Success rates and predictors of outcomes of type I tympanoplasty performed by residents in a teaching tertiary hospital

Several technical variations of tympanoplasty have been reported,usually involving the type of graft and/or instruments *** studies have focused on the outcome of type-1 tympanoplasty specifically in teaching *** aimed to describe the results of type-1 tympanoplasty performed by residents,and to investigate potential predictive factors of surgical *** do so,we did a retrospective analysis of medical records of patients who underwent type-1 tympanoplasty in a tertiary university *** evaluated the tympanic membrane closure and audiometric outcomes during the first year of follow-up,and compared the results according to some clinical and surgical factors.130 operated ears were included in the *** closure rate reached 84.12%after one month and 72.72%after twelve months of *** mean air-bone gap was 22.98 dB preoperatively,and reached 10.55 dB after *** closure rates were 85%and 57.14%for those operated by endoscopic and microscopic-assisted approaches,respectively(p?0.004).The use of cartilage grafts and time without otorrhea of more than three months prior to surgery were also predictors of surgical success(p?0.002 and 0.041,respectively).Gender,age,perforation size,contralateral disease,operated side,reoperation,and degree of hearing loss did not significantly interfere with *** showed good overall results when performed by residents,although inferior to those reported by experienced *** use of the endoscope,cartilage grafting,and longer preoperative time without otorrhea were predictors of surgical success in this scenario.

Isabel grape(IG)products have high contents of phenolic compounds and fiber recognized for their positive impacts on microorganisms associated with health benefits to *** study evaluated the effects of goat yogurts formulated with ingredients from IG integral valorization on the growth and metabolism of different probiotic strains,as well as on the population of selected bacterial groups and metabolic activity of human colonic microbiota in *** yogurts with IG ingredients(IGI)stimulated the growth of tested Lactobacillus and Bifidobacterium probiotic strains during a 48-h cultivation,as well as decreased the pH values and enhanced the organic acid *** yogurts with IGI increased the population of Lactobacillus *** Bifidobacterium *** a 24-h in vitro colonic fermentation.A stable Firmicutes:Bacteroidetes ratio close to 1 was found in media with goat yogurt formulations during the colonic fermentation,being similar to the effect caused by *** yogurt formulations with IGI caused increased production of short-chain fatty acids and sugar consumption during colonic *** yogurts with IGI should be a valuable strategy for development of novel added-value foods with beneficial effects on gut microbiota and human health.

phylogeographic patterns of endemic species are critical keys to understand its adaptation to future climate ***,based on chloroplast DNA,we analyzed the genetic diversity of two endemic and endangered tree species from the Brazilian savanna and Atlantic forest(Eremanthus erythropappus and Eremanthus incanus).We also applied the climate-based ecological niche modeling(ENM)to evaluate the impact of the Quaternary climate(last glacial maximum*21 kyr Bp(thousand years before present)and Mid-Holocene*6 kyr Bp)on the current haplotype ***,we modeled the potential effect of future climate change on the species distribution in 2070 for the most optimistic and pessimistic *** primer/enzyme combination(SFM/HinfI)revealed polymorphism with very low haplotype diversity,showing only three different *** haplotype 1 has very low frequency and it was classified as the oldest,diverging from six mutations from the haplotypes 2 and *** *** populations are structured and differ genetically according to the areas of *** general,the populations located in the north region are genetically different from those located in the *** genetic structuring was observed for *** ENM revealed a large distribution during the past and a severe decrease in geographic distribution of *** and *** from the LGM until present and predicts a drastic decline in suitable areas in the *** reduction may homogenize the genetic diversity and compromise a relevant role of these species on infiltration of groundwater.

海岸沙地提供了诸多宝贵的生态系统服务,例如耕地保护、水分供应和生物多样性保护,因此海岸沙地代表了重要的栖息地保护区。因为海岸沙地环境恶劣、水资源匮乏、养分贫瘠,所以土壤微生物群落对维持植物多样性至关重要。就海岸沙地的生态系统保护而言,外来入侵植物是主要威胁。本文探讨了沿海地区入侵植物食用日中花(Carpobrotus edulis)对细菌群落功能及结构的各种影响。沙地中土壤有机质含量低,微生物活性有限,所以沙地是一种颇具挑战性的底物。因此,在评估食用日中花对生态系统的影响之前需要对细菌提取进行优化,并开展功能评估。本文首先从群落水平生理特征(CLpp)的角度,比较了采用不同土壤储量、样品量、提取液等12种提取方法对沙地群落功能活性的影响,然后又探讨了食用日中花入侵的沙地菌落功能(利用Biolog Ecoplates)与结构变化(利用pCR-DGGE)。血细胞计数发现盐溶液持续增殖了细菌细胞(p≤0.001)。主成分分析(pCA)结果显示,在有限时间框架(0–24 h)内,碳底物消耗没有显著变化。通过对比天然区与非天然区(种间)及天然区(种内)之间食用日中花入侵后的变化,发现了一种与众不同的碳底物利用模式,食用日中花入侵对群落功能的影响因地而异,而与之相辅相成的是,pCR-DGGE结果表明原沙地的菌落结构不同于食用日中花入侵后沙地的菌落结构。

Effects of probiotic supplementation on abdominal pain severity in pediatric patients with irritable bowel syndrome:a systematic review and meta-analysis of randomized clinical trials

Background probiotic supplementation has been used to alleviate abdominal pain in children and adolescents with irritable bowel syndrome(IBS),but the evidence is not ***,a systematic review and meta-analysis of randomized clinical trials(RCTs)were performed to investigate the effects of probiotic supplementation on abdominal pain in pediatric patients with *** pubMed/MEDLINE,Web of Science,Scopus,Cochrane Library,and Embase were the available databases searched to find relevant randomized clinical trials up to April *** effect size was expressed as weighted mean differenee(WMD)and 95% confidence interval(CI).Results Seven RCTs with 441 participants were included,from which the meta-analysis demonstrated that probiotic supplementation has a significant effect on reducing abdominal pain in pediatric patients with IBS(WMD=-2.36:95%CI-4.12 to-0.60;p=0.009).Although our study involved children and adolescents(≤18 years),the effects of probiotic supplementation seem to be more potent in patients under 10 years old(WMD=-2.55;95%CI-2.84 to -2.27)compared to patients aged 10-18 years(WMD=-1.70;95%CI-2.18 to -1.22).The length of supplementation longer than four weeks was more effective(WMD=-2.43;95%CI-2.76 to -2.09).Conclusion probiotic supplementation can reduce abdominal pain in pediatric patients with IBS.

Dendrochronology and climatic signals in the wood of Nectandra oppositifolia from a dense rain forest in southern Brazil

Nectandra is one of the most representative genera of Lauraceae in the subtropical Atlantic Forest of Brazil. The objective of this work was to study the dendrochronological potential of Nectandra oppositifolia Nees and Mart. from two sites in Santa Catarina State in southern Brazil. A tree-ring chronology of 34 trees was developed. The time span ranged from 1843 to 2013. The oldest and youngest trees were 171 and 47 years-old and the average length of the series was 103 years. Average diameter and annual increment were 20.64 cm and0.74 mm a-1; diameter and age were unrelated. The species has good dendrochronological potential with an intercorrelation of 0.61 between sites, indicating the existence of a synchronous pattern in the development of the trees. The climate response of the species could be seen by negative tree growth effects for previous hot and current rainy growth seasons.

Is Robotic Arm Assisted Total Hip Arthroplasty More Bone preserving than Conventional Hip Replacements and Hip Resurfacing?

Background: Total hip arthroplasty (THA) is a common procedure that is increasingly being performed in younger patients. Deep acetabular reaming will result in more bone loss and the need for large acetabular components to be implanted. It can also lead to impingement, loosening, an altered center of rotation, and intraoperative periprosthetic fracture. The purpose of this study is to determine whether the single ream, robotic arm-assisted (RAA) THA can preserve a greater volume of bone stock compared to conventional hip replacement and resurfacing. Methods: We prospectively recruited 69 patients who had undergone primary THA using the Stryker Trident Acetabular System®in combination with the Stryker RAA System (MAKO)®and compared their mean reaming weight (g) with that of conventional hip replacement and resurfacing, as measured by Brennan et al. Comparison of acetabular reaming during hip resurfacing versus uncemented THA (J Orthop Surg. 2009;17(1): 42-46). Results: The mean reaming weight using the MAKO system was 9.08 g, which was 29% less than the reaming weight using uncemented THA and hip resurfacing of 12.75 g. None of the acetabular cups required screw fixation. During the 35-month follow-up period, there were no complications related to cup placement or positioning. Conclusions: The use of RAA THA results in statistically significant preservation of acetabular bone compared to conventional hip replacement and resurfacing. This approach reflects the increased precision offered by RAA single reaming. Surgeons may consider utilizing RAA THA, particularly in younger patients, to better preserve bone stock as this could potentially impact future revision procedures.
