Adverse events related to colonoscopy: Global trends and future challenges

Colonoscopy is a widely used method for diagnosing and treating colonic disease. The number of colonoscopies is increasing worldwide, and concerns about associated adverse events are growing. Large-scale studies using big data for post-colonoscopy complications have been reported. A colon perforation is a severe complication with a relatively high mortality rate. The perforation rate, as reported in large studies(≥ 50,000 colonoscopies) published since 2000, ranges from 0.005-0.085%. The trend in the overall perforation rate in the past 15 years has not changed significantly. Bleeding is a more common adverse event than perforation. Recent large studies(≥ 50,000 colonoscopies) have reported postcolonoscopy bleeding occurring in 0.001-0.687% of cases. Most studies about adverse events related to colonoscopy were performed in the West, and relatively few studies have been conducted in the East. The incidence of post-colonoscopy complications increases in elderly patients or patients with inflammatory bowel diseases. It is important to use a unified definition and refined data to overcome the limitations of previous studies. In addition, a structured training program for endoscopists and a systematic national management program are needed to reduce post-colonoscopy complications. In this review, we discuss the current trends in colonoscopy related to adverse events, as well as the challenges to be addressed through future research.

Bifunctional hybrid anodes(Bhas),which are both a high-performance active host material for lithium-ion storage as well as a guiding agent for homogeneous lithium metal nucleation and growth,exhibit significant potential as anodes for next-generation high-energy-density lithium-ion batteries(LIBs).In this study,sulfur-doped hard carbon nanosphere assemblies(S-HCNAs)were prepared through a hydrothermal treatment of a liquid organic precursor,followed by high-temperature thermal annealing with elemental sulfur for application as Bhas for *** a carbonate-based electrolyte containing fluoroethylene carbonate additive,the S-HCNAs showed high lithium-ion storage capacities in sloping as well as plateau voltage sections,good rate capabilities,and stable *** addition,high average Coulombic efficiencies(CEs)of~96.9%were achieved for dual lithium-ion and lithium metal storage *** the LIB full-cell tests with typical NCM811 cathodes,the S-HCNA-based Bhas containing~400 mA h g^(−1) of excess lithium led to high energy and power densities of~500Wh kg^(−1) and~1695Wkg^(−1),respectively,and a stable cycling performance with~100%CEs was achieved.

Effect of Doubling the Ensemble Size on the Performance of Ensemble Prediction in the Warm Season Using MOGREPS Implemented at the KMA

Using the Met Office Global and Regional Ensemble Prediction System （MOGREPS） implemented at the Korea Meteorological Administration （KMA）, the effect of doubling the ensemble size on the performance of ensemble prediction in the warm season was evaluated. Because a finite ensemble size causes sampling error in the full forecast probability distribution function （PDF）, ensemble size is closely related to the efficiency of the ensemble prediction system. Prediction capability according to doubling the ensemble size was evaluated by increasing the number of ensembles from 24 to 48 in MOGREPS implemented at the KMA. The initial analysis perturbations generated by the Ensemble Transform Kalman Filter （ETKF） were integrated for 10 days from 22 May to 23 june 2009. Several statistical verification scores were used to measure the accuracy, reliability, and resolution of ensemble probabilistic forecasts for 24 and 48 ensemble member forecasts. Even though the results were not significant, the accuracy of ensemble prediction improved slightly as ensemble size increased, especially for longer forecast times in the Northern Hemisphere. While increasing the number of ensemble members resulted in a slight improvement in resolution as forecast time increased, inconsistent results were obtained for the scores assessing the reliability of ensemble prediction. The overall performance of ensemble prediction in terms of accuracy, resolution, and reliability increased slightly with ensemble size, especially for longer forecast times.

Background: The optimal treatment strategy for biliary tract cancer(BTC) after curative?intent resection remains con?troversial. The purpose of this study was to evaluate the efficacy of fluoropyrimidine?based adjuvant chemotherapy for BTC patients undergoing microscopically margin?negative(R0) ***: We retrospectively analyzed the clinical data of BTC patients who underwent curative?intent R0 resection. Patients were eligible if they received either fluoropyrimidine?based adjuvant chemotherapy or observation after R0 ***: A total of 153 patients were included. In the entire patient cohort, no significant differences were observed in 5?year overall survival(OS) rates(48.4% vs. 39.6%, P = 0.439) or 3?year recurrence?free survival(RFS) rates(49.1% vs. 39.5%, P = 0.299) between patients who received fluoropyrimidine?based adjuvant chemotherapy or observation. However, for patients with stages Ⅱ and Ⅲ BTC, chemotherapy significantly improved 5?year OS rate(52.4% vs. 35.6%, P = 0.002) and 3?year RFS rate(55.5% vs. 39.1%, P = 0.021) compared with ***: Fluoropyrimidine?based adjuvant chemotherapy may prolong the survival of patients with stages Ⅱ and Ⅲ BTC after R0 resection.

An experimental investigation was carried out to study the energy absorption characteristics of thin-walled square tubes subjected to dynamic crushing by impact loading to develop the optimum structural members. Here, the controller is introduced to improve and control the absorbed energy of thin-walled square tubes in this paper. When the controller were used, the experimental results of crushing of square tubes controlled by the controller＇s elements showed a good candidate for a controllable energy absorption capability in impact crushing.

Casting defects that are closely related to entrapped air bubbles and metallic oxides occur very frequently in the sand mold casting process. Many researchers have shown that these defects can be reduced by adopting an appropriate gating system design. However, it is difficult for field engineers to identify a specific gating system that is more appropriate for their products. In this study, we tried to draw a comparison between two gating system designs with and without a ceramic foam filter. The ceramic foam filter was added to the horizontal runner just after the down sprue to prevent air bubble generation and reduce turbulence without a change of gating system design. The water modeling experiment was conducted with four different amounts of the initial volumes of water in the reservoir to verify the effects of initial pouring velocity. The results of the experiment applying the filter showed remarkably changed flow characteristics. The use of the filter was found to convert the flow pattern of water in the desired direction. The ceramic foam filter performed well to reduce flow velocity and stabilize the water *** flow pattern without a filter can be improved significantly even with the the use of just a 10 PPI irregular filter. Although the study confirmed that use of the filter may change the flow characteristics, it needs to be noted that the use of the ceramic filter alone cannot solve all the problems caused by a poorly designed gating system.

The effect of streamwise vortices generated from passive vortex generators was investigated to manipulate the separation bubble behind the vertical fence. The experiments were carried out in a circulating water channel and the velocity fields were measured using 2D and stereoscopic PIV *** distance between the vortex generator and fence and the effect of the Reynolds number were investigated. In addition,the effect of boundary layer thickness was also investigated. The averaged recirculation lengths were compared with that of uncontrolled fence flow. The results showed the oscillatory variation of recirculation region appeared under the existence of vortex generators. The reduction of the separation bubble became larger when the fence was submerged in the thick boundary layer with increasing the distance between the generator and fence. When the boundary layer is thin,vortex generator can only suppress the separation bubble under the specific condition.
