Molecular pathogenesis and clinical conse-quences of iron overload in liver cirrhosis

BACKGROUND: The liver, as the main iron storage compart-ment and the place of hepcidin synthesis, is the central organ involved in maintaining iron homeostasis in the body. Exces-sive accumulation of iron is an important risk factor in liver disease progression to cirrhosis and hepatocellular carcinoma. Here, we review the literature on the molecular pathogenesis of iron overload and its clinical consequences in chronic liver diseases. DATA SOURCES: PubMed was searched for English-language articles on molecular genesis of primary and secondary iron overload, as well as on their association with liver disease pro-gression. We have also included literature on adjuvant thera-peutic interventions aiming to alleviate detrimental effects of excessive body iron load in liver cirrhosis. RESULTS: Excess of free, unbound iron induces oxidative stress, increases cell sensitivity to other detrimental factors, and can directly affect cellular signaling pathways, resulting in accelerated liver disease progression. Diagnosis of liver cirrhosis is, in turn, often associated with the identiifcation of a pathological accumulation of iron, even in the absence of genetic background of hereditary hemochromatosis. Iron depletion and adjuvant therapy with antioxidants are shown to cause signiifcant improvement of liver functions in patients with iron overload. Phlebotomy can have beneifcial effects on liver histology in patients with excessive iron accumulation combined with compensated liver cirrhosis of different etiology. CONCLUSION: Excessive accumulation of body iron in liver cirrhosis is an important predictor of liver failure and avail-able data suggest that it can be considered as target for adju-vant therapy in this condition.

Liver steatosis correlates with iron overload but not with HFE gene mutations in chronic hepatitis C

BACKGROUND: Liver steatosis and iron overload, which are frequently observed in chronic hepatitis C (CHC), may contribute to the progression of liver injury. This study aimed to evaluate the correlation between liver steatosis and iron overload in Polish patients with CHC compared to non- alcoholic fatty liver disease (NAFLD) and HFE-hereditary hemochromatosis (HH) patients. METHODS: A total of 191 CHC patients were compared with 67 NAFLD and 21 HH patients. Liver function tests, serum markers of iron metabolism, cholesterol and triglycerides were assayed. The inflammatory activity, fibrosis, iron deposits and steatosis stages were assessed in liver specimens. HFE gene polymorphisms were investigated by PCR-RFLP. RESULTS: Liver steatosis was associated with obesity and diabetes mellitus. This disease was confirmed in 76/174 (44%) CHC patients, most of whom were infected with genotype 1. The average grade of steatosis was higher in NAFLD patients. CHC patients had significantly higher iron concentrations and transferrin saturations than NAFLD patients. Compared with CHC patients, HH patients had higher values of serum iron parameters and more intensive hepatocyte iron deposits without differences in the prevalence and intensity of liver steatosis. In the CHC group, lipids accumulation in hepatocytes was significantly associated with the presence of serummarkers of iron overload. No correlation between the HFE gene polymorphism and liver steatosis in CHC patients was found. CONCLUSIONS: Liver steatosis was diagnosed in nearly half of CHC patients, most of whom were infected with genotype 1. The intensity of steatosis was lower in CHC patients than that in NAFLD patients because of a less frequent diagnosis of metabolic syndrome. Only in CHC patients were biochemical markers of iron accumulation positively correlated with liver steatosis; these findings were independent of HFE gene mutations.

Since the discovery of penicillin by Fleming in 1928, the knowledge of the antibiotics’ spectrum and mechanism of action has been steadily increasing. Antibiotics became an effective tool in the fight against many pathogens, changing the prognosis of outcome for many serious diseases. It is already known that antibiotics not only have the antibacterial activity, but many of them—e.g. macrolides, sulphonamides and tetracyclines—have immunomodulating effect, affecting functions of lymphocytes, macrophages and costimulatory molecules, macrophage migration and phagocytosis, as well as proinflammatory cytokine secretion. The expanding knowledge of the effects of antibiotics on the immune system has brought with it new applications of antibiotics in organ transplantation, invasive cardiology and treatment of autoimmune diseases such as rheumatoid arthritis or asthma.

In this paper, using Sadovskii’s fixed point theorem and properties of the measure of noncompactness, we obtain asymptotic stability results for solutions of nonlinear differential equation with variable delay. Results presented in this paper extend some previous results due to Burton?[1], Becker and Burton?[2], Ardjouni and Djoudi [3], and Jin and Luo?[4].

In this paper we prove Leray-Schauder and Furi-Pera types fixed point theorems for a class of multi-valued mappings with weakly sequentially closed graph. Our results improve and extend previous results for weakly sequentially closed maps and are very important in applications, mainly for the investigating of boundary value problems on noncompact intervals.

加深对来华留学生对汉语学习动机的了解,能够提高汉语教学的质量。分析了不同留学生对汉语学习动机后,教师便可选择既合适又具有针对性的教学目标、方法以及教材等,从而能够更有效地进行当下的汉语教学活动。由给天津大学多个教学系不同教学班的留学生进行问卷调查,本研究分析并总结留学生汉语学习动机的主要特点。此外,根据所分析出的特点,本研究提出多方面的培养及激发汉语学习动机的教学建议。从而,本课题为了解留学生学习汉语的动机以及进行汉语教学任务提供了参考和助益。本文的内容分为五大部分:引言、理论基础及文献研究综述、汉语学习动机调查、培养及激发留学生汉语学习动机的教学建议以及结论。第一章,引言,介绍本文研究的缘由、目的、价值以及基本框架。第二章,理论基础及文献研究综述,介绍国内外主要的基础理论及相关研究的结果与贡献。第三章,汉语学习动机调查,介绍本文调查的概况,并且从不同的角度对调查结果进行分析与讨论。本研究发现工具型动机及内部动机为本调查对象最强的汉语学习动机。除此之外,本研究还发现不同汉语学习动机类型以及对待汉语、课堂内外学习不同环节的态度与个人差异因素各个互动的关联,并对此进行了一定的分析。第四章,培养及激发留学生汉语学习动机的教学建议,提出从语言、学习者、学习环境三大方面的教学建议。结论,为本文研究的总结,同时提出对将来相关研究的补充与设想。

＜正＞Synchronous scanning fluorescence spectroscopy techniques were applied to characterization of fluorescence of selected beers. The spectra were recorded using back face and right angle geometries. The spectra of all beers studied reveal presence of similar fluorescent components. The fluorescence spectra of diluted beers exhibit a relatively intense short-wavelength emission band ascribed to amino-acids （tyrosine, tryptophan and phenylalanine）, while for neat beers broad emission bands in the long-wavelength region are observed, which may originate from vitamins of the B group. Classification of beers based on their synchronous fluorescence spectra was performed using linear discriminant analysis. Good discrimination between beers was obtained. The results presented demonstrate the capability of the fluorescence techniques to characterise beers and differentiate between their brands and varieties.

目的评估术前经血糖水平校正的最大标准吸收值(GC-SUVmax)在预测早期非小细胞肺癌（NSCLC）患者手术后预后中的价值。方法该回顾性研究的患者入组标准是:组织学证实是TNM分期为I期的NSCLC患者,术前行18F-FDG PET/CT显像,手术切除病灶并在门诊随访。排除标准是:患者患有除NSCLC以外的其他恶性肿瘤、糖尿病或使用过新辅助疗法。随访期是术后5年。结果 2006年至2008年间共33例患者（男16例,女17例）符合入组标准,其SUVmax和GC-SUVmax有显著相关性（ρ=0.97）。5年总体生存率是70%（95%CI:56%87%）。5年内死亡患者的GC-SUVmax[10.6（8.3,14.4）]显著高于5年内未死亡患者的GC-SUVmax[6.4（3.0,9.8）,P=0.04]。两者SUVmax的差异也有统计学意义[10.4（8.0,12.9）和6.6（3.0,8.8）,P=0.047]。第5年GC-SUVmax的ROC曲线下面积是0.70（95%CI:0.50～0.90）,而第5年SUVmax的ROC曲线下面积是0.71（95%CI:0.51～0.91）,P=0.75。结论 NSCLC患者术前肿瘤对FDG的摄取可预测术后的存活情况。GC-SUVmax作为SUVmax的校正值,可以更好地避免血糖对肿瘤FDG摄取的影响。但是,该研究中即便存在这种影响,其效果也很小。
