effect of a region-wide incorporation of an algorithm based on the 2012 international consensus guideline on the practice pattern for the management of pancreatic cystic neoplasms in an integrated health system

AIM To examine the practice pattern in Kaiser Permanente Southern California(KPSC), i.e., gastroenterology(GI)/surgery referrals and endoscopic ultrasound(eUS), for pancreatic cystic neoplasms(PCNs) after the regionwide dissemination of the PCN management *** Retrospective review was performed; patients with PCN diagnosis given between April 2012 and April 2015(18 mo before and after the publication of the algorithm) in KPSC(integrated health system with 15 hospitals and 202 medical offices in Southern California) were ***2558(1157 pre-and 1401 post-algorithm) received a new diagnosis of PCN in the study period. There was no difference in the mean cyst size(pre-19.1 mm vs post-18.5 mm, P = 0.119). A smaller percentage of PCNs resulted in eUS after the implementation of the algorithm(pre-45.5% vs post-34.8%, P er proportion of patients were referred for GI(pre-65.2% vs post-53.3%, P ery consultations(pre-24.8% vs post-16%, P er the implementation. There was no significant change in operations for PCNs. Cost of diagnostic care was reduced after the implementation by 24%, 18%, and 36% for eUS, GI, and surgery consultations, respectively, with total cost saving of 24%.CONCLUSION In the current healthcare climate, there is increased need to optimize resource utilization. Dissemination of an algorithm for PCN management in an integrated health system resulted in fewer eUS and GI/surgery referrals, likely by aiding the physicians ordering imaging studies in the decision making for the management of PCNs. This translated to cost saving of 24%, 18%, and 36% for eUS, GI, and surgical consultations, respectively, with total diagnostic cost saving of 24%.

Differential effects of Alternative Glycoforms of IgG on Human Monocytes and Macrophages: Sialylated IgG Induces Novel expression Signatures of Cell Surface Markers, Cytokines, and Chemokines

The effector functions elicited by the fragment crystallizable (Fc) region of immunoglobulin G (IgG) antibodies are subject to variation by the presence of terminal sialic acid (Sia) residues at asparagine-297 (Asn-297). We have previously shown that the sialic acid-containing (Sia+) fraction of intravenous immune globulin (IVIG) influences cell surface marker expression and cytokine/ chemokine secretion during the differentiation and maturation of human dendritic cells (DC). The present study examined the effects of Sia+ IgG on human peripheral blood mononuclear cell (PBMC)-derived monocyte and macrophage surface marker expression and cytokine/chemokine secretion. Sia+ IgG induced increased expression of CD80 and dendritic cell immunoreceptor (DCIR) on monocytes, whereas the expression of HLA-DR was decreased. In addition, the production of IL-6, TNFα, IL-1β, and CXCL1 by monocytes was profoundly increased by treatment with Sia+ IgG. Sia+ IgG also increased the expression of cell surface markers associated with macrophage polarization (e.g. CD40 and CD206) on monocytes. In macrophage-colony stimulating factor (MCSF) generated macrophages, Sia+ IgG induced increased production of numerous cytokines/ chemokines including IL-6, TNFα, CXCL1, and IL-10, and the expression of the macrophage surface marker CD163. Our data extended prior observations of Sia+ IgG on DC function and showed that Sia+ IgG was able to differentially modulate multiple pathways in monocytes and macrophages. Our data indicate that the Sia+ fraction of IVIG possesses the ability to influence inflammatory processes in multiple immune cell types and induces novel signatures in cell surface marker expression and cytokine/chemokine production.

Food and Nutritional Insecurity before and during the COVID-19 Pandemic in Households of Women Belonging to a Social Program

Introduction: According to the National Health and Nutrition Survey, the Mexican households in 2018 had food insecurity at 55.5% while in 2012 it was 70%. Food insecurity is a global health problem and now with the COVID-19 pandemic, it has increased. Objective: To compare the levels of food and nutritional insecurity in women’s households from two Social Impulse Centers of the Secretary for Human Social Development (SEDESHU) before and during the COVID-19 pandemic. Methods: A comparative and cross-sectional study was carried out in 41 women participating in a nutritional intervention program in two social promotion centers. Food security was measured with the Latin American and Caribbean Food Security Scale (ELCSA), comparing it in a period from January to April 2020: before and during the health contingency. Socio-economic and demographic variables were also measured, as well as access to water during the COVID-19 pandemic. Results: Food insecurity levels during the COVID-19 contingency were found to have increased by more than 60% (from 31.7% to 93%);the main risk factors associated with unemployment and access to healthy food. The COVID-19 contingency reflects impacts on households, but because women participated in the nutritional intervention program, it could influence by observing that, at least, the use of food was adequate since apparently food waste was minimal. e="font-famil

Anti-CD163-dexamethasone conjugate inhibits the acute phase response to lipopolysaccharide in rats

AIM: To study the effect of a new anti-CD163-dexamethasone conjugate targeting activated macrophages on the hepatic acute phase response in rats. MeTHODS: Wistar rats were injected intravenous with either the CD163 targeted dexamethasone-conjugate(0.02 mg/kg) or free dexamethasone(0.02 or 1 mg/kg) 24 h prior to lipopolysaccharide(LPS)(2.5 mg/kg intraperitoneal). We measured plasma concentrations of tumour necrosis factor-a(TNF-a) and interleukin 6(IL-6) 2 h post-LPS and liver m RNAs and serum concentrations of the rat acute phase protein a-2-macroglobulin(a-2-M) 24 h after LPS. Also, plasma concentrations of alanine aminotransferase and bilirubin were measured at termination of the study. Spleen weight served as an indicator of systemic steroid ***: The conjugate halved the a-2-M liver m RNA(3.3 ± 0.6 vs 6.8 ± 1.1, P erum protein(201 ± 48 μg/mL vs 389 ± 67 μg/mL, P = 0.04) after LPS compared to low dose dexamethasone treated animals, while none of the free dexamethasone doses had an effect on liver m RNA or serum levels of a-2-M. Also, the conjugate reduced TNF-a(7208 ± 1977 pg/mL vs 21583 ± 7117 pg/mL, P = 0.03) and IL-6(15685 ± 3779 pg/mL vs 25715 ± 4036 pg/mL, P = 0.03) compared to the low dose dexamethasone. The high dose dexamethasone dose decreased the spleen weight(421 ± 11 mg vs 465 ± 12 mg, P ed to controls, an effect not seen in any other ***: Low-dose anti-CD163-dexamethasone conjugate effectively decreased the hepatic acute phase response to LPS. This indicates an anti-inflammatory potential of the conjugate in vivo.

Deep understanding of the impact of photon polarization on pair production is essential for the efficient generation of laser-driven polarized positron beams and demands a complete description of polarization effects in strong-field QeD *** fully polarization-resolved Monte Carlo simulations,we investigate correlated photon and electron(positron)polarization effects in the multiphoton Breit–Wheeler pair production process during the interaction of an ultrarelativistic electron beam with a counterpropagating elliptically polarized laser *** show that the polarization of e−e+pairs is degraded by 35%when the polarization of the intermediate photon is resolved,accompanied by an∼13%decrease in the pair ***,in this case,the polarization direction of energetic positrons at small deflection angles can even be reversed when high-energy photons with polarization parallel to the laser electric field are involved.
