AIM: To predict node-positive disease in colon cancer using computed tomography(CT).METHODS: American Joint Committee on Cancer stage Ⅰ-Ⅲ colon cancer patients who underwent curavtiveintent colectomy between 2007-2010 were identified at a single comprehensive cancer center. All patients had preoperative CT scans with original radiology reports from referring institutions. CT images underwent blinded secondary review by a surgeon and a dedicated abdominal radiologist at our institution to identify pericolonic lymph nodes(LNs). Comparison of outside CT reports to our independent imaging review was performed in order to highlight differences in detection in actual clinical practice. CT reviews were compared with final pathology. Results of the outside radiologist review, secondary radiologist review, and surgeon review were compared with the final pathologic exam to determine sensitivity, specificity, positive and negative predictive values, false positive and negative rates, and accuracy of each review. Exclusion criteria included evidenceof metastatic disease on CT, rectal or appendiceal involvement, or absence of accompanying imaging from referring ***: From 2007 to 2010, 64 stageⅠ-Ⅲ colon cancer patients met the eligibility criteria of our study. The mean age of the cohort was 68 years, and 26(41%) patients were male and 38(59%) patients were female. On final pathology, 26 of 64(40.6%) patients had nodepositive(LN+) disease and 38 of 64(59.4%) patients had node-negative(LN-) disease. Outside radiologic review demonstrated sensitivity of 54%(14 of 26 patients) and specificity of 66%(25 of 38 patients) in predicting LN+ disease, whereas secondary radiologist review demonstrated 88%(23 of 26) sensitivity and 58%(22 of 38) specificity. On surgeon review, sensitivity was 69%(18 of 26) with 66% specificity(25 of 38). 背景与目的对牙龈过度显露的治疗方法通常包括LeFort嵌入或上颌牙龈切除术。笔者提供一种新的方法，通过降低提上唇肌功能及复位上唇的方式来减少牙龈的显露程度。方法自2008年2月至2009年3月，我们对14例牙龈过度显露的女性患者进行了手术。分别于术前和术后6个月拍照记录。要求患者最大程度地笑，测量患者牙龈显露的最大值，并使用ImageJ软件进行分析。患者在局部麻醉下进行手术。利用骨膜剥离器，将牙龈黏膜从上颌骨上分离下来。钝性剥离上唇肌肉下方的皮肤和皮下组织。通过系带成形术延长上唇。切开双侧提上唇肌并分离。结果术后所有患者恢复良好。术前牙龈显露程度为（5．22±1．48）mm，而术后6个月为（1．91±1．50）mm。牙龈显露程度减少1．59±1．83mm，平均（1．59±4．83）mm。结论本方法可以有效减少笑时牙龈的显露程度，本组患者均取得满意疗效。

Background: This document includes recommendations and guidelines issued by a group of Mexican researchers and specialists gathered in the First National Colloquium for the Diagnosis and Management of Chronic Myeloid Leukaemia (CML) by initiative of Instituto Nacional de Cancerología and with the support of the Leukaemia Department of the MD Anderson Cancer Center. Mexico lacks of updated information taken from its own reality on the diagnosis and treatment of CML and other haematological disorders;besides, there are no national guidelines. Aim: To publish a consensus document with guidelines for the management of CML adjusted to the national environment and overall characteristics. Method: The participants answered a DELPHI questionnaire about the overall aspects of the disease, aiming to target controversial topics, discuss them in the colloquium, and to agree on the best ones. After those meetings, a final document was drawn up. Results: The group presents recommendations for definition, diagnosis, prognosis, monitoring, and treatment of CML in Mexico. Conclusions: Having consensus guidelines for the clinical management of CML in our country will enable the consensual practice of Mexican specialists regarding the clinical approach to CML, as well as optimize the resources which allow the rational planning of the medical care strategies.

灰霉病是导致水果采后巨大经济损失的主要病害,其致病菌为葡萄孢菌。葡萄孢菌通常以潜伏侵染的形式存在,一旦病原菌发展蔓延则引起健康果实的迅速腐烂变质。因此,开发快速、灵敏的葡萄孢菌检测和定量分析方法势在必行。本文介绍了一种用于测定苹果(Red Delicious)、鲜食葡萄(Pink Moscatel)和梨(William’s)组织中葡萄孢菌的间接竞争酶联免疫吸附法(ELISA)。该方法的原理是基于水果组织中葡萄孢菌抗原与交联固定在微量滴定板表面的葡萄孢菌纯化抗原对于单克隆抗体结合位点的竞争作用。为了验证该方法的有效性,进行了如选择性、线性分析、精密度、准确性、灵敏性等大量的参数测定。该方法对于葡萄孢菌抗原的检测限为0.97μ***-1,为了保证测定的稳定性,事先将纯化抗原固定长达4个月。应用该方法可以对尚未表现症状的任何水果中的葡萄孢菌进行检测定量分析。通过与一项DNA定量测试结果进行对比,表明ELISA法的测定结果与DNA分析结果具有很好的相关性。本研究结果表明,新开发的测试方法实现了对无发病症状的果实中葡萄孢菌的检测分析,具有成本低廉、操作简便、快捷等优点,可将其应用于世界各国对水果等植物的病害检疫领域。

Introduction: Post-hospital syndrome (PHS) is defined as a period of vulnerability during the first 30 days after a patient is discharged from hospital, in which multiple factors come into play. Hyponatremia is the most frequent hydroelectrolytic disorder in hospitalized patients and may be related to the appearance of PHS. Objective: The objective is to estimate the prevalence of PHS that is assessed as the rate of readmissions in the first 30 days after discharge, in patients with hyponatremia. Material and Methods: It is a descriptive observational study of patients with hyponatremia who were discharged from 1 September 2010 to 2 February 2020 at the Internal Medicine Service of the Hospital University of San Juan (Alicante, Spain). Results: Of the 25 included patients, 5 (20%) were readmitted within a month of discharge, after a mean of 11.4 days (standard deviation [SD] 5.1). The overall mortality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatremia was undetermined. The most frequently recorded etiology for the condition was pharmacological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The most widely used corrective measure was drug withdrawal, in 16 patients (64%). Water intake restriction was the most common treatment after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. Conclusion: Hyponatremia may be the cause of PHS, which could increase the rate of early readmission. Hyponatremia is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system to optimize its management and, in future studies, to assess its impact on PHS.

环境强化(Environmental enrichment,EE)多次被证实具有身心治疗作用,通过为实验动物提供复杂的居住条件,能够改善它们的活跃度、行为和交互能力。使用Tcf4Het/+ApcMin/+介导的结肠肿瘤发生模型,发现EE显著改善了肿瘤小鼠的存活率,EE对小鼠肿瘤负荷的影响性别差异。Tcf4Het/+ApcMin/+雄性小鼠的循环型炎性细胞因子和诱导型核激素受体(NHR)信号表达显著降低,这两者都是伤口修复过程中的常见因子。有趣的是,EE激活了肿瘤伤口修复能力,该过程是通过血运重建,浆细胞募集和IgA分泌,在伤口修复过程中利用周细胞更换腺体肿瘤结构,使肠道微生物菌群正常化。EE依赖性变化可能是Tcf4Het/+ApcMin/+介导的结肠肿瘤雄性小鼠存活率显著改善的重要基础。本研究凸显了EE在未来设计结肠癌患者治疗策略中的重要作用。
