Dear Editor, The current standard 12-core systematic transrectal ultrasound （TRUS）-guided biopsy in prostate cancer diagnosis has the disadvantages of overdetecting indolent tumors, while failing to identify clinically significant cancers in up to 35%.1 Advances in imaging techniques, notably the multiparametric magnetic resonance imaging （mpMRI）, allow clinically significant prostate cancers to be detected with more precise localization） Fusion of MRI and real-time TRUS enables targeted biopsy of suspicious lesions, overcoming human error, and inability of biopsy site tracking in cognitive fusion？ Multiple Western series have suggested a higher detection rate, higher specificity, and better risk stratification with fusion biopsy.

This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

Objective:To report the clinical maternal and fetal outcomes of pregnant women with coronavirus disease 2019(COVID-19),along with any associated pregnancy complications,in Hong Kong,China,and to assess the impact of COVID-19 vaccination on these ***:This prospective registry-based observational study included pregnant women who were recruited through convenient sampling and had a laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection with a cycle threshold(Ct)value result available on admission to eight local hospitals in Hong Kong,*** on clinical symptoms,laboratory results,medical treatments,delivery timing and mode,and pregnancy complications were extracted from the Hospital Authority’s electronic medical record ***,fetal,and pregnancy outcomes were compared between unvaccinated pregnant women with COVID-19 and those who had received at least one dose of COVID-19 vaccine before *** continuous variables and categorical variables were analyzed using the Mann-Whitney U test and the Pearson’s chi-squared test respectively.A P value less than 0.05 was considered statistically ***:A total of 164 pregnant women were included,of whom 78(47.56%)were ***-19 was diagnosed before 28 weeks’gestation in 30(18.29%),while 134(81.71%)were diagnosed at or after 28 weeks’***-two(37.80%)women received at least one dose of COVID-19 *** were no significant differences between vaccinated and unvaccinated groups in the time interval between COVID-19 diagnosis and delivery,the Ct value,and the gestational age at infection onset or delivery(P>0.05).The majority of women were symptomatic at diagnosis regardless of vaccination status 55(88.71%)in vaccinated group vs.78(76.47%)in unvaccinated group(P=0.052).Symptoms did not significantly differ between groups except for cough(62.90%vs.47.06%,P=0.049).The overall rate of severe COVID-19 in pregnant women was *** t

BACKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

Many cities have pledged to achieve carbon *** urban water industry can also contribute its share to a carbon-neutral *** a multi-city time-series analysis approach,this study aims to assess the progress and lessons learned from the greenhouse gas(GHG)emissions management of urban water systems in four global cities:Amsterdam,Melbourne,New York City,and *** cities are advanced in setting GHG emissions reduction targets and reporting GHG emissions in their water *** four cities have reduced the GHG emissions in their water industries,compared with those from more than a decade ago(i.e.,the latest three-year moving averages are 13%–32%lower),although the emissions have“rebounded”multiple times over the *** emissions reductions were mainly due to various engineering opportunities such as solar and mini-hydro power generation,biogas valorization,sludge digestion and incineration optimization,and aeration system *** cities have recognized the many challenges in reaching carbon-neutrality goals,which include fluctuating water demand and rainfall,more carbon-intensive flood-prevention and water-supply strategies,meeting new air and water quality standards,and revising GHG emissions accounting *** study has also shown that it is difficult for the water industry to achieve carbon neutrality on its own.A collaborative approach with other sectors is needed when aiming toward the city’s carbon-neutrality *** an approach involves expanding the usual system boundary of the water industry to externally tap into both engineering and non-engineering opportunities.

Background and Aims:We aimed to perform a network meta-analysis(NWM)to examine comparative effectiveness of non-selective beta blockers(NSBBs)on prophylaxis of gastroesophageal variceal bleeding(GVB)and mortality ***:MEDLINE(OVID)and EMBASE databases were searched for eligible randomized clinical trials(RCTs)from inception to July 3,*** of interest included primary/secondary prophylaxis of GVB,failure to achieve hepatic venous pressure gradient(HVPG)decremental response,liver-related and all-cause mortality.A Bayesian NWM was performed to derive relative risk(RR)with 95%credible intervals(CrIs).The ranking probability of each NSBB was assessed by surface under cumulative ranking curve(SUCRA).Results:Thirty-three RCTs including 3,188 cirrhosis patients with gastroesophageal varices were *** with placebo,nadolol ranked first for reducing variceal bleeding[RR:0.25,(95%CrI:0.11–0.51);SUCRA:0.898],followed by carvedilol[RR:0.33,(95%CrI:0.11–0.88);SUCRA:0.692]and propranolol[RR:0.52,(95%CrI:0.37–0.75);SUCRA:0.405].Carvedilol was more effective than propranolol in achieving HVPG decremental response[RR:0.43,(95%CrI:0.26–0.69)].Carvedilol ranked first for reducing all-cause mortality[RR:0.32,(95%CrI:0.17–0.57);SUCRA:0.963],followed by nadolol[RR:0.48,(95%CI:0.29–0.77);SUCRA:0.688],and propranolol[RR:0.77,(95%CI:0.58–1.02);SUCRA:0.337].Similar findings were observed for liver-related *** ranked the *** RR of adverse events was 4.38,(95%CrI:0.33–161.4);SUCRA:0.530,followed by propranolol[RR:7.54,(95%CrI:1.90–47.89);SUCRA:0.360],and nadolol[RR:18.24,(95%CrI:91.51–390.90);SUCRA:0.158].Conclusions:Carvedilol is the preferred NSBB with better survival benefit and lower occurrence of adverse events among patients with gastroesophageal varices.

The embolization of gastric varices is an established technique for acute bleeding in patients with portal hypertension. Here, we report an attempt to embolize a gastrorenal shunt to facilitate esophagectomy in a patient with an esophageal malignancy. To our knowledge, this is the first case in the literature to highlight the role of interventional medicine in the treatment of patients with esophageal malignancy.

Microwave steam pyrolysis(MSP)is an innovative thermochemical approach to converting biomass into high-quality biochar using steam to improve the dielectric heating of microwave *** shows high fixed carbon and carbon contents at a maximum temperature of 550℃in 10 *** MSP achieved a heating rate of 112℃/min from 200℃to 400℃to produce biochar ***,the thermal properties of biochar in microwave heating were investigated in this study to explore its potential as a microwave heat-absorbent *** is able to perform volumetric and controllable heating to the ***,biochar shows good microwave heat absorbency,storing and transferring heat *** temperature profile of three different sizes of biochar was investigated to examine the efficiency of biochar in heat absorption from microwave *** was found that the powder form of biochar showed a higher heat transfer rate of 40℃/min and a low cooling rate of 7.5℃/*** presented results are useful for evaluating the application of biochar as a promising medium for heat storage systems.

In this investigation,a picosecond laser was employed to fabricate surface textures on a Stavax steel substrate,which is a key material for mold fabrication in the manufacturing of various polymer *** main types of surface textures were fabricated on a Stavax steel substrate:periodic ripples,a two-scale hierarchical two-dimensional array of micro-bumps,and a micro-pits array with *** wettability of the laser-textured Stavax steel surface was converted from its original hydrophilicity into hydrophobicity and even super-hydrophobicity after exposure to *** results clearly show that this super-hydrophobicity is mainly due to the surface *** ultrafast laserinduced catalytic effect may play a secondary role in modifying the surface chemistry so as to lower the surface *** laser-induced surface textures on the metal mold substrates were then replicated onto polypropylene substrates via the polymer injection molding *** surface wettability of the molded polypropylene was found to be changed from the original hydrophilicity to *** developed process holds the potential to improve the performance of fabricated plastic products in terms of wettability control and easy cleaning.

BACKGROUND： Acupuncture has been used as a treatment for cognitive impairment. OBJECTIVE： This review assesses clinical evidence for or against acupuncture as a treatment for cognitive impairment. This review also discusses the proposed mechanism（s） that could link acupuncture to improved cognitive function. METHODS： We searched the literature using PolyUone search from its inception to January 2013, with full text available and language limited to English. Levels of evidence were examined using Oxford Centre for Evidence-based Medicine-Levels of Evidence （March, 2009）. RESULTS： Twelve studies met the inclusion criteria： 3 human studies and 9 animal studies. Levels of evidence ranged from level lb to level 5. CONCLUSION： Most animal studies demonstrated a positive effect of acupuncture on cognitive impairment. However, the results of human studies were inconsistent. Further high-quality human studies with greater statistical power are needed to determine the effectiveness of acupuncture and an optimal protocol.
