Reducing Hospital Lengths of Stay by Discharge Status

This study evaluated the impact of length of stay reduction by discharge status in the hospitals of Syracuse, New York. It focused on the two largest inpatient services, adult medicine and adult surgery, between 2008 and 2018. In the Syracuse hospitals, the adult medicine mean length of stay declined by 0.12 days, resulting in a savings of 14,154 patient days during the ten year period. The adult surgery mean stays declined by 0.91 days, resulting in a savings of 22,639 patient days. The reductions in stays were accompanied by differences in utilization by discharge status. For discharges to self care, the changes in mean stays increased the number of days saved for adult medicine from 5111 to 13,264 and the number of days saved for adult surgery from 4355 to 13,862. These changes were brought about through internal efficiencies within the hospitals. For discharges to nursing homes, the reductions in stays caused the number of excess days to decline from 13,631 to 8695 for adult medicine and from 9150 to 5075 for adult surgery. These changes were brought about through cooperative efforts with long term care providers in the community.

Improving Hospital Stays with Community Wide Programs

This study described a series of programs implemented in Syracuse, New York to support the movement of long term acute care patients to skilled nursing facilities. The Difficult to Place Program involved the identification of these patients and the communication of information concerning them between hospitals and nursing homes on a continuing basis. These patients involved approximately 20 percent of new admissions to nursing homes. The Subacute Programs included services such as intravenous therapy and offsite transportation that were not originally available in area nursing homes. The Subacute Program stimulated the development of these services in long term care. The Complex Care Programs have included services for patients with high severity of illness such as multiple intravenous antibiotic therapy and high cost medications. The Subacute and Complex Care Programs included 5 - 6 percent of Difficult to Place patients. The study demonstrated that these programs reduced the number of annual adult medicine and adult surgery patient days by 2288 between 2011 and 2017.

Who Am I on Facebook？ Usage and Motivation Through User Eyes

This article discusses different modes of using Facebook and the motivation for its users. The analysis aims to clarify how and why users might resort to one or more usage modes when joining this social network and emphasizes the relationship between interactions mediated by this social network and user identity. For this we conducted semi-structured interviews with 10 young Facebook users-five males and five females-all residing in Brasilia-DF, Brazil. The results obtained were compared with those found in previous research. We used theoretical derived data from social psychology and socio-cultural anthropology for the data analysis and conducted an accurate literature review. The conclusion goes beyond the distinction between motivation and usage and expresses a critical analysis of Facebook usage purposes thus indicating widespread use that can extend to other specifics depending on the interests of the participants. Furthermore, there is motivation behind Facebook which is not yet mentioned in existing research which attracts enthusiasts to Facebook and is directly related to the issue of identity in the contemporary world.

Stemness features in liver cancer

Heterogeneity is a cardinal hallmark of cancer, including primary liver cancer (PLC), and occurs at different layers including putative cell-of-origin. Current evidence suggests that within cellular subpopulations in PLC there are stem-like cells, the cancer stem cells (CSCs). The CSC concept has been recently proposed as an explanation of such intra-tumor heterogeneity. According to this model, CSCs are responsible for tumor initiation, recurrence, metastasis as well as drug-resistance. However, although the CSC hypothesis is intriguing and supported by a large number of experimental studies, there are still open questions regarding the origin of putative CSCs. Since chemo-resistance and recurrence represent major issues in PLC treatment, the development of new therapeutic strategies is needed, for which a good understanding of tumor behavior and in particular of CSCs biology is an imperative prerequisite. In this review we summarize the regulatory pathways that support CSC features in PLC. Moreover, we highlight the key features of hepatic CSC, in terms of enhanced drug-resistance, increased metastatic potential and metabolic rearrangement. Knowledge of the molecular mechanisms underlying CSC biology may provide novel options for PLC combination therapies.

Design of the readout electronics for the DAMPE Silicon Tracker detector

The Silicon Tracker （STK） is one of the detectors of the DAMPE satellite used to measure the incidence direction of high energy cosmic rays. It consists of 6 X-Y double layers of silicon micro-strip detectors with 73728 readout channels. It is a great challenge to read out the channels and process the huge volume of data in the harsh environment of space. 1152 Application Specific Integrated Circuits （ASIC） and 384 ADCs are used to read out the detector channels. 192 Tracker Front-end Hybrid （TFH） modules and 8 identical Tracker Readout Board （TRB） modules are designed to control and digitalize the front signals. In this paper, the design of the readout electronics for the STK and its performance are presented in detail.

The Role of Sentinel Lymph Node Biopsy in Thin Melanoma (Breslow Thickness ≤ 0.75 mm and 0.76 mm - 1.0 mm Respectively): Our Results and Review of the Literature

Introduction: The Sentinel Lymph Node Biopsy (SLNB) in melanoma is an important tool of staging. The impact on overall survival still remains unclear. The guidelines in regard to depth, taking in mind where SLNB staging benefits do not outweigh the risks of the procedure, are constantly reviewed and modified. Patients and Methods: From 2010 to 2015, 104 patients with thin melanoma Stage IA with presence of adverse or high risk features and from IB only TIb, N0, M0 (American Joint Committee on Cancer, AJCC Melanoma Staging and Classification 7th Edition 2009) were included and divided into 2 groups: group A: 68 patients with Breslow ≤ 0.75 mm and group B: 36 patients with Breslow 0.76 - 1.0 mm. Initially all patients underwent excision of the primary site and subsequently wide local excision and SLNB. We analyzed the histopathology reports of SLNB procedures in both groups. Results: There was no positive SLN in group A (0%). 4 patients from group B had positive SLN (11.1%) and underwent Completion Lymph Node Dissection (CLND). The total percentage of positive SLNs from both groups was 3.8%. Conclusions: Our findings justify the SLNB procedure in thin melanomas of 0.76 - 1.0 mm. In melanomas ≤ 0.75 mm, SLNB should be considered on an individual basis when “high-risk features” are present. More comparable studies should be evaluated in order to accurately define the threshold value of Breslow thickness where SLNB is safely deemed unnecessary.

Speciation and Mobility of Phosphate in the Eutrophic Ponds at Prospect Park, Brooklyn, New York, USA

The water system at the Prospect Park ponds of Brooklyn, NY has been receiving chronic loads of orthophosphate from the municipal water since the mid ’90s. The ponds are currently eutrophic with degraded water quality and high levels of P in the sediment. In this study, the distribution and speciation of P within the water column was examined. The dominant form of P in the water was found as soluble reactive phosphate (SRP) and the temporal change of SRP indicated significant internal loading from the sediment during late summer in the ponds downstream. Lake sediment was investigated for P fractionation and phosphate bound to Fe-hydroxides was the dominant form of P in the sediment. Both external and internal P pools are important and should be taken into consideration when planning for restoration.

背景氟喹诺酮类药物通常治疗对一线抗结核药物不耐受的药物敏感性肺结核患者以及感染耐药菌的患者。尽管这类药物的应用逐渐增加，但有关它们在临床试验之外的耐受性方面的信息较少。目的比较含左氧氟沙星方案与标准疗法的重大不良事件总的发生率。方法含左氧氟沙星的方案的病例在年龄、性别上与对照（标准一线结核药物）病人相匹配。患者从2001年至2004年地方结核资料库中选出验明合格。药物安全性评估是用不良事件的性质，与研究药物有关的可能性及严重性。只有主要副反应，也就是严重或中等度的不良事件且明确分类与结核药物可能相关才考虑作为分析用。结果在3年的研究期间，102例患者接受左氧氟沙星，358例患者接受一线药物的活动性结核治疗。
