Endoscopic papillectomy: Data of a prospective observational study

AIm:To investigate the clinical value of endoscopic papillectomy indicated by feasibility and safety of the procedure in various diseases of the papilla in a representative number of patients in a setting of daily clinical and endoscopic practice and care by means of a systematic prospective observational study. mETHODS:Through a defined time period, all consecutive patients with tumor-like lesions of the papilla, who were considered for papillectomy, were enrolled in this systematic bicenter prospective observational study, and subdivided into 4 groups according to endoscopic and endoscopic ultrasonography (EUS) findings as well as histopathological diagnosis:adenoma; carcinoma/ neuroendocrine tumor (NET)/lymphoma; papilla into which catheter can not be introduced; adenomyomatosis, respectively. Treatment results and outcome were characterized by R0 resection, complication, recurrence rates and tumor-free ***:Over a 7-year period, 58 patients underwent endoscopic papillectomy. main symptoms prompting to diagnostic measures were unclear abdominal pain in 50% and cholestasis with and without pain in 44%. Overall, 54/58 patients [inclusion rate, 93.1%; sex ratio, males/females = 25/29 (1:1.16); mean age, 65 (range, 22-88) years] were enrolled in the study. Prior to papillectomy, EUS was performed in 79.6% (n = 43/54). Group 1 (adenoma, n = 24/54; 44.4%):91.6% (n = 22/24) with R0 resection; tumor-free survival after a mean of 18.5 mo, 86.4% (n = 19/22); recurrence, 13.6% (n = 3/22); minor complications, 12.5% (n = 3/24). Group 2 (carcinoma/NET/lymphoma, n = 18/54; 33.3%):75.0% (n = 10/18) with R0 resection; tumor-free survival after a mean of 18.5 (range, 1-84) mo, 88.9% (n = 8/9); recurrence, 11.1% (n = 1/9). Group 3 (adenomyomatosis, n = 4/54; 7.4%). Group 4 (primarily no introducible catheter into the papilla, n = 8; 14.8%). Currently,there is no effective treatment for amyotrophic lateral sclerosis(ALS),despite the limited efficacy of riluzole[1]and edaravone[2].SOD1(coding for the Cu/Zn superoxide dismutase)is the second most frequent genetic cause for ALS only after C9orf72 in patients with European ancestry while being the most frequent in Asian ALS populations[3],multiple therapeutic approaches have targeted SOD1-related ALS,including the antisense oligonucleotide tofersen with promising results in a recent phase I/II trial[4].

An explainable AI framework for robust and transparent data-driven wind turbine power curve models

In recent years,increasingly complex machine learning methods have become state-of-the-art in modelling wind turbine power curves based on operational *** these methods often exhibit superior performance on test sets,they face criticism due to a perceived lack of transparency and concerns about their robustness in dynamic,non-stationary environments encountered by wind *** this work,we address these issues and present a framework that leverages explainable artificial intelligence methods to gain systematic insights into data-driven power curve *** its core,we propose a metric to quantify how well a learned model strategy aligns with the underlying physical principles of the *** novel tool enables model validation beyond the conventional error metrics in an automated *** demonstrate,for instance,its capacity as an indicator for model generalization even when limited data is ***,it facilitates understanding how decisions made during the machine learning development process,such as data selection,pre-processing,or training parameters,affect learned *** a result,we obtain physically more reasonable models,a prerequisite not only for robustness but also for meaningful insights into turbine operation by domain *** latter,we illustrate in the context of wind turbine performance *** summary,the framework aims to guide researchers and practitioners alike toward a more informed selection and utilization of data-driven wind turbine power curve models.

Wear dependent virtual flow rate sensor for progressing cavity pumps with deformable stator

This contribution presents a novel wear dependent virtual flow rate sensor for single stage single lobe progressing cavity pumps. We study the wear-induced material loss of the pump components and the impact of this material loss on the volumetric efficiency. The results are combined with an established backflow model to implement a backflow calculation procedure that is adaptive to wear. We use a laboratory test setup with a highly abrasive fluid and operate a pump from new to worn condition to validate our approach. The obtained measurement data show that the presented virtual sensor is capable of calculating the flow rate of a pump being subject to wear during its regular operation.

Analyzing voting power in decentralized governance:Who controls DAOs?

We empirically study the state of three prominent DAO governance systems on the Ethereum blockchain:Compound,Uniswap and Ethereum name service(ENS).In particular,we examine how the voting power is distributed in these *** a comprehensive dataset of all governance token holders,delegates,propos-als,and votes,we analyze who holds the voting power and how this power is being used to influence governance *** we reveal that the majority of voting power is concentrated in the hands of a small number of addresses,we rarely observe these powerful entities overturning a vote by choosing a different outcome than that of the overall community and less influential voters.

Conception of a new 4-quadrant hydrogen compressed air energy storage power plant with an integrated electrolyzer

A hydrogen compressed air energy storage power plant with an integrated electrolyzer is ideal for large-scale,long-term energy storage because of the emission-free operation and the possibility to offer multiple ancillary services on the German energy *** paper defines analyzes such a storage concept and conducts an extensive comparison with four additional storage concepts based on various *** results show that the combination of storing compressed air and hydrogen offers a higher efficiency than storing only hydrogen and lower specific investment costs than storing only compressed *** result is confirmed with analysis of the optimal sizing of each power plant component for simultaneous participation on multiple energy markets with a linear optimization dispatch *** hydrogen compressed air energy storage(HCAES)power plant can utilize more revenue possibilities than a hydrogen energy storage because of the higher round-trip efficiency and the combination of the air compressor and the integrated electrolyzer during charging *** integration of the electrolyzer,however,offers a couple of challenges itself because of the highly flexible operation mode.A new concept for the controllable 24-pulse diode-thyristor rectifier of the electrolyzer is presented,that uses mostly common components while offering little to no grid harmonics and a long *** flexible integrated electrolyzer allows for the 4-quadrant operation of the storage power plant.

CO_(2)capture costs of chemical looping combustion of biomass:A comparison of natural and synthetic oxygen carrier

Chemical looping combustion has the potential to be an efficient and low-cost technology capable of contributing to the reduction of the atmospheric concentration of CO_(2) in order to reach the 1.5/2°C goal and mitigate climate *** this process,a metal oxide is used as oxygen carrier in a dual fluidized bed to generate clean CO_(2) via combustion of *** commonly,natural ores or synthetic materials are used as oxygen carrier whereas both must meet special requirements for the conversion of solid *** oxygen carriers are characterized by higher reactivity at the expense of higher costs versus the lower-cost natural *** determine the viability of both possibilities,a techno-economic comparison of a synthetic material based on manganese,iron,and copper to the natural ore ilmenite was *** synthetic oxygen carrier was characterized and tested in a pilot plant,where high combustion efficiencies up to 98.4%and carbon capture rates up to 98.5%were *** techno-economic assessment resulted in CO_(2) capture costs of 75 and 40€/tCO_(2) for the synthetic and natural ore route respectively,whereas a sensitivity analysis showed the high impact of production costs and attrition rates of the synthetic *** synthetic oxygen carrier could break even with the natural ore in case of lower production costs and attrition rates,which could be reached by adapting the production process and recycling *** comparison to state-of-the-art technologies,it is demonstrated that both routes are viable and the capture cost of CO_(2) could be reduced by implementing the chemical looping combustion technology.

The improvement in the efficiency of inverted perovskite solar cells(PSCs)is significantly limited by undesirable contact at the NiO_(x)/perovskite *** this study,a novel microstructure-control technology is proposed for fabrication of porous NiO_(x)films using Pluronic P123 as the structure-directing agent and acetylacetone(AcAc)as the coordination *** synthesized porous NiO_(x)films enhanced the hole extraction efficiency and reduced recombination defects at the NiO_(x)/perovskite ***,without any modification,the power conversion efficiency(PCE)of the PSC with mAPbl_(3)as the absorber layer improved from 16.50%to 19.08%.moreover,the PCE of the device composed of perovskite Cs0.05(mA_(0.15)FA_(0.85))_(0.95)Pb(I_(0.85)Br_(0.15))_(3)improved from 17.49%to 21.42%.Furthermore,the application of the fabricated porous NiO_(x)on fluorine-doped tin oxide(FTO)substrates enabled the fabrication of large-area PSCs(1.2 cm^(2))with a PCE of 19.63%.This study provides a novel strategy for improving the contact at the NiO_(x)/perovskite interface for the fabrication of high-performance large-area perovskite solar cells.

Hyperparathyroidism(HPT)is a condition in which one or more parathyroid glands produce increased levels of parathyroid hormone(PTH),causing disturbances in calcium *** commonly HPT presents with asymptomatic hypercalcemia but the clinical spectrum may include disturbances reflecting the combined effects of increased PTH secretion and *** tumors are rare,benign,tumor-like bone lesions,occurring in 1.5%to 4.5%of patients with HPT,as a complication of an uncontrolled disease pathway,and are nowadays rarely seen in clinical *** tumor can appear either as a solitary or multifocal lesion and usually presents as an asymptomatic swelling or a painful exophytic ***,it can cause a pathological fracture or skeletal pain and be radiologically described as a lytic bone *** diagnosis of a brown tumor in HPT is typically confirmed by assessing the levels of serum calcium,phosphorus,and *** when present,brown tumor is quite pathognomonic for HPT,the histologic finding often suggests a giant cell tumor,while clinical presentation might suggest other more frequent pathologies such as metastatic *** of brown tumors frequently focuses on managing the underlying HPT,which can often lead to regression and resolution of the lesion,without the need for surgical ***,in refractory cases or when dealing with large symptomatic lesions,surgical treatment may be necessary.
