Sulforaphane Attenuates CD36-mediated Platelet Hyperreactivity through Modulating cAMP/PKA/NOX2 Signaling in Hyperlipidemic Conditions

Hyperlipidemia is a risk factor for clinically significant thrombotic events in cardiovascular diseases. Platelet reactivity in hyperlipidemic conditions is enhanced when platelet scavenger receptor CD36 recognizes oxidized lipids in oxidized low-density lipoprotein (ox-LDL) particles， a process that induces atherothrombosis. Sulforaphane (SFN) is a dietary isothiocyanate enriched in cruciferous vegetables and exerts multiple biological activities. The current study sought to investigate the efficacy of SFN on platelet hyperreactivity under hyperlipidemic conditions in vitro and in vivo. Using a series of platelet functional assays in human platelets in vitro， we demonstrated that SFN attenuated ox-LDL-increased platelet aggregation and activation (surface CD62P expression). Mechanistically， studies using pharmacological inhibitors clarified that these inhibitory effects of SFN were mainly modulated by down-regulating CD36-mediated activation of Src kinases， leading to enhanced activation of cAMP/PKA signaling， and resultant inhibition of NAD phosphate oxidase 2 (NOX2)-dependent generation of reactive oxygen species (ROS). Moreover， 12-week supplementation of SFN-enriched broccoli sprout extract (BSE， 0.06% diet) in hyperlipidemic C57BL/6J mice also decreased platelet hyperreactivity. Studies using pharmacological inhibitors of CD36， PKA and NOX2 showed that the efficacy of BSE supplementation was mainly through modulating CD36-mediated the cAMP/PKA/NOX2 signaling. Thus， through modulating the cAMP/PKA/NOX2 pathway and attenuating CD36-mediated platelet hyperreactivity， SFN may play important protective roles in atherothrombosis under hyperlipidemic conditions.

一种新型膈肌移动度测量方法的验证研究:经相控阵探头评估肝肾及脾肾切面膈肌移动度

目的探讨使用经肝肾/脾肾切面超声测量膈肌移动度(DE)作为评估膈肌功能新方法的准确性及可重复性。方法通过招募方式纳入12名健康受试者。每位受试者均由4名不同经验水平的操作者分别采用传统方法(肝下/脾下切面)和新方法(肝肾/脾肾切面)测量DE,其中2名有经验的操作者为重症临床医师,膈肌超声操作大于50例;另外2名无经验的操作者为呼吸治疗师,膈肌超声操作小于10例,均接受2 h理论及操作培训,考核通过后再开始试验操作。操作者首先应用传统方法,采用1.5~6.0 MHz凸阵探头,选择M超模式,将取样线垂直于膈肌置于移动度最大处,于受试者平静呼吸时测量肝下/脾下切面DE;然后应用新型测量方法,采用1.6~4.5 MHz相控阵探头,经腋中线观察膈肌沿头尾方向的移动情况,选择解剖M超模式,将取样线垂直于膈肌置于肾脏中点水平,于受试者平静呼吸时测量肝肾/脾肾切面DE。肝下及肝肾切面用于评估右侧膈肌,脾下及脾肾切面用于评估左侧膈肌。采用Deming法对不同切面测量的DE进行相关性分析;采用Bland-Altman法进行一致性分析,临床可接受的一致性界限定义为不存在固定偏倚和比例偏倚,且测量值之间差异的2个标准差小于测量值平均值的40%,即%一致性界限=测量值之间差异的2个标准差/测量值平均值×100%。结果4名操作者分别对12名受试者进行4个切面DE的图像扫查,其中肝肾切面及脾肾切面DE获取率均高达100%(48/48),肝下切面次之〔91.7%(44/48)〕,脾下切面最低〔66.7%(32/48)〕,提示对于左侧膈肌的评估,脾肾切面DE获取率显著高于传统脾下切面(P0.05),且均显著高于传统方法经肝下切面和脾下切面测量的DE(cm:肝肾切面比肝下切面为1.64±0.10比1.44±0.09,脾肾切面比脾下切面为1.55±0.14比1.09±0.14,均P40%)。4名操作者在肝下切面、脾下切面、肝肾切面和脾肾切面测量的DE差异均无统计学意义(cm:肝下切面分别为1.49±0.34、1.44±0.37、1.43±0.30、1.40±0.27,脾下切面分别为1.10±0.36、1.05±0.18、1.09±0.22、1.06±0.26,肝肾切面分别为1.67±0.43、1.57±0.34、1.63±0.32、1.66±0.36,脾肾切面分别为1.45±0.33、1.48±0.34、1.50±0.24、1.65±0.26,均P>0.05);根据临床可接受的一致性界限范围,4名操作者在4个切面测量的DE一致性均较好(%一致性界限均<40%)。结论经肝肾/脾肾切面测量DE是一种准确、可重复性好、获取率高的新方法,可作为传统经肝下/脾下切面测量DE的替代方法。

目的基于“人口公平性”维度分析上海农村中医药服务的资源配置情况,为进一步优化上海农村卫生资源配置及中医药服务发展提供依据。方法采用内容分析法对农村中医药服务政策文本进行计量分析。采用洛伦兹曲线、基尼系数、泰尔指数对上海农村中医药卫生资源公平性进行评价。结果从2016年2月22日至2022年9月,全国农村中医药服务政策文件共纳入27份71条;上海1235家样本村卫生室2019年中医诊疗设备、中医适宜技术、中医诊疗人次、中医执业(助理)医师数、中药配置按人口配置的基尼系数分别为0.17、0.20、0.40、0.80、0.80;总泰尔指数介于0.0906~0.3969,泰尔指数贡献率除中医诊疗人次外,其余四组指标组内大于组间,与基尼系数的结果保持一致。结论上海农村中医药服务发展在全国处于前列,中医药发展政策环境良好,但在某些方面存在卫生资源配置不公平。影响上海农村中医药卫生资源配置公平性的主要原因是区域内的不公平。

Fragmentation/disassembly of fiber-like micelles generated by living crystalline-driven self-assembly(CDSA)is usually encountered in aqueous media,which hinders the applications of ***,we report the generation of uniform fiber-like micelles consisting of a𝜋Л-conjugated oligo(p-phenylenevinylene)core and a crosslinking silica shell with grafted poly(ethylene glycol)(PEG)chains by the combination of living CDSA,silica chemistry and surface grafting-onto *** to the presence of crosslinking silica shell and the outmost PEG chains,the resulting micelles exhibit excellent dispersity and colloidal stability in PBS buffer,BSA aqueous solution and upon heating at 80℃ for 2 h without micellar fragmentation/*** micelles also show negligible cytotoxicity toward both HeLa cervical cancer and HEK239T human embryonic kidney cell ***,micelles with Ln of 156 nm show the“stealth”property with no significant uptake by HeLa cells,whereas some certain amounts of micelles with Ln of 535 nm can penetrate into HeLa cells,showing length-dependent cellular uptake *** results provide a route to prepare uniform,colloidally stable fiber-like nanostructures with tunable length and functions derived for biomedical applications.

Study on creep deformation and energy development of underground surrounding rock under four‐dimensional support

There is an urgent need to develop optimal solutions for deformation control of deep high‐stress roadways,one of the critical problems in underground *** previously proposed four‐dimensional support(hereinafter 4D support),as a new support technology,can set the roadway surrounding rock under three‐dimensional pressure in the new balanced structure,and prevent instability of surrounding rock in underground ***,the influence of roadway depth and creep deformation on the surrounding rock supported by 4D support is still *** study investigated the influence of roadway depth and creep deformation time on the instability of surrounding rock by analyzing the energy *** elastic strain energy was analyzed using the program redeveloped in *** numerical simulation results indicate that the combined support mode of 4D roof supports and conventional side supports is highly applicable to the stability control of surrounding rock with a roadway depth exceeding 520 *** the increase of roadway depth,4D support can effectively restrain the area and depth of plastic deformation in the surrounding ***,4D support limits the accumulation range and rate of elastic strain energy as the creep deformation time increases.4D support can effectively reduce the plastic deformation of roadway surrounding rock and maintain the stability for a long deformation period of 6 *** confirmed by in situ monitoring results,4D support is more effective for the long‐term stability control of surrounding rock than conventional support.

Characteristics of Elderly-Onset Rheumatoid Arthritis Patients With COVID-19

This study aimed to investigate the clinical characteristics of elderly-onset rheumatoid arthritis(EORA)patients with *** ret-rospectively enrolled EORA patients diagnosed with COVID-19 from December 2022 through February 2023 and followed them *** characteristics of the patients and clinical data on COVID-19 were collected,and EORA-related data at the onset of rheu-matoid arthritis and during COVID-19 episodes were evaluated.A total of 102 EORA outpatients were enrolled and stratified into the COVID-19 group(n=60)and the non–COVID-19 group(n=42).In the COVID-19 group,56(93.3%)had symptoms,8(13.3%)had viral pneumonia,and 4(6.7%)developed severe *** logistic regression analysis showed that lack of COVID-19 vac-cination(P=0.016 and P=0.030,respectively)and having interstitial lung disease(P=0.013 and P=0.001,respectively)were inde-pendent risk factors for viral pneumonia and severe *** with the general population,EORA patients did not show significantly higher susceptibility to COVID-19 or a higher risk of severe ***,EORA patients with interstitial lung disease require special consideration during COVID-19 episodes.
