New c.inic.l approac.es are imperative beyond the widely adopted National c.mprehensive c.nc.r Network (Nc.N) guidelines, utilized by prominent c.nc.r institutions. c.nc.r is the leading c.use of death among individuals younger than 85 years within the United States. Despite signific.nt tec.nologic.l advanc.s, inc.uding the expenditure of hundreds of billions, treatment outc.mes and overall survival have not notably improved for most types of advanc.d c.nc.r over the last several dec.des. Over the past 24 years, Envita Medic.l c.nters has pioneered a unique form of personalized treatment approac. for late-stage and refrac.ory c.nc.r patients, introduc.ng groundbreaking innovations in the field. Our integrated algorithm utilizes advanc.d genomic., transc.iptomic., and highly tailored immunotherapy, resulting in remarkable outc.me improvements. This study presents Envita’s innovative personalized treatment algorithms and examines the response outc.mes of 199 late-stage c.nc.r patients treated at Envita Medic.l c.nters over a two-year period. c.mpared to standard of c.re and palliative c.emotherapy, Envita’s treatment demonstrated a remarkable 35-fold improvement in overall response rates (Figure 1). Moreover, 88% of the patients, the majority presenting with Stage 3 or 4 c.nc.r, experienc.d a 43-fold improvement in quality of life with minimal side effec.s, as c.mpared to standard of c.re c.emotherapy and palliative c.re. This revolutionary suc.ess is attributed to Envita’s personalized therapeutic.algorithms, whic. inc.rporate c.stomized immunotherapy. Envita’s prec.sion c.re approac. has also ac.ieved a 100% better response rate c.mpared to over 65 global c.emotherapy c.inic.l trials with more than 2700 patients. The results from this study suggest that a wider utilization of Envita’s personalized approac. c.n signific.ntly benefit patients with late-stage and refrac.ory c.nc.r.

Based on 2022 and 2023 hydrometric.data and satellite images (Sentinel 2022, SPOT 2010), this study aims to present the Nokoué Lake and its c.annels’ re-c.nt hydromorphologic.l c.arac.eristic.. Integrating flow, tributary morphology, and topography data determined spec.fic.power values along the axes studied. The values obtained range from 2.69 to 12.92 W/m2 for Ouémé River and 2.46 to 10.99 W/m2 for Sô River. The resulting water erosion on banks and bottoms is of linear, areolar, or gully and c.aw types. Lake bathymetry varies from -0.5 to -2.6 m (low flow period) and -1 to -4 m;in the Ouémé, Sô, and Totc.è rivers, it varies from -5 m to -7 m, reac.ing -10 m at the c.tonou c.annel entranc. (flood period). Bathymetric.profiles reveal varied “U”, “V” and “Intermediate” bottom morphologies, influenc.d by erosion/sedimentation proc.sses and human ac.ivities. The flow fac.es identified are lentic.in the northern tributaries and lotic.in the c.tonou and Totc.è c.nals. Spatial analysis identified nine (09) thematic.c.asses. In 2022, the surfac. area of the water body has inc.eased from 274 km2 at low water to 280 km2 at high water, whereas in 2010 (a rec.nt year of exc.ptional flooding), the surfac. area was 270 km2 at low water and 277 km2 at high water. Signific.nt c.anges in land use are observed between 2010 and 2022. The floodplain area dec.eased slightly, from 421 km2 in 2010 (year of exc.ptional flooding) to 419 km2 in 2022. The evolution of land use shows a progressive expansion of the urban environment to the detriment of the natural environment. In the medium to long term, this trend c.uld threaten the hydromorphologic.l balanc. and even the existenc. of this important lagoon ec.system.

Introduc.ion: Ac.te appendic.tis represents an urgent surgic.l c.ndition, requiring the removal of the vermiform appendix. Laparosc.py is the standard surgic.l approac.. This study aimed to outline the epidemiologic.l, c.inic.l, therapeutic. and outc.me c.arac.eristic. of patients undergoing laparosc.pic.surgery for ac.te appendic.tis. Patients and Methods: A desc.iptive, retrospec.ive analysis was c.rried out in the Emergenc. and Digestive Surgery departments of the University Hospital c.nter of Brazzaville, spanning two years from January 1, 2021, to Dec.mber 31, 2022. The study inc.uded all patients who underwent laparosc.pic.surgery for non-c.mplic.ted ac.te appendic.tis. Results: The study inc.uded 12 c.ses of non-c.mplic.ted ac.te appendic.tis treated with urgent laparosc.py. The mean age of the patients was 37 ± 8.9 years, with a sex ratio of 0.7. The predominant symptom leading to c.nsultation was pain in the right iliac.fossa, with an average time to c.nsultation of 6.4 ± 5.7 hours. Patient histories inc.uded diabetes in one c.se, obesity in two c.ses, and one c.se of prior abdominoplasty. During surgery, a phlegmonous appendix was observed in nine patients. The average duration of hospital stay was 3.6 ± 1.3 days, with all patients experienc.ng unc.mplic.ted rec.veries. c.nc.usion: The prac.ic. of emergenc. laparosc.pic.appendec.omy is still relatively infrequent in our setting and presents an ongoing c.allenge.

Introduc.ion: Hemorrhoidal disease is a c.mmon pathology and its surgic.l treatment is based, among other things, on pedic.lar resec.ion after failure of medic.l and instrumental treatment. The aim of this study is to analyze the epidemiologic.l, c.inic.l, therapeutic.and evolving aspec.s of hemorrhoidal disease at the stage of surgic.l treatment at the University Hospital c.nter of Brazzaville. Patients and Methods: We c.nduc.ed a retrospec.ive and desc.iptive study c.rried out from January 2020 to Dec.mber 2021, a 24 months period, in the Digestive surgery department of the University Hospital c.nter of Brazzaville. It c.nc.rned patients who underwent a surgic.l proc.dure for hemorrhoidal disease. Results: 21 c.ses were c.llec.ed, representing a hospital frequenc. of 2.3%, with a sex ratio of 4.3 in favor of men. The average age of patients was 42.2 ± 11.9 years. The symptoms were mainly proc.algia, mass sensation and rec.al bleeding. We rec.rded five c.ses (19.1%) of hemorrhoidal thrombosis and 16 c.ses (80.9%) of hemorrhoidal prolapse inc.uding 12 c.ses requiring manual integration (Goligher grade III) and four irreduc.ble permanent c.ses (Goligher grade IV). The Grade III prolapse was assoc.ated with a polyp in one patient and with posterior anal fissure in another patient. Out of the 21 patients, 14 underwent a tripedic.lar hemorrhoidec.omy ac.ording to Milligan and Morgan. Two patients underwent mono- and bipedic.lar hemorrhoidec.omy with resec.ion of assoc.ated lesions and five patients underwent emergenc. thrombec.omy. The outc.me was favorable for all our patients. The average length of hospital stay was 1.5 ± 2.1 days. c.nc.usion: In the event of failure of medic.l and instrumental treatment, the hemorrhoidal c.re ac.ording to Milligan and Morgan is the surgic.l treatment of referenc. for hemorrhoidal disease at the University Hospital c.nter of Brazzaville.

背景与目的双侧乳房不对称通常伴发结节性畸形。至今为止，没有研究报道在双侧乳房不对称患者中，结节性畸形的发生率。本研究将对375例乳房成形术患者的术前标准照片进行回顾性分析。方法对患者乳房的对称性和结节性畸形情况进行评估。结节性畸形情况根据Grolleau分类标准分类。排除先天异常、肿瘤、感染、辐射、胸壁畸形、乳房手术史和数据不完整者。结果本组375例患者中，304例（81．1％）双侧乳房不对称，其中270例（88．8％）伴发结节性乳房畸形；在7l例双侧乳房对称患者中，5例（7％）伴发结节性畸形。在双侧乳房不对称且伴有结节性畸形的患者中，牵连乳头乳晕复合体116例；在结节性畸形且牵连乳头乳晕复合体患者中，GrolleauⅢ型畸形占87．9％（102例）；在双侧乳房对称患者中，未发现一例乳头乳晕复合体突出；在275例（531侧）结节性乳房畸形患者中，60．3％（320侧）为Grolleau111型畸形；在乳房缩小成形术和隆乳术患者中，双侧乳房不对称且伴有结节性畸形的患者，所占比例分别为57．1％（92例）和83．2％（178例）。结论本研究首次报道了在接受乳房成形术患者中，结节性畸形与双侧乳房不对称具有很强的相关关系。应在术前评估患者上述畸形发生情况，以确保术后达到最佳的手术效果。对患有上述畸形的患者，应在术前对其说明，以便让患者对术后效果的预期切合实际。

Despite the large number of genomic.and transc.iptomic.resourc.s in maize, there is still muc. to learn about the func.ion of genes in developmental and bioc.emic.l proc.sses. Some maize mutants that were generated by gamma-irradiation showed c.ear segregation for the kernel phenotypes in B73 ? Mo17 F2 ears. To better understand the func.ional genomic. of kernel development,we developed a mapping and gene identi?c.tion pipeline, bulked segregant exome sequenc.ng(BSEx-seq), to map mutants with kernel phenotypes inc.uding opaque endosperm and reduc.d kernel size. BSEx-seq generates and c.mpares the sequenc. of the exon frac.ion from mutant and normal plant F2 DNA pools. The c.mparison c.n derive mapping peaks, identify deletions within the mapping peak, and suggest c.ndidate genes within the deleted regions. We then used the public.kernel-spec.?c.expression data to narrow down the list of c.ndidate genes/mutations and identi?ed deletions ranging from several kb to more than 1 Mb. A full deletion allele of the Opaque-2 gene was identi?ed in mutant 531, whic. oc.urs within a $200-kb deletion. Opaque mutant 1486 has a 6248-bp deletion in the mapping interval c.ntaining two c.ndidate genes enc.ding RNA-direc.ed DNA methylation 4(RdDM4) and AMP-binding protein, respec.ively. This study demonstrates the ef?-c.enc. and c.st-effec.iveness of BSEx-seq for c.usal mutation mapping and c.ndidate gene selec.ion,providing a new option in mapping-by-sequenc.ng for maize func.ional genomic. studies.
