Prevalence and outcomes of pancreatic cystic neoplasms in liver transplant recipients

AIM To determine the prevalence,characteristics and clinical course of pancreatic cystic neoplasms(PCNs) in liver transplantation(LT) *** We retrospectively studied consecutive patients who underwent LT between January 1998 to April 2016. Clinical and laboratory data were obtained from patient medical records. Imaging findings on computed tomography and magnetic resonance cholangiopancreatography were reviewed by two *** During the study period,872 patients underwent cadaveric LT. Pancreatic cysts were identified in 53/872(6.1%) and 31/53(58.5%) were PCNs [28 intraductal papillary mucinous neoplasm(IPMN),2 mucinous cystic neoplasm(MCN),1 serous cystadenoma]. Patients with PCNs exhibited less male predominance(55% vs 73%,P = 0.03) compared to patients without pancreatic cysts. Thirteen patients(42%) were diagnosed with PCN pre-LT while 18 patients(58%) developed PCN post-LT. The median size of PCNs was 13 mm [interquartile range(IQR) 10-20 mm]. All IPMNs were side-branch type. Most PCNs were found in the head and body of pancreas(37% each),followed by the tail(25%). Five patients underwent further evaluation with endoscopic ultrasound. Progress imaging was performed on 81% of patients. PCNs remained stable in size and number in all but 2 patients. During a median follow up of 39 mo(IQR 26-58 mo),the 2(6%) patients with MCN underwent pancreatectomy. No PCN patient developed pancreatic adenocarcinoma,while 5 died from illnesses unrelated to the PCN. Among patients without PCN,1/841(0.1%) developed pancreatic *** The prevalence of PCNs in LT recipients was similar to the general population(3.6%,31/872). Side-branch IPMNs do not appear to have accelerated malignant potential in post-LT patients,indicating the current surveillance guidelines are applicable to this group.

Introduction: Cranioencephalic exploration has always played a major role in CT scans. In the Central Africa Republic (CAR), the lack of cross-sectional imaging before the year 2020 meant that no study had focused on cranioencephalic lesions. The aim of this study was to contribute to improving the management of cranioencephalic pathologies in CAR. Patients and Method: The study took place at the Bangui National Medical Imaging Centre (CNIMB). It was a retrospective study over a two-year period (March 1, 2021 to February 30, 2023). All patients referred for cranioencephalic CT scans were included, regardless of age or sex. Results: 1745 CT scans were performed, 575 of which were cranioencephalic CT scans. The majority of patients were male (53%). Most lived in the capital Bangui (90.9%). Patients aged 61 and over were the most representative. The distribution of patients by requesting department showed that the reception and emergency department was one of the least requesting departments. The main abnormalities observed were strokes, 82.1% of which were ischaemic strokes and 17.9% haemorrhagic strokes. Strokes were followed by degenerative lesions. Post-traumatic injuries included haemorrhagic contusions (38.3%), subdural haematomas in 20.5% of cases, and extradural haematomas (9.3%). Craniofacial lesions (fractures) were observed in 45.8% of cases. Conclusion: Cranioencephalic scans accounted for 1/3 of CT examinations performed during the study period. It revealed pathologies that could not be detected by conventional means. All in all, CT scans contributed to the diagnosis of cerebral pathologies.

Background: In Central African Republic, a study carried out in 2019 highlighted the limits of conventional radiography in the diagnosis of non-traumatic lower back pain in adults. Objective: The objective of this study is to show the value of CT scanning in the exploration of non-traumatic pathologies of the lumbar spine in adults. Patients and Method: Retrospective and descriptive cross-sectional study covering the files of patients aged at least 18 years old, sent for lumbar scanning from March 1 to December 31, 2021 at the medical imaging center. Results: In total, 593 scan examinations were performed including 159 (26.8%) examinations of the lumbar spine. The average age of the patients was 49.84 years. The majority were male (66%). 127 (79.8%) were referred mainly to the neurology and rheumatology departments. Common low back pain (n = 97, 61%) was the main reason for requesting a CT scan. The lumbar scan was performed without contrast product in 88.7% of cases. In total, 148 (93.1%) examinations were pathological and dominated by overall disc overhang (62.2%) and somatic and interapophyseal osteoarthritis (16.9%). The other lesions were spondylodiscitis (7.4%), tumor-like lesions (3.4%) and narrow lumbar canal (1.4%).

Clustering is one of the recently challenging tasks since there is an *** amount of data in scientific research and commercial applications. High quality and fast document clustering algorithms are in great demand to deal with large volume of data. The computational requirements for bringing such growing amount data to a central site for clustering are complex. The proposed algorithm uses optimal centroids for *** clustering based on Particle Swarm Optimization(PSO).PSO is used to take advantage of its global search ability to provide optimal centroids which aids in generating more compact clusters with improved accuracy. This proposed methodology utilizes Hadoop and Map Reduce framework which provides distributed storage and analysis to support data intensive distributed applications. experiments were performed on Reuter's and RCV1 document dataset which shows an improvement in accuracy with reduced execution time.

This article reviews biological invasions in which predation （or its absence） plays a major role in the success of the invader. examples are described in which the invader out-competes native species for the same food, and cases in which the in-vader consumes valued native species. In many instances, better predator avoidance by the invasive species or the absence of predators in the new habitat contributes to the success of the invaders; in other cases native or introduced predators appear to be able to keep the invasive species in check. A relatively new management approach in the US is the idea of adding another trophic level - to have humans act as the predators and consume the invasive species. This approach is being utilized in Florida and throughout the Caribbean against the lionfish, but could be extended to other fishes, as well as to various invasive crustaceans and mollusks. This idea is controversial, and current regulations prohibiting the possession of individuals of the invasive species （e.g., mitten crabs or snakefish） would preclude the development of a fishery for them [Current Zoology 57 （5）： 613~524, 2011].

Natural regeneration responses to thinning and burning treatments in ponderosa pine forests and implications for restoration

Understanding naturally occurring pine regeneration dynamics in response to thinning and burning treatments is necessary not only to measure the longevity of the restoration or fuels treatment,but also to assess how well regeneration meets forest sustainability guidelines and whether natural regeneration is sufficient for maintaining a sustainable forest structure and composition.A synthesis review was carried out on the effects of mechanical thinning and prescribed burn treatments on natural pine regeneration response in frequent-fire ponderosa pine forests across the western United *** focus was on site-specific variability in pine regeneration dynamics,temporal trends in regeneration presence and abundance,and response to treatment as described in the current literature using 29 studies that met our evidence-based review *** showed that the effects of thinning and burning treatments on regeneration depended on time since *** thinning,prescribed burning,and thinning plus burn treatments all increased seedling density,but there was high variability among sites and *** were mixed results in the short-term(ears)with both increasing and decreasing regeneration,and a general increase in regeneration 11−20 years *** long-term studies(>20 years)concluded that stands can return to pre-treatment densities in terms of total trees per hectare and forest floor duff levels when there are no maintenance treatments *** studies showed the average ponderosa pine seedling presence,survival and growth found in today’s forests to be at a high density;this combined with missed fire cycles could contribute to future fire risk and reduce the efficacy of maintaining fuel reduction goals.

The larval stages of the cestode parasites belonging to the genus echinococcus grow within internal organs of humans and a range of animal *** resulting diseases,collectively termed echinococcoses,include major neglected tropical diseases of humans and *** larvae are outwardly protected by the laminated layer(LL),an acellular structure that is unique to this *** LL is based on a fibrillar meshwork made up of mucins,which are decorated by galactose-rich *** addition,in the species cluster termed *** sensu lato,the LL features nano-deposits of the calcium salt of myo-inositol hexakisphosphate(Insp_(6)).The main purpose of our article is to update the immunobiology of the *** recent advances in this area are(i)the demonstration of LL“debris”at the infection site and draining lymph nodes,(ii)the characterization of the decoy activity of calcium Inp6 with respect to complement,(iii)the evidence that the LL mucin carbohydrates interact specifically with a lectin receptor expressed in Kupffer cells(Clec4F),and(iv)the characterization of what appear to be receptor-independent effects of LL particles on dendritic cells and *** information is missing on the immunology of this intriguing structure:we discuss gaps in knowledge and propose possible avenues for research.
