碳排放统计核算是贯彻落实“碳达峰”、“碳中和”目标的重要基础。针对港区缺少统一的核算体系,依据《陆上交通运输企业温室气体排放核算方法与报告指南(试行)》和《建筑碳排放计算标准》,明确港口碳排放的核算范围和核算内容,将燃料、电力、热力、自来水和空调制冷剂以及绿地作为港区碳排放的核算内容,计算和归纳核算内容的碳排放因子。以某集装箱码头为例对碳排放核算体系进行应用,并依据碳排放核算结果提出降碳路径。结果表明,该方法计算快速,可为港区碳排放核算提供科学的依据。

目的评估全关节镜下弹性固定Latarjet手术治疗合并严重骨缺损的肩关节前向不稳的疗效。方法回顾分析2020年6月—2023年2月采用全关节镜下弹性固定Latarjet手术治疗的15例符合选择标准的合并严重骨缺损的肩关节前向不稳患者临床资料。其中男11例,女4例;年龄20~54岁,平均31.1岁。三维CT示关节盂骨缺损16.3%~35.2%,平均24.4%。肩关节脱位3~8次,平均4.2次。病程6~21个月,平均10.6个月。记录手术时间、术中出血量;手术前后采用疼痛视觉模拟评分(VAS)评估疼痛缓解情况,Rowe评分、WalchDuplay评分、美国肩肘外科协会(ASES)评分评价肩关节功能恢复情况;评估肩关节各向活动度(range of motion,ROM),包括主动前屈、体侧外旋、外展90°外旋和内旋。术后6个月和末次随访时行三维CT检查,观察移植骨块吸收情况、骨块与关节盂位置以及骨块愈合情况。结果所有患者手术均顺利完成,手术时间85~195 min,平均123.0 min;术中出血量20~75 mL,平均26.5 mL。15例患者均获随访,随访时间13~32个月,平均18.7个月。随访期间患者均未出现肩关节感染、关节僵硬、血管神经损伤等严重并发症。1例术后3个月出现移植骨部分吸收、骨不连,但末次随访时肩关节疼痛较术前改善,未发生肩关节再脱位等情况;其余患者未发生明显骨块骨折、肩关节脱位等情况。术后6个月随访时骨块均已愈合。末次随访时,患者VAS评分、Rowe评分、Walch-Duplay评分和ASES评分均较术前改善,差异有统计学意义(P0.05)。结论全关节镜下弹性固定Latarjet手术治疗合并严重骨缺损的肩关节前向不稳,术后临床功能改善明显,影像学检查提示移植骨块位置满意。

目的比较关节镜下外排张力带固定与切开复位内固定治疗肱骨大结节骨折的疗效。方法采用回顾性队列研究分析2019年9月至2022年6月浙江省台州医院收治的55例肱骨大结节骨折患者的临床资料,其中男24例,女31例;年龄26~80岁[(61.7±10.5)岁]。35例采用切开复位钢板螺钉内固定治疗(切开组),20例采用关节镜下外排锚钉张力带固定治疗(关节镜组)。比较两组手术时间;比较两组术前、术后1个月及末次随访时视觉模拟评分(VAS)、肩关节美国肩肘外科协会(ASES)评分、Constant-Murley评分、肩关节主动活动度(前屈、外展及后伸);末次随访时观察两组骨折愈合情况;比较两组术后并发症发生情况。结果患者均获随访12~29个月[(16.9±4.0)个月]。两组手术时间差异无统计学意义(P>0.05)。两组术前VAS、ASES评分、Constant-Murley评分、肩关节主动活动度差异均无统计学意义(P>0.05)。术后1个月关节镜组VAS为3(2,3)分,低于切开组的4(3,4)分(P0.05)。术后1个月及末次随访时关节镜组ASES评分为(70.6±4.2)分、(90.2±3.7)分,均高于切开组的(64.7±6.4)分、(87.5±4.9)分(P0.05);末次随访时关节镜组Constant-Murley评分为(94.1±3.1)分,高于切开组的(89.2±4.7)分(P<0.01)。术后1个月及末次随访时关节镜组前屈[(52.7±12.3)°、(140.0±16.9)°]、外展[(57.4±8.6)°、(125.0±14.3)°]、后伸[16(15,19)°、25(20,30)°]活动度均大于切开组前屈[(42.2±5.2)°、(110.9±14.0)°]、外展[(52.8±6.0)°、(103.7±11.7)°]、后伸[10(10,20)°、16(15,25)°]活动度(P<0.05或0.01)。末次随访时两组骨折均已愈合。两组术后均未见明显切口感染、关节僵硬等并发症。切开组2例患者术后1~3个月出现内固定失效,行翻修手术;6例患者术后3~6个月出现肩关节僵硬,经门诊康复治疗。关节镜组术后并发症发生率[0%(0/20)]明显低于切开组[23%(8/35)](P<0.05)。结论与切开复位钢板螺钉内固定相比,关节镜下外排张力带固定治疗肱骨大结节骨折,具有早期疼痛缓解明显、肩关节功能改善更佳、肩关节活动度恢复更好、并发症更少等优点。

目的评估改良斜方肌下束转位联合自体腘绳肌腱桥接治疗后上型不可修复性巨大肩袖撕裂的临床疗效。方法回顾性分析2019年6月至2022年11月采用全关节镜下改良斜方肌下束转位联合自体腘绳肌腱桥接治疗后上型不可修复性巨大肩袖撕裂患者59例,男11例、女48例;年龄(66.70±6.10)岁(范围55~79岁)。记录术前和末次随访时肩关节主动上举、前屈、体侧外旋、外展外旋和内旋角度,术前、术后3、6个月和末次随访时Constant-Murley评分、疼痛视觉模拟评分(visual analogue scale,VAS)、美国肩肘外科协会(American Shoulder and Elbow Surgeons,ASES)评分。术后3、6和12个月行MR检查评估肩袖愈合及再撕裂情况。结果手术均顺利完成,使用锚钉数量为(3.2±0.5)枚。随访时间为(15.9±5.7)个月。体侧外旋角度由术前12.0°±19.4°改善至末次随访时的35.3°±19.6°,外展外旋角度由术前33.4°±22.4°改善至末次随访时的43.4°±23.1°,外展角度由术前121.7°±47.9°改善至末次随访时的136.4°±40.6°,前屈上举角度由术前117.6°±45.8°改善至末次随访时的139.5°±33.7°,差异均有统计学意义(P<0.05);末次随访时患侧肩关节活动度与健侧比较,差异均有统计学意义(P<0.05)。术前、术后3、6个月及末次随访时VAS评分分别为(4.8±1.8)分、(2.3±1.3)分、(1.6±1.3)分和(0.3±0.8)分,差异有统计学意义(F=104.868,P<0.001);ASES评分分别为(42.0±14.9)分、(59.2±6.3)分、(61.2±12.5)分和(71.7±14.6)分,差异有统计学意义(F=47.287,P<0.001);Constant-Murley评分分别为(54.3±17.1)分、(41.6±9.6)分、(52.1±13.3)分和(76.4±13.0)分,差异有统计学意义(F=44.082,P<0.001)。1例患者取肌腱处出现局部皮下血肿,1例发生下肢深静脉血栓形成,经对症治疗后好转;术后未发生肩关节感染、关节僵硬、血管神经损伤等严重并发症。随访期间2例出现移植物完全撕裂、7例移植物部分撕裂,再撕裂发生率为15.3%。结论全关节镜下改良斜方肌下束转位联合自体腘绳肌腱桥接治疗后上型不可修复性巨大肩袖撕裂术后肩关节疼痛明显缓解,肩关节活动及功能评分改善,手术并发症较少。

Laboratory research and pharmacoepidemiology provide support for metformin as a potential antitumor ***,the lack of a clear understanding of the indications of metformin limits its ***,we performed a genome-wide CRISPR knockout negative screen to identify potential targets that might synergize with ***-generation sequencing of pooled genomic DNAs isolated from surviving cells after 18 days of metformin treatment(T18)compared to those of the untreated cells at day 0(T0)yielded candidate *** of a group of cyclin-dependent kinases(CDKs),including CDK1,CDK4,and CDK6,confirmed the results of the *** treatment of the CDKs inhibitor abemaciclib with metformin profoundly inhibited tumor viability in vitro and in *** cell cycle parameters were not further altered under the combination treatment,investigation of the metabolome revealed significant changes in cell metabolism,especially with regard to fatty acid oxidation,the tricarboxylic acid cycle and aspartate *** changes appeared to be mediated through inhibition of the mTOR ***,our study suggests that the combination of CDKs inhibitor with metformin could be recognized as a potential therapy in future clinical applications.

油轮单点系泊系统采用孤点式布置,无陆域设施可依托,发生事故时仅依靠机动船舶进行应急防护,因此最大可信事故消防和溢油的应急设施配置至关重要。依据《油气化工码头设计防火规范》,结合单点系泊特点,引入池火和安全防护距离的理念,提出消防船的能力配备要求;针对装卸作业时无固定围油栏的特点,提出自行配置的溢油应急设施达到一级防护能力要求;通过对比《船舶溢油应急能力评估导则》《油轮单点系泊作业安全要求》2个行业标准,提供了各项溢油应急设施的计算方法和最低配置要求。

Innate immunity plays critical antiviral roles. The highly virulent avian influenza viruses (AIVs) H5N1, H7N9, and H5N6 can betterescape host innate immune responses than the less virulent seasonal H1N1 virus. Here, we report a mechanism by whichtranscriptional readthrough (TRT)-mediated suppression of innate immunity occurs post AIV infection. By using cell lines, mouselungs, and patient PBMCs, we showed that genes on the complementary strand (“trans” genes) influenced by TRT were involved inthe disruption of host antiviral responses during AIV infection. The trans-TRT enhanced viral lethality, and TRT abolishmentincreased cell viability and STAT1/2 expression. The viral NS1 protein directly bound to SSU72, and degradation of SSU72 inducedTRT. SSU72 overexpression reduced TRT and alleviated mouse lung injury. Our results suggest that AIVs infection induce TRT byreducing SSU72 expression, thereby impairing host immune responses, a molecular mechanism acting through the NS1-SSU72-trans-TRT-STAT1/2 axis. Thus, restoration of SSU72 expression might be a potential strategy for preventing AIV pandemics.
