Optimal sequential therapy using tyrosine kinase inhibitors as the first-line treatment in patients with metastatic renal cell carcinoma: A nationwide multicenter study

Objective:The purpose of the study was to identify the best sequence of therapy beginning with a tyrosine kinase inhibitor(Tki)as the first-line therapy for patients with metastatic renal cell carcinoma(mRCC)in terms of overall survival(OS),progression-free survival(PFS),and rates of discontinuation and adverse effects during the treatment ***:This is a retrospective,nationwide multicenter study of patients with mRCC after diagnosis at 10 different tertiary medical centers in Korea from January 1992 to December *** focused on patients at either“favorable”or“intermediate”risk according to the International mRCC Database Consortium criteria,and they were followed up(median 335 days).Finally,a total of 1409 patients were selected as the study *** generated a Cox proportional hazard model adjusted for covariates,and the different therapy schemes were statistically tested in terms of OS as well as *** addition,frequencies of discontinuation and adverse events were compared among the therapy ***:Of the primary patterns of treatment sequences(24 sequences),“sunitinib epazopanib”and“sunitinibeeverolimuseimmunotherapy”showed the most beneficial results in both OS and PFS with significantly lower hazards than“sunitinib”,which is the most commonly treated agent in *** that the“TkieTki”structure showed relatively higher discontinuation rates with higher adverse effects,the overall beneficial sequence would be“sunitinibeeverolimuseimmunotherapy”.Conclusion:Among several sequential therapy starting with Tkis,“sunitinibeeverolimuse immunotherapy”was found to be the best scheme for mRCC patients with“favorable”or“intermediate”risks.

Analysis of expanded criteria to select candidates for active surveillance of low-risk prostate cancer

We aimed to analyze the value of each criterion for clinically insignificant prostate cancer （PCa） in the selection of men for active surveillance （AS） of low-risk PCa. We identified 532 men who were treated with radical prostatectomy from 2006 to 2013 who met 4 or all 5 of the criteria for clinically insignificant PCa （clinical stage 〈 T1, prostate specific antigen [PSA] density 0.15, biopsy Gleason score 〈 6, number of positive biopsy cores 〈 2, and no core with 〉 50% involvement） and analyzed their pathologic and biochemical outcomes. Patients who met all 5 criteria for clinically insignificant PCa were designated as group A （n -- 172）, and those who met 4 of 5 criteria were designated as group B （n = 360）. The association of each criterion with adverse pathologic features was assessed via logistic regression analyses. Comparison of group A and B and also logistic regression analyses showed that PSA density 0.15 ng ml-1 and high （〉7） biopsy Gleason score were associated with adverse pathologic features. Higher （〉 Tlc） clinical stage was not associated with any adverse pathologic features. Although 〈 3 positive cores were not associated with any adverse pathology, 〉4 positive cores were associated with higher risk of extracapsular extension. Among potential candidates for AS, PSA density ~ 0.15 ng ml-~ and biopsy Gleason score ~ 6 pose significantly higher risks of harboring more aggressive disease. The eligibility criteria for AS may be expanded to include men with clinical stage T2 tumor and 3 positive cores.

Novel ternary metals-based telluride electrocatalyst with synergistic effects of high valence non-3d metal and oxophilic Te for pH-universal hydrogen evolution reaction

Electrocatalyst designs based on oxophilic foreign atoms are considered a promising approach for developing efficient pH-universal hydrogen evolution reaction(HER)electrocatalysts by overcoming the sluggish alkaline HER ***,we design ternary transition metals-based nickel telluride(Mo WNi Te)catalysts consisting of high valence non-3d Mo and W metals and oxophilic Te as a first demonstration of non-precious heterogeneous electrocatalysts following the bifunctional *** Mo WNi Te showed excellent HER catalytic performance with overpotentials of 72,125,and 182 mV to reach the current densities of 10,100,and 1000 mA cm^(-2),respectively,and the corresponding Tafel slope of 47,52,and 58 mV dec-1in alkaline media,which is much superior to commercial Pt/***,the HER performance of Mo WNi Te is well maintained up to 3000 h at the current density of 100 mA cm^(-2).It is further demonstrated that the Mo WNi Te exhibits remarkable HER activities with an overpotential of 45 mV(31 mV)and Tafel slope of 60 mV dec-1(34 mV dec-1)at 10 mA cm^(-2)in neutral(acid)*** superior HER performance of Mo WNi Te is attributed to the electronic structure modulation,inducing highly active low valence states by the incorporation of high valence non-3d transition *** is also attributed to the oxophilic effect of Te,accelerating water dissociation kinetics through a bifunctional catalytic mechanism in alkaline *** functional theory calculations further reveal that such synergistic effects lead to reduced free energy for an efficient water dissociation process,resulting in remarkable HER catalytic performances within universal pH environments.

Early experience of the compression anastomosis ring (CAR^(TM) 27) in left-sided colon resection

AIM:To evaluate clinical validity of the compression anastomosis ring(CARTM27) anastomosis in left-sided colonic resection. METHODS:A non-randomized prospective data collection was performed for patients undergoing an elective left-sided colon resection,followed by an anastomosis using the CARTM27 between November 2009 and January *** criteria of the use of the CARTM27 were anastomoses between the colon and at or above the intraperitoneal *** primary short-term clinical endpoint,rate of anastomotic leakage,and other clinical outcomes,including intraand postoperative complications,length of operation time and hospital stay,and the ring elimination time were evaluated. RESULTS:A total of 79 patients(male,43;median age,64 years) underwent an elective left-sided colon resection,followed by an anastomosis using *** was performed laparoscopically in 70 patients,in whom two patients converted to open procedure(2.9%) .There was no surgical *** an intraoperative complication,total disruption of the anastomosis occurred by premature enforced tension on the proximal segment of the anastomosis in one *** ring was removed and another new CARTM27 anastomosis was *** patient with sigmoid colon cancer showed postoperative anastomotic leakage after 6 d postoperatively and temporary diverting ileostomy was *** date of expulsion of the ring could not be recorded because most patients were not aware that the ring had been *** patients manifested clinical symptoms of anastomotic stricture. CONCLUSION:Short-term evaluation of the CARTM27 anastomosis in elective left colectomy suggested it to be a safe and efficacious alternative to the standard hand-sewn or stapling technique.

Improvement of oxidation resistance in graphite for MgO-C refractory through surface modification

Graphite, used as a carbon source in a conventional magnesia-carbon(MgO-C) refractory, was modified with an acid reagent, resulting in a negative charge on the surface of graphite, to enhance the coating efficiency of aluminum(Al) phase, which was compared to the pristine graphite through its dispersibity and oxidation behavior. The graphite particles with and without surface modification were added, respecticely, in an Al(NO3)3 suspension used as a coating reagent, and then filtered at room temperature. The modified graphite shows better disperbility than the pristine graphite, indicating that the coating efficiency of Al precursor is enhanced in the modified graphite. With respect to oxidation behavior, the modified graphite without the coating layer is totally reacted with oxygen at heat treatment of 900 °C in air. However, the Al-coated graphite starts to react with oxygen at heat treatment of 900 °C and fully reacted with oxygen at heat treatment of 1000 °C, showing the gray and white colors, respectively. It is verified that the Al layer is individually and uniformly formed on the surface of graphite and the oxidation resistance of graphite is enhanced owing to the increased coating efficiency of Al precursor.

A new damage quantification approach for shear-wall buildings using ambient vibration data

This paper presents a new approach to estimate damage seventy tor slaear-wall DUlltllngs using alagona~ terms of a modal flexibility matrix estimated from dynamic properties. This study aims to provide a fundamental concept for quantifying the damage of realistic buildings by investigating an idealized shear-wall building. Numerical studies were performed on a 5-story shear-wall building model to validate the applicability of the presented approach, using two damage patterns. With the numerical simulations, the proposed approach accurately determined the damage ratio of the specimens. Experiments were also conducted on a 5-story shear-wall building model for which the system parameters were almost the same as those in numerical simulations. The estimated damage-quantification results from the experimental validations demonstrated that the performance of the presented method for shear-wall buildings was both suitable and accurate.

Fabrication of periodically aligned vertical singlecrystalline anatase TiO2 nanotubes with perfect hexagonal open-ends using chemical capping materials

A vertically aligned anatase TiO2 （A-TiO2） nanotube array has been fabricated by coating a ZnO nanorod （NR） template with a TiO2 precursor solution. After coating, the ZnO NR cores were selectively etched in an acidic environment to form TiO2 nanotubes （NTs）. More specifically, after growing the ZnO NRs via a hydrothermal method, one drop of the TiO2 precursor solution was cast to coat the ZnO NRs, the tops of which were previously covered with chemical capping materials by electrostatic interaction, and then the sample was sintered. Finally, the sample was immersed in an acidic solution resulting in selective etching of the ZnO NR cores. Thus, only TiO2 NTs remained on the substrate. The capping material is effectively used to create a perfect, hexagonal open-ended TiO2 NT array, which interestingly extends onset absorption towards the visible region.

Adoption of wide-bandgap microcrystalline silicon oxide and dual buffers for semitransparent solar cells in building-integrated photovoltaic window system

We focused on developing penetration-type semitransparent thin-film solar cells(STSCs) using hydrogenated amorphous Si(a-Si:H) for a building-integrated photovoltaic(BIPV) window system. Instead of conventional p-type a-Si:H, p-type hydrogenated microcrystalline Si oxide(p-μc-SiOx:H) was introduced for a wide-bandgap and conductive window layer. For these purposes, we tuned the CO2/SiH4 flow ratio(R) during p-μc-SiOx:H deposition. The film crystallinity decreased from 50% to 13% as R increased from 0.2 to 1.2. At the optimized R of 0.6, the quantum efficiency was improved under short wavelengths by the suppression of p-type layer parasitic absorption. The series resistance was well controlled to avoid fill factor loss at R = 0.6. Furthermore, we introduced dual buffers comprising p-a-SiOx:H/i-a-Si:H at the p/i interface to alleviate interfacial energy-band mismatch. The a-Si:H STSCs with the suggested window and dual buffers showed improvements in transmittance and efficiency from 22.9% to 29.3% and from 4.62% to 6.41%, respectively, compared to the STSC using a pristine p-a-Si:H window.

Coral-Like Yolk–Shell-Structured Nickel Oxide/Carbon Composite Microspheres for High-Performance Li-Ion Storage Anodes

In this study, coral?like yolk–shell?structured NiO/C composite microspheres(denoted as CYS?NiO/C) were prepared using spray pyrolysis. The unique yolk–shell structure was characterized, and the formation mechanism of the structure was proposed. Both the phase separation of the polyvinylpyrrolidone and polystyrene(PS) colloidal solution and the decompo?sition of the size?controlled PS nanobeads in the droplet played crucial roles in the formation of the unique coral?like yolk–shell structure. The CYS?NiO/C microspheres delivered a reversible discharge capacity of 991 mAh g^(-1) after 500 cycles at the current density of 1.0 A g^(-1). The dis?charge capacity of the CYS?NiO/C microspheres after the 1000 th cycle at the current density of 2.0 A g^(-1) was 635 mAh g^(-1), and the capacity retention measured from the second cycle was 91%. The final discharge capacities of the CYS?NiO/C microspheres at the current densities of 0.5, 1.5, 3.0, 5.0, 7.0, and 10.0 A g^(-1) were 753, 648, 560, 490, 440, and 389 mAh g^(-1), respectively. The synergetic e ect of the coral?like yolk–shell structure with well?defined interconnected mesopores and highly conductive carbon resulted in the excellent Li+?ion storage properties of the CYS?NiO/C microspheres.
