An origin of the along-arc compositional variation in the Izu-Bonin arc system

The Izu-Bonin arc system is sediment-poor(~400 m thick with no accretionary prism)and,therefore,the influence of the altered oceanic crust(AOC)is most likely the source of the documented along-arc lava compositional variations,especially in Pb ***-Bonin arc lava geochemistry suggests an influx of subduction component from an Indian-type ***,samples drilled from the western Pacific geochemical reference site at Integrated Ocean Drilling Program Site 1149 implies subduction of a Pacific-type *** solve the apparent discrepancy of slab input versus arc output in this arc system,samples of the AOC were dredged from vertical fault scarps of the subducting Pacific Plate along a transect from 27.5°N to 34.50 *** range from tholeiitic to mildly alkalic mid-ocean ridge basalts as well as trachybasalts,basaltic trachyandesites,tephrites,and *** ratios also exhibit a range of values(87Sr/86Sr=0.70282-0.70673,143 Nd/144 Nd=0.512552-0.513174,206Pb/204 Pb=18.43-20.00,207 Pb/204 Pb=15.40-15.67,208Pb/204Pb=37.75-39.55).Our results suggest that there is a geochemical variation in the AOC that is neither completely due to seawater or hydrothermal alteration,nor to petrogenetic ***,this variation is the result of the Pacific-Izanagi Ridge system tapping into a heterogeneous,plume-polluted mantle source during the Mid-Cretaceous volcanic *** observed Pacific-type AOC is not responsible for the Indian-type Pb isotopic signature of Izu-Bonin arc *** leads us to propose an alternative scenario where the Izu-Bonin arc lava Indian-type Pb isotopic signature originates from slab-derived fluids interacting and adsorbing Pb from an Indian-type mantle wedge through zone-refining.

Improving hospital-based trauma care for road traffic injuries in Malawi

BACKGROUND: The mortality rate from road traffic injuries has increased in sub-Saharan Africa as the number of motor vehicles increase. This study examined the capacity of hospitals along Malawi's main north-south highway to provide emergency trauma ***: Structured interviews and checklists were used to evaluate the infrastructure, personnel, supplies, and equipment at all four of Malawi's central hospitals, ten district hospitals, and one mission hospital in 2014. Most of these facilities are along the main north-south highway that spans the ***: Between july 2013 and March 2014, more than 9 200 road traffic injuries(RTIs) and 100 RTI deaths were recorded by the participating hospitals. All of the hospitals reported staff shortages, especially during nights and weekends. Few clinicians had completed formal training in emergency trauma management, and healthcare workers reported gaps in knowledge and skills, especially at district hospitals. Most central hospitals had access to the critical supplies and medications necessary for trauma care, but district hospitals lacked some of the supplies and equipment needed for diagnosis, treatment, and personal ***: The mortality and disability burden from road traffi c injuries in Malawi(and other low-income countries in sub-Saharan Africa) can be reduced by ensuring that every central and district hospital has a dedicated trauma unit with qualified staff who have completed primary trauma care courses and have access to the equipment necessary to save lives.

In a previous transcriptomic study of human bone marrow stromal cells(BMSCs,also known as bone marrow-derived^mesenchymal stem cells#,)/SFRP2 was highly over-represented in a subset of multipotent BMSCs(skeletal stem cells,SSCs),which recreate a bone/marrow organ in an in vivo ectopic bone formation *** modulate WNT signaling,which is essential to maintain skeletal homeostasis,but the specific role of SFRP2 in BMSCs/SSCs is ***,we evaluated Sfrp2 deficiency on BMSC/SSC function in models of skeletal organogenesis and *** skeleton of Sfrp2-def\c\ent(KO)mice is overtly normal;but their BMSCs/SSCs exhibit reduced colony-forming efficiency,reflecting low SSC self-renewal/***2 KO BMSCs/SSCs formed less trabecular bone than those from WT littermates in the ectopic bone formation ***,regeneration of a cortical drilled hole defect was dramatically impaired in Sfrp2 KO ***2-deficient BMSCs/SSCs exhibited poor in vitro osteogenic differentiation as measured by Runx2 and Osterix expression and calcium ***,activation of the Wnt co-receptor;Lrp6,and expression of Wnt target genes,Axin2,C-myc and Cyclin Dl,were reduced in Sfrp2-deficient BMSCs/*** of recombinant Sfrp2 restored most of these activities,suggesting that Sfrp2 acts as a Wnt *** demonstrate that Sfrp2 plays a role in self-renewal of SSCs and in the recruitment and differentiation of adult SSCs during bone ***2 is also a useful marker of BMSC/SSC multipotency,and a factor to potentially improve the quality of ex vivo expanded BMSC/SSC products.

A cadaveric model for transesophageal echocardiography transducer placement training: A pilot study

BACKGROUND: Transesophageal echocardiography(TEE) is used in the emergency department to guide resuscitation during cardiac arrest. Insertion of a TEE transducer requires manual skill and experience, yet in some residency programs cardiac arrest is uncommon, so some physicians may lack the means to acquire the manual skills to perform TEE in clinical practice. For other infrequently performed procedural skills, simulation models are used. However, there is currently no model that adequately simulates TEE transducer insertion. The aim of this study is to evaluate the feasibility and efficacy of using a cadaveric model to teach TEE transducer placement among novice ***: A convenience sample of emergency medicine residents was enrolled during a procedure education session using cadavers as tissue models. A pre-session assessment was used to determine prior knowledge and confidence regarding TEE manipulation. Participants subsequently attended a didactic and hands-on education session on TEE placement. All participants practised placing the TEE transducer until they were able to pass a standardized assessment of technical skill(SATS). After the educational session, participants completed a post-session assessment. RESULTS: Twenty-five residents participated in the training session. Mean assessment of knowledge improved from 6.2/10 to 8.7/10(95% confidence interval [CI] of knowledge difference 1.6–3.2, P<0.001) and confidence improved from 1.6/5 to 3.1/5(95% CI of confidence difference 1.1–2.0, P<0.001). There was no relationship between training level and the delta in knowledge or ***: In this pilot study, the use of a cadaveric model to teach TEE transducer placement methods among novice users is feasible and improves both TEE manipulation knowledge and confidence levels.
