AIM:To compare a first diagnostic procedure of transbronchial needle aspiration(TBNA)with selection of endoscopic ultrasound-guided fine-needle aspiration(EUS-FNA)or TBNA for mediastinal ***:Sixty-eight consecutive patients with mediastinal lymphadenopathy on computed tomography(CT),who required cytopathological diagnosis,were *** first 34 underwent a sequential approach in which TBNA was performed first,followed by EUS-FNA if TBNA was *** next 34 underwent a selective approach where either TBNA or EUS-FNA was selected as the first procedure based on the CT ***:The diagnostic yield of TBNA as the first diagnostic procedure in the sequential approach was 62%.In the selective approach,the diagnostic yield of the first procedure was 71%.There was no significant difference in the overall diagnostic yield,but there were significantly fewer combined procedures with the selective ***:Selecting either EUS-FNA or TBNA as the first diagnostic procedure achieved a comparable diagnostic yield with significantly fewer procedures than performing TBNA first in all patients.

AIM:To examine whether acupuncture can prevent prolonged postoperative ileus(PPOI)after intraperitoneal surgery for colon cancer. METHODS:Ninety patients were recruited from the Fudan University Cancer Hospital,Shanghai,China. After surgery,patients were randomized to receive acupuncture(once daily,starting on postoperative day 1, for up to six consecutive days)or usual *** was defined as an inability to pass flatus or have a bowel movement by 96 h after *** main outcomes were time to first flatus,time to first bowel movement, and *** outcomes were quality of life(QOL)measures,including pain, nausea,insomnia,abdominal distension/fullness,and sense of well-being. RESULTS:No significant differences in PPOI on day 4 (P=0.71)or QOL measures were found between the *** were also no group differences when the data were analyzed by examining those whose PPOI had resolved by day 5(P=0.69)or day 6(P= 0.88).No adverse events related to acupuncture were reported. CONCLUSION:Acupuncture did not prevent PPOI andwas not useful for treating PPOI once it had developed in this population.

During the follow up pathological fractures are common complications that can be accompanied by Implant failure and defect situations that are most challenging in their *** SUMMARY We present a case of a 52-year-old female suffering from a pathological fracture of the humeral shaft 10 yr after resection of a Leiomyosarcoma and postoperative radiotherapy. She developed implant failure after retrograde nailing and another failure after revision to double plate fixation. In a two-stage revision, the implants were removed and the huge segmental defect created after debridement was bridged by a compound osteosynthesis with nancy nails and bone cement for formation of the induced membrane. Due to the previous radiotherapy treatment,20 cm of the humeral shaft were declared devascularized but were left in situ as a scaffold. In the second stage, a vascularized fibula graft was used in combination with a double plate fixation and autologous spongiosa grafts for final *** This combinatory treatment approach led to a successful clinical outcome and can be considered in similar challenging cases.

为提升浮托安装的效率与成功率,提出一种连续变载模型试验方法,将压载水舱模块化,配合自研的压载调节系统,灵活调节压载分布,能快速连续地调整船舶重量和重心位置分布。同时利用HydroD软件和连续变载模型试验方法,进行波浪作用下的驳船运动响应研究。对比数值计算与试验结果,固有周期最大误差仅为3.3%。自研的压载调节系统可在2%误差内快速实现压载调节,调节效率较高。相较于传统压载调节方法,连续变载模型试验方法可真实模拟动态压载调节过程,实现全过程连续监测,为开展相关有变载需求的船舶模型试验提供参考。

As part of a collaborative study between the 3rd Institute of Oceanography, Xiamen, and the U-K. Universities of East Anglia and St. Andrews, a 4-day experiment was conducted on a ridge- and -runnel beach of north Norfolk on the North Sea coast- Detailed surveys were made of every low water of an area 10m by 30m and electromagnetic current meters were used to measure wave- induced currents over the ridge and in the runnel- The locations of bedform fields were noted, fluorescent sand was used to follow the sand movement and, at the end of the experiment, lacquer peels were taken of the top 0-2 m of a vertical section through the ridge- and- runnel-During the 4 days 0-75 m3 of sand per metre width of beach accreted in the runnel and a similar volume was eroded from the upper foreshore. The dominant shoreward transport, identified qualitatively by the movement of the fluorescent sand, suggests the ridge- and- runnel system migrated shorewards up the general beach slope of 1 by 2 - 3 m.
