To advance preclinical testing of novel targeted drugs in colorectal cancer (CRC) we established a panel of 133 mouse xenograft models from fresh tumor specimens of 239 patients with CRC of all four UICC stages. A subgroup of 67 xenograft models was treated with cetuximab, bevacizumab and oxaliplatin as single agents. Mutation status of kRAS (G12, G13, A146T), BRAF (V600E) and PIk3CA (E542k, E545k, H1047R) was assessed in all xenografts by allelespecific real-time PCR. kRAS codon 61 was assessed by conventional sequencing. AREG and EREG expression levels were analyzed by real-time PCR expression assays. In the treatment experiment we observed response rates of 27% (18/67) for cetuximab, 3% (2/67) for bevacizumab, and 6% (4/67) for oxaliplatin. Classification based on kRAS, BRAF and PIk3CA mutation status identified 15 of the responders (sensitivity 83%, confidence interval at p = 0.05 (CI): 59% - 96%), and 38 nonresponders (specificity 78%, CI: 63% - 88%). If any mutation except in kRAS codon 13 were considered, the classifier reached sensitivity of 94% and specificity of 69%. We improved specificity of the classifiers to 90% and 86% respectively by adding AREG and EREG RNA expression thresholds retrospectively. In patient-derived xenograft models, we found a predictive classifier for response to cetuximab that is more accurate than established biomarkers. We confirmed its potential performance in primary human tumors. For patients, the classifier’s sensitivity promises increased response rates and its specificity limits unnecessary toxicity. Given the scope of our xenograft models across all UICC stages, this applies not only to mCRC but also to the adjuvant setting of earlier stages. The xenograft collection allows to mimic randomized phase II trials and to test novel drugs effectively as single agents or in combinations. It also enables the development of highly accurate companion diagnostics as demonstrated by us for cetuximab.

CONTRALATERAL SUPPRESSION OF DISTORTION PRODUCT OTOACOUSTIC EMISSION IN CHILDREN WITH AUDITORY PROCESSING DISORDERS

Previous research has demonstrated that the amplitude of evoked emissions decreases in human sub-jects when the contralateral ear is stimulated by noise. The medial olivocochlear bundle (MOCB) is be-lieved to control this phenomenon. Recent research has examined this effect in individuals with auditory pro-cessing disorders (APD), specifically with difficulty understanding speech in noise. Results showed tran-sient evoked otoacoustic emissions (TEOAEs) were not affected by contralateral stimulation in these sub-jects. Much clinical research has measured the function of the MOCB through *** study will use an alternative technique, distortion product otoacoustic emissions (DPOAEs), to examine this phenomenon and evaluate the function of the MOCB. DPOAEs of individuals in a control group with normal hearing and no significant auditory processing difficulties were compared to the DPOAEs of children with signifi-cant auditory processing *** showed that the suppression effect was observed in the control group at 2 kHz with 3 kHz of narrowband noise. 目的 :观察抗肝抗原自身抗体在我国不同类型肝病患者中的存在状况 ;探讨自身免疫性肝脏疾病的自身抗体特征。方法 :间接免疫荧光法初筛 14 12例肝功能异常血清 ,从中选择 2 2 8例 :①自身免疫性肝病组 4 2例 :初步诊断为AIH 18例、PBC 2 1例、PSC 3例。②HAV组 2 3例 ;③HBV组 70例 ;④HCV组 33例 ;⑤非甲～非戊型肝炎组 6 0例。结合Westernblot、酶免疫条带技术等分别检测ANA、AMA、SMA、LkM 1、LC 1、SLA LP和AMA M2亚型、dsDNA及ENA类多种抗体。结果 :14 12例中诊断AIH、PBC和PSC者分别为送检标本的 12 7‰ ,14 9‰和 2 1‰。 2 2 8例血清中 2例LkM 1阳性和 2例SLA LP阳性 ;按AIH的分型标准 ,自身免疫性肝病组属于I型AIH者 14例 (78% ) ,II型 2例 (11% ) ,III型 2例 (11% ) ;AIH患者ANA抗体未见特定的荧光类型。PBC患者AMA和M2全部阳性 ;其ANA以核膜型为主 (7 14 )。NonA～E组 4例AMA和M2阳性 ,3例SMA高滴度阳性 ,4例出现SS A、SS B或dsDNA等抗体。结论 :三型自身免疫性肝炎在中国都存在 ,肝抗原自身抗体和ANA及AMA的分型检测有助于自身免疫性肝病的诊断与治疗 ;少数非甲～非戊型肝炎应考虑自身免疫性肝病诊断。

心脏骤停（SCA）后预后极差，其事件后存活率不足10％，尤其是院外发生的SCA。而罹发SCA多数是因心血管病所致，其中缺血性心脏病（IHD）又高居榜首。然而既往关于在罹发SCA前，其IHD能得已确诊与其继后存活率间关系尚不清楚，现就此进行调研分析。受测对象为1466例罹发SCA的患者，均龄（65±16）岁，女性33％，研究中均评估罹发SCA前的合并症，包括已确诊的IHD（冠心病）、先前心肌梗死（MI）、先前血管再成形术和其他合并症，以及SCA前伍用药物等。其中罹发SCA前被确诊的IHD589例（41％）。最后在校正诸混杂影响因素及变量和IHD诸公认危险因素等后观察比较分析，先前确诊的IHD与未确诊者间，继后存活率间差异。
