Linear Accelerator Based External Beam Radiotherapy in Glomus Jugulare Tumour: A Retrospective Review from a Tertiary Cancer Hospital in kenya

Objective: Tumours originating from jugular bulb, carotid bifurcation, Vagus nerve are collectively called Paragangliomas. They are slow growing, essentially benign tumours, but can be detrimental if untreated. There is limited evidence on the effectiveness of fractionated radiotherapy in the management ofs Glomus jugulare tumours. The aim of this study is to determine the efficacy of Linear accelerator based fractionated external beam radiotherapy on unilateral inoperable Glomus jugulare paragangliomas. Method: This is a retrospective analysis of all the 12 cases of inoperable, unilateral Glomus jugulare tumours treated during the period 2011-2016 at a tertiary cancer centre in kenya. Minimum follow up duration was 3 years. Patient characteristics, disease staging, immediate complications and therapeutic efficacy were analysed from the case files. Results: The 12 patients diagnosed with inoperable Glomus jugulare tumours reported in this period were treated with external beam radiotherapy to a tumour dose of 54 Gy in 30 fractions over a period of 6 weeks using IMRT technique in 6 MV Linear accelerator. 2/3rd of the patients were females in 5th and 6th decade of life. Onset of first symptom to initiation of treatment was found to be 1.7 years. Headache, earache, and tinnitus were the main complaints. No major side effects were recorded during therapy. Mean length of the tumour in its maximum dimension at the time of diagnosis was 4.5 cm. At the end of one-year post therapy, a mean reduction of 6.5 mm in the tumour length was observed, (Range: 0 - 15 mm). Tumour size remained static for a year and thereafter a slow growth pattern of 1mm per year was observed. Conclusion: Fractionated external beam radiotherapy is an effective and non-invasive treatment for advanced, inoperable Glomus jugulare paragangliomas. Trigonelline protects the cardiocyte from hydrogen peroxide induced apoptosis in H9c2 cells

Objective:To elucidate the key parameters associated with hydrogen peroxide induced oxidative stress and investigates the mechanism of trigonelline(TG)for reducing the H_2O_2induced toxicity in H9c2 ***:Cytotoxicity and antioxidant activity of TG was assessed by EZ-CYTOX *** extraction and cDNA synthesized according to the kit manufacture *** was measured by the Flowcytometry,general PCR and ***:It was found that the TG significantly rescued the morphology of the H9c2 *** of cells with TG attenuated H_2O_2 induced cell deaths and improved the antioxidant *** addition,TG regulated the apoptotic gene caspase-3,caspase-9 and anti-apoptotic gene ***-XL during H_2O_2 induced oxidative stress in H9c2 *** results were comparable with quercetin *** evident,flow cytometer results also confirmed the TG significantly reduced the H_2O_2 induced necrosis and apoptosis in H9c2 ***,further increment of TG concentration against H_2O_2 could induce the necrosis and apoptosis along with H_2O_***:It is suggested that less than 125μM of TG could protect the cells from H_2O_2 induced cell damage by down regulating the caspases and up regulating the Bcl-2 and Bcl-XL ***,we suggest the trigonelline could be useful for treatment of oxidative stress mediated cardiovascular diseases in future.

A cAMP and CREB-mediated feed-forward mechanism regulates GSk3β in polycystic kidney disease

Glycogen synthase kinase 3β(GSk3β),a serine/threonine protein kinase,is commonly known to be regulated at the level of its ***,in some diseases including polycystic kidney disease(PkD),GSk3βexpression is increased and plays a pathophysiological *** current studies aimed to determine the mechanism for the increased GSk3βexpression in PkD and its significance to disease *** mouse models of PkD,increases in renal GSk3βcorresponded with increases in renal cAMP levels and disease *** vivo and in vitro studies revealed that GSk3βis a cAMP-responsive gene,and elevated cAMP levels,as seen in PkD,can increase GSk3β*** normal mice,vasopressin signaling induced by water deprivation increased GSk3βexpression,which decreased following *** of the GSk3βpromoter revealed five potential binding sites for the transcription factor,cAMP response element binding protein(CREB).CREB was found to bind to GSk3βpromoter and essential for cAMP-mediated regulation of GSk3β.Importantly,this regulation was demonstrated to be part of a feedforward loop in which cAMP through CREB regulates GSk3βexpression,and GSk3βin turn positively regulates cAMP ***3βor CREB inhibition reduced transepithelial fluid secretion and cyst expansion in ***,disruption at any point of this destructive cycle may be therapeutically useful to reduce cyst expansion and preserve renal function in PkD.

Nowadays, mobile agents are an effective paradigm for accessing the information in distributed applications, especially in a dynamic network environment such as Internet businesses. In such kind of Internet based applications, access must be secure and authentication takes a vital role to avoid malicious use of the system. This kind of security has been provided by several previously proposed algorithms based on RSA digital signature cryptography. However, the computational time for performing encryption and decryption operations in the past literatures is very high. In this paper, we propose an anonymous authentication scheme which potentially reduces the overall computation time needed for verifying the legitimacy of the users. Comparing with previous anonymous authentication schemes, our proposed scheme provides more security and it is effective in terms of computation cost. The experimental results show that the proposed method authenticates the users with low computational time significantly.

Underdiagnosed Distal Colonic Volvuli on MDCT Imaging

Volvuli affecting the distal colon, specifically the splenic flexure onwards is underdiagnosed pathologies with non-specific clinical features. Delay in diagnosing these pathologies can lead to significant morbidity and mortality in terms of ischemia and perforation of bowel. Multi detector computed tomography (MDCT) with intravenous contrast is an accurate and reliable imaging modality allowing prompt diagnosis and management. We present an overview of the radiological features found in acute presentations of different types of colonic volvuli.

Administration of grape seed extract alleviates age-associated decline in ubiquitin-proteasome system and cardiomyocyte apoptosis in rats

Effective clearance of oxidized, damaged, and/or misfolded proteins in the cell by the ubiquitin-proteasome system (UPS) is critical for cell homeostasis, survival and function. We hypothesized that in the aging heart, generation of free radicals could impair UPS where the associated build-up of polyubiquitinated proteins could trigger programmed cell death. To test this, we used young (4 months old) and aged (24 months old) rats to analyze polyubiquitinated proteins, proteasome activity and programmed cell death in the ventricular tissue samples. Our studies reveal excessive deposition of polyubiquitinated proteins in the ventricular tissue extracts of old rats when compared to younger rats. The increased ubiquitination was accompanied by a significant decrease in 20S proteasome activity. Since the loss of proteasome-mediated clearance of ubiquitinated proteins is linked to programmed cell death, we measured TUNEL activity in aged rat heart and compared with younger animals. Aged animal hearts showed a substantial increase in programmed cell death as evidenced by TUNEL positive nuclei and DNA fragmentation. Analyses of cell death/survival pathways support our findings in terms of age-associated increase in the nuclear localization of p53, Bax/Bcl2 ratio and cleaved (active) caspase-3 and decreased expression of cellular inhibitor of apoptosis (cIAP1). Administration of grape seed extract (GSE) as a source of antioxidants significantly reduced these age-associated deleterious changes suggesting that free radicals primarily contribute to impaired UPS function and increased programmed cell death and that administration of antioxidants during aging could protect cardiac muscle cells and preserve ventricular function.
