Critical limb ischemia is a medical condition that decreases blood flow and limb oxygen supply;this disease in its late stages of progression leads to only two possible options: either surgical bypass revascularization or limb amputation. We investigated a novel method using autologous transplantation of progenitor cells derived from mobilized peripheral blood bone marrow mononuclear cells to evaluate its long-term effect as a cell therapy to induce neo-angiogenesis and restore blood flow in the affected ischemic limbs. A total of 20 ischemic limbs from critical limb ischemia diagnosed patients, non candidates to surgical revascularization were transplanted with autologous progenitor cells by either intramuscular combined with intravenous (group A) or intramuscular (group B) procedure. Patients were monitored during 31 months. Treatment efficacy was evaluated according to the following parameters: ankle brachial index which increased at a range of 0.29-1.0 in group A and 0.40-0.90 in group B;pain-free walking distance which increased at a range of 50-600 m in group A and 50-300 m in group B;and blood perfusion (measured by Laser Doppler) which increased at a range of 48-299 in group A and 135-225 in group B. We achieved 90% treated ischemic limbs free of amputation in both transplanted groups. Results here described provide a safe, efficient and minimally invasive therapy with progenitor cells to induce angiogenesis and preserve limbs from amputation in CLI diagnosed patients.

Characterizing the diversity and structure of host–parasite communities is crucial to understandingtheir eco-evolutionary *** and related haemosporidian parasites are responsible forfitness loss and mortality in bird species ***,despite exhibiting the greatest ornithologicalbiodiversity,avian haemosporidians from Neotropical regions are quite ***,we analyze the genetic diversity of bird haemosporidian parasites(Plasmodium andHaemoproteus)in 1,336 individuals belonging to 206 bird species to explore for differences in diversityof parasite lineages and bird species across 5 well-differentiated Peruvian *** 70 different haemosporidian lineages infecting 74 bird *** showed that 25 out ofthe 70 haplotypes had not been previously ***,we also identified 81 new host–parasite interactions representing new host records for these haemosporidian *** outcomesrevealed that the effective diversity(as well as the richness,abundance,and Shannon–Weaver index)for both birds and parasite lineages was higher in Amazon basin ***,we also showed that ecoregions with greater diversity of bird species also had highparasite richness,hence suggesting that host community is crucial in explaining parasite *** parasites were found in ecoregions with lower bird diversity,implying that the abundanceand richness of hosts may shape the exploitation strategy followed by haemosporidian *** outcomes reveal that Neotropical region is a major reservoir of unidentified *** studies analyzing host distribution and specificity of these parasites inthe tropics will provide important knowledge about phylogenetic relationships,phylogeography,and patterns of evolution and distribution of haemosporidian parasites.

在地中海国家扩大灌溉土地的面积,应当考虑气候与土地利用变化对水资源的影响。本文利用降水和温度资料,分析了20世纪中叶以来西班牙比利牛斯山脉中部流域径流的变化趋势。降水与径流之间关系的年变化表明,径流在所研完期间的后半时段相对较低,这一趋势与土地利用变化一致。在月水平上,降水量在10月、4月和7月明显增加,而在3月则有下降:温度在1月和2月有所增加,而在4月则趋于下降。但过去50年间径流在大部分月份中呈显著下降趋势,土地利用和植被覆盖的变化是解释年平均径流下降大约30%的惟一非气候因素。

BACKGROUND: This study aimed to compare the rates of open and laparoscopic cholecystectomies and outcomes in patients with or without type 2 diabetes mellitus (T2DM) in Spain from 2003 to 2013. METHODS: We collected all cases of open and laparoscopic cholecystectomies using national hospital discharge data and evaluated the annual cholecystectomy rates stratiifed by T2DM status. We analyzed tendency for in-hospital mortality (IHM). We also analyzed the impact of T2DM on IHM in patients who underwent cholecystectomies. RESULTS: We identiifed 611 533 cholecystectomies (71.3%laparoscopic) in the patients, in whom 78 227 (12.8%) patients had T2DM. The rates of open cholecystectomies were 3-fold higher (130.0/105 vs 41.1/105) in patients with T2DM than in those without T2DM, and the rate of laparoscopic cholecys-tectomies was almost 2-fold higher (195.2/105 vs 111.8/105) in patients with T2DM. Efficacy of Alzer<sup>®</sup>and Diamel, Nutritional Supplements, in the Prevention of Severe Diabetic Macular Edema

Introduction: Macular edema is the main cause of low vision in diabetic patients. Laser photocoagulation continues to be the treatment of choice in conjunction with the use of steroids and anti-angiogenics, but these treatments include possible ocular complications. The nutritional supplement Alzer (whose primary active ingredient is Ginkgo biloba, a powerful antioxidant that acts on vascular factors and oxidative damage, which are two of the mechanisms implicated in the pathogenesis of diabetic macular edema), which has been used on other non-diabetic macular conditions, along with the Diamel nutritional supplement has been shown to be effective on glycemic control and could represent a treatment alternative for mild to moderate macular edema by reducing the thickness of the macular retina and preventing the progression of other more advanced clinical presentations that are harder to treat. Objective: Identify the effect of Alzer along with Diamel in reduction of the thickness of the macular retina among patients with mild to moderate diabetic macular edema. Materials and Methods: A phase II double-blind clinical trial was conducted in 64 patients with non-severe diabetic macular edema over the course of non-proliferative diabetic retinopathy, who attended the ophthalmology service of the Institute of Endocrinology of Havana from January 2016 to December 2016. To investigate physicochemical stability of sevofuranein dimethyl sulfoxide using gas chromatography with a fame ionization detector and nuclear magnetic reso-nance （NMR）.METHODSUndiluted sevoflurane, plus dilutions 1：2, 1：5, 1：10, 1：25, and 1：50 in dimethyl sulfoxide were prepared in a vertical laminar fow cabinet class Ⅱ type B and stored at different temperatures （23 ℃, 6 ℃, and -10 ℃） for 45 d. Sterile 1 mL polypropylene amber syringes to minimize light degradation, caps and needles were used. The presence of sevofurane and its degradation products in the samples was determined by gas chroma-tography with flame ionization detector （260 ℃, 40min）, and by 1H, 19F, and proton-decoupled 19F nuclearmagnetic *** The gas chromatography analysis showed sevofluraneand dimethyl sulfoxide （DMSO） retention times were 2.7and 13.0 min, respectively. Pure DMSO injection into thecolumn resulted in two additional peaks at 2.1 and 2.8min. The same sevofurane peak at 2.7 min was observedin all the dilutions at -10 ℃, 4 ℃ and 25 ℃. The NMRspectra showed signals consistent with the sevoflurane structure in all the dilutions at -10 ℃, 4 ℃ and 25 ℃. In the 1H spectrum, two signals corresponding to the sevoflurane molecule were observed at 5.12 and 4.16 parts per million （ppm5）. In the 19F-NMR spectrum, two signals were observed at -76.77 ppm and -157.13 ppm. In the 19F NMR CPD, two signals were observed at -76.77 ppm and -157.13 ppm. The first one showed a doublet （JF-F = 3.1 Hz） which integrated by six fluorine nuclei from the hexafluoro-isopropyl group. The second signal was integrated by a fuorine atom and showed a septuplet （JF-F = 3.1 Hz）.CONCLUSIONThis study shows that different concentrations ofsevofurane in dimethyl sulfoxide retain their chemicalcomposition after exposure to different temperaturesfor a period of 45 d.
