Increasing pancreatic cancer is not paralleled by pancreaticoduodenectomy volumes in Brazil:A time trend analysis

Background: Currently, surgical resection represents the only curative treatment for pancreatic cancer(PC), however, the majority of tumors are no longer resectable by the time of diagnosis. The aim of this study was to describe time trends and distribution of pancreaticoduodenectomies(PDs) performed for treating PC in Brazil in recent years. Methods: Data were retrospectively obtained from Brazilian Health Public System(namely DATASUS) regarding hospitalizations for PC and PD in Brazil from January 2008 to december 2015. PC and PD rates and their mortalities were estimated from DATASUS hospitalizations and analyzed for age, gender and demographic characteristics. Results: A total of 2364 PDs were retrieved. Albeit PC incidence more than doubled, the number of PDs increased only 37%. Most PDs were performed in men(52.2%) and patients between 50 and 69 years old(59.5%). Patients not surgically treated and those 70 years or older had the highest in-hospital mortality rates. The most developed regions(Southeast and South) as well as large metropolitan integrated municipalities registered 76.2% and 54.8% of the procedures, respectively. LMIM PD mortality fluctuated, ranging from 13.6% in 2008 to 11.8% in 2015. Conclusions: This study suggests a trend towards regionalization and volume-outcome relationships for PD due to PC, as large metropolitan integrated municipalities registered most of the PDs and more stable mortality rates. The substantial differences between PD and PC increasing rates reveals a limiting step on the health system resoluteness. Reduction in the number of hospital beds and late access to hospitalization, despite improvement in diagnostic methods, could at least in part explain these findings.

Moringa oleifera Lam(Moringaceae) is a plant with high nutritional and medicinal value. Native to India, it is now widely distributed throughout tropical and subtropical regions of the world. Its different parts are sources of proteins, vitamins and minerals and present different pharmacological and biotechnological potential. Moreover, M. oleifera seeds are widely used in water and effluent treatment, for their coagulation, flocculation and sedimentation properties, their ability of improving water quality, by reducing organic matter and microbial load, with special applicability in intensive animal production systems, such as aquaculture. In addition, due to its high nutritional value and several medicinal properties, this tree may act as a nutritional and medical alternative for socially neglected populations. In this context, this review gathers information on M. oleifera, emphasizing its chemical constituents, nutritional, pharmacological and antimicrobial properties, applications in the treatment of water effluents, and ecological and social aspects.

Although the high diversity of plant species in the rupestrian fields has been primarily attributed to the existence of a set of distinct habitats, few studies support this assertion. The present study aimed to further investigate the relationship between physical and chemical attributes of soils with the diversity of plant species in this unique ecosystem. The rupestrian field is a unique vegetation formation that covers some of the southeastern Brazilian mountains in the transition of the Atlantic rain forest and the Cerrado(savanna). Different habitats occur according to soil characteristics(e.g., presence of rocks, sand, fertility, hydrology, etc.). These attributes ultimately influence the vegetation that is highly adapted to the harsh edaphic and climatic mountain conditions. Five distinct habitats were studied by us: rocky outcrops, peat bogs, sandy bogs, quartz gravel fields, and "cerrado"(savanna). A floristic survey indicated that four families are found at greater frequency: Poaceae, Asteraceae, Cyperaceae, and Leguminosae. The greatest diversity of plant species was found in the rocky outcrops habitat, followed by cerrado, peat bog, quartz gravel grassland, and sandy bogs, respectively. The main difference in the floristic composition among these habitats was related to the dominant species. Trachypogon spicatus(Poaceae) was the most dominant species in the rocky outcrops, Axonopus siccus(Poaceae) in the peat bogs, Lagenocarpus rigidus(Cyperaceae) in the sandy bogs, Schizachyrium tenerum(Poaceae) in the cerrado, while Vellozia sp. 8(Velloziaceae) dominated the vegetation in the quartz gravel grassland. This study demonstrated that physical and chemical soil properties strongly related the diversity of plant species occurring in the different habitats of rupestrian fields.

Sugarcane mosaic virus(SCMV)is the main etiological agent of sugarcane mosaic disease,which affects sugarcane and other grass *** the extensive characterization of quantitative trait loci controlling resistance to SCMV in maize,the genetic basis of this trait in sugarcane is largely ***,a genome-wide association study was performed and machine learning coupled with feature selection was used for genomic prediction of resistance to SCMV in a diverse sugarcane *** single-nucleotide polymorphisms(SNPs)explained up to 29.9%of the observed phenotypic variance and a 73-SNP set predicted resistance with high accuracy,precision,recall,and F1 scores(the harmonic mean of precision and recall).Both marker sets were validated in additional sugarcane genotypes,in which the SNPs explained up to 23.6%of the phenotypic variation and predicted resistance with a maximum accuracy of 69.1%.Synteny analyses suggested that the gene responsible for the majority of SCMV resistance in maize is absent in sugarcane,explaining why this major resistance source has not been identified in this ***,using sugarcane RNA-Seq data,markers associated with resistance to SCMV were annotated,and a gene coexpression network was constructed to identify the predicted biological processes involved in *** network allowed the identification of candidate resistance genes and confirmed the involvement of stress responses,photosynthesis,and the regulation of transcription and translation in resistance to *** results provide a practical marker-assisted breeding approach for sugarcane and identify target genes for future studies of SCMV resistance.

Effects of corn gluten feed inclusion at graded levels in a corn-soybean diet on the ileal and fecal digestibility of growing pigs

Background： This study aimed to determine the effect of the inclusion of corn gluten feed （CGF） on the apparent and standardized ileal digestibility of protein and amino acids and the apparent ileal and total tract digestibility of energy in growing pigs. The study was performed using 16 barrows （weight, 45.3 ±4.5 kg） that were fitted with a T cannula at the terminal ileum. There were four treatments： a corn-soybean diet without CGF and three corn-soybean diets containing increasing levels of CGF （65, 130, and 195 g/kg）. Data were analyzed according to a randomized complete block design, four blocks with four pigs each （one pig per treatment）. The trend of the response （linear or quadratic） was determined using orthogonal contrasts, and when a linear effect was determined, a linear equation was obtained. Results： The results showed that the inclusion up to 195 g/kg of CGF in the corn-soybean diet did not diminish the ileal digestibility （apparent and standardized） of protein and amino acids （P 〉 0.05）, except that of phenylalanine, cystine, and proline. A linear decrease （P 〈 0.05） per gram of CGF added to the diet in the apparent and standardized ileal digestibility of phenylalanine （0.011 and 0.015 percentage units, respectively）, cystine （0.048 and 0.043 percentage units, respectively）, and proline （0.045 and 0.047 percentage units, respectively） was noted. Similarly, ileal digestibility of dry matter and energy were adversely affected （reduced by 0.028 and 0.025 percentage units, respectively, per gram of CGF increment in the diet）. A significant （P 〈 0.05） linear reduction in total tract digestibility with increase in CGF amount in the diet was observed for energy （0.027 percentage units）, dry matter （0.027 percentage units）, crude protein （0.020 percentage units）, and neutral detergent fiber （0.041 percentage units） per gram of CGF added to the diet. Grade 4 febrile neutropenia and Fournier's Syndrome associated with triple therapy for hepatitis C virus:A case report

The use of triple therapy for hepatitis C not only increases the rate of sustained virological responses compared with the use of only interferon and ribavirin(RBV) but also leads to an increased number of side effects. The subject of this study was a 53-year-old male who was cirrhotic with hepatitis C virus genotype 1 A and was a previous null non-responder. We initially attempted retreatment with boceprevir(BOC), Peginterferon and RBV, and a decrease in viral load was observed in the 8th week. In week 12, he presented with disorientation, flapping, fever, tachypnea, arterial hypotension and tachycardia. He also exhibitedleucopenia with neutropenia. Cefepime and filgrastim were initiated, and treatment for hepatitis C was suspended. A myelogram revealed hypoplasia, cytotoxicity and maturational retardation. After 48 h, he developed bilateral inguinal erythema that evolved throughout the perineal area to the root of the thighs, with exulcerations and an outflow of seropurulent secretions. Because we hypothesized that he was suffering from Fournier's Syndrome, treatment was replaced with the antibiotics imipenem, linezolid and clindamycin. After this new treatment paradigm was initiated, his lesions regressed without requiring surgical debridement. Triple therapy requires knowledge regarding the management of adverse effects and drug interactions; it also requires an understanding of the importance of respecting the guidelines for the withdrawal of treatment. In this case report, we observed an adverse event that had not been previously reported in the literature with the use of BOC.

AIM：To investigate the cytotoxic effects of spray-dried extracts of Phyllanthus niruri in combination with cis- platin on two cancer cell lines. METHODS： Colorectal carcinoma （HT29） and human hepatocellular carcinoma （HepG2） cells were treated with spray-dried extracts of Phyllanthus niruri （SdePN） either alone or in combination with cisplatin at differ- ent concentrations （0.5 mg/mL and 1 mg/mL） for 4 h and 24 h. To verify and quantify cancer cells treated with these products as well as identify the cell cycle stage and cell viability, we stained the cells with prop- idium iodide and assessed them by flow cytometry. The percentage of cells in different cell cycle phases was quantified and data were expressed as histo- grams. Significant differences between groups were determined using analysis of variance and Bonferroni＇s test, as indicated. A value of P 〈 0.05 was considered to be statistically significant. RESULTS： SdePN had significantly different cyto- toxic effects on HT29 （2.81 4- 0.11 vs 3.51 4- 1.13, P 〉 0.05） and HepG2 （5.07± 0.3 vs 15.9 ± 1.04, P 〈 0.001） cells when compared to control cells for 4 h. SdePN also had significantly different cytotoxic effects on HT29 （1.91 ± 0.57 vs 4.53± 1.22, P 〉 0.05） and HepG2 （14.56 ± 1.6 vs 35.67 ± 3.94, P 〈 0.001） cells when compared to control cells for 24 h. Both cell lines were killed by cisplatin in a dose-dependent manner compared to control cells （HepG2 cells for 4 h： 10.78 ± 1.58 vs 53.89 ± 1.53, P 〈 0.001; 24 h： 8.9 ± 1.43 vs 62.78 ± 1.87, P 〈 0.001 and HT29 cells for 4 h： 9.52 ±0.913 vs 49.86 ± 2.89, P 〈 0.001; 24 h： 11.78 ± 1.05 vs 53.34 ± 2.65, P 〈 0.001）. In HT29 cells, pretreat- ment with SdePN and subsequent treatment with cis-platin resulted in a greater number of cells being killed （12.78 ± 1.01 vs 93.76 ± 1.6, P 〈 0.001）.
