Impact of endoscopic ultrasound-guided fine needle biopsy for diagnosis of pancreatic masses

AIM： To evaluate the diagnostic accuracy of histological evaluation of pancreatic tissue samples obtained by a modified method for recovering and processing the endoscopic ultrasound （EUS）-guided fine needle aspiration （FNA） material in the differential diagnosis of pancreatic solid masses. METHODS： Sixty-two consecutive patients with pancreatic masses were prospectively studied. EUS was performed by the linear scanning Pentax FG-38UX echoendoscope. Three FNAs （22G needle） were carried out during each procedure. The materials obtained with first and second punctures were processed for cytological study. Haterials of the third puncture were recovered into 10% formol solution by careful injection of saline solution through the needle, and processed for histological study. RESULTS： Length of the core specimen obtained for histological analysis was 6.5±5.3 mm （range 1-22 mm）. Cytological and histological samples were considered as adequate in 51 （82.3%） and 52 cases （83.9%）, respectively. Overall sensitivity of both pancreatic cytology and histology for diagnosis of malignancy was 68.4%. Conbary to cytology, histology was able to diagnose tumours other than adenocarcinomas, and all cases of inflammatory masses. Combination of cytology and histology allowed obtaining an adequate sample in 56 cases （90.3%）, with a global sensitivity of 84.21%, specificity of 100% and an overall accuracy of 90.32%. The complication rate was 1.6%.CONCLUSION： Adequate pancreatic core specimens for histological examination can be obtained by EUS-guided FNA. This technique is mainly useful for the diagnosis of different types of pancreatic tumours and evaluation of benign diseases.

To evaluate factors that influence the diagnostic accuracy of endoscopic ultrasound (EUS)-guided tissue acquisition for lymph node enlargement in the absence of an on-site pathologist. METHODSA retrospective analysis of patients who underwent EUS-guided tissue acquisition for the pathological diagnosis of lymph node enlargement between April 2012 and June 2015 is reported. Tissue acquisition was performed with both cytology and biopsy needles of different calibers. The variables evaluated were lymph node location and size, number of passes and type of needle used. Final diagnosis was based on surgical histopathology or, in non-operated cases, on EUS-guided tissue acquisition and imaging assessment with a minimum clinical follow-up of 6 mo. RESULTSDuring the study period, 168 lymph nodes with a median size of 20.3 mm (range 12.5-27) were sampled from 152 patients. Ninety lymph nodes (53.6%) were located at mediastinum, and 105 (62.5%) were acquired with biopsy needles. The final diagnosis was benign/reactive origin in 87 cases (51.8%), malignant in 65 cases (38.7%), and lymphoma in 16 cases (9.5%). The sensitivity, specificity, positive predictive value and negative predictive value for the detection of malignancy were 74.1%, 100%, 100% and 80.6%, respectively. The overall accuracy was 87.5% (95%CI: 81.7-91.7). No variables were independently associated with a correct final diagnosis according to the multivariate analysis. CONCLUSIONEUS-guided tissue acquisition is a highly accurate technique for assessing lymph node enlargement. None of the variables evaluated were associated with diagnostic accuracy.

Optical vortices are structures of the electromagnetic field with a spiral phase ramp about a point-phase singularity,carrying orbital angular momentum(OAM). Recently, OAM has been imprinted to short-wavelength radiation through high-order harmonic generation(HHG), leading to the emission of attosecond twisted beams in the extreme-ultraviolet(XUV) regime. We explore the details of the mapping of the driving vortex to its harmonic spectrum. In particular, we show that the geometry of the harmonic vortices is convoluted, arising from the superposition of the contribution from the short and long quantum paths responsible of HHG. Finally, we show how to take advantage of transverse phase-matching to select twisted attosecond beams with different spatiotemporal properties.

The earliest stages of plant succession on severely disturbed sites usually follow highly unpredictable trajectories. However, in the Popocatépetl volcano area(50 km SE of Mexico City),the development of physiognomically distinct primary plant communities suggests the occurrence of various successional trajectories only 10 years after the onset of colonization of a temperate forest on lahars. To characterize plant communities and determine the environmental factors that drive the differences observed between plant communities and their successional trajectories, we monitored 64 circular sample plots(3.14 m2) from 2002 to 2011. We examined the plant communities' composition and structure in terms of their species richness and abundance, plant cover, and maximum stem height,and recorded 13 environmental factors related to the volcanic deposit characteristics, microclimate, soil,flow dynamics and gravitational processes. A cluster analysis of the species abundance data showed that,by 2011, six plant community types(CT's) had established, including grasslands, and open, dense and very dense shrub lands. As these communities developed over the same period of time and within the same overall ecosystem, then these plant community types were interpreted as different stages of the same successional trajectory. Two sequential main stages that drive regeneration were identified from this successional trajectory: a) the first four years are characterized by a steady increase in species richness and physiognomic development(plant size and coverage), mostly dominated by Baccharis conferta, Eupatorium glabratum and Senecio barbajohannis; b) from the sixth year onwards, a continued increase in the abundance of those same species led to the development of the dense shrubland *** in the availability of soil resources and disturbances linked to recent lahar flows were the main factors accounting for such differences.

Hydroclimate affects the radial growth responses of trees, but the drivers of their spatial and population variability are not sufficiently understood. We addressed this issue by sampling several conifer populations located at the same latitude, but at different longitude and elevation in western(W) and eastern(E) Mexican regions. We used dendroecology to disentangle how earlywood width(EW), latewood width(LW) and adjusted LW(LWadj),i.e. the residuals after removing EW influences on LW, responded to climate variables(temperature and precipitation), climate indices(Southern Oscillation Index, SOI, Nino 3.4, Pacific Decadal Oscillation, PDO) and a drought index(Standardised Precipitation-Evapotranspiration Index, SPEI). The W species(Pinus herrerae Martinez, Pinus durangensis Martinez, Abies durangensis Martínez and Cupressus lusitanica Mill.) showed lower growth rates than the E species(Pinus hartwegii Lindl., Picea mexicana Martinez, Pseudotsuga menziesii(Mirb.) Franco and Abies vejari Martinez). Growth in W benefits mostly from high precipitation in the prior winter and current spring and it is limited by high temperatures in spring, whereas growth in the E showed similar but weaker ***, positive(negative) correlations were found in radial growth with the Nino 3.4(SOI) and the PDO from the prior to current autumns, which were again stronger in absolute terms in the W than in the E regions,excepting SOI in summer. In the W, P. durangensis and C. lusitanica were the least and most responsive species to spring drought, respectively;whilst P. menziesii and A. vejari were very responsive to spring drought compared to P. hartwegii in the E. Our results suggest greater responsiveness to hydroclimate and atmospheric patterns in the W than in the E region. The objectives of this research were to compare the performance of each individual nondestructive sensor with the destructive sensor,and to apply sensor fusion technique to explore whether a combination of sensors would give better results than a single sensor for classification of peach *** were carried out with four peach varieties namely Royal Glory,Caterina,Tirrenia and *** this research,the three nondestructive firmness sensors acoustic firmness,low-mass impact and micro-deformation impact were used to measure firmness.A Bayesian classifier was chosen to provide a classification into three categories,namely soft,intermediate and *** level fusion technique was performed by using identity declaration provided by each *** data fusion system processed the information of the sensors to output the fused *** result of the high level fusion was compared with the classification provided by an unsupervised algorithm based on destructive reference *** fusion process of the nondestructive sensors provided some improvements in the firmness classification;the error rate varied from 25%to 19%for individual ***,the results of fusion process by using three sensors decreased the error rate from 19%to 13%.This research demonstrated that the fused systems provided more complete and complementary information and,thus,were more effective than individual sensors in the firmness classification of peaches.

Planetary Gear Profile Modification Design Based on Load Sharing Modelling

In order to satisfy the increasing demand on high performance planetary transmissions, an important line of research is focused on the understanding of some of the underlying phenomena involved in this mechanical system. Through the development of models capable of reproduce the system behavior, research in this area contributes to improve gear transmission insight, helping developing better maintenance practices and more efficient design processes. A planetary gear model used for the design of profile modifications ratio based on the levelling of the load sharing ratio is presented. The gear profile geometry definition, following a vectorial approach that mimics the real cutting process of gears, is thoroughly described. Teeth undercutting and hypotrochoid definition are implicitly considered, and a procedure for the incorporation of a rounding arc at the tooth tip in order to deal with corner contacts is described. A procedure for the modeling of profile deviations is presented, which can be used for the introduction of both manufacturing errors and designed profile modifications. An easy and flexible implementation of the profile deviation within the planetary model is accomplished based on the geometric overlapping. The contact force calculation and dynamic implementation used in the model are also introduced, and parameters from a real transmission for agricultural applications are presented for the application example. A set of reliefs is designed based on the levelling of the load sharing ratio for the example transmission, and finally some other important dynamic factors of the transmission are analyzed to assess the changes in the dynamic behavior with respect to the non-modified case. Thus, the main innovative aspect of the proposed planetary transmission model is the capacity of providing a simulated load sharing ratio which serves as design variable for the calculation of the tooth profile modifications.

Currently, more than 7% of admissions to acute care hospitals am related with AEMs （adverse events to medications）. AEMs are the sixth cause of death, causing a cost of over $5.6 million dollars （USD） per hospital per year. There is an estimate that between 19% and 23% of hospitalized patients will have an adverse effect within the first 30 days after being discharged, 14.3% will be re-admitted and 70% of these events will be related to a medication prescription. Fortunately, at least 58% of these AEMs are preventable, since they result from a lack of information on the medication, prescription and dosage errors and from the abuse and underuse of the same. Polymedicated patients, especially the elderly with multiple pathologies and/or chronic patients that need to be admitted into the hospital more frequently, usually to internal medicine, neurology, psychiatry, rehabilitation and intensive care, are precisely the most liable to suffer from medication errors. It must be the objective to aim for the increase in the patient safety standards when it comes to medications.
