BACKGROUND Cetuximab in combination with oral fluoropyrimidine(FP)remains controversial in metastatic colorectal cancer(mCRC).In view of the regional variation in the tolerability of FP,we conducted a retrospective analysis to compare oral FP with infusional FP in combination with cetuximab in Chinese *** To compare the efficacy and safety profile of cetuximab in combination with oral FP and infusional FP in Chinese population in the real-world *** A retrospective cohort study was done to analyse consecutive patients with Kras wild-type mCRC who received first-line treatment with cetuximab and FP-based chemotherapy in our unit from January 2010 to December ***-five eligible patients were *** median follow-up of our cohort was 65.0 *** The median progression-free survival(mPFS)and median overall survival(moS)of the entire cohort were 9.66 mo(95%CI:7.72–12.5)and 25.8 mo(95%CI:18.7–35.6),*** oral FP and infusional FP,there was no statistical significant difference in the mPFS[9.79 mo(95%CI:7.49–12.7)vs 9.63 mo(95%CI:6.34–13.4);P=0.72]and moS[25.8 mo(95%CI:15.2–35.6)vs 26.3 mo(95%CI:18.7–41.2);P=0.63].Grade 3 or above adverse events were reported in 28.4%of patients,being similar with oral and infusional FP,and included 10.5%of neutropenia and 2.1%of diarrhoea *** The current analysis demonstrates comparable efficacy and safety profiles of cetuximab in combination with oral and infusional FP in Chinese *** results expand treatment options for Chinese patients and invite revision of existing treatment guidelines to incorporate oral FP-based chemotherapy plus cetuximab.

Bayesian meta-analysis of regional biomass factors for Quercus mongolica forests in South Korea

Indirect approaches to estimation of biomass factors are often applied to measure carbon flux in the forestry sector. An assumption underlying a country-level carbon stock estimate is the representativeness of these factors. Although intensive studies have been conducted to quantify biomass factors, each study typically covers a limited geographic area. The goal of this study was to employ a meta-analysis approach to develop regional bio- mass factors for Quercus mongolica forests in South Korea. The biomass factors of interest were biomass conversion and expansion factor （BCEF）, biomass expansion factor （BEF） and root-to-shoot ratio （RSR）. Our objectives were to select probability density functions （PDFs） that best fitted the three biomass factors and to quantify their means and uncertainties. A total of 12 scientific publications were selected as data sources based on a set of criteria. Fromthese publications we chose 52 study sites spread out across South Korea. The statistical model for the meta- analysis was a multilevel model with publication （data source） as the nesting factor specified under the Bayesian framework. Gamma, Log-normal and Weibull PDFs were evaluated. The Log-normal PDF yielded the best quanti- tative and qualitative fit for the three biomass factors. However, a poor fit of the PDF to the long right tail of observed BEF and RSR distributions was apparent. The median posterior estimates for means and 95 % credible intervals for BCEF, BEF and RSR across all 12 publica- tions were 1.016 （0.800-1.299）, 1.414 （1.304-1.560） and 0.260 （0.200-0.335）, respectively. The Log-normal PDF proved useful for estimating carbon stock of Q. mongolica forests on a regional scale and for uncertainty analysis based on monte Carlo simulation.

Phantom limb is a disabling neuropsychiatric condition among amputees resulting in pain and disturbance that impact their functions,quality of life,and *** pharmacological approaches appeared to be ineffec-tive,the emergence and integration of X-reality,including virtual reality,augmented reality,and mixed reality,might elevate the effectiveness of mirror therapy in managing phantom *** objective of this study is to review X-reality for managing phantom pain.A systematic search was conducted on PubMed,Scopus,Web of Science,PsycINFO,Embase,and ***(n=16)studies containing 66 lower-limb and 53 upper-limb amputees were included for the review over the thematic framework of amputee characteristics and intervention designs,while thirteen(n=13)studies were further proceeded for the *** found eleven studies on virtual reality(n=11),four studies on marker-based augmented reality(n=4)and one study on mixed reality(n=1)with a total of 40 game/task themes involving,motor skills,motor control,and ***,all these interventions adopted the movement representation strategies with different ***,the X-reality interventions reduced the pain level of the amputees(mean difference:-2.30,95%CI,-3.38 to-1.22),especially the virtual reality subgroup(mean difference:-2.83,95%CI,-4.43 to-1.22).However,there were substantial heterogeneity and partially explained by the subgroup analysis on publication *** strength of evidence was limited by case reports and case series in this review.

Differing coping mechanisms, stress level and anorectal physiology in patients with functional constipation

AIM: To investigate coping mechanisms, constipation symptoms and anorectal physiology in 80 constipated subjects and 18 ***: Constipation was diagnosed by Rome Ⅱ *** ability and anxiety/depression were assessed by validated questionnaires. Transit time and balloon distension test were ***: 34.5% patients were classified as slow transit type of constipation. The total colonic transit time (56 h vs 10 h, P＜0.0001) and rectal sensation including urge sensation (79 mL vs 63 mL, P = 0.019) and maximum tolerable volume (110 mL vs95 mL, P = 0.03) differed in patients and controls. Constipated subjects had significantly higher anxiety and depression scores and lower SF-36 scores in all categories. They also demonstrated higher scores of'monitoring' coping strategy (14+6 vs9+3, P = 0.001),which correlated with the rectal distension sensation (P = 0.005), urge sensation (P=0.002), and maximum tolerable volume (P = 0.035). The less use of blunting strategy predicted slow transit constipation in both univariate (P = 0.01) and multivariate analysis (P = 0.03).CONCLUSION: Defective or ineffective use of coping strategies may be an important etiology in functional constipation and subsequently reflected in abnormal anorectal physiology.

In an effort to understand the molecular events contributing to the cytotoxicity activity of resveratrol(RSV), we investigated its effects on human lung adenocarcinoma epithelial cell line A549 at different concentrations. Cellular nucleoside metabolic profiling was determined by an established liquid chromatography-mass spectrometry method in A549 cells. RSV resulted in significant decreases and imbalances of deoxyribonucleoside triphosphates(d NTPs) pools suppressing subsequent DNA synthesis. Meanwhile, RSV at high concentration caused significant cell cycle arrest at S phase, in which cells required the highest dNTPs supply than other phases for DNA replication. The inhibition of DNA synthesis thus blocked subsequent progression through S phase in A549 cells,which may partly contribute to the cytotoxicity effect of RSV. However, hydroxyurea(HU), an inhibitor of RNR activity, caused similar dNTPs perturbation but no S phase arrest, finally no cytotoxicity effect. Therefore, we believed that the dual effect of high concentration RSV, including S phase arrest and DNA synthesis inhibition, was required for its cytotoxicity effect on A549 cells. In summary,our results provided important clues to the molecular basis for the anticancer effect of RSV on epithelial cells.

Kinematic Evolution of the Nyakong-Manyi Shear Zone(Adamawa,Cameroon): Constraints from Field Observations and Microstructures,and Implication for Metamorphic P-T-t Estimation

The Nyakong-Manyi Shear Zone(NMSZ) is a NE-SW elongated corridor found to the northwest of the Foumban-Bankim Shear Zone(FBSZ) along the Central Cameroon Shear Zone. Controversial chronology models has been proposed for the kinematic evolution of the sinistral and dextral shear phases in the Tikar Plain, thus in the FBSZ;early dextral and late sinistral shear phases for some authors and early sinistral and late dextral shear for others. moreover, the NMSZ kinematic evolution implication on the mylonitization P-T-t path in the area seem to be problematic and the present paper aim is to clear enough those problems;since this shear zone is the main mylonitic corridor that registered the left and right lateral movement in this area. The NMSZ comprises amphibolites, protomylonites, strict sensus mylonites(garnet-kyanite-sillimanite mylonite and garnet-pyroxene mylonite), ultramylonites kyanite-sillimanite and garnet-kyanite-sillimanite gneiss. Field structures testify that the investigated area recorded three deformation phases:(i) the D1deformation phase which is marked by NW-SE to N-S trending S1metamorphic foliation with low to moderate dips(15°–45°) that was transposed during the D2phase, is responsible for a regional metamorphism whose mineral paragenesis is garnet-kyanite-sillimanite;(ii) the early sinistral NNE-SSW to NE-SW shear phase D2marked by S2metamorphic and mylonitic foliations;responsible for, L2stretching mineral lineation, F2fold axes and B2boudins structures;(iii) the late dextral NE-SW shear phase D3, characterized by F3folds, B3boudins and ductile dextral C3shear planes. Mineral paragenesis garnet + kyanite + sillimanite and microstructures within gneiss testify that this rock underwent high grade regional metamorphism whose peak conditions are estimated at 11.5–13.5 kbar/850–900 ℃. Intravenous fluid selection rationales in acute clinical management

BACKGROUND: Intravenous fluid(IVF) is commonly used in acute clinical management. This study aimed to review the choice and primary considerations in IVF prescriptions and to evaluate the adequacy of guidelines and trainings on it in the New Territories West Cluster(NTWC) of Hong ***: This is a descriptive study based on data collected from an online survey. Data were processed by SPSS for statistical analysis. This study focused on a general description and doctor-nurse between group comparison. Participants were asked the choice of IVF for nine acute clinical scenarios and provide reason. A 1–10 scale was used to assess the sufficiency of guideline, training and information, and time for revision on IVF ***: 0.9% sodium chloride was the most familiar IVF(36%), followed by 5% Dextrose solution(26%). In the nine scenarios, the most chosen IVF was 0.9% sodium chloride(37%–61%). There was significant difference in the choice of IVF between doctors and nurses in 7 cases. The second most chosen IVF for doctors was Plasma-Lyte A while that for nurses was Gelofusine. Departmental practice was the most chosen reason to account for the prescription. The adequacy of guideline, information and training, and time for revision was rated 5. Doctors had significantly more time at work than nurses to update knowledge in IVF prescription(5.41 versus 4.57).CONCLUSION: 0.9% sodium chloride was mostly chosen. The choice of IVF was mainly based on departmental practice. Adequacy of guideline, information and training, and time for revision on IVF prescription were average, indicating significant training deficit.

Profiles and removal efficiency of polycyclic aromatic hydrocarbons by two different types of sewage treatment plants in Hong Kong

Sewage discharge could be a major source of polycyclic aromatic hydrocarbons （PAHs） in the coastal waters. Stonecutters Island and Shatin Sewage Treatment Works （SCISTW and STSTW） in Hong Kong, adopted chemically enhanced primary treatment and biological treatment, respectively. This study aimed at （1） determining the removal efficiencies of PAHs, （2） comparing the capabilities in removing PAHs, and （3） characterizing the profile of each individual PAHs, in the two sewage treatment plants （STPs）. Quantification of 16 PAHs was conducted by a Gas Chromatography. The concentrations of total PAHs decreased gradually along the treatment processes （from 301 ± 255 and 307 ± 217 ns/L to 14.9 ± 12.1 and 63.3 ± 54.1 ng/L in STSTW and SCISTW, respectively）. It was noted that STSTW was more capable in removing total PAHs than SCISTW with average total removal effidency 94.4% ± 4.12% us. 79.2% ± 7.48% （p 〈 0.05）. The removal of PAHs was probably due to sorption in particular matter, confirmed by the higher distribution coefficient of individual and total PAHs in solid samples （dewatered sludge contained 92.5% and 74.7% of total PAHs in SCISTW and STSTW, respectively） than liquid samples （final efffluent-total contained 7.53% and 25.3% of total PAHs in STSTW and SCISTW, respectively）. Despite the impressive capability of STSTW and SCISTW in removing PAHs, there was still a considerable amount of total PAHs （1.85 and 39.3 ks/year, respectively for the two STPs） being discharged into Hone Kon coastal waters, which would be an environmental concern.

A Traditional Chinese Medicine Approach in Treating Depression by Promoting Liver Qi Circulation: A Western Medicine Perspective

Depression is a prevalent mental illness in developed countries. In Western medicine, experimental and clinical investigations have demonstrated that depression is associated with the dysregulation of neurotransmitter signaling, and symptoms of depression can be alleviated by therapeutic intervention. However, patients taking antidepressant drugs often experience serious side effects and high relapse rates. On the other hand, traditional Chinese medicine (TCM) views depression as a manifestation of liver qi stagnation. Practitioners of TCM have long been treating depression with herbs that promote qi circulation in the liver. In this article, we offer a hypothesis stating the biochemical basis of the linkage between liver qi stagnation and depression. Liver qi is involved in the processing of macronutrients into molecules to fuel energy metabolism in brain neurons, as well as the synthesis of plasma proteins that maintain blood circulation to the brain, thereby enabling these fuel molecules to be delivered to the brain. In cases of liver qi stagnation, the failure in delivering sufficient fuel molecules to the brain disrupts mitochondrial ATP production in neurons. Because neurotransmitter release and neurotropin transport are driven by ATP, the deficiency in release and transport processes resulting from insufficient ATP production could lead to depression. Therefore, if liver qi stagnation is causally related to the pathogenesis of depression, the promotion of liver qi circulation by Chinese herbs might offer a promising prospect for the effective treatment of depression.
