%vs -0.24%after 2 min,and 0.18%vs-0.29%after 5 min, P=0.034,P=0.044).Significant differences of the RPP were not found between conventional transoral and transnasal small-caliber *** Study 2,crossover analysis showed statistically significant increases of the RPP at 2 min after intubation and the end of endoscopy (26.8 and 34.6 vs 3.1 and 15.2,P=0.044,P=0.046), and decreases of SpO2(-0.8%vs-0.1%,P=0.042) during EGD with transoral conventional in comparison with transnasal small-caliber ***,for bedridden patients with PEG feeding,who were examined in the supine position,transoral conventional EGD more severely suppressed cardiopulmonary function than transnasal small-caliber *** were also significant increases in the markers of inflammation, blood leukocyte counts and serum CRP values,in bedridden patients after transoral conventional EGD,but not after transnasal small-caliber EGD perfo

We review our studies on how to identify the most appropriate models of diseases, and how to determine their parameters in a quantitative manner given a short time series of biomarkers, using intermittent androgen deprivation therapy of prostate cancer as an example. Recently, it has become possible to estimate the specific parameters of individual patients within a reasonable time by employing the information concerning other previous patients as a prior. We discuss the importance of using multiple mathematical methods simultaneously to achieve a solid diagnosis and prognosis in the future practice of personalized medicine.

Nitrogen Discharged from the Earth’s Interior Regions

The abundant nitrogen in the Earth’s atmosphere can be interpreted as the result of endothermic nuclear transmutation of carbon and oxygen atom pairs in (Ca, D) CO3 or CaCO3 aragonite lattice of Earth’s crust from the Archean era to the present time, by physical catalytic help of excited electrons e* generated by stick sliding due to plate tectonics and geoneutrinos ν by the radioactive decay of elements such as uranium and thorium in Earth’s mantle: through a nuclear attraction effect that is due to deuteron catalysis of nitrogen formation. The relationship between the critical temperature T and the critical pressure P for the nuclear transmutation is expressed as 7253 × e-0.014P, and the formation of nitrogen in the mantle is possible at temperatures ≥ 2510 K and pressure ≥ 58 GPa.
