Relationship between clinical belonging,professional identity,and nursing information ability among nursing interns:Model construction

BACKGROUND Clinical belonging refers to the feeling that clinical medical staff feel recognized and accepted by others or groups.The level of clinical belonging of nursing interns affects students’learning motivation and confidence,which in turn affects their clinical practice behavior.AIM To explore the effects of professional identity and nursing information ability on clinical belonging among nursing interns and establish a relationship model for these factors.METHODS The researchers used the convenience sampling method to select 682 nursing interns from China.The survey was conducted using a general information questionnaire,clinical sense of belonging scale,nursing information ability self-assessment scale,and a nursing student professional identity questionnaire.The mediating effect of nursing information ability between their professional identity and clinical sense of belonging was analyzed using SPSS 21.0 and the path analysis in structural equation modeling.RESULTS The total scores of clinical belonging,professional identity,and nursing information ability of nursing interns were(104.29±13.11)points,(57.89±7.16)points,and(70.29±6.20)points,respectively.Nursing information ability had a direct effect on the clinical sense of belonging(effect value=0.46,P<0.05).Occupational identity had a direct effect(effect value=0.52,P<0.05)and an indirect effect(effect value=0.21,P<0.05)on clinical belonging.CONCLUSION Nursing administrators in nursing colleges and hospitals should take effective measures to improve the professional identity and nursing information ability of nursing interns,as well as the clinical sense of belonging among nursing interns.

Intelligent diagnosis of retinal vein occlusion based on color fundus photographs

AIM:To develop an artificial intelligence(AI)diagnosis model based on deep learning(DL)algorithm to diagnose different types of retinal vein occlusion(RVO)by recognizing color fundus photographs(CFPs).METHODS:Totally 914 CFPs of healthy people and patients with RVO were collected as experimental data sets,and used to train,verify and test the diagnostic model of RVO.All the images were divided into four categories[normal,central retinal vein occlusion(CRVO),branch retinal vein occlusion(BRVO),and macular retinal vein occlusion(MRVO)]by three fundus disease experts.Swin Transformer was used to build the RVO diagnosis model,and different types of RVO diagnosis experiments were conducted.The model’s performance was compared to that of the experts.RESULTS:The accuracy of the model in the diagnosis of normal,CRVO,BRVO,and MRVO reached 1.000,0.978,0.957,and 0.978;the specificity reached 1.000,0.986,0.982,and 0.976;the sensitivity reached 1.000,0.955,0.917,and 1.000;the F1-Sore reached 1.000,0.9550.943,and 0.887 respectively.In addition,the area under curve of normal,CRVO,BRVO,and MRVO diagnosed by the diagnostic model were 1.000,0.900,0.959 and 0.970,respectively.The diagnostic results were highly consistent with those of fundus disease experts,and the diagnostic performance was superior.CONCLUSION:The diagnostic model developed in this study can well diagnose different types of RVO,effectively relieve the work pressure of clinicians,and provide help for the follow-up clinical diagnosis and treatment of RVO patients.

Impact of Annealing on the Melt Recrystallization of a-PDLA/α-PLLA Double-layered Films

Poly(lactic acid)(PLA) as a bio-based polymer with biodegradability and biocompatibility has attracted much attention. To manipulate its properties for different applications, regulation of crystal structure and crystalline morphology becomes an attractive research topic. In this work, the structure evolution of layered samples containing an amorphous poly(D-lactide)(PDLA) layer and a crystalline poly(L-lactide)(PLLA)layer with highly oriented edge-on α lamellar crystals after annealing at 150 ℃ or/and after melt-recrystallization has been studied by AFM, FTIR,and TEM combined with electron diffraction. The results demonstrate that melt recrystallization of the as-prepared sample leads to the formation of abundant randomly oriented PLA stereo-complex(PLA SC) crystals. Annealing at 150 ℃ results in the formation of a small amount of oriented PLA SC crystals at the interface. These PLA SC crystals show great impact on the recrystallization behavior of sample after melting at 190 ℃ and then crystallizing at 90 ℃. First, they impede the mutual diffusion of the overlying PDLA and underlying PLLA, and thus reduce their stereocomplexation ability as manifested by the decreased amount of PLA SC crystals. Second, they act as substrate to initiate the epitaxial crystallization of the overlying PDLA and underlying PLLA, which ensures the production of a highly oriented structure of PDLA and PLLA after melt recrystallization again.

Polypropylene(PP)is considered to be a rather promising material for HVDC cable insulation.Grafting modification has proved to be an effective method on the electrical property improvements of PP.This paper reports on the highlyenhanced electrical properties of styrene-grafted polypropylene(PP-g-St)and their related mechanism.The influence of grafting styrene on crystallization structures,thermal properties and electrical properties of PP-g-St are characterized and analyzed.These results show that PP-g-St exhibits remarkably enhanced DC volume resistivity,suppressed space charge accumulation and improved DC breakdown strengths as compared to PP.Crystallization observation shows PP-g-St has small,imperfect spherulites with indistinct boundaries,which is strongly correlated to enhanced electrical properties.This study provides an effective grafting strategy to design high-performance HVDC cable insulation materials for bulk power transmission systems.

BACKGROUND Diffusion-weighted imaging(DWI)has been developed to stage liver fibrosis.However,its diagnostic performance is inconsistent among studies.Therefore,it is worth studying the diagnostic value of various diffusion models for liver fibrosis in one cohort.AIM To evaluate the clinical potential of six diffusion-weighted models in liver fibrosis staging and compare their diagnostic performances.METHODS This prospective study enrolled 59 patients suspected of liver disease and scheduled for liver biopsy and 17 healthy participants.All participants underwent multi-b value DWI.The main DWI-derived parameters included Mono-apparent diffusion coefficient(ADC)from mono-exponential DWI,intravoxel incoherent motion model-derived true diffusion coefficient(IVIM-D),diffusion kurtosis imaging-derived apparent diffusivity(DKI-MD),stretched exponential model-derived distributed diffusion coefficient(SEM-DDC),fractional order calculus(FROC)model-derived diffusion coefficient(FROC-D)and FROC model-derived microstructural quantity(FROC-μ),and continuous-time random-walk(CTRW)model-derived anomalous diffusion coefficient(CTRW-D)and CTRW model-derived temporal diffusion heterogeneity index(CTRW-α).The correlations between DWI-derived parameters and fibrosis stages and the parameters’diagnostic efficacy in detecting significant fibrosis(SF)were assessed and compared.RESULTS CTRW-D(r=-0.356),CTRW-α(r=-0.297),DKI-MD(r=-0.297),FROC-D(r=-0.350),FROC-μ(r=-0.321),IVIM-D(r=-0.251),Mono-ADC(r=-0.362),and SEM-DDC(r=-0.263)were significantly correlated with fibrosis stages.The areas under the ROC curves(AUCs)of the combined index of the six models for distinguishing SF(0.697-0.747)were higher than each of the parameters alone(0.524-0.719).The DWI models’ability to detect SF was similar.The combined index of CTRW model parameters had the highest AUC(0.747).CONCLUSION The DWI models were similarly valuable in distinguishing SF in patients with liver disease.The combined index of CTRW parameters had the highest AUC.

Fabrication of a nano-sized ZSM-5 zeolite with intercrystalline mesopores for conversion of methanol to gasoline

Carbon deposition during methanol to hydrocarbons leads to the quick deactivation of ZSM-5 catalyst and it is one of the major problems for this technology. Decreasing the crystal size or introducing mesopores into ZSM-5 zeolites can improve its diffusion property and decrease the coke formation. In this paper, nano-sized ZSM-5 zeolite with intercrystalline mesopores combining the mesoporous and nano sized structure was fabricated. For comparison, the mesoporous ZSM-5 and nano-sized ZSM-5 were also prepared. These catalyst samples were characterized by XRD, BET, NH3-TPD, TEM, Py-IR and TG techniques and used on the conversion of methanol to gasoline in a fixed-bed reactor at T=405 degrees C, WHSV =4.74 h(-1) and P=1.0 MPa. It was found that the external surface area of the nano-sized ZSM-5 zeolite with intercrystalline mesopores reached 104 m(2)/g, larger than that of mesoporous ZSM-5 (66 m(2)/g) and nano sized ZSM-5 (76 m(2)/g). Catalytic lifetime of the nano-sized ZSM-5 zeolite with intercrystalline mesopores was 93 h, which was only longer than that of mesoporous ZSM-5 (86 h), but shorter than that of nano sized ZSM-5 (104 h). Strong acidity promoted the coke formation and thus decreased the catalytic lifetime of the nano-sized ZSM-5 zeolite with intercrystalline mesopores though it presented large external surface that could improve the diffusion property. The special zeolite catalyst was further dealuminated to decrease the strong acidity. After this, its coke formation rate was slowed and catalytic lifetime was prolonged to 106 h because of the large external surface area and decreased weak acidity. β弛豫行为及应力弛豫是探索非晶合金微观结构非均匀性的重要切入口.β弛豫行为及应力弛豫与非晶合金扩散、玻璃转变行为以及塑性变形等密切相关,探究它们之间关联有重要科学意义.本文以典型的La基非晶合金作为研究载体,系统地探究了通过不同冷却速率所得块体样品与条带样品的β弛豫特征及应力弛豫行为.结果表明,冷却速率是控制玻璃体系能量状态,进而影响其物理力学性能的重要参数.冷却速率越大,低温内耗更大,对应β弛豫激活能更小,β弛豫行为在温度谱中展宽更大,表明高冷却速率导致原子移动性更大,微观尺度结构非均匀程度更高.基于应力弛豫测量与分析,发现高冷却速率变形特征时间减小,归一化应力衰减幅度更大,在外加力场作用下更易变形,变形单元更易激活以容纳结构形变.本研究为进一步厘清非晶合金β弛豫、变形及微观结构非均匀性之间关联提供了思路.

Analysis of the Effect of Shenghua Zhuyu Herbal Acupoint Plaster in the Treatment of Postpartum Malodor

Objective:To assess the clinical effect of Shenghua Zhuyu ointment,which was applied onto the Zhongji,Qihai,and Guanyuan acupoints,on postpartum lochia.Methods:We randomly divided 166 patients who were admitted in 2018-2021 into an observation group and a control group.The control group was treated with conventional clinical methods,whereas the observation group was treated by applying Shenghua Zhuyu ointment on the Zhongji,Qihai,and Guanyuan acupoints.The amount and duration of lochia,abdominal pain,hospitalization time were observed;B-scan ultrasonography was performed to check whether the placenta was discharged.Results:The treatment received by the observation group of patients was shown to be more effective than that of the control group.Besides,the experimental group had less lochia than that of the control group.The observation group also had shorter duration of lochia and hospital stay than those of the control group(P<0.05).The placenta excretion was better,and the degree of abdominal pain was lower in the observation group than those of the control group(P<0.05).Conclusion:The acupoint application of Shenghua Zhuyu herbal acupoint plaster has a significant impact on retained placenta,in which it can aid the discharge of residual placenta and improve the patients’physical functions and symptoms.

Development and validation of tongue imaging-based radiomics tool for the diagnosis of insomnia degree:a two-center study

Background:Traditional Chinese medicine(TCM)is commonly used for the diagnosis and treatment of insomnia,with tongue diagnosis being particularly important.The aim of our study was to develop and validate a novel tongue imaging-based radiomics(TIR)method for accurately diagnosing insomnia severity.Methods:This two-center analysis prospectively enrolled 399 patients who underwent tongue imaging between July and October 2021 and divided them into primary and validation cohorts by study center.Here,we referred to the Insomnia Severity Index(ISI)standard and the degree of insomnia was evaluated as absent,subthreshold,moderate,or severe.For developed the TIR diagnostic tool,a U-Net algorithm was used to segment tongue images.Subsequently,seven imaging features were selected from the extracted high-throughout radiomics features using the least absolute shrinkage and selection operator algorithm.Then,the final radiomics model was developed in the primary cohort and tested in the independent validation cohort.Finally,we assessed and compared the diagnostic performance differences between TCM tongue diagnosis and our TIR diagnostic tool with the ISI gold standard.The confusion matrix was calculated to evaluate the diagnostic performance.Results:Seven tongue imaging features were selected to build the TIR tool,with showing good correlations with the insomnia degree.The TIR method had an accuracy of 0.798,a macro-average sensitivity of 0.78,a macro-average specificity of 0.906,a weighted-average sensitivity of 0.798,and a weighted specificity of 0.916,showing a significantly better performance compared to the average performance of three experienced TCM physicians(mean accuracy of 0.458,P<0.01).Conclusions:The preliminary study demonstrates the potential application of TIR in the diagnosis of insomnia degree and measurement of sleep health.The integration of quantitative imaging analysis and machine learning algorithms holds promise for advancing both of TCM and precision sleep medicine.

[目的]水土漏失控制是喀斯特地区石漠化防治的关键,因此对水土漏失机理及其影响因素进行探究。[方法]通过室内模拟试验,研究水压力(0.3,1.3,2.3,5.5 m水头)、岩溶裂隙宽度(0.25,0.50,0.75,1.00 cm)和土壤团聚体粒径(0~1.0,1.0~2.0,2.0~5.0,0~5.0 mm)对峰丛洼地水土漏失过程的影响。[结果]水压力和裂隙宽度越大、团聚体粒径越小或粒组缺失的土壤,上覆土层越易被击穿。土层被击穿时间约需2~20 min。在土层被击穿后,水漏失速率迅速增大并逐渐趋于稳定,土壤漏失速率则急剧增大至峰值后减小并趋于其稳定值,且水土漏失速率及其累积漏失量显著大于未击穿土层。水土漏失量随着水压力或裂隙宽度的增大而增大。当裂隙宽度为0.25,0.50,0.75,1.00 cm时,1.3~5.5 m水压力时的累积水漏失量分别比0.3 m水压力时增大1.2~13.2,2.4~131.0,2.1~167.3,75.8~141.9倍。累积土壤漏失量随团聚体粒径的增大而显著减小。在大团聚体缺失时土壤漏失量随着裂隙宽度的增大而减小,而在小团聚体缺失时则随之增大。当裂隙宽度为0.25,0.5,0.75,1.0 cm时,1.0~2.0,2.0~5.0 mm团聚体粒组的土壤漏失量分别比0~1.0 mm粒组减小98.4%和99.1%,46.3%和83.7%,43.2%和74.0%及41.1%和27.1%。[结论]上覆土层的稳定状况决定峰丛洼地水土漏失过程,水力条件、土壤性质及岩溶裂隙发育程度是影响其稳定的关键因素。
