New radiolarian ages show that the island arc-related Acoje block of the Zambales Ophiolite Complex is possibly of Late Jurassic to Early Cretaceous *** dating of its plutonic and volcanichypabyssal rocks yielded middle Eocene *** the other hand,the paleontological dating of the sedimentary carapace of the transitional mid-ocean ridge-island arc affiliated Coto block of the ophiolite complex,together with isotopic age datings of its dikes and mafic cumulate rocks,also yielded Eocene *** offers the possibility that the Zambales Ophiolite Complex could have:(1)evolved from a Mesozoic arc(Acoje block)that split to form a Cenozoic back-arc basin(Coto block),(2)through faulting,structurally juxtaposed a Mesozoic oceanic crust with a younger Cenozoic lithospheric fragment or(3)through the interplay of slab rollback,slab break-off and,at a later time,collision with a microcontinent fragment,caused the formation of an island arc-related ophiolite block(Acoje)that migrated trench-ward resulting into the generation of a back-arc basin(Coto block)with a limited subduction *** Meso-Cenozoic ophiolite complex is compared with the other oceanic lithosphere fragments along the western seaboard of the Philippines in the context of their evolution in terms of their recognized environments of generation.

Clinical value of rapid urine trypsinogen-2 test strip, urinary trypsinogen activation peptide, and serum and urinary activation peptide of carboxypeptidase B in acute pancreatitis

AIM: To assess the usefulness of urinary trypsinogen-2 test strip, urinary trypsinogen activation peptide (TAP),and serum and urine concentrations of the activation peptide of carboxypeptidase B (CAPAP) in the diagnosisof acute ***: Patients with acute abdominal pain and hospitalized within 24 h after the onset of symptoms were prospectively studied. Urinary trypsinogen-2 was considered positive when a clear blue line was observed (detection limit 50 μg/L). Urinary TAP was measured using a quantitative solid-phase ELISA, and serum and urinary CAPAP by a radioimmunoassay ***: Acute abdominal pain was due to acute pancreatitis in 50 patients and turned out to be extrapancreatic in origin in 22 patients. Patients with acute pancreatitis showed significantly higher median levels of serum and urinary CAPAP levels, as well as amylase and lipase than extrapancreatic controls. Median TAP levels were similar in both groups. The urinary trypsinogen-2 test strip was positive in 68% of patients with acute pancreatitis and 13.6% in extrapancreatic controls (P<0.01). Urinary CAPAP was the most reliable test for the diagnosis of acute pancreatitis (sensitivity 66.7%, specificity 95.5%, positive and negative predictive values 96.6% and 56.7%, respectively), with a 14.6 positive likelihood ratio for a cut-off value of 2.32 nmol/***: In patients with acute abdominal pain,hospitalized within 24 h of symptom onset, CAPAP in serum and urine was a reliable diagnostic marker of acute pancreatitis. Urinary trypsinogen-2 test strip showed a clinical value similar to amylase and *** TAP was not a useful screening test for the diagnosis of acute pancreatitis.

Enzymes of Entomopathogenic Fungi, Advances and Insights

Entomopathogenic fungi (EF) are recognized biological control agents of insects. Basically, the entomopathogenic fungi pathogen activity depends on the ability of its enzymatic equipment, consisting of lipases, proteases and chitinases, which are in charge of breaking down the insect’s integument. Lipases are the first enzymes synthesized by the entomopathogenic fungi. Recently, a cytochrome P450 subfamily, referred as CYP52XI and MrCYP52 has been identified in Beauveria bassiana and Metarhizium robertsii, respectively. These break down long-chain alkenes and fatty acids to become initial nutrients. Subsequently, subtilisin type (Pr1) proteases sintetize;these enzymes are considered as virulence indicators and they are regulated by a signal transduction mechanism activated by the protein kinase A (PKA) mediated by AMPc. Through the employment of genetic engineering, it has been possible to increase virulence producing Pr1 recombinants with Androctonus australis neurotoxins or with chitinases, reducing the insect’s time of death. In the course of time, the Pr1 protease gene has presented evolutionary adaptations by gene duplication or horizontal transfer infecting different orders of insects. In the same way, the entomopathogenic fungi chitinases have presented a functional diversification. Currently, these have been phylogenetically classified into three subgroups, in accordance to the catalytic site domain and the chitin binding domain. The chitinolytic activity has increased through a directed evolution processes and genetic recombination with Bombyx mori chitinase. Recently, enzymes have been employed as control agents for insects and phytopathogenic fungi (disease originator) opening new potentialities in order to improve the entomopathogenic fungi use. Post-transplant plasma cell hepatitis in a liver transplant patient treated with pegylated interferon plus ribavirin

The recurrence of hepatitis C after a liver transplant remains an important cause of graft loss and retransplant. Antiviral therapy with peginterferon plus ribavirin (PEG-INF/RBV) can achieve a sustained viral response and histological improvement in a high percentage of cases. However, this treatment is not exempt from important side effects or from the possibility of precipitating rejection, with the resulting graft loss. We report the case of a liver transplant patient who received treatment with PEG-INF/RBV and developed plasma cell hepatitis as the presenting form of rejection.

Assessment of Beauveria bassiana and Their Enzymatic Extracts against Metamasius spinolae and Cyclocephala lunulata in Laboratory

The application of enzymatic extracts and conidia of Beauveria bassiana in Metamasius spinolae and Cyclocephala lunulata was evaluated. The variables were mortality and time of death. In M. spinolae, mortality with extracts 29%, conidia 27% and the combination of both 31%, all had a time of death of four days. Although with different symptoms, used enzymatic extracts: contraction and softening of the joints;by conidia: mycelium in the joints;in the combination of conidia and enzymatic extracts: abundant aerial mycelium. In C. lunulata, 100% mortality in all treatments;Time of death: enzymatic extracts and extracts with conidia 1.2 days;conidia 2.8 days. Symptoms were different, enzymatic extracts: melanization and internal tissue lysis;enzymatic extract and conidia: mycelium emerged and melanization;conidia: mycelium emerged. Enzymatic extracts showed insecticidal activity in M. spinolae and C. lunulata. These results suggest the potential of enzymatic extracts as biocontrol agents to improve the use of entomopathogenic fungi.

Caregiver Burden in the Management of Frail Elderly Patients with Diabetes in Internal Medicine

Aim: Advanced age and fragility often lead to dependence, making caregiver assistance necessary for performing the activities of daily living. Patients with diabetes are characterized by an elevated presence of comorbidities and subsequent frailty—a relevant consideration when making decisions regarding treatment goals. Health professionals may overlook the caregiver experience when assessing the circumstances surrounding frail elderly patients with diabetes, but this factor can have a large impact on relevant caregiver and patient outcomes. Methods: An observational, retrospective study was carried out to assess the impact of improved management in diabetes on caregiver burden. We assessed patients’ performance of activities of daily living using the Barthel scale. Our primary outcome was caregiver burden, as assessed using the Zarit scale. Results: At baseline, 81.4% of the caregivers reported severe overburden (>55 points on the Zarit scale). At three months, the proportion of caregivers reporting this level of burden had dropped to 76.7% and remained stable at six months (77.1%). In contrast, 8.5% of the caregivers reported moderate overburden (47 to 55 points) at baseline, compared to 13.3% at three months and 12.5% at six. The proportion of caregivers reporting no overburden (Conclusions: Caregiver burden remained persistently high, decreasing only slightly after six months.

Acute Respiratory Distress Syndrome (ARDS) Case Report after a Caesarean Section Related to the Immune Reconstitution Inflammatory Syndrome

During pregnancy, a cellular immunosuppressant status is produced. It is characterized by anti-inflammatory cellular responses that allow embryonic implantation. The reversal of these changes during the postpartum period may result in overt clinical manifestations of otherwise quiescent or latent infections. We present an Acute Respiratory Distress Syndrome (ARDS) case after a caesarean section related to the Immune Reconstitution Inflammatory Syndrome (IRIS) in a Guinean black woman. The patient was treated with large doses of corticosteroids showing a great clinical and radiological improvement.

Improving outcomes in pancreatic cancer: Key points in perioperative management

This review focused in the perioperative management of patients with pancreatic cancer in order to improve the outcome of the disease. We consider that the most controversial points in pancreatic cancer management are jaundice management, vascular resection and neoadjuvant therapy. Preoperative biliary drainage is recommended only in patients with severe jaundice, as it can lead to infectious cholangitis, pancreatitis and delay in resection, which can lead to tumor progression. The development of a phase Ⅲ clinical trial is mandatory to clarify the role of neo-adjuvant radiochemotherapy in pancreatic adenocarcinoma. Venous resection does not adversely affect postoperative mortality and morbidity, therefore, the need for venous resection should not be a contraindication to surgical resection in selected patients. The data on arterial resection alone, or combined with vascular resection at the time of pancreatectomy are more heterogeneous, thus, patient age and comorbidity should be evaluated before a decision on operability is made. In patients undergoing R0 resection, arterial resection can also be performed.

Investigation of Chemical Bath Deposition of ZnO Nanocrystals Thin Films Using Different ZnNO3 Concentration

In this work, thin films growth at different ZnNO3 concentrations: 0.02 M, 0.03 M and 0.04 M by chemical bath deposition (CBD) as-prepared on glass substrates at 80 °C ± 2 °C. Patterns Diffraction x-ray (DRX) shown peaks 2θ = (31.72, 32.769, 34.36, 36.10, 47.44, 56.52, 62.78, 67.84 and 69.02), at concentration 0.02 M show formation from hydrozincite [Zn5(CO3)2(OH)6]. The optical band gap shifts from 3.8 eV to 3.6 eV. Photoluminescence (PL) spectra show intensity variation of the green emission (GE) may be resulted from variation of the intrinsic defect in ZnO films, such zinc vacancy (VZn), oxygen vacancy (VO), and interstitial zinc (Zni), interstitial oxygen (Oi). The presence of defects in our samples is responsible for the observed broad visible PL bands; this indicates reductions of oxygen vacancies and increase of oxygen interstitials. It could be believed that the presence of defects in samples is responsible for the observed broad visible PL band. The concentration increased the visible defect luminescence, which might be due to improvement to sample stoichiometric. The presence of both green emission (GE) and yellow emission (YE) can be seen through the deconvolution of the defect increasing the concentration, the position of the GE shifted from 1.96-2.45 eV and the increase of its intensity. The shifts of these two visible PL bands might be due to changes in local environments of the defects centers in samples due to increased concentration. Therefore, the GE relate to donor levels dominated due to increase of oxygen vacancies. ZnO is an n-type semiconductor and it means that most defects are Zn interstitials and oxygen vacancies. Raman spectra showed increase of the 437 cm-1 (E2 mode) peak intensity due concentration 0.4 M.

Conversion from calcineurin inhibitors to mTOR inhibitors stabilizes diabetic and hypertensive nephropathy after liver transplant

AIM: To investigate if conversion to the mammalian target of rapamycin inhibitors(mTORi) improves renal function in diabetic and/or hypertensive liver transplant patients immunosuppressed with tacrolimus or ***: The study included 86 liver graft recipients immunosuppressed with mTORi treatment after orthotopic liver transplantation(OLT), including all liver recipients with worsening renal function before conversion to mTORi(n = 55 patients) and recipients with normal renal function who converted to m TORi for other reasons(n = 31 patients). We identified patients with diabetes mellitus(n = 28), arterial hypertension(n = 27), proteinuria(n = 27) and all three factors(n = 8)(some patients have hypertension and diabetes and no proteinuria). The primary endpoint was evolution in renal function defined as the development in plasma creatinine as a function of diabetes mellitus(DM), hypertension(HT) or proteinuria. We required elevated serum creatinine for at least two weeks to define renal ***: Only patients that converted because of renal failure with plasma creatinine levels > 1.5 mg/dL showed an improvement of renal function(2.14 to 1.77 mg/dL)(P = 0.02). Patients with DM showed no improvement of serum creatinine levels(1.31 mg/dL to 1.37 mg/dL) compared with non DM patients(1.31 mg/dL to 1.15 mg/dL)(P = 0.01), HT patients(1.48 mg/dL to 1.5 mg/dL) with non HT patients(1.21mg/d L to 1.08 mg/dL) and patients with proteinuria(1.44 mg/dL to 1.41 mg/dL) and no proteinuria(1.31 mg/dL to 1.11 mg/dL). CONCLUSION: In OLT recipients with diabetes or hypertensive nephropathy, conversion to m TORi does not improve renal function but stabilizes plasma levels of creatinine. Proteinuria is not a contraindication to conversion to m TORi; it also stabilizes renal function. Conversion to m TORi should only be avoided in patients with diabetes, hypertension and proteinuria.
