Introduction: Congenitally corrected transposition of the great arteries (CCTGA) is a rare heart disease that encompasses an atrioventricular and ventriculoarterial discordance and accounts for less than 1 percent of congenital heart diseases. Objective: To present an atypical case of a man with complex congenital heart disease and conduction anomalies. Case Presentation: This is a case of a 34-year-old patient who came to the hospital with 1 week of dyspnea on exertion and episodes of lipothymia. The patient was referred to our hospital after an electrocardiogram from his primary care with evidence of blocked atrial fibrillation. During the initial evaluation blocked atrial fibrillation was confirmed with a rescue ventricular rate of 38 bpm. A magnetic resonance confirmed the presence of the atrioventricular and ventriculoarterial discordance, the aorta had a left anterior position, perimembranous ventricular septal defect with a right to left shunt, biventricular systolic dysfunction, moderate tricuspid, and mitral regurgitation, interventricular septal intramyocardial and biatrial fibrosis, left atrial dilation, and dilation of the pulmonary artery. After pacemaker placement, the patient has an improvement in his clinical symptoms and quality of life. The main objective of this research is to recognize several geological structures associated with the shear zones of the MFFS（Magallanes-Fagnano fault system） by the analysis and interpretation of gravimetric anomalies. Besides, to compare the gravimetrical response of the cortical blocks that integrate the region under study, which is related to the different morphotectonic domains recognized in the region. This research was developed employing data obtained from World Gravity 1.0, which includes earth and satellite gravity data derived from the EGM2008 model. The study and interpretation of the MFFS from the analysis and processing of the gravimetric data, allowed appreciation of a noticeable correlation with the most superficial cortical structure.

Minimally Invasive Surgical Approach for Spinal Canal Tumors—Technique Description and Experience from a Reference Center

Background: Spinal canal tumors are difficult to diagnose and treat. The traditional surgical approaches for attempting a complete excision of these lesions frequently involve big incisions and tissue dissection with high risk of postop instability and cerebrospinal fluid leakage. Also, there is a risk of neurological worsening, sometimes irreversible. Methods: We present our experience in a patient series with spinal canal tumors and describe the surgical approach with minimally invasive techniques (MIS). All of them were performed by the Neurosurgery team of the Hospital Universitario San ignacio during the period of 2011-2016. Results: We reviewed forty patients with spinal canal tumors surgically treated with MIS techniques. 15 patients (37.5%) had Meningioma diagnosis (complete resection in 11 (73.3%), subtotal in 3 (20%) and biopsy in one patient), 10 patients (25%) with Schwannomas reached complete resection in 70% and subtotal in 30%. 5 patients had spinal cord metastasis, with complete resection in 4 patients (80%) and subtotal in 1 (20%). Other included ependymoma, astrocytoma, and miscellaneous. No patient has had cerebrospinal fluid leakage and no postoperative fusion has been required. Conclusions: The minimally invasive approach allowed complete tumors removal in a high number of patients and good postoperative results. These findings are similar and in some cases, better than the reported with traditional techniques. This MIS technique provides encouraging results. It requires a wide learning curve and a high degree of surgical experience.

由于气候的变化直接或间接地影响了诸如生物多样性、雪盖、冰川、径流过程、水的可利用性等各种不同的关键要素,人们认为地中海群山区的生物群落正在遭受威胁。本文中,基于不同排放情景(排放情景特别报告)和大气-海洋耦合总体循环模型(AOGCMs,Atmosphere-Ocean-Coupled General CirculationModels),我们对两个时期即2055(2040～2069年间)和2085(2070～2099年间)期间地中海群山区的温度、降水量、春季降水量的变化进行了评价。最后,将地中海群山区和欧洲地区非地中海群山区的未来气候变化趋势的预测结果进行了对比。预计到2055期间,温度将增高1.4～5.1℃,而到2085期间将增温1.6～8.3℃。气候模型也预测地中海群山区的降水量将减少,且主要集中于春季(2085期间,排放情景Alfi下减少17%,B1下减少4.8%)。而与之相反,欧洲非地中海群山的年降水量和春季降水量并不会减少。将预期的气候变化对自然和人文特性的意义结合到一个综合框架中,以得到一个关于未来趋势及其后果的更广阔的视角。
