目的探讨奈玛特韦/利托那韦(Paxlovid)对肾功能不全新型冠状病毒感染(COVID-19)患者肾功能的影响。方法收集2022年3月23日至2023年4月30日在首都医科大学附属北京地坛医院住院且应用Paxlovid治疗的肾功能不全COVID-19患者临床资料,对Paxlovid相关急性肾损伤(AkI)的发生情况和Paxlovid治疗前后血尿素、血清肌酐(Scr)、血尿酸和估算肾小球滤过率(eGFR)的变化进行回顾性分析。结果共有386例患者纳入分析,男性220例(57.0%),女性166例(43.0%);中位年龄79岁;COVID-19分型为无症状感染者42例(10.9%),轻型175例(45.4%),普通型126例(32.6%),重型34例(8.8%),危重型9例(2.3%);轻度肾功能不全246例(63.7%),中度肾功能不全110例(28.5%),重度肾功能不全30例(7.8%)。386例患者中16例(4.1%)在Paxlovid治疗期间至停药后48-h内发生AkI,其中5例(1.3%)被判定与Paxlovid相关,均为≥70岁老年人且并用了肾毒性药物。386例患者Paxlovid治疗后与治疗前比较,Scr[(133.8±9.3)μmol/L比(111.7±6.6)μmol/L]、血尿酸[(335.9±7.1)μmol/L比(291.9±5.8)μmol/L]和eGFR[(63.4±1.1)ml/(min·1.73 m2)比(69.1±1.2)ml/(min·1.73 m2)]均有改善,差异有统计学意义(均PkI的发生。

A number of studies have been conducted on the improvement of the sound quality of electrical artificial laryngeal speech,the speech produced has been difficult to hear compared to a natural *** this reason,it is necessary to effectively improve the frequency characteristics of the input *** the present study,to improve the sound quality of vocalization using an electrical artificial larynx,first,a comparison of the frequency characteristics between the real and artificial voices was conducted,and three filters that can make the frequency characteristics of the artificial voice closer to those of a real voice were ***,the influence of these filters on the quality of the artificial voice was investigated via physical measurement and a subjective evaluation experiment targeted at Japanese five *** was found that the intelligibility of artificial/a/and/o/sounds was improved,whereas little improvement was observed in the case of/i/,/u/,and/e/.The obtained results confirmed the effect of optimizing the input signal into the vibration speaker and indicated areas for further improvement.

Clustering is one of the recently challenging tasks since there is an *** amount of data in scientific research and commercial applications. High quality and fast document clustering algorithms are in great demand to deal with large volume of data. The computational requirements for bringing such growing amount data to a central site for clustering are complex. The proposed algorithm uses optimal centroids for *** clustering based on Particle Swarm Optimization(PSO).PSO is used to take advantage of its global search ability to provide optimal centroids which aids in generating more compact clusters with improved accuracy. This proposed methodology utilizes Hadoop and Map Reduce framework which provides distributed storage and analysis to support data intensive distributed applications. experiments were performed on Reuter's and RCV1 document dataset which shows an improvement in accuracy with reduced execution time.

Ionomics is a high-throughput elemental profiling approach to study the molecular mechanistic basis underlying mineral nutrient and trace element composition （also known as the ionome） of living organisms. Since the concept of ionomics was first introduced more than 10 years ago, significant progress has been made in the identification of genes and gene networks that control the ionome. In this update, we summa- rize the progress made in using the ionomics approach over the last decade, including the identification of genes by forward genetics and the study of natural ionomic variation. We further discuss the potential application of ionomics to the investigation of the ecological functions of ionomic alleles in adaptation to the environment.

Male-mediated developmental toxicity has been of concern for many years. The public became aware of male-mediated developmental toxicity in the early 1990s when it was reported that men working at Sellafield might be causing leukemia in their children. Human and animal studies have contributed to our current understanding of male-mediated effects. Animal studies in the 1980s and 1990s suggested that genetic damage after radiation and chemical exposure might be transmitted to offspring. With the increasing understanding that there is histone retention and modification, protamine incorporation into the chromatin and DNA methylation in mature sperm and that spermatozoal RNA transcripts can play important roles in the epigenetic state of sperm, heritable studies began to be viewed differently. Recent reports using molecular approaches have demonstrated that DNA damage can be transmitted to babies from smoking fathers, and expanded simple tandem repeats minisatellite mutations were found in the germline of fathers who were exposed to radiation from the chernobyl nuclear power plant disaster. In epidemiological studies, it is possible to clarify whether damage is transmitted to the sons after exposure of the fathers. Paternally transmitted damage to the offspring is now recognized as a complex issue with genetic as well as epigenetic components.

china is physically and socio-economically susceptible to global warming-derived high temperature extremes because of its vast area and high urban population density. This article presents a scenario-based analysis method for high temperature extremes aimed at illustrating the latter＇s hazardous potential and exposure across china. Based on probability analysis, high temperature extreme scenarios with return periods of 5, 10, 20, and 50 years were designed, with a high temperature hazard index calculated by integrating two differen- tially-weighted extreme temperature indices （maximum temperature and high temperature days）. To perform the exposure analysis, a land use map was employed to determine the spatial distribution of susceptible human activities under the different scenarios. The results indicate that there are two heat-prone regions and a sub-hotspot occupying a relatively small land area. However, the societal and economic consequences of such an environmental im- pact upon the North china Plain and middle/lower Yangtze River Basin would be substantial due to the concentration of human activities in these areas.

Objective:Inhibition of HIV-1 integrase is an important strategy for the treatment of HIV and ***-fore, HIV-1 integrase inhibitory activity of 3-aroyl-1,1-dioxo-1,4,2-benzodithiazines has been analyzed with different physicochemical ***:In the present work,quantitative structure activity relationship studies were performed on a series of benzodithiazines as HIV-1 integrase inhibitors using the modeling software Win CAche version *** linear regression analysis was performed to derive quantitative structure activity relationship models which were further evaluated for statistical significance and predictive power by internal and external ***:The best QSAR models were having good correlation coefficient (r) with low standard error of estimation(See) and cross validated square of correlation coefficient (q^2 ).The robustness of the models was checked by Y-randomization test and they were identified as good predictive *** model for HIV integrase(wt) inhibitory activity of benzodithiazines suggest that the increase of dipole moment(Z) of molecules leads to reduce 3’processing and strand transfer inhibitory activity, substitution with high electro positive groups is conducive for the 3’processing inhibitory activity,and the increase in heat of formation is favorable for 3 -processing and strand transfer inhibitory ***: The model for HIV integrase(C65s) inhibitory activity of benzodithiazines suggest that the increase of dipole moment(X) of molecules leads to reduce 3’processing and strand transfer inhibitory activity,and the substitution with high hydrophobic groups is conducive for the 3’processing and strand transfer inhibitory.

Purpose-The purpose of this paper is to improve the privacy in healthcare datasets that hold sensitive *** a stop to privacy divulgence and bestowing relevant information to legitimate users are at the same time said to be of differing ***,the swift evolution of big data has put forward considerable ease to all chores of *** far as the big data era is concerned,propagation and information sharing are said to be the two main *** several research works performed on these aspects,with the incremental nature of data,the likelihood of privacy leakage is also substantially expanded through various benefits availed of big ***,safeguarding data privacy in a complicated environment has become a major ***/methodology/approach-In this study,a method called deep restricted additive homomorphic elGamal privacy preservation(DR-AHePP)to preserve the privacy of data even in case of incremental data is *** entropy-based differential privacy quasi identification and DR-AHePP algorithms are designed,respectively,for obtaining privacy-preserved minimum falsified quasi-identifier set and computationally efficient privacy-preserved ***-Analysis results using Diabetes 130-US hospitals illustrate that the proposed DR-AHePP method is more significant in preserving privacy on incremental data than existing *** analysis of state-of-the-art works with the objective to minimize information loss,false positive rate and execution time with higher accuracy is ***/value-The paper provides better performance using Diabetes 130-US hospitals for achieving high accuracy,low information loss and false positive *** result illustrates that the proposed method increases the accuracy by 4%and reduces the false positive rate and information loss by 25 and 35%,respectively,as compared to state-of-the-art works.
