Objective: To evaluate the association of socio-demographic and clinical characteristics, and risk factors with occurrence of pressure ulcers in institutionalized elderly people. Method: This cross-sectional, analytical study, in quantitative approach, performed from the database analysis linked to the project “Pressure Ulcer in institutionalized elderly people: Association of incidence with the risk factors to functional and nutritional assessment” developed in six long-stay institutions for the elderly people in João Pessoa. Data were analyzed using the Statistical Package for Social Sciences (SPSS) version 20.0. Associations were made through chi square test and Odds Ratio. Results: The clinical conditions of significant associations with the occurrence of pressure ulcers were the variables neurological disorders (p = 0.011) and visual impairment (p = 0.005). As for risk factors, the most important was fecal incontinence (p Conclusion: The analysis of the associations pointed out problems that require intervention in order to prevent health implications of the elderly people and to minimize the risk and occurrence of this injury.

Evaluation of Prophylaxis with Norfloxacin and Ceftriaxone versus Ciprofloxacin during the Period of Neutropenia after Haematopoietic Stem Cell Transplantation:Retrospective Analysis of Two Sequencial Cohorts

Background: The aim of this study was to evaluate the occurrence of febrile neutropenia (FN) during aplasia after haematopoietic stem cell transplantation (HSCT) in patients who received anti-bacterial prophylaxis with norfloxacin/ceftriaxone or ciprofloxacin, between September 2006 and May 2009. Methods: We retrospectively examined all patients undergoing HSCT at S?o Paulo Hospital during the study period. Results: Of the 107 studied patients, 71 (66.3%) were included in the analysis. The exclusion criteria were as follows: occurrence of fever or infection before the transplantation (17), prophylactic use of norfloxacin (6), fever of probable non-infectious aetiology (2), and absence of prophylaxis during the aplasia period (11). Thirty-eight patients received norfloxacin and ceftriaxone as prophylaxis (Group I) and 33 received ciprofloxacin (Group II). The groups were comparable with respect to age, underlying diseases, comorbidities, and status of the underlying disease. Patients in Group II used antibiotics in a significantly higher frequency than those in Group I (66.7% vs 33.8%, p = 0.017), for 30 days before transplantation. The number of previous hospitalisations, length of hospitalisation until transplantation, type of transplant, progenitor cells used, number of CD34 cells infused, type of conditioning, and graft-versus-host disease prophylaxis were similar in both groups. After the transplantation, 28.9% and 39.4% of the patients in Groups I and II, respectively, presented Grade III and IV mucositis (p = 0.448). Neutropenia of <500 neutrophils/mm3 lasted an average of 9.5 and 9.3 days in Groups I and II, whereas neutropenia of <100 neutrophils/mm3 lasted between 6.6 and 5.7 days. FN occurred in 78.9% of patients in Group I and 81.8% in Group II, with no significant difference in the classification of these episodes. We investigated the effects of formulations based on phosphite products on gas exchange and activity of antioxidant enzymes in coffee plants. Seedlings of the Mundo Novo cultivar were submitted to various treatments composed of different formulations of with potassium phosphite (ADB 120), manganese phosphite (Reforce Mn), and fungicide (PrioriXtra®). For coffee seedlings, the combination of potassium phosphite and citrus by-products, isolated or in a combination with other products elicited the antioxidant system. Besides the high activity of antioxidant enzymes, the photosynthetic rates were higher than other treatments. The better performance of coffee seedlings treated with those formulations occurred even in absence of pathogens.

Relevant Aspects of Expert Interest in the Perception of the Facial Aesthetic Damage

Forensic aesthetic facial damage—AFD expert’s valuation is a concerning matter in court as expressively vary in between experts. In fact, differences can be quite significant suggesting examiners age, sex and professional qualification may influence this valuation. Aiming to offer a systematic and more objective evaluation, some methods for assessing AFD had been proposed lately. Known as Aesthetic Impairment Impact Perception (AIPE), Cobo-Plana’s revised methodology (2010) was idealized to minimize the examiner’s subjectivity by means of answering an ordered sequence of questions related to the specific damage perception. This research aims to access AFD perception differences (simulated cases) in between Lawyers, Heath professionals (Physicians and Dental surgeons) and general professionals (laypeople) under AIPE methodology in order to allow a better comprehension of AFD when dealing with forensic expertise cases. Within Groups perceptions varied (p < 0.05) when evaluating lesions below “important” severity grade. Raters over 35 Years age scored higher AFD lesions, and rater’s sex (as an independent variable) did not express any significant difference in AFD scoring. The tendency of under-rating AFD lesions below very important severity grade was observed being a matter of major concern considered their prevalence and the effective consequences involved to the victim. These results recommend extra caution when performing AFD evaluation and consequently expertise reports, claiming a more judicious analysis based on consolidated standards and suggest, examiners to be more perceptive to physical damage evaluation in order to include social perceptions and reactions, mainly, victims feelings and constrains over their aesthetic loss.

Prevalence of Low Levels of Vitamin D in Type 2 Diabetes at the City of Mangueirinha, Paraná, Southern Brazil

Background: Diabetes mellitus is a worldwide epidemic. In 2002 there were 173 million diabetic adults worldwide, and these numbers are expected to reach up to 300 million people by 2030. Meanwhile, vitamin D deficiency has its worldwide prevalence directly influenced by factors as solar radiation, skin color, latitude and seasons, cultural habits of populations such as clothing and food, and these factors are important to explain the different prevalences of vitamin D deficiency in the world. Methods: A prospective cross-sectional cohort study was conducted with patients in the outpatient clinic of the Health Unit of the city of Mangueirinha, Paraná, Southern Brazil. Fifty-four type 2 diabetic patients were evaluated (38 women and 16 men), aged 55.8 ± 12.6 years. The following variables were evaluated: age, ethnicity, presence of type 2 diabetes (DM2), hypertension, dyslipidemia, weight, BMI, WC, blood pressure, blood glucose, glycated hemoglobin, ionized calcium, PTH, 25-OH Vit, total cholesterol, HDL, LDL, urea, creatinine, uric acid and red cell/hematocrit. Results: mean BMI was 30.2 ± 4.4 kg/m2, indicating class 1 obesity in this population. Fasting glucose levels were approximately 169.8 ± 74.5 mg/dL. The 25-OH vitamin D values for this population were 23.4 ± 8.3 ng/mL, and 13% of them showed 25-OH vitamin D levels above 30 mg/dL. Fifty percent of those patients had vitamin D levels lower than 30 mg/dL, and 37% had less than 20 mg/dL. Conclusions: this study suggests that vitamin D is associated with low levels of vitamin D in type 2 diabetic patients. Supplementation of vitamin D should be considered in diabetic patients, when levels under 30 mg/mL are found.

The aim of the present work was to present the outcomes of the patients with Y-chromosome microdeletions treated by intracytoplasmic sperm injection （ICSI）, either using fresh （TESE） or frozen-thawed （TESE-C） testicular sperm and ejaculated sperm （EJAC）. The originality of this work resides in the comparisons between the different types of Y-microdeletions （AZFa, AZFb, and AZFc） and treatments, with detailed demographic, stimulation, embryological, clinical, and newborn （NB） outcomes. Of 125 patients with Y-microdeletions, 33 patients presented severe oligozoospermia （18 performed ICSI with ejaculated sperm） and 92 secretory azoospermia （65 went for TESE with 40 having successful sperm retrieval and performed ICSI）. There were 51 TESE treatment cycles and 43 TESE-C treatment cycles, with a birth of 19 NB （2 in AZFa/TESE-C, 12 in AZFc/TESE, and 5 in AZFc/TESE-C）. Of the 29 EJAC cycles, there was a birth of 8 NB （in AZFc）. In TESE and EJAC cycles, there were no significant differences in embryological and clinical parameters. In TESE-C cycles, there was a significant lower oocyte maturity rate, embryo cleavage rate and mean number of embryos transferred in AZFb, and a higher mean number of oocytes and lower fertilization rate in AZFc. In conclusion, although patients with AZFc microdeletions presented a high testicular sperm recovery rate and acceptable clinical outcomes, cases with AZFa and AZFb microdeletions presented a poor prognosis. Due to the reported heredity of microdeletions, patients should be informed about the infertile consequences on NB and the possibility of using preimplantation genetic diagnosis for female sex selection.

Long-Term Impact in Hepatitis C Virus Infection in Post Renal Transplantation

Purpose: The authors have evaluated the impact of the kidney transplantation associated with chronic hepatitis C virus (HCV) infection, analyzing the complications, patients and graft survival. Methods: Retrospective study with 40 kidney transplant recipients with HCV infection and 40 kidney transplant recipients without HCV infection in the same post transplantation period. Results: the average follow-up after transplantation was 12.3 ± 4.5 years in patients with HCV infection and 12.5 ± 2.9 years in patients without HCV infection (p = 0.49). There was no statistical difference related to age and gender of the recipient nor donor age and type. The current renal function in patients with HCV infection was 47.3 ± 24.9 mL/min and 54.9 ± 27.2 mL/min in the HCV negative group (p = 0.54). The incidence of graft and patient survival was similar in both groups. The main cause of death in both groups was bacterial infection (10% in patients with HCV infection and 12.5% in HCV negative patients (p = 0.63). The most common complication in the two groups were acute allograft rejection and bacterial infection. The incidence of diabetes mellitus did not differ statistically in both groups. Abnormal liver enzymes levels and cirrhosis were observed only in patients with HCV infection. Conclusion: HCV infection did not impact patient or graft survival and post-transplant complications were similar in both groups during a mean follow-up period of 12 years.
