Introduction: Hypopharyngeal cancers are ubiquitous cancers. These are very lymphophilic cancers;cervical lymph node metastases are present in 60% to 80% of patients at the time of diagnosis. Alcohol and tobacco have been implicated in their genesis. Objective: To study the epidemiological, clinical and paraclinical aspects of hypopharyngeal cancer in the ENT department of the CHU Gabriel Toure. Patients and Methods: It is a retrospective and prospective quantitative study covering a period of 4 years from June 2014 to July 2018;all patients who presented with hypopharyngeal cancer were included. Results: Thirty-five (35) patients were collected during the study period. The mean age of the patients was 44 ± 14 years with extremes of 16 and 78 years. The sex ratio was 0.47. Dysphagia was the reason for consultation at 100%. The average consultation time was 14 months, with extremes of 3 to 26 months. Smok.ng intoxication was found in 13 patients. The piriform sinus was interested in 52%. More than half of the patients were received at the advanced stage (T3 - T4). The histology was dominated by the well-differentiated epidermal carcinoma (91%). Conclusion: Hypopharyngeal cancer is common and occurs at all ages. T3 - T4 tumors are bad factors, hence early diagnosis and appropriate management are important for improving the prognosis.

Digestive hemorrhage by rupture of esophageal Varices is common and has a pejorative prognosis in our context. Purpose: The main purpose of this work.was to study the digestive bleeding by esophageal varices and prognosis value of blood transfusion in the Hospital of Gabriel Touré. Methodology: It was a prospective study that took.place in the service of Hepato-gastroenterology of Gabriel Touré Hospital from June 2016 to May 2017 and from November 2017 to August 2018. Results: At the end of the study, 77 patients met the inclusion criteria out of 1396 patients hospitalized during the same period. Varices bleeding represented a prevalence of 5.5% among hospitalized patients during the same period. The average age of our patients was 46.58 ± 15.09 years. The male sex was more reported in our study with a prevalence of 67.5%. At admission, 63.2% had clinical anemia, 58.4% low arterial pressure and 50.6% hemoglobin rate less than 7 g/dL. Blood transfusion was indicated in 47 patients (61%). The mortality rate was 23.4% and was comparable in both groups (p = 0.0990). Early rebleeding was significantly observed in the case of transfusion (p = 0.0452). Hepatic encephalopathy was the leading cause of death of our patients with 72.2%. Conclusion: Digestive bleeding by esophageal varices is a worsen complication in cirrhosis in hospital setting. Transfusion has not significantly improved the prognosis of our patients.

The nephroblastoma is the third pediatric cancer in Mali, this study aimed to describe the prevalence and prognosis of nephroblastoma relapses. Methods: It was a descriptive retrospective study over a 10-year period from January 2005 to March 2015. We collected children aged 0 to 15 years followed for relapse of nephroblastoma in the pediatric oncology unit of university hospital center (UHC) Gabriel Toure. Results: The frequency of relapse of nephroblastoma was 7.4% (19 cases) whose mean age was 42 months with a sex ratio of 1.3. The relapse occurred before the end of the postoperative course in 16% of cases (3 patients). It was local recurrence in 52% of cases (10 patients), pulmonary 16% (3 patients), and hepatic 11% (2 patients). According to the SIOP classification, 47% of patients were diagnosed in stage III (9 patients) and 21% (4 patients) in stage IV. The tumor was high risk.in 37%. Palliative chemotherapy was performed in 63% of the patients (12 patients) and the remaining seven were put on a high risk.diet. The overall survival at 5 years was 21% or 4 patients. Conclusion: Our results showed all the difficulties in the management of nephroblastoma relapses in our context.

Malnutrition is a major public health problem in Mali, along with the country’s political and security instability. We initiated this work.with the objective of determining the frequency as well as the risk.factors favoring the occurrence of malnutrition in children aged 6 - 59 months hospitalized in the pediatrics department of the Gabriel Touré University Hospital in Bamako, country reference service to identify potential interventions to plan. A cross-sectional study was carried out over a period of 4 months. A bivariate logistic regression analysis allowed us to identify risk factors with degree of significance if p 0.05. During the study period, 2888 children were hospitalized, including 348 aged 6 to 59 months, or a frequency of 12.04%. One in two children was malnourished, i.e. a frequency of 50%. The objective of our study was to evaluate hepatitis B virus (HBV) infection in an urban population. This longitudinal study was conducted in Bamak. District and k.ti Commune. After a preparatory phase, the persons who accepted the protocol were assessed for HBsAg. HBsAg carriers had blood collection for HBeAg assay, viral load assessment, genotyping, DNA mutation testing, and severity of hepatic fibrosis and necrosis. At the end of this study, 1475 persons were included, of which 195 had HBsAg positive confirmed, that is to say 13.97%. The mean age of HBsAg positive patients was 35.11 ± 11.12 years with a sex ratio of 2.68. HBeAg was found in 8.9% of the patients tested for this antigen. The viral load was undetectable in 10.52% of patients and greater than 2000 IU/mL in 32.24% of cases. Fibrosis ≥ F2 and necrosis ≥ A2 were found in respectively 19.72% and 6.80% of cases. Genotype E was found in 91.6 patients and an R249S mutation observed in 39.04% of cases. Conclusion: HBV infection has a serious impact on socio-economic development in Mali because it affects mainly the young male population, hence the need to organize preventive measures effectively.

Objective: To study the pulmonary complications of major sick.e cell syndromes in children aged 6 months to 15 years followed at the Department of Pediatrics of Gabriel Toure University Hospital. Materials and methods: This was a retrospective study from January 1st, 2017 to February 28, 2018 and a prospective study from March 1, 2018 to December 31, 2019, concerning sick.e cell disease children followed at the Gabriel Touré University Hospital. All children with sick.e cell disease confirmed by hemoglobin electrophoresis, aged 6 months to 15 years with a pulmonary complication and admitted to pediatrics were included. Were not included: 1) Infants and children with sick.e cell disease not presenting a pulmonary complication coming only to their routine follow-up. 2) Sick.e cell patients over 16 years of age and those whose parents do not consent. 3) Those who have not done electrophoresis. Results: During the study period, we were able to include 45 sick.e cell children with a pulmonary complication. The frequency of pulmonary complications was 13.76%, involving homozygotes in 91.11% of cases. The age group 6 - 10 years predominated with 46.66% and the sex ratio was 0.45. Vaso-occlusive crisis was the most frequent reason for consultation with 35.56%. Respiratory distress (80%), fever (66.67%), crepitus rales (64.44%) and chest pain (60%) were the most frequent clinical signs. The main pulmonary complication was acute chest syndrome with 86.67%. Treatment was based on hydration (91.91%), analgesics (91.91%) and antibiotics (73.33%). Lethality was 4.44%. Conclusion: Pulmonary complications of sick.e cell disease are serious and constitute the main cause of mortality in our context.

The blood count is an easily achievable routine exam and will it have specifics in the event of a neonatal bacterial infection? Hence, the present study with the objective of determining the profile of the hemogram of newborns hospitalized for early bacterial neonatal infection. Material and methods: This was a cross-sectional study that took.place from June 27 to September 03, 2016 in the neonatology department of teaching hospital Gabriel Toure. Included were all neonates hospitalized for early neonatal bacterial infection (ENBI) and who had a blood count. Results: We included 227 patients, 64.8% of whom were premature. The sex ratio was 1.4. The infants were less than 24 hours old in 93.6% of the cases. The mean hemoglobin level was 16.435 g/dl [8.8 - 22.26]. Erythrocytopenia was found in 18.5% of cases. Anemia was present in 17% of newborns. The average leuk.cyte was 15.228·103/mm3 [1.4 - 72]. Hyperleuk.cytosis and leuk.penia were found in 12.32% and 6.6% respectively. Neutropenia and lymphopenia were present in 14.5% and 30.8%. There was a correlation between leuk.cytosis of negative blood cultures (23/27) (p = 0.030). For Neutrophils, neutrophilia was more observed in term neonates and neutropenia in premature infants (p = 0.03). Monocytosis was present in 13.6% of cases. One quarter (25.5%) of newborns had thrombocytopenia. Conclusion: Hematological variations did not allow a specific profile of newborns hospitalized for early neonatal bacterial infection to be identified.

The objectives of this work.were to evaluate the surgical activities carried out in the general surgery department of the Reference Health Center of Commune I of Bamak., to describe the sociodemographic characteristics of the operated patients, to determine the main pathologies encountered and to evaluate qualitatively the result of the treatment. In order to improve performance, and the quality of care, and to identify common pathologies in the surgical department, we undertook.a retrospective study on surgical activities from January 2009 to December 2010. At the end of this study, out of 474 men and 187 women (equal sex ratio 2.53);we were able to determine the frequency of surgical pathologies. Farmers, housewives and pupils/students were the most represented with 25.9% respectively;20% and 13.3%. The most frequently observed pathologies were wall hernia (44.8%), prostate adenoma (12%) and acute appendicitis (10.5%). The average length of hospitalization was 3.43 days. Infectious complications affected 25 patients (3.8% of cases) and a death rate of 0.45% (i.e. 3 patients). The average cost of care was 53,500 FCFA. Indeed, the reality of surgical practice in health centers was not the same because of the level of sk.lls of practicing surgeons.

Subcutaneous Emphysema Associated with Pneumomediastinum and Complicated Pneumopericardium in a 14-Month-Old Infant

Pneumomediastinum is sometimes observed in adult patients but its occurrence in pediatric patients (especially infants) is very rare. We here report a 14-month-old male infant who had subcutaneous emphysema, pneumomediastinum, and importantly, pneumopericardium. He had no particular past histories. He abruptly had cough, fever, and eruption on the abdomen. Computed tomography and echocardiography revealed pneumomediastinum and pneumopericardium. Antibiotics, rest, and supportive therapy ameliorated the condition. We must be aware that pneumomediastinum, and importantly pneumopericardium, can be present in a pediatric patient with subcutaneous emphysema. The infant’s symptoms disappeared under strict monitoring of respiratory status, nasal oxygen therapy and antibiotic therapy.
