Aim:to analyze the significance of RTI Tier 2 metaphonological and alphabetic principle program for Brazilian schoolchildren at risk for ***:a bibliographic review was carried out in the Scielo and Pubmed databases using the descriptors“intervention studies”,“reading”,“dyslexia”,and“learning”in Portuguese and English for the elaboration of RTI Tier 2 metaphonological and alphabetic principle program for schoolchildren at risk for *** program was designed based on the selection of strategies for metaphonological skills,knowledge of the alphabet,and grapheme-phoneme correspondence described in national and international *** stimuli(real words)and visual stimuli(pictures)were selected to be used in the intervention program *** stimuli were selected from a word bank according to length,regularity,and *** intervention program designed was composed of the following skills:letter-sound knowledge,syllable knowledge,syllabic segmentation,phonemic segmentation,syllabic addition,phonemic addition,syllabic subtraction,phonemic subtraction,syllabic substitution,phonemic substitution,syllabic combination,and phonemic *** program was structured with collective strategies and was applied in small groups of four to five schoolchildren aged six years and 11 months to seven years and 11 months in 50-minute ***:The results revealed that there was a change in the response pattern,showing better performance in the skills of the Early Identification of Reading Problems Protocol,demonstrating significance and positive change in post-testing situations when compared to ***:the program developed in this study proved to be effective and,in this way,it can be used as an intervention instrument based on scientific evidence that helps in the acquisition of skills necessary for the development of reading.

AIM To systematically review the incidence of ipsilateral graft re-rupture and contralateral anterior cruciate ligament(ACL) rupture following its reconstruction, with special attention to the femoral drilling technique. METHODS Systematic review and meta-analysis of high-level prospective studies searched in MEDLINE database following PRISMA statement. The rate of ipsilateral graft re-rupture and contralateral rupture in patients submitted to either transtibial(TT) technique(isometric) or anteromedial(AM) technique(anatomic) was compared. RESULTS Eleven studies met the criteria and were included in final analysis. Reconstructions using the AM technique had a similar chance of contralateral ACL rupture when compared to the chance of ipsilateral graft failure(OR = 1.08, P = 0.746). In reconstructions using TT technique, the chance of contralateral ACL rupture was approximately 1.5 times higher than ipsilateral graft failure(OR = 1.49, P = 0.048). Incidence of contralateral lesions were similar among the techniques TT(7.4%) and AM(7.0%)(P = 0.963), but a trend could be noticed with a lower incidence of lesion in the ipsilateral limb when using the TT technique(4.9%) compared to the AM technique(6.5%)(P = 0.081).CONCLUSION ACL reconstruction by TT technique leads to lower incidence of graft re-injury than contralateral ACL lesion. There is no difference between the chance of re-injury after AM technique and the chance of contralateral ACL lesion(native ligament) with either technique.

The evaluation of the new cultivars adaptation and yield potential of Mangifera indica L. provides tools to assist and improve the mango production in different climates conditions. This study aimed to evaluate the phenological and reproductive development of five mango cultivars (Bourbon, Haden, Palmer, Parwin and Tommy Atkins), on climate conditions in dry land and four production cycles (from 08/2007 to 01/2011), at S?o Manuel, S?o Paulo state, Brazil. The variables were total plant height, trunk height, stem diameter and the first insertion diameter, fruits physical characteristics, yield and harvest period, flowering seasonality and morphological characterization of the inflorescences. It was found that the four-year-old plants of Haden cv. reach 4.0 m high and 0.20 m of trunk diameter. Bourbon cultivar had the highest flowering period, from April to October. Tommy Atkins cv. had productivities of 14779.07 kg·ha-1 in the fourth cycle and the productivity of Bourbon, Haden and Palmer cultivars was alternated.

利率(国家间的利差)如果上升,债务就会依据经济基本面的情况,自发产生滚雪球效应。这是市场的一个不完美之处,因为我们不能确保市场自身会远离“差的”均衡而达到“好的”均衡。我们提出了一个政策干预机制来解决国际间资本流动的结构性弱点。这个政策干预机制的基础是一个简单的分类,它把危机的根源分为三种:信心(利差和货币危机)、基本面(实际增长率)和经济政策(初始赤字)。然后,从理论上提出了“第一贷款人”(lender of first resort,以下简称LFR)适用的一些环境。本文提出的政策机制希望通过IMF的一些支持来增强事前激励,从而化解避免危机。本文还介绍了“集体行动条款”(collective action clauses---CACs)在减少自我实现性债务危机(self-fullfilling debtcrises,文中有时也翻译为自发性债务危机)中的潜在作用。

Surgical Technique Used for Portal Vein Thrombosis when Thrombectomy Is Not Possible During Liver Transplantation

Portal vein thrombosis (PVT) was for a long time considered a barrier to liver transplantation. The aim of this study is to demonstrate the surgical technical options for portal vein reconstruction during liver transplantation in patients with PVT in which thrombectomy was not possible. Between September 1991 and March 2009, 420 liver transplanted patients were retrospectively analyzed, identifying 29 cases with PVT (6.9%). Preoperative diagnosis, preoperative risk factors, surgical technique options to treat various forms (grades) of PVT, postoperative recurrence and actuarial survival rates were studied. In three cases of PVT grade II and in one case PVT grade III the thrombectomy was insufficient, requiring some surgical technique options (13.79%). In two cases placement of iliac vein graft was performed, in one an anastomosis of the portal vein with collateral splenorenal vein and in the other with left gastric varicose. The actuarial survival rate for patients without PVT and patients with PVT and effective thrombectomy was 73.8% while those with PVT who needed some type of surgical option was 75%. Our results suggest that actuarial survival rates were similar among patients with PVT or PVT with effective thrombectomy when compared with PVT that required some surgical options.

本研究旨在探讨生长激素(GH)缺乏(GHD)儿童长期接受重组人GH(rhGH)替代治疗是否导致血浆甲状腺激素水平变化，并探讨其变化机制。对象与方法 20名GHD儿童，研究开始时14例已用GH治疗至少6个月，6例尚未用GH治疗。所有患儿均于促甲状腺激素释放激素(TRH)注射前及注射后15、30及60 min采血，测定有GH替代治疗(每日应用rhGH 0.1 U/kg体重至少6个月)和无GH替代治疗(6例GH开始治疗前，14例GH停药30～60 d后)两种不同治疗状态下患者血清游离T4(FT4)、T3、促甲状腺激素(TSH)和反T3(rT3)浓度。其中有8例甲状腺功能减退(甲减)患者，GH替代治疗期间甲状腺激素治疗剂量不变。所有患儿进入研究前临床及生化指标示甲状腺功能正常。结果根据基础TSH水平将患儿分为两组。第1组TSH＞0.4 mU/L，12例，第2组TSH＜0.05 mU/L，8例。GH替代治疗期间两组患者血清FT4水平均降低［分别为(17.0±1.1)比(14.3±0.9)mU/L，P<0.001和(18.0±1.7)比(14.2±1.7)mU/L，P<0.01］，两组间无差别；而T3水平均升高［分别为(1.8±0.1)比(2.4±0.2)nmol/L，P<0.001和(1.9±0.3)比(2.4±0.2)nmol/L，P<0.05］；rT3水平均降低［分别为(0.35±0.03)比(0.25±0.03)nmol/L，P<0.01和(0.48±0.06)比(0.34±0.06)nmol/L，P<0.01］。第1组患者GH替代治疗期间基础TSH水平及TRH刺激的TSH峰值无改变，TRH刺激的TSH释放曲线下面积无改变。两组仅1例患儿出现低FT4血症和高T3血症，6例患儿出现低rT3血症。结论 GHD儿童长期GH替代治疗可降低血清FT4水平和rT3水平，升高血清T3水平，且此改变与TSH无关，而是由外周T4向T3转化增加所致。GHD儿童长期GH替代治疗不会导致甲减。讨论 GHD儿童长期GH替代治疗不导致甲减，但可引起有诊断和生理意义的血清甲状腺激素水平的变化。由于GH替代治疗降低血清FT4水平，可使更多的患儿误诊为甲减，而中枢性甲减的诊断主要依赖FT4水平，因此建议GH治疗前血清FT4水平正常的GHD患儿于GH治疗期间应重新测定其FT4水平。另外，由于GH替代治疗也升高血清T3水平(即使患儿血清FT4水平低于正常)，GH替代治疗后血清T3水平无助于这些患儿中枢性甲减的诊断。接受GH治疗的GHD患儿rT3水平降低的意义仍需探讨。
