Efficacy of transjugular intrahepatic portosystemic shunts in treating cirrhotic esophageal-gastric variceal bleeding

BACKGROUND Esophageal-gastric variceal bleeding(EGVB)represents a severe complication among patients with cirrhosis and often culminates in fatal ***-tional therapy,a rapidly developing treatment modality over the past few years,has found widespread application in clinical practice due to its minimally inva-sive ***,whether transjugular intrahepatic portosystemic shunt(TIPS)treatment has an impact on patient prognosis remains *** A retrospective study was conducted on ninety-two patients presenting with cirrhotic EGVB who were admitted to our hospital between September 2020 and September *** on the different modes of treatment,the patients were assigned to the study group(TIPS received,n=50)or the control group(per-cutaneous transhepatic varices embolization received,n=42).Comparative ana-lyses were performed between the two groups preoperatively and one month postoperatively for the following parameters:Varicosity status;hemodynamic parameters[portal vein flow velocity(PVV)and portal vein diameter(PVD);platelet count(PLT);red blood cell count;white blood cell count(WBC);and hepatic function[albumin(ALB),total bilirubin(TBIL),and aspartate transaminase(AST)].The Generic Quality of Life Inventory-74 was utilized to assess quality of life in the two groups,and the 1-year postoperative rebleeding and survival rates were *** Following surgical intervention,there was an improvement in the incidence of varicosity compared to the preoperative status in both ***,the study group exhibited more pronounced enhancements than did the control group(Plues,with the study cohort achieving better outcomes(P<0.05).PLT and WBC counts were elevated postoperatively in the two groups,with the study cohort displaying higher PLT and WBC counts(P<0.05).No differences were detected between the two groups in terms of serum ALB,TBIL,or AST lev

Surgical techniques to prevent delayed gastric emptying after pancreaticoduodenectomy

Background: Delayed gastric emptying(DGE) is one of the most common complications after pancreaticoduodenectomy(PD). DGE represents impaired gastric motility without significant mechanical obstruction and is associated with an increased length of hospital stay, increased healthcare costs, and a high readmission rate. We reviewed published studies on various technical modifications to reduce the incidence of DGE. Data sources: Studies were identified by searching Pub Med for relevant articles published up to December 2022. The following search terms were used: “pancreaticoduodenectomy”, “pancreaticojejunostomy”, “pancreaticogastrostomy”, “gastric emptying”, “gastroparesis” and “postoperative complications”. The search was limited to English publications. Additional articles were identified by a manual search of references from key articles. Results: In recent years, various surgical procedures and techniques have been explored to reduce the incidence of DGE. Pyloric resection, Billroth II reconstruction, Braun's enteroenterostomy, and antecolic reconstruction may be associated with a decreased incidence of DGE, but more high-powered studies are needed in the future. Neither laparoscopic nor robotic surgery has demonstrated superiority in preventing DGE, and the use of staplers is controversial regarding whether they can reduce the incidence of DGE. Conclusions: Despite many innovations in surgical techniques, there is no surgical procedure that is superior to others to reduce DGE. Further larger prospective randomized studies are needed.

Earth-abundant Zn-dipyrrin chromophores for efficient CO2 photoreduction

The development of strong sensitizing and Earth-abundant antenna molecules is highly desirable for CO2reduction through artificial photosynthesis. Herein, a library of Zn–dipyrrin complexes（Z-1-Z-6） are rationally designed via precisely controlling their molecular configuration to optimize strong sensitizing Earth-abundant photosensitizers. Upon visible-light excitation, their special geometry enables intramolecular charge transfer to induce a charge-transfer state, which was first demonstrated to accept electrons from electron donors. The resulting long-lived reduced photosensitizer was confirmed to trigger consecutive intermolecular electron transfers for boosting CO2-to-CO conversion. Remarkably, the Earth-abundant catalytic system with Z-6 and Fe-catalyst exhibits outstanding performance with a turnover number of >20 000 and 29.7% quantum yield, representing excellent catalytic performance among the molecular catalytic systems and highly superior to that of noble-metal photosensitizer Ir（ppy）2（bpy）+under similar conditions. Experimental and theoretical investigations comprehensively unveil the structure–activity relationship, opening up a new horizon for the development of Earth-abundant strong sensitizing chromophores for boosting artificial photosynthesis.

Preliminary Study on Spatial Variability and Distribution of Soil Available Microelements in pinggu County, Beijing, China

The objectives of this study were to explore the spatial distribution of soil available microelements and to provide a firm foundation for scientifically formulated fertilizer. 1 076 soil samples were collected from pinggu County of Beijing, China, and the contents of available Cu, Zn, Fe, and Mn in topsoil （0-20 cm） and subsoil （20-40 cm） were analyzed respectively. Descriptive statistics and geostatistics were used to analyze the data. The mean values of topsoil available Cu, Zn, Fe, and Mn were 1.96, 2.51, 17.32, and 10.91 mg kg^-1, respectively, which were at medium or at slightly above medium level. The topsoil content of the soil available microelements was more than that of subsoil. The result of semivariance analysis showed that the topsoil available Fe and Mn were spatially correlated at a larger distance of 21.6 and 45.3 km, and the available Cu and Zn were at a shorter distance of 3.1 and 2.9 km, respectively. The subsoil spatial correlation distances were obviously different from the topsoil, which were in the order of available Mn 〉 Cu 〉 Zn = Fe. The Kriging method was applied to calculate the unobserved points and was used to generate the contour map. The results showed that the structural factors, such as topology, soil types, and soil properties, were the main factors influencing the spatial distribution of topsoil available Fe and Mn. Topsoil available Cu and Zn contents were mainly affected by land use, and their contents in orchard and in vegetable land were greatly higher than in field. The spatial distribution of subsoil microelements was similar to that of the topsoil, which exhibited vertical distribution patterns. Fertilizer and pesticide application resulted in the higher contents of available Cu and Zn in pinggu County. The higher content of available Zn is potentially harmful to soil environmental quality, which should be inspected.

Excessive electrolyte decomposition seriously determines the performance of high-voltage Mg batteries. In Nazar's work, the strategy using a zeolite polymer membrane as a protective interface was proposed to restrain the electrolyte decomposition. Manganese-based oxide electrode materials suffer from severe Jahn-Teller(J-T)distortion,leading to severe cycle instability in sodium ion ***,it is difficult to adjust the electron at d orbitals exactly to a low spin state to eliminate orbital degeneracy and suppress J-T distortion *** article constructed concentration-controllable Mn/O coupled vacancy and amorphous network in Mn_(3)O_(4) and coated it with nitrogen-doped carbon aerogel(Mn_(3-x)O_(4-y)@NCA).The existence of Mn/O vacancies has been confirmed by scanning transmission electron microscopy(STEM)and positron annihilation lifetime spectroscopy(PALS).Atomic absorption spectroscopy(AAS)and X-ray photoelectron spectroscopy(XPS)determine the most optimal ratio of Mn/O vacancies for sodium ion storage is 1:*** functional theory(DFT)calculations prove that Mn/O coupled vacancies with the ratio of 1:2could exactly induce a low spin states and a d~4 electron configuration of Mn,suppressing the J-T distortion *** abundant amorphous regions can shorten the transport distance of sodium ions,increase the electrochemically active sites and improve the pseudocapacitance *** the synergetic effect of Mn/O coupled vacancies and amorphous regions,Mn_(3-x)O_(4-y)@NCA exhibits an energy density of 37.5 W h kg^(-1)and an ultra-high power density of 563 W kg^(-1)in an asymmetric *** sodium-ion batteries,it demonstrates high reversible capacity and exceptional cycling *** research presents a new method to improve the Na^(+)storage performance in manganese-based oxide,which is expected to be generalized to other structural distortion.
