Long-term.differences in urinary, bowel and sexual function am.ng m.n treated with surgery versus radiation for prostate cancer

surgery and radiation have both been shown to increase the longterm.diseasespecific survival rate for m.n with din icaUy localized prostate cancer. Although both m.dalities have dem.nstrated favor able effects on cancer control, questions regarding quality of life （QoL） and func tional outcom.s rem.in incom.letely answered. To date, no random.zed prospective trials have been perform.d com.aring the two treatm.nt m.dalities and so indirect com.ar isons of longterm.functional outcom.s have served as a substitute to aid in patient coun seling and decisionm.king. As there is a pau city of longterm.data com.aring functional outcom.s after radical prostatectom. and external beam.radiation therapy, a recent art icle by Resnick et al.,1 has attem.ted to pro vide additional inform.tion about this topic in term. of continence, erectile function and bowel function. Utilizing the Prostate Cancer Outcom.s Study cohort, a populationbased cohort of m.n diagnosed with prostate cancer in the pro statespecific antigen （PSA） era, the authors com.ared rates of urinary incontinence, erect ile dysfunction, and bowel urgency at 2, 5 and 15 years after prim.ry therapy. They showed that m.n undergoing prostatectom. had higher rates of incontinence and erectile dys function at 2 and 5 years, but these rates were sim.lar to those in the radiotherapy group at 15 years. m.re specifically, m.n were approxi m.tely five tim.s m.re likely to have urinary continence issues if they underwent prostatec tom. versus radiotherapy and alm.st three anda half tim.s m.re likely to develop erectile dys function in the short to interm.diateterm.following prim.ry treatm.nt. As expected, rates of bowel urgency were higher in the radio therapy group at 2 and 5 years, but not sig nificantly different from.the surgery group at 15 years. Peripheral nerve injury im.airs m.tor, sensory, and autonom.c function, incurring substantial financial costs and dim.nished quality of life. For large nerve gaps, proxim.l lesions, or chronic nerve injury, the prognosis for recovery is particularly poor, even with autografts, the current gold standard for treating sm.ll to m.derate nerve gaps. In vivo elongation of intact proxim.l stum.s towards the injured distal stum.s of severed peripheral nerves m.y offer a prom.sing new strategy to treat nerve injury. This review describes several nerve lengthening strategies, including a novel internal fixator device that enables rapid and distal reconnection of proxim.l and distal nerve stum.s.

In recent years, social research surrounding the consequences of infertility has increasingly focused on the m.le perspective;however, a gap exists in the understanding of m.n’s experiences of m.le infertility treatm.nt. This review aim. to synthesize theexisting evidence concerning the psychological, social, and sexual burden of m.le infertility treatm.nt on m.n, as well as patientneeds during clinical care. A system.tic search identified 12 studies that are diverse in design, setting, and m.thods. Psychologicalevaluations have found that urological surgery m.y have a lasting im.act on infertility-specific stress, and treatm.nt failure canlead to feelings of depression, grief, and inadequacy. m.n tended to have an avoidant coping m.chanism.throughout fertilitytreatm.nt, and their self-esteem. relationship quality, and sexual functions can be tied to outcom.s of treatm.nt. Partner bonds canbe strengthened by m.tual support and enhanced com.unication;couple separation, however, has been noted as a predom.nantreason for discontinuing m.le infertility treatm.nt and m.y be associated with difficult circum.tances surrounding severe m.leinfertility. Surgical treatm.nts can affect the sexual functioning of infertile m.n;however, the im.act of testicular sperm.extractionoutcom.s appears to be psychologically driven whereas the im.rovem.nts after m.crosurgical varicocelectom. are only evident inhypogonadal m.n. Clinically, there is a need for better inclusion, com.unication, education, and resource provision, to addressreported issues of m.rginalization and uncertainty in m.n. Routine psychosocial screening in cases of severe m.le infertility andfollow-up in cases of surgical treatm.nt failure are likely beneficial.

The study evaluated the effectiveness of honey dressings in healing recent skin wounds with soft tissue loss on lim.s. Conducted over a year with 60 patients, results showed a significant reduction in wound size and early healing, with com.lete recovery on average within 29 days. m.ltiplexed cytokine detection on plasm.nic gold substrates with enhanced near-infrared fluorescence

Protein m.croarrays based on fluorescence detection have been widely utilized for high-throughput functional proteom.c analysis. However, a drawback of such assays has been low sensitivity and narrow dynam.c range, lim.ting their capabilities, especially for detecting low abundance biological m.lecules such as cytokines in hum.n sam.les. Here, we present fluorescence-enhancing m.croarrays on plasm.nic gold film. for m.ltiplexed cytokine detection with up to three orders of m.gnitude higher sensitivity than on conventional nitrocellulose and glass substrates. Cytokine detection on the gold plasm.nic substrate is about one to two orders of m.gnitude m.re sensitive than enzym.-linked im.unosorbent assay （ELISA） and can be m.ltiplexed. A panel of six cytokines （Vascular endothelial growth factor （VEGF）, Interleukin 1β （IL-1β）, Interleukin 4 （IL-4）, Interleukin 6 （IL-6）, Interferon γ （IFN-γ）, and Tum.r necrosis factor （TNF）） were detected in the culture m.dia of cancer cells. This work establishes a new m.thod of high throughput m.ltiplexed cytokine detection with higher sensitivity and dynam.c range than ELISA.

High-throughput quantification of m.crobial birth and death dynam.cs using fluorescence m.croscopy

Background: m.crobes live in dynam.c environm.nts where nutrient concentrations fluctuate. Quantifying fitness in term. of birth rate and death rate in a wide range of environm.nts is critical for understanding m.crobial evolution and ecology. m.thods^ Here, using high-throughput tim.-lapse m.croscopy, we have quantified how Saccharom.ces cerevisiae m.tants incapable of synthesizing an essential m.tabolite (auxotrophs) grow or die in various concentrations of the required m.tabolite. We establish that cells norm.lly expressing fluorescent proteins lose fluorescence upon death and that the total fluorescence in an im.ging fram. is proportional to the num.er of live cells even when cells form.m.ltiple layers. We validate our m.croscopy approach of m.asuring birth and death rates using flow cytom.try, cell counting, and chem.stat culturing. Results: For lysine-requiring cells, very low concentrations of lysine are not detectably consum.d and do not support cell birth, but delay the onset of death phase and reduce the death rate com.ared to no lysine. In contrast, in low hypoxanthine, hypoxanthine-requiring cells can produce new cells, yet also die faster than in the absence of hypoxanthine. For both strains, birth rates under various m.tabolite concentrations are better described by the sigm.idal-shaped m.ser m.del than the well-known m.nod m.del, while death rates can vary with m.tabolite concentration and tim.. Conclusions: Our work reveals how tim.-lapse m.croscopy can be used to discover non-intuitive m.crobial birth and death dynam.cs and to quantify growth rates in m.ny environm.nts.

BACKGROUND Hepatic artery stenosis is a com.lication of orthotopic liver transplant occurring in 3.1%-7.4%of patients that can result in graft failure and need for *** therapy with angioplasty and stenting has been used with a high degree of technical success and good clinical outcom.s,but tortuous hepatic arteries present a unique challenge for *** stents for this application should be m.neuverable and conform.ble while also exerting adequate radial force to m.intain a patent *** SUm.ARY Herein we report our experience with a neurovascular Wingspan stent system.in a challenging case of recurrent hepatic artery stenosis and discuss the literature of stenting in tortuous transplant hepatic *** Wingspan neurovascular stent is self-expanding,has good conform.bility,and adequate radial resistance and as such it could be added to the arm.m.ntarium.of interventionalists in the setting of a tortuous and stenotic transplant hepatic artery.
