Total knee arthroplasty using computer assisted navigation in patients with severe valgus deformity of the knee

Background Severe valgus deformity often has bone defect and laxity of the medial ligamentous, and total knee arthroplasty in severe valgus knee is, in most cases, more challenging for surgeons. The usefulness of a computer assisted navigation system in reestablishing the mechanical axis has been well established. Hence, the interest for surgeons is how the navigation system makes the procedure of total knee arthroplasty with severe valgus knee easier. Methods From june 2006 to March 2008 in Department of Joint Surgery, Shanghai Sixth People＇s Hospital, 6 patients （7 knees） with severe valgus knee underwent total knee arthroplasty using the Stryker Navigation system, which is an active wireless and imageless system. All the patients were followed up for 12 to 18 months after surgery. The X-ray radiographs for whole limbs were obtained on all patients to determine preoperative and postoperative alignments. Results A primary, posterior stabilized prosthesis was utilized in all cases. The average preoperative overall mechanical axis of the seven knees was 19.6°±4.6° of valgus （range 16°to 29°）, and the average postoperative mechanical axis was 0.4°±0.7° （range 0.8° varus to 1.4° valgus ）. Conclusions The navigation system is a very effective and useful tool for accurate intraoperative restoration of alignment in the face of significant deformity with valgus knee. To prevent component malposition, we did not reduce the knee before solidification of bone cement but controlled alignment using the navigation system up to implantation of the final component.

The permeability index is one of the important production indicators to monitor the operation of blast *** is crucial to grasp the trends of changes in the new permeability index in *** the complex vibration spectrum of the permeability index,a prediction model of the permeability index based on the VMD-PSO-BP(variational mode decomposition-particle swarm optimization-back propagation)method was ***,the key factors that affect the permeability index of blast furnace were studied from multiple ***,the permeability index was divided into multiple sub-modes based on the difference of frequency bands by the VMD algorithm,and a PSO-BP prediction model was established for each ***,the prediction results of each sub-mode were summed to obtain the final *** results show that the composite prediction accuracy by using the VMD algorithm is 3%higher than that of the traditional prediction method,which has better applicability.

Short-term efficacy of natural orifice specimen extraction surgery for low rectal cancer

BACKGROUND This case-control study compared the short-term clinical efficacy of natural orifice specimen extraction surgery(NOSES) using a prolapsing technique and the conventional laparoscopic-assisted approach for low rectal *** To further explore the application value of the transanal placement of the anvil and to evaluate the short-term efficacy of NOSES for resecting specimens of low rectal cancer, as well as to provide a theoretical basis for its extensive clinical *** From june 2015 to june 2018, 108 consecutive laparoscopic-assisted low rectal cancer resections were performed at our center. Among them, 26 specimens were resected transanally using a prolapsing technique(NOSES), and 82 specimens were resected through a conventional abdominal wall small incision(LAP). A propensity score matching method was used to select 26 pairs of matched patients, and their perioperative data were *** The baseline data were comparable between the two matched groups. All 52 patients underwent the surgery successfully. The operative time, blood loss,number of harvested lymph nodes, postoperative complication rate,circumferential margin involvement, postoperative follow-up data, and postoperative anal function were not statistically significant. The NOSES group had shorter time to gastrointestinal function recovery(2.6 ± 1.0 d vs 3.4 ± 0.9 d, P= 0.006), shorter postoperative hospital stay(7.1 ± 1.7 d vs 8.3 ± 1.1 d, P = 0.003),lower pain score(day 1: 2.7 ± 1.8 vs 4.6 ± 1.9, day 3: 2.0 ± 1.1 vs 4.1 ± 1.2, day 5: 1.7± 0.9 vs 3.3 ± 1.0, P < 0.001), a lower rate of additional analgesic use(11.5% vs61.5%, P = 0.001), and a higher satisfaction rate in terms of the aesthetic appearance of the abdominal wall after surgery(100% vs 23.1%, P < 0.001).CONCLUSION NOSES for low rectal cancer can achieve satisfactory short-term efficacy and has advantages in reducing postoperative pain, shortening the length of postoperative hospital stay, and improving pat

Obstructive Sleep Apnea Affecting Platelet Reactivity in Patients Undergoing Percutaneous Coronary Intervention

β-Chiral sulfones are structural motifs widely found in natural products and bioactive molecules, while also serving as important intermediates for the synthesis of valuable chiral scaffolds. In contrast to the rapid growth of sulfur dioxide insertion chemistry over the last decade, enantioselective catalytic variants for accessing β-chiral sulfones, especially those bearing a quaternary carbon stereocenter, remain rare. Herein, we report an enantioselective copper/bisoxazoline catalyzed oxysulfonylation of alkenes to yield isoxazolinyl-containing β-chiral sulfones bearing quaternary carbon stereocenters with moderate to excellent yields and up to 96:4 er. The advantage and irreplaceability of this sulfur dioxide insertion catalytic system have been demonstrated by achieving transformations clearly unavailable with the previously reported sulfonyl chloride system. Additionally,the recoverability of the new-developed bisoxazoline ligand L9 and its reusability without any erosion of the catalytic activity and enantioselectivity further demonstrate the usefulness of the protocol.

Sexual,physical and overall adverse effects in patients treated with 5a-reductase inhibitors:a systematic review and meta-analysis

Postfinasteride syndrome(PFS)is a term coined to characterize a constellation of reported undesirable sexual,physical,and neuropsychiatric side *** the present study,we conducted the meta-analysis to demonstrate whether the use of 5α-reductase inhibitors(5ARIs)increases the risk of PFS-like adverse effects.A search of studies published until May 10,2020,was performed using PubMed,EMBASE,and the Cochrane *** included randomized controlled trials with at least one comparison between male patients receiving 5ARIs versus placebo for the treatment of benign prostatic hyperplasia(BPH)or androgenetic alopecia(AGA),and identified 34 studies from 28 articles that met our eligibility *** the random-effects model,the overall use of 5ARIs exhibited a 1.87-fold risk of PFS-like adverse effects during the trial(95%confidence interval[CI]:1.64-2.14).Regarding specific types of adverse effects,the use of 5ARIs had a 1.89-fold risk of sexual adverse effects(95%CI:1.74-2.05)and was associated with an increased risk of physical adverse effects(relative risk[RR]:1.31,95%CI:0.80-2.15),albeit without statistical *** meta-analysis helped to better define the adverse effects caused by *** concluded that the overall use of 5ARIs significantly increased the risk of PFS-like adverse effects in men with AGA or BPH during *** awareness of and education on the PFS-like adverse effects are necessary for clinicians.

Diagenetic evolution and reservoir quality of the Oligocene sandstones in the Baiyun Sag, Pearl River Mouth Basin, South China Sea

The Oligocene Zhuhai sandstones are significant reservoirs for hydrocarbons in the Baiyun Sag, South China *** effective appraisal, exploration and exploitation of such a deep-water hydrocarbon sandstone, samples of five wells from depths of 850 m to 3 000 m were studied. A series of comprehensive petrographic and geochemical analyses were performed to unravel the diagenetic features and their impact on the reservoir ***, the sandstones are dominated by feldspathic litharenites and lithic arenites with fine to medium grain sizes and moderate to good sorting. The reservoir quality varies greatly with a range of porosity from 0.2% to 36.1% and permeability from 0.016 ×10~(–3) μm~2 to 4 301 ×10~(–3) μm~2, which is attributed to complex diagenetic evolution related to sedimentary facies;these include compaction, cementation of calcite, dolomite, siderite and framboidal pyrite in eogenetic stage;further compaction, feldspar dissolution, precipitation of ferrocalcite and ankerite, quartz cements, formation of kaolinite and its illitization, precipitation of albite and nodular pyrite, as well as hydrocarbon charge in mesogenetic stage. The dissolution of feldspar and illitization of kaolinite provide internal sources for the precipitation of quartz cement, while carbonate cements are derived from external sources related to interbedded mudstones and deep fluid. Compaction is the predominant factor in reducing the total porosity, followed by carbonate cementation that leads to strong heterogeneity. Feldspar dissolution and concomitant quartz and clay cementation barely changes the porosity but significantly reduces the *** high-quality reservoirs can be concluded as medium-grained sandstones lying in the central parts of thick underwater distributary channel sandbodies(>2 m) with a high content of detrital quartz but low cement.

PPP1R14A is Associated with Immunotherapy Resistance in Head and Neck Squamous Cell Carcinoma Identified by Single-Cell and Bulk RNA-Sequencing

An Efficient Optimization Design Framework for Low-Resistance Shape of Bionic Amphibious Robot

To realize the low-resistance shape optimization design of amphibious robots,an efficient optimization design framework is proposed to improve the geometric deformation flexibility and optimization *** the proposed framework,the free-form deformation parametric model of the flat slender body is established and an analytical calculation method for the height constraints is *** method is introduced to carry out the high-precision resistance calculation and a constrained Kriging-based optimization method is built to improve the optimization efficiency by circularly infilling the new sample points which satisfying the ***,the shape of an amphibious robot example is optimized to get the low-resistance shape and the results demonstrate that the presented optimization design framework has the advantages of simplicity,flexibility and high efficiency.
