The gonarthrosis with congenital dislocation of the patella is rare condition. This paper will report about total knee arthroplasty (TkA) for the valgus knee with bilateral congenital dislocation of the patella. A 52-year-old woman presented to our hospital with progressive pain in bilateral knees. Both knee showed severe valgus deformity and lateral dislocation of the patella. Anteroposterior weight-bearing radiographs showed osteoarthritic changes in the lateral compartment with 7 valgus deformity at the right and 15 at the left. A bilateral TkA with cruciate retained components (NAkASHIMA FINE) was performed. Good tracking of the patella was achieved by using a lateral parapatellar approach and vastus medialis plication. There are no normative guidelines of operative procedure about TkA for osteoarthritis of valgus knee with congenital dislocation of the patella. With some referencing to current literature, strategies to good patella tracking are discussed.

Background: The geriatric nutritional risk index (GNRI) has been developed as a tool to assess the nutritional risk. The triglyceride to high-density lipoprotein cholesterol (TG/HDL-C) ratio has been shown to be a predictor of cardiovascular (CV) outcomes in the general population. Objectives: The aim of this study was to determine whether the combination of GNRI and TG/HDL-C ratio is a predictor of all-cause mortality and CV deaths in maintenance hemodialysis (MHD) patients. Methods: We performed a retrospective, observational cohort study in which we enrolled 341 MHD patients from a single center in Japan who had been followed up for a mean of 48.0 ± 12.7 months. The outcomes were defined as the occurrence of all-cause mortality and CV deaths during the follow-up period. Baseline GNRI and TG/HDL-C ratios were investigated for associations with outcomes by using Cox proportion hazards models adjusted for demographic parameters. Results: Overall, 101 of the subjects had died, of whom 52 died due to CV events during the mean follow-up period of 48.0 ± 12.7 months. The patients were grouped into four categories according to a median GNRI Conclusion: The combination of GNRI and TG/HDL-C ratio is an easily accessible marker for predicting all-cause mortality and CV deaths in MHD patients.

This research aimed to clarify the relationship between nursing students’ levels of structural knowledge and assessment skills. Participants were 17 third-year university students majoring in nursing who participated individually in the experiments. The experiments included a nursing-scene task, free-recall task, and sorting task. Experiments were conducted before and after their clinical practice. Each student’s level of structural knowledge was calculated from the free-recall and sorting task responses, and each student’s assessment skill was calculated from the nursing-scene task responses. Levels of structural knowledge were significantly higher in post-examination compared with pre-examination (p p n = 5) had significantly higher scores for their conclusions and reasons than the low-structured group (n = 5) (p p knowledge of students in the high-structured group seemed to enable them to acquire and activate highly related information during assessment. Students in the high-structured group apparently made assessments not only by using information given but also by over viewing information comprehensively.

Reconstruction Using a Free Vascularized Fibular Graft after Frozen Autograft Reconstruction for Osteosarcoma of the Distal Tibia: A Case Report

Recently we have been performing biological reconstruction for malignant bone tumors of the extremities using frozen autografts. Here we present a case treated with free vascularized fibular graft (FVFG) after this method. A 23-year-old man developed osteosarcoma in his left distal tibia. There was nonunion after frozen autograft reconstruction, which we treated with FVFG. Twenty-four months later, bridging between the host bone and the frozen autograft was achieved. Our department has achieved bone union in almost all cases, but we sometimes encounter cases of nonunion after this method because of delayed blood supply. In these instances, reconstruction using FVFG may represent an attractive choice for salvage treatment.

Impressions of Hazmat Suits Based on Color

Background: Commonly, skin and clothing surface contamination screening (radiation survey) is performed for residents of an area to confirm the levels of residual contamination after nuclear power disasters. Examiners for such a screening often wear white radiation hazmat suits. However, it has been reported that examinees tend to feel tension or anxiety. In this study, we focus on the color of the radiation hazmat suit as one measure to reduce residents’ tension or anxiety. This study aims to determine the common conceptions of radiation hazmat suits based on color. Methods: An anonymous self-administered questionnaire survey was conducted with a total of 262 participants consisting of nursing students, administrative staff of a university, general citizens, and mothers raising their children in the Tohoku region in Japan. The common impressions of radiation hazmat suits, which are typically white, were measured using the Multiple Affective States Scale. This scale can measure the affective state brought about by clothing. The color of white (W) was defined as a standard color commonly used for hazmat suits;in order to determine the common impressions of radiation hazmat suits based on color, the following additional colors were shown to the participants: salmon pink (P), ivory (I), and light green (G). Results: There were no sex differences regarding common impressions of radiation hazmat suits. For white radiation hazmat suits, a high score reflecting the characteristic of “capable” was obtained, while the scores for “cheerful” and “calm” were low, and those for “depressed” and “tense” were high. The colors that showed the highest score for “calm” as a positive affirmative state were I and P. W and P reflected a high “ashamed” score, and the “tense” score for G was higher for the participants in their 20s than for those in their 30s and older. Endoscopic removal and trimming of distal self-expandable metallic biliary stents

AIM:To evaluate the efficacy and safety of endoscopic removal and trimming of self-expandable metallic stents(SEMS).METHODS:All SEMS had been placed for distal biliary ***-seven endoscopic procedures were performed in 19 patients in whom SEMS(one uncovered and 18 covered) removal had been attempted,and 8 patients in whom stent trimming using argon plasma coagulation(APC) had been attempted at Tokyo Medical University *** APC settings were:voltage 60-80 W and gas flow at 1.5 L/***:The mean stent indwelling period for all patients in whom stent removal had been attempted was 113.7 ± 77.6 d(range,8-280 d).Of the 19 patients in whom removal of the SEMS had been attempted,the procedure was successful in 14(73.7%) without procedure-related adverse *** indwelling period in the stent removable group was shorter than that in the unremovable group(94.9 ± 71.5 d vs 166.2 ± 76.2 d,P = 0.08).Stent trimming was successful for all patients with one minor adverse event consisting of self-limited *** time ranged from 11 to 16 ***:Although further investigations on larger numbers of cases are necessary to accumulate evidence,the present data suggested that stent removal and stent trimming is feasible and effective for stent-related complications.

Successful treatment of multiple lung metastases of hepatocellular carcinoma by combined chemotherapy with docetaxel, cisplatin and tegafur/uracil

We report the successful treatment of multiple lung metastases after hepatic resection for hepatocellular carcinoma (HCC) with combined docetaxel, cisplatin (CDDP), and enteric-coated tegafur/uracil (UFT-E). A 68-year-old man was diagnosed with multiple lung metastases of HCC 7 mo after partial hepatectomy for HCC. Oral UFT-E was given daily and docetaxel and CDDP were given intra-arterially (administered just before the bronchial arteries) every 2 wk via a subcutaneous injection port. One month after starting chemotherapy, levels of tumor marker, protein induced by vitamin k absence(PIVkA--), decreased rapidly,and after a further month, chest X-ray and computed tomography revealed the complete disappearance of multiple liver metastases. Two years after the combined chemotherapy, HCC recurred in the liver and was treated but no pulmonary recurrence occurred. In the absence of a standardized highly effective therapy, this combined chemotherapy with docetaxel, CDDP and UFT-E may be an attractive option for multiple lung metastases of HCC.
