Feasibility of pancreatectomy following high-dose proton therapy for unresectable pancreatic cancer

AIM To review surgical outcomes for patients undergoing pancreatectomy after proton therapy with concomitant capecitabine for initially unresectable pancreatic *** From April 2010 to September 2013,15 patients with initially unresectable pancreatic cancer were treated withproton therapy with concomitant capecitabine at 1000 mg orally twice daily. All patients received 59.40 Gy(RBe) to the gross disease and 1 patient received 50.40 Gy(RBe) to high-risk nodal targets. There were no treatment interruptions and no chemotherapy dose reductions. Six patients achieved a radiographic response sufficient to justify surgical exploration,of whom 1 was identified as having intraperitoneal dissemination at the time of surgery and the planned pancreatectomy was aborted. Five patients underwent resection. Procedures included:Laparoscopic standard pancreaticoduodenectomy(n = 3),open pyloris-sparing pancreaticoduodenectomy(n = 1),and open distal pancreatectomy with irreversible electroporation(IRe) of a pancreatic head mass(n = 1). ReSULTS The median patient age was 60 years(range,51-67). The median duration of surgery was 419 min(range,290-484),with a median estimated blood loss of 850 cm^3(range,300-2000),median ICU stay of 1 d(range,0-2),and median hospital stay of 10 d(range,5-14). Three patients were re-admitted to a hospital within 30 d after discharge for wound infection(n = 1),delayed gastric emptying(n = 1),and ischemic gastritis(n = 1). Two patients underwent R0 resections and demonstrated minimal residual disease in the final pathology specimen. One patient,after negative pancreatic head biopsies,underwent IRe followed by distal pancreatectomy with no tumor seen in the specimen. Two patients underwent R2 resections. Only 1 patient demonstrated ultimate local progression at the primary site. Median survival for the 5 resected patients was 24 mo(range,10-30).CONCLUSION Pancreatic resection for patients with initially unresectable cancers is feasible after

Use of Pearson’s Chi-Square for Testing equality of Percentile Profiles across Multiple Populations

In large sample studies where distributions may be skewed and not readily transformed to symmetry, it may be of greater interest to compare different distributions in terms of percentiles rather than means. For example, it may be more informative to compare two or more populations with respect to their within population distributions by testing the hypothesis that their corresponding respective 10th, 50th, and 90th percentiles are equal. As a generalization of the median test, the proposed test statistic is asymptotically distributed as Chi-square with degrees of freedom dependent upon the number of percentiles tested and constraints of the null hypothesis. Results from simulation studies are used to validate the nominal 0.05 significance level under the null hypothesis, and asymptotic power properties that are suitable for testing equality of percentile profiles against selected profile discrepancies for a variety of underlying distributions. A pragmatic example is provided to illustrate the comparison of the percentile profiles for four body mass index distributions.

nfertility, impotence, and emasculation - psychosocia ：ontexts for abandoning reproduction

From a Darwinian perspective we live to reproduce, but in various situations genetic males elect not to reproduce by choosing medical treatments leading to infertility, impotence, and, in the extreme, emasculation. For many men, infertility can be psychologically distressing. However, for certain genetic males, being infertile may improve their quality of life. examples include （1） men who seek vasectomy, （2） individuals with Gender Dysphoria （e.g., transwomen, and modern day voluntary eunuchs）, （3） most gay men, and （4） men treated for testicular and prostate cancer. Men who desire vasectomy typically have a Darwinian fitness W 〉1 at the time of their vasectomies; i.e., after they have their desired number of offspring or consider themselves past an age for parenting newborns. In contrast, prostate and testicular cancer patients, along with individuals with extreme Gender Dysphoria, do not necessarily seek to be sterile, but accept it as an unavoidable consequence of the treatment for their condition undertaken for survival （in case of cancer patients） or to achieve a better quality of life （for those with Gender Dysphoria）. Most gay men do not father children, but they may play an avuncular role, providing for their siblings＇ offspring＇s welfare, thus improving their inclusive fitness through kin selection. In a strictly Darwinian model, the primary motivation for all individuals is to reproduce, but there are many situations for men to remove themselves from the breeding populations because they have achieved a fitness W 〉1, or have stronger medical or psychological needs that preclude remaining fertile.

Quantifying relations between altered hydrology and fish community responses for streams in Minnesota

Background:Altered hydrology is a stressor on aquatic life,but quantitative relations between specific aspects of streamflow alteration and biological responses have not been developed on a statewide scale in *** sub-sets regression analysis was used to develop linear regression models that quantify relations among five categories of hydrologic metrics(i.e.,duration,frequency,magnitude,rate-of-change,and timing)computed from streamgage records and six categories of biological metrics(i.e.,composition,habitat,life history,reproductive,tolerance,trophic)computed from fish-community samples,as well as fish-based indices of biotic integrity(FIBI)scores and FIBI scores normalized to an impairment threshold of the corresponding stream class(FIBI_BCG4).Relations between hydrology and fish community responses were examined using three hydrologic datasets that represented periods of record,long-term changes,and short-term changes to flow regimes in streams of ***:Regression models demonstrated significant relations between hydrologic explanatory metrics and fish-based biological response metrics,and the five regression models with the strongest linear relations explained over 70%of the variability in the biological metric using three hydrologic metrics as explanatory ***-based biological metrics demonstrated the strongest linear relations to hydrologic *** most commonly used hydrologic metrics were related to bankfull flows and aspects of flow ***:Final regression models represent paired streamgage records and biological samples throughout the State of Minnesota and encompass differences in stream orders,hydrologic landscape units,and watershed *** methods can support evaluations of stream fish communities and facilitate targeted efforts to improve the health of fish *** also can be applied to locations outside of Minnesota with continuous streamgage data and fish-community sampl

Invertibility is one of the desirable properties of moving average processes. This study derives consequences of the invertibility condition on the parameters of a moving average process of order three. The study also establishes the intervals for the first three autocorrelation coefficients of the moving average process of order three for the purpose of distinguishing between the process and any other process (linear or nonlinear) with similar autocorrelation structure. For an invertible moving average process of order three, the intervals obtained are , -0.5ρ2ρ1<0.5.

Conspecific negative density dependence(CNDD)is a potentially important mechanism in maintaining species *** previous evidence showed habitat heterogeneity and species’dispersal modes affect the strength of CNDD at early life stages of trees(e.g.,seedlings),it remains unclear how they affect the strength of CNDD at later life *** examined the degree of spatial aggregation between saplings and trees for species dispersed by wind and gravity in four topographic habitats within a 25-ha temperate forest dynamic plot in the Qinling Mountains of central *** used the replicated spatial point pattern(RSPP)analysis and bivariate paircorrelation function(PCF)to detect the spatial distribution of saplings around trees at two scales,15 and 50 m,*** the signal was not apparent across the whole study region(or 25-ha),it is distinct on isolated areas with specific characteristics,suggesting that these characteristics could be important factors in ***,we found that the gravity-dispersed tree species experienced CNDD across habitats,while for wind-dispersed species CNDD was found in gully,terrace and low-ridge *** study suggests that neglecting the habitat heterogeneity and dispersal mode can distort the signal of CNDD and community assembly in temperate forests.

entameba histolytica is a protozoan that usually causes procto-colitis. e. histolytica amebiasis is endemic in tropical countries but exceedingly rare in the US Amebic liver abscess commonly presents with fever, abdominal distension, right upper quadrant pain/tenderness, hepatomegaly, and tachycardia. We discuss the presentation/diagnosis of amebiasis in a child who had visited Mexico and the difficulty of early diagnosis when there is no evidence of intestinal infection. Ultrasonography and/or CT were essential diagnostic tools and an IgG antibody study confirmed the diagnosis. Management included oral metronida-zole and abscess drainage.

Tai Ji Quan as antihypertensive lifestyle therapy:A systematic review and meta-analysis

Background:Professional health organizations are not currently recommending Tai Ji Quan alongside aerobic exercise to treat *** aimed to examine the efficacy of Tai Ji Quan as antihypertensive lifestyle ***:Tai Ji Quan interventions published in english and Chinese were included when they involved healthy adults,reported pre-and postintervention blood pressure(BP),and had a non-exercise/non-diet control *** systematically searched 11 electronic databases for studies published through July 31,2018,yielding 31 qualifying controlled ***(1)evaluated the risk of bias and methodological study quality,(2)performed meta-regression analyses following random-effects assumptions,and(3)generated additive models representing the largest possible clinically relevant BP ***:Participants(n=3223)were middle-aged(56.6±15.1 years of age,mean±SD)adults with prehypertension(systolic BP(SBP)=136.9±15.2 mmHg,diastolic BP(DBP)=83.4±8.7 mmHg).Tai Ji Quan was practiced 4.0±1.4 sessions/week for 54.0§10.6 min/session for 22.3±20.2 ***,Tai Ji Quan elicited significant reductions in SBP(-11.3 mmHg,95%CI:-14.6 to-8.0;d+=-0.75)and DBP(-4.8 mmHg,95%CI:-6.4 to-3.1;d+=-0.53)***(pes with hypertension,Tai Ji Quan trials published in english elicited SBP reductions of 10.4 mmHg and DBP reductions of 4.0 mmHg,which was half the magnitude of trials published in Chinese(SBP reductions of 18.6 mmHg and DBP reductions of 8.8 mmHg).Conclusion:Our results indicate that Tai Ji Quan is a viable antihypertensive lifestyle therapy that produces clinically meaningful BP reductions(i.e.,10.4 mmHg and 4.0 mmHg of SBP and DBP reductions,respectively)among individuals with *** magnitude of BP reductions can lower the incidence of cardiovascular disease by up to 40%.
